NOTE
Si ricorda, al fini della domanda di insinuazione al passivo, che:
1) La domanda di insinuazione deve essere redatta in lingua italiana e la documentazione a
sostegno delle proprie ragioni creditorie, se a suo tempo redatta in lingua non italiana, dovrà
essere accompagnata da apposita traduzione in lingua italiana, documento per documento,
pena l'inutilizzabilità.
La domanda deve essere presentata presso la Cancelleria del Tribunale di Busto Arsizio (VA),
Largo Gaetano Giardino 4, 21052 Busto Arsizio (VA), a mano oppure tramite raccomandata.
2) Nel caso non venisse eletto domicilio nel comune di Busto Arsizio, "tutte le notificazioni
posteriori si fanno al creditore presso la Cancelleria del Tribunale" (art. 93, comma 2, del R. D.
16/03/1942 n° 267 ).
3) Nella domanda deve essere indicato il titolo da cui il credito deriva, l'ammontare espresso in
moneta di conto = EURO (per i crediti in altra valuta la data del cambio da utilizzare per la
conversione è il 30.11.2004), le ragioni dell'eventuale prelazione richiesta (art. 2741 codice
civile) e la documentazione a supporto.

Please, note the following items:
1) The Proof of Claim has to be written in Italian, together with supporting documents, which
must also be translated into Italian, each document individually under penalty of rejection
because unusable.
Each creditor may put forth his/her claims by filing exclusively with the Clerk of the Court of
Busto Arsizio - Largo Gaetano Giardino 4, 21052 Busto Arsizio (VA), Italy - by hand or by
registered mail, a proof of claim in bankruptcy petition.
2) In case the creditor doesn’t choose a domicile inside the municipality of Busto Arsizio, “all
notifications after the proof of claim will be made to the Clerk of the Court of Busto Arsizio”
(according to the art. 93.2 of the Italian bankruptcy Law).
3) In the proof of claim, it’s need to specify the cause of the credit, the amount in currency =
Euro (regarding the credits in other currency, for the conversion in Euro it’s need to make
reference to the change as of November 30, 2004), the reasons of the preference rights if
applicable (art. 2741 of the Italian civil code) and the supporting documents.
