Grosseto (Italia) DETERMINAZIONE COLORIMETRICA DEI METABOLITI REATTIVI DELL'OSSIGENO (DERIVATI DEI RADICALI LIBERI) NEL PLASMA E NEL SIERO Principio I radicali liberi dell’ossigeno sono atomi singoli o raggruppati, uno dei quali, almeno, appartiene all’ossigeno e possiede uno o più elettroni spaiati in uno dei suoi orbitali più esterni. A causa della loro estrema reattività, i radicali liberi tendono a reagire praticamente con qualsiasi molecola organica vengono a contatto, generando, così, derivati o metaboliti reattivi dell’ossigeno (reactive oxygen metabolites, ROM). Questi ultimi, pur dotati di un discreto potere ossidante, sono relativamente più stabili dei radicali che li hanno generati e, pertanto, possono essere quantificati. Nel d-ROMs test, i ROM (principalmente gli idroperossidi, ROOH), contenuti nel campione biologico da analizzare, in presenza di ferro (liberato dalle proteine plasmatiche da un tampone acido, il reagente R2 del kit), generano, per la reazione di Fenton, radicali alcossilici (R−O*) e perossilici (R− OO*) che, reagendo con un’ammina aromatica sostituita (A−NH2, contenuta in una miscela cromogena, il reagente R1 del kit), ossidano quest’ultima trasformandola in un derivato colorato in rosa ([A−NH2*]+), secondo le reazioni: 1A) R−OOH + Fe2+→ R−O* + Fe3+ + OH1B) R−O* + A−NH2 → R−O- + [A−NH2*]+ Per i radicali alcossilici 2A) R−OOH + Fe3+→ R−OO* + Fe2+ + H+ 2B) R−OO* + A−NH2 → R−OO- + [A−NH2*]+ Per i radicali perossilici Tale derivato, infine, viene quantificato fotometricamente. Infatti, l’intensità del colore sviluppato sarà direttamente proporzionale alla concentrazione dei ROM, in accordo con la legge di Lambert-Beer. In queste condizioni essi rimangono stabili fino alla data di scadenza riportata sulla confezione. Campione Il test può essere effettuato sia su siero che su plasma eparinato. Non usare plasma−citratato o plasma−EDTA. Preparazione del calibratore Aprire il flaconcino contenente il calibratore con le dovute precauzioni, evitando qualsiasi perdita del contenuto. Aggiungere, quindi, 1 mL di acqua distillata. Riporre il tappo di gomma sul flaconcino e agitare delicatamente, evitando di fare schiuma, per consentire al liofilizzato di sciogliersi completamente e correttamente. Lasciare riposare per 15 minuti e, quindi, capovolgere il flaconcino un paio di volte. Prima dell’ uso assicurarsi che tutto il siero liofilo si sia disciolto. Condizioni di lavoro e modalità di analisi Lunghezza d'onda Cammino ottico Temperatura Modalità 505 (500 – 510) nm Cinetica 1 cm 25 – 30 – 37 °C o 546 (540 – 550) nm End-point In ciascuna seduta analitica, dopo aver portato i reagenti alla temperatura di lavoro, bisogna preparare sempre un bianco reagente e uno calibratore (o calibratore) a titolo noto. La concentrazione del calibratore é riportata nella sua etichetta. Nella procedura cinetica si procede secondo lo schema: Reagente R2 Reagente R1 H2O distillata Calibratore Campione Bianco reagente 1 mL 10 µL 10 µL − − Calibratore 1 mL 10 µL − 10 µL − Campione 1 mL 10 µL − − 10 µL Le soluzioni così preparate vanno mescolate delicatamente e lasciate ad incubare a 37°C per 1 minuto. Terminata l’incubazione, esse vanno sottoposte a lettura fotometrica, misurando l’assorbanza a 505 nm o 546 nm sia immediatamente che successivamente, nelle stesse condizioni di lavoro (37°C), dopo 1, 2 e 3 min. Ai valori di assorbanza ottenuti per lo calibratore e per il campione bisogna sottrarre, quindi, il valore di assorbanza del bianco reagente. Diacron srl Via Zircone, 8 – 58100 Grosseto Tel. 0564 467064 I risultati del test saranno espressi in Unità Carratelli (U CARR) applicando la seguente formula: U CARR = (∆Abs/min) x F dove: • (∆Abs/min) sono le differenze medie dei valori di assorbanza misurati a 1, 2, 3 e 3 minuti; F è un fattore di correzione con un valore predeterminato calcolato utilizzando il siero di controllo a titolo noto (valore indicativo a 37 °C: 9000). Nella procedura end-point si procede secondo lo schema: Bianco reagente Calibratore Campione Reagente R2 1 mL 1 mL 1 mL Reagente R1 10 µL 10 µL 10 µL H2O distillata 5 µL − − Calibratore − 5 µL − Campione − − 5 µL Le soluzioni così preparate vanno mescolate delicatamente e lasciate ad incubare a 37°C per 90 minuti. Appena terminata l’incubazione, esse vanno sottoposte a lettura fotometrica, misurando l’assorbanza a 505 nm o a 546 nm. Ai valori di assorbanza ottenuti per il campione e per lo calibratore bisogna sottrarre, quindi, il valore di assorbanza del bianco reagente (“azzeramento con bianco reagente”). Anche in questo caso, i risultati del test saranno espressi in U CARR, applicando la seguente formula: Abs campione U CARR = x [calibratore] Abs calibratore dove: • • (Abs campione) e (Abs standard) sono i valori di assorbanza misurati (per il campione e per lo calibratore, rispettivamente); [calibratore] è la concentrazione dello calibratore. 