Le maggiori problematiche sono dovute ad un’inappropriata alimentazione o sovralimentazione o insufficiente riposo. La dieta, nei soggetti diabetici, durante il Ramadan non dovrebbe differire significativamente da un dieta bilanciata ed adeguata finalizzata al mantenimento del peso corporeo. Molti studi hanno dimostrato che il 50-60% dei soggetti che digiunano mantengono il loro peso corporeo durante il mese, mentre il 20-25% subisce un calo ponderale. Raccomandazioni nutrizionali La pratica comune di ingestione di grandi quantità di alimenti che ricchi di grassi e carboidrati, in particolare al pasto tramonto, dovrebbe essere evitata. Si raccomanda di aumentare l’introito di liquidi soprattutto durante le ore di “non digiuno”. Il pasto del mattino dovrebbbe contenere carboidrati complessi e essere ritardato il piu possibile prima di iniziare il digiuno. Riassumendo: alcuni giorni fa giorni fa Open Heart ha pubblicato una revisione e meta-analisi che mette in discussione le raccomandazioni presenti da decenni nelle linee guida USA e UK sulle dosi di grassi saturi e insaturi da includere nella dieta per prevenire le malattie cardiovascolari. Nel 1977 e nel 1983 i governi degli Stati Uniti e del Regno Unito, rispettivamente, hanno introdotto linee guida dietetiche nazionali che consigliano la riduzione del consumo dei grassi, soprattutto saturi, con l'ambizione di ridurre malattia coronarica. Fino ad oggi, nessuna analisi è stata effettuata per valutare l'effettiva utilità di queste raccomandazioni. Questo studio analizza i lavori pubblicati fino al 1983 su cui dovrebbero essere state basate tali Linee Guida. Sei studi rispettavano i criteri di inclusione e sono stati considerati, per un totale di 2.467 soggetti. Cinque dei sei studi riguardavano la prevenzione primaria e solo uno quella secondaria. La durata media è stata di 5,4 ± 3,5 anni. La mortalità per tutte le cause e per le malattie cardiovascolari, in particolare, non presentava differenze significative tra i gruppi. The paper reports the change of diet, lipids, and lipoproteins produced during Ramadan in one Tunisian population. During Ramadan, the study subjects consumed more proteins, cholesterol, vitamin E (p<0.01), and polyunsaturated fatty acids (p<0.05). At the same time, they exhibited an increase in total cholesterol, low-density lipoprotein-cholesterol (p<0.01) and apoprotein B (p<0.05) and a decrease in the ratio of apoprotein AI to apoprotein B (p<0.01). All assayed saturated fatty acids were unaffected by Ramadan fasting while three unsaturated fatty acids (C18:1cis9, C18:2n-6, and C30:4n-6) increased significantly. For the study subjects, Ramadan was clearly associated with a change of diet and biochemical profile but its effective impact on atherosclerosis risk was unclear, perhaps, because other non-alimentary changes ought to be considered too. Future studies considering the no-alimentary factors, such as sleep and physical activity, would be useful to clarify the contribution of dietary change in the observed modification of biological profile.
