PERFORMANCE CHARACTERISTICS Precision: Within-run precision was assessed using Control Plasmas N and P, on an automatic and a manual instrument. The results are shown in the following table. APTT (English) Activated Partial Thromboplastin Time (APTT) Reagent System reagents for Coagulyzer® analyzers CG0021 CG0022 Within-run Precision Results: Sample 10x2mL 10x4mL INTENDED USE The APTT is composed of liquid soy phosphatides with plasma activator for use in the determination of activated partial thromboplastin time (APTT) in citrated plasma and related coagulation procedures. SUMMARY The main use for the APTT is for the screening of coagulation defects and the presence of inhibitors. The test is prolonged by the deficiencies of factors VIII, IX, X, XI and XII and defects of the contact phase, e.g. prekallikrein, high molecular weight kininogen. It also may be prolonged by gross defects of factor II, V and fibrinogen. With reliable APTT systems, specific and non-specific inhibitors of intrinsic clotting factors are detected. The APTT is also the most widely used method for the laboratory monitoring of heparin administration (1,2). The APTT is also used as in the ECAT study as an overall assessment of the intrinsic clotting system, Accelerated APTT have been reported after operations, oral contraceptive administration and withdrawal or oral anticoagulation (3). Coagulyzer® 100 (Automatic) Coagulyzer® 1,2,4 (Manual) Control Plasma N 1.21 % 3.40 % Control Plasma P 1.50 % 3.18 % Heparin Sensitivity: Numerous variables can affect the behavior of heparinized plasma, when evaluated by the APTT test. As a general guideline, when increasing amounts of Heparin were added to normal plasma, the following results were obtained using the APTT on a photo-optical instrument. Heparin Concentration (NIBSC – unfractionated) (units / mL) REAGENT The APTT is a preparation of soy phospholipids and ellagic acid activator with buffer, stabilizers and preservatives. The reagent is provided ready to use. Mix reagent by gentle inversion (5 to 8 times) before use. PRECAUTIONS Do not ingest. Avoid contact with skin, eyes or clothing. STORAGE AND STABILITY The APTT is stable according to the following specifications. DO NOT FREEZE. Unopened 2 – 8°C Refer to vial label Open vial 2 – 8°C 7 days On-board Coagulyzer®100 18 - 26°C 5 days For Coagulyzer® 1,2,4 Instruments 18 - 26°C 5 days SPECIMEN COLLECTION AND PREPARATION Test plasma should be prepared from citrated whole blood without heparin, EDTA or oxalate. To obtain the plasma, carefully mix 1 part sodium citrate solution (0.11 mol/L) with 9 parts venous blood, avoiding the formation of foam. Immediately centrifuge for 10 minutes at approximately 3000 rpm (1500 x g), remove the supernatant plasma and keep at + 15 to + 25°C until use in the test. In the US please refer to the NCCLS Document H21-A2, entitled “Collection, Transport and Processing of Blood Specimens for Coagulation Testing and Performance of Coagulation Assays”(4). The samples must not have any contact with glass. Do not incubate samples at 37°C for longer than 5 minutes to avoid the loss of factors V and VII. MATERIALS PROVIDED APTT MATERIALS REQUIRED BUT NOT PROVIDED 1. Calcium Chloride 0.025M (REF CG0024) 2. Control Plasmas N and P (REF CG0081 and CG0082) PROCEDURE This procedure pertains to manual or semi-automated coagulation systems. Refer to your instrument manual for more detailed instrument specific instructions. WARRANTY This product is warranted to perform in accordance with its labeling and literature. Analyticon disclaims any implied warranty of merchantability or fitness for any other purpose. Purchaser must calibrate and determine the suitability of Analyticon‘s products for their specific applications. In no event will Analyticon be liable for any consequential damages arising out of aforesaid express warranty. Sample, control or standard plasma 50µl APTT. 120s Incubation at 37°C (in the measuring block). 50µl Calcium Chloride (0.025M) as start reagent (pre-warmed at 37°C). The clotting time in seconds will be recorded automatically by the analyzer. Refer to the instrument operator’s manual for further information. QUALITY CONTROL Reliability of test results should be monitored by use of Control Plasma N-P. A control range should be established by the laboratory to determine the allowable variation in day to day performance for each level of control. Failure to meet quality control specifications should be investigated and resolved. Control Plasma N Control Plasma P (REF CG0081) (REF CG0082) Normal Values Abnormal Values In case the values of Control Plasma are out of the given ranges, check: Calibration, reagent, stability of sample/controls or analyzer. CALCULATION OF RESULTS For best results, duplicate samples are recommended. APTT results should be reported as clotting time in seconds. Calculate the mean clotting time of duplicate samples and controls. Differences between duplicate results should be less than 5%. Repeat the test if necessary. LIMITATIONS Stress, exercise, pregnancy, the post-partum state and surgical operations result in acceleration of the test. Prothrombic changes associated with recent deep-vein thrombosis, thromboembolic disorders and oestrogen administration may result in accelerated clotting times (5). Acquired pathological states such as liver disease, disseminated intravascular coagulation (DIC), and drug toxicity cause prolongation. Certain drugs, including oral anticoagulants, heparin and thrombolytic agents, prolong the APTT. EXPECTED VALUES Analyzer Method Normal Range (in seconds) Coagulyzer® 1,2,4 Manual 25 - 39 Coagulyzer® 100 Automated 25 - 39 Therapeutic ranges for monitoring anticoagulation therapy will vary from laboratory to laboratory; therefore it is essential that each laboratory establish relevant APTT ranges for its respective patient population. Abnormal results obtained with a plasma from a patient not on anticoagulant therapy may indicate a factor deficiency or the presence on an inhibitor. The result may also be due to the effects of certain drugs and medications. Additional procedures such as the PT test and mixing studies using factor deficient plasma are usually required. PRECAUCIONES No ingerir. NO CONGELAR.