1 U CARR = 0.08 mg H2O2/dL Il metodo è lineare fino a 500 U CARR Nota: in ambedue i procedimenti, è possibile operare con una miscela di lavoro preparata mescolando il reagente R1 con il reagente R2 nel rapporto di 1:100 e utilizzando il campione come starter. Tale miscela è stabile per circa 12 ore se conservata a 2 − 8°C, al riparo dalla luce. Interpretazione dei risultati Il valore di riferimento del test, determinato su un campione di circa 5.000 soggetti sani, è compreso fra 250 e 300 U CARR (corrispondenti all’intervallo compreso fra 20.00 e 24.00 mg H 2O2/dL), indipendentemente dal sesso e dall’età. Tuttavia, i neonati presentano valori sensibilmente più bassi, le gravide più alti. E’ consigliabile, comunque, che ogni laboratorio determini i propri valori di riferimento. Valori superiori a 300 U CARR configurano, dopo una fascia border-line (301 − 320 U CARR), livelli progressivamente crescenti di stress ossidativo, come indicato in tabella: ROM ROM Stress ossidativo (U CARR) (mg H2O2/dL) (gravità) Condizione border-line 300 − 320 24.08 − 25.60 Stress ossidativo lieve 321 − 340 25.68 − 27.20 Stress ossidativo medio 341 − 400 27.28 − 32.00 Stress ossidativo elevato 401 − 500 32.08 − 40.00 > 500 > 40.00 Stress ossidativo elevatissimo Range normale: 250 – 300 U CARR (pari a 20.08 − 24.00 mg H2O2/dL) Valori inferiori a 250 U CARR possono osservarsi in atleti oppure in corso di trattamento corticosteroideo o antiossidante. Stabilità del calibratore Il calibratore liofilizzato è stabile fino alla data di scadenza indicata sulla confezione se conservato a +2 / +8 °C. Una volta ricostituito, il siero é stabile per circa 10 giorni se conservato a +2/+8 °C o per 10 mesi se congelato a −20 °C. Se il prodotto deve essere congelato, si consiglia di suddividerlo in piccole aliquote, prima di procedere al congelamento. Quando necessario, scongelare una piccola aliquota e mescolarla capovolgendola più volte con delicatezza. Grosseto (Italy) COLORIMETRIC DETERMINATION OF REACTIVE OXYGEN METABOLITES (FREE RADICALS–DERIVED COMPOUNDS) IN BLOOD PLASMA AND SERUM Principle Free radicals of oxygen are either single or grouped atoms, one of which, at least, belongs to the oxygen and it possesses one or more “unpaired” electrons in its/their external orbital(s). Because their greatest reactivity, practically free radicals have the potential to “attack” any organic molecule, thus generating a class of compounds which are called Reactive Oxygen Metabolites (ROMs). Despite their fair oxidant power, ROMs are relatively more stable compared to their “parent” free radicals and, therefore, they can be adequately detected and quantified. In the d-ROMs test, ROMs (hydroperoxides, ROOH, primarily) of a biological sample, in presence of iron (that is released from plasma proteins by an acidic buffer, the R2 reagent of kit), are able to generate alkoxyl (R−O*) and peroxyl (R−OO*) radicals, according to the Fenton’s reaction. Such radicals, in turn, are able to oxidize an alkyl-substituted aromatic amine (A−NH2, that is solubilized in a chromogenic mixture, the R1 reagent of kit) thus transforming them in a pink-colored derivative ([A−NH2*]+), according to the reactions: 1A) R−OOH + Fe2+→ R−O* + Fe3+ + OH1B) R−O* + A−NH2 → R−O- + [A−NH2*]+ For alkoxyl radicals 2A) R−OOH + Fe3+→ R−OO* + Fe2+ + H+ 2B) R−OO* + A−NH2 → R−OO- + [A−NH2*]+ For peroxyl radicals Finally, this colored-derivative is photometrically quantified. Indeed, the intensity of developed colour is directly proportional to the concentration of ROMs, according to the Lambert-Beer’s law. In such conditions, if kept away from direct sunlight, they remain stable until the expiry