USL 3 Pistoia Anziani in RSA… . Anziani in RSA… Solving the problem of malnutrition among older people is not only a public health imperative; it could also yield important economic benefits. Malnutrition is a significant and under-recognized problem in older adults. Malnutrition prevalence of older adults ranges from 17% to 65% depending on the setting and the method of assessment. ANDJ 2012;112:376-381 The ‘hidden’ problem of malnutrition “Malnutrition does not show up in the streets in Europe. Instead malnutrition is a hidden health problem residing at home Ljungqvist O & de Man F. Nutr Hosp 2009; 24(3):368-370. Older adults in long term-care facilities are at greatest nutritional risk, with malnutrition more likely amongst older residents and/or those who require a higher level of care Malnourished older adults have increased hospital admissions and length of stay; increased incidence of pressure ulcers, falls, and hip fractures and poorer quality of life. The severity of malnutrition is a strong predictor of morbidity even when controlling for severity of illness and malnourished older adults have increased mortality rates ANDJ 2012;112:376-381 Vecchi da morire… Malnutrition is a neglected area of health policy and clinical practice. It is almost as if this ancient clinical knowledge has been overwhelmed by the excitement generated by new treatments and drugs. Vecchi da morire… Earlier identification and appropriate nutrition support may help to reverse or halt the malnutrition trajectory and the negative outcomes associated with poor nutritional status A nutrition screening process is recommended to help detect people at malnutrition risk. Screening should be a simple process for identifying those at risk of malnutrition. Nutrition screening does not require nutrition expertise and can be performed by any staff member. In the absence of nutrition screening, malnutrition may not be recognized or treated. “Over 70 screening tools were reviewed in 2005, including many for the elderly, but excluding many so-called ‘local’ screening tools” Elia M & Stratton R. Identificare lo stato nutrizionale Mini Nutritional Assessment (MNA) 2. Milano: MASSON; 1993. 2009 Apart from situations contraindicating oral feeding, nutritional support should, as a priority, be initiated by providing dietary advice and/or fortified foods, if possible in collaboration with a dietitian… It may also be necessary to modify the food texture according to the person’s chewing and swallowing abilities and to assist feeding when required… 2009 The simplest and safest way for nutritional support is eating more ordinary food, in some cases fortified by adding cream, milk, oil and similar ingredients, resulting in increased energy and protein intake Portion sizes should be reduced and the energy and protein density of the food increased (energy-dense menus). The use of snacks or supplemental foods between regular meals is important, especially in older people, because activities connected with food intake can positively stimulate the rehabilitation process. The standardized nutritional care protocol adds approximately 600 kcal and 12 grams of protein to the daily intake of the malnourished Hinke Kruizenga Dutch Dietetic Association, 2006 Appetite for life! An adequate amount of protein intake with each meal is essential Although specialized supplementation with amino acid products may be beneficial or necessary in specific elderly or clinical populations, the most practical means of promoting skeletal muscle protein anabolism for the majority of older adults is to include a moderate serving of protein of high biological value during each meal. Paddon-Jones et al 2009 Appetite for life! Although the role of dietary protein in the prevention of sarcopenia remains unclear, a protein intake moderately greater than that amount may be beneficial to enhance muscle protein anabolism and reduce progressive loss of muscle mass with age. Some experts suggest that a protein intake of 1.0 to 1.6 g/kg daily is safe and adequate to meet the needs of healthy older adults Houston DK, Nicklas BJ, Ding J, et al. Although lack of caloric ingestion is a hallmark of all cachectic illnesses, the role of caloric supplementation in its management is controversial. Vecchi da morire… Fat provides more calories per gram than does protein or carbohydrate. As such it may be considered an excellent method to deliver calories in anorectic persons Fat is a potent releaser of the satiety hormone, cholecystokinin (CCK). For this reason, excess quantities of fat should be avoided Vecchi da morire… Published evidence on polypharmacy relations with quality of diet in elderly people is scarce High number of drugs in use means also increased risk of drug–nutrient interactions leading to decreased absorption of essential nutrients Curr Opin Clin Nutr Metab Care 2012, 15:1–6 Nutrition patient safety issues Analysis of nutrition related patient safety incident between 1st January 2009 and 31st December 2009 • All incidents reported as death and severe harm – 597 • A sample of 300 incidents reported as moderate, low or no harm – from a pool of 38,437 incidents • Following analysis approx 20% of the incidents were identified as nutrition related patient safety incidents UK National Patient Safety Ageny Reported deaths and severe harm Pressure sores Choking Lack of assessment Nil by Mouth – prolonged periods of time Inappropriate diet Dehydration Artif icial Nutrition Naso-gastric tube Transf er of care Lack of assistance Medications Nil by Mouth – not observed Allergy Catering services Delay in assessment Delay in treatment Discharge UK National Patient Safety Ageny Reported moderate, low and no harm Artificial nutrition Nil by Mouth – prolonged period of time Inappropriate diet Lack of assessment Nil by Mouth – not observed Choking Catering services Missed meals Pressure sores Dehydration Nasogastric tube Weighing Scales UK National Patient Safety Ageny Appetite for life!