date found on the packaging. Sample d-ROMs test can be carried out either on blood (heparinized) plasma or serum. Do not use neither citrate−plasma nor EDTA–plasma. Calibrator Preparation Open the control serum vial carefully avoiding any loss off material. Then reconstitute with 1 mL of freshly distilled water. Replace the rubber stopper vial and shake gently, avoiding any foam, to allow the complete and correct dissolution. Leave to stand for 15 minutes and then invert the vial twice. Before the use, make sure that all traces of dry material are dissolved. Work conditions and procedure Wavelenght Optical path Temperature Mode 505 (500 − 510) nm Kinetics 1 cm 25 – 30 – 37 °C End-point o 546 (540 − 550) nm Before carrying out the test, when the reagents are at working temperature, a blank reagent and a calibrator (or calibrator) with assigned value should be prepared for each series of assays. This value is reported in the sticker on the control serum vial. According to the kinetic mode, the following procedure should be performed: Reagent blank Calibrator Sample R2 reagent 1 mL 1 mL 1 mL R1 reagent 10 µL 10 µL 10 µL Distilled H2O 10 µL − − Calibrator − 10 µL − Sample − − 10 µL All the solutions should be delicately mixed and after 1 min of incubation at 37 °C they can undergo photometric reading, by measuring their absorbance (at 505 nm or 546 nm) immediately and after 1, 2 and 3 min, in the same operating conditions (37 °C). Afterwards, the absorbance value of reagent blank must be subtracted from that of the calibrator and the samples. Finally, the results of d-ROMs test will be expressed as Carratelli Units (CARR U), according to the following formula: CARR U = (∆Abs/min) x F where: • • (∆Abs/min) are the mean differences of the absorbances recorded at 1, 2, and 3 min; F is a correction factor with an assigned value – approximately 9000 at 37 °C (according to the results obtained with the calibrator). According to the end-point mode, the following procedure should be performed: Reagent blank Calibrator Sample R2 reagent 1 mL 1 mL 1 mL R1 reagent 10 µL 10 µL 10 µL Distilled H2O 5 µL − − Calibrator − 5 µL − Sample − − 5 µL All the solutions should be delicately mixed and after 90 min of incubation at 37 °C they must undergo immediately photometric reading, by measuring their absorbance (at 505 nm or 546 nm), in the same operating conditions (37 °C). Afterwards, the absorbance value of reagent blank must be subtracted from that of the calibrator and the samples (“zero adjustment with the reagent blank”). The results of d-ROMs test will be expressed as Carratelli Units (CARR U), according to the following formula: Abs sample CARR U = X [calibrator] Abs calibrator where: • Abs sample and Abs calibrator are the measured absorbance values (for the sample and the calibrator, respectively); • [calibrator ] is the concentration of calibrator. 1 CARR U = 0.08 mg H2O2/dL The method is linear up to 500 CARR U Note. In both the procedures, it is possible to operate with a “working solution” obtained by mixing R1 reagent with R2 reagent (according a ratio of 1:100). Therefore, the sample can be used as “starter”. Such working solution is stable up to 12 hours, if stored at 2 − 8°C and protected from direct sunlight. Interpretation of results Reference values of d-ROMs test, as calculated on a sample of approximately 5.000 apparently healthy subjects, are between 250 and 300 CARR U (i. e. between 20.00 and 24.00 mg H2O2/dL), independently on gender and age. However, newborns exhibits very low values while pregnant women have more high values. In any case, every laboratory should determine its own reference value. Calibrator Stability The lyophilized control serum is stable up to the expiry date when stored at +2 / +8 °C. The expiry date is illustrated on the side of each pack. Once reconstituted the components of the serum are stable up to 10 days if stored at + 2 / + 8 °C or up to 10 months if frozen at −20 °C. If the product has to be frozen, we suggest to split up them in small aliquots, before proceeding to the freezing. When necessary, defrost a small aliquot and mix them delicately by inversion before using. Once defrosted, each aliquot of reconstituted serum can be used only one time. References Cornelli U, et al.
