Understanding Prostate Cancer Risk Groups What Is a Risk Group? Multiple studies have indicated that patients with slightly different characteristics can be organized into groups that have similar outcomess. For example a Low Risk patient with a T1c, Gleason Score 6 cancer and a PSA of 5 will behave very similarly to a patient with a T2a, Gleason Score 6 and PSA of 8. By grouping patients with similar results, large numbers of patients can be analyzed for the results of a treatment. By grouping these patients, we are able to compare the success of any treatment regimen. CARCINOMA DELLA PROSTATA A BASSO RISCHIO: CHE COSA SIGNIFICA? Why Evaluate by Risk Groups Instead of Just Stage? The standard cancer staging system (TNM) does not take into account the influence of PSA or Gleason Score on prognosis. Both the PSA blood test and Gleason Score are also strongly predictive of response to treatment. Risk group classifications take into account the three most important predictors; the digital rectal exam (clinical T stage), the pre-treatment PSA and the biopsy Gleason Score. Risk groups allow one to separate patients into similar groups with the same prognosis in order to compare results of one treatment to another. Il rischio di progressione di malattia nei tumori della prostata definiti “a basso rischio” (T1-T2a, Gleason score ≤6; PSA <10 ng/mL) non trattati è REALE se l’attesa di vita è di almeno 10-15 anni. BRACHITERAPIA: Tecniche LDR/HDR: CAREFUL PATIENT SELECTION BRACHITERAPIA: Criteri di selezione del PZ Technical feasibility: prostate volume < 50-60 cc No interference with pubic arc Functional index (uroflowmetry, IPSS) “RISULTATI OTTIMALI SOLO IN PAZIENTI ACCURATAMENTE SELEZIONATI!” no recent TURP / TUIP BRACHITERAPIA: Indicazioni BRACHITERAPIA: Elevata conformazione della distribuzione di dose BRACHITERAPIA Risultati LDR (Reviews sistematiche) LDR Brachytherapy for LowLow-risk PC: 10-yr bNED = 87%-94% BRACHITERAPIA Risultati HDR (Reviews sistematiche) BRACHITERAPIA: Tossicità Ottimo profilo di tossicita’ e qualità di vita: HDR anche meglio di LDR (Reviews sistematiche) BRACHITERAPIA Risultati (LDR/HDR) (Linee Guida AIOM) • In linea generale la Brachiterapia si pone come alternativa terapeutica al trattamento chirurgico del tumore prostatico localizzato a basso rischio (Livello di Evidenza-Grado di raccomandazione:3D) • Periodo libero da progressione biochimica di malattia a 5 anni (5-yr FFBP/bNED): 70-82% (TUTTI GLI STADI!) • Risultati simili a quelli di Chirurgia ed RT esterna • Tutti studi monoistituzionali retrospettivi • Assenza di studi randomizzati che dimostrino una differenza nel controllo locale tra BRT, CHIR ed RTE • Studio ACOSOG-Z007 chiuso nel 2009 per mancanza di accrual BRACHITERAPIA: Tossicità (Linee Guida AIOM) • Effetti collaterali più importanti della BRT a livello di uretra prostatica e bulbo penieno • Incidenza di sintomi irritativi ostruttivi elevata nei primi 6 mesi dall’impianto (cateterizzazione temporanea: 5-30%) • Stenosi uretrale tardiva: 5-7% • Deficit erettile dopo 3-6 anni: 50% (> con età e dose al bulbo) • Studio ACOSOG-Z007: BRT meglio di CHIR a 5 anni per funzione erettile ed urinaria RADIOTERAPIA ESTERNA Moderne Tecnologie di “Delivery” RADIOTERAPIA ESTERNA CON MACCHINE DI ULTIMA GENERAZIONE (Tomotherapy) Distribuzioni di dose altamente conformate di tipo “brachiterapico” RADIOTERAPIA ESTERNA NEL CARCINOMA PROSTATICO A BASSO RISCHIO: Risultati D’Amico et al,JAMA. 1998;280:969-974… Low-risk patients had estimates of 5-year PSA outcome after treatment with RP, RT, or implant with or without neoadjuvant androgen deprivation that were not statistically different. RT ESTERNA (EBRT) While it appears on the graphs that Brachytherapy does the best overall, it is apparent that most patients can do well with any treatment. Therefore, for cancer control purposes, we advise most patients that they cannot make a bad decision. RADIOTERAPIA ESTERNA NEL CARCINOMA PROSTATICO A BASSO RISCHIO: Risultati RUOLO ANCORA NON DEFINITO DELLA DOSEESCALATION (>70 Gy) NEL BASSO RISCHIO RADIOTERAPIA ESTERNA : Tossicità ACUTA TOSSICITA’ RTE { Proctite acuta Cistite acuta Cistite cronica TARDIVA { Proctite cronica Incontinenza fecale Stenosi uretrale Disfunzione erettile La 3D-CRT, oggi impiegata di routine in tutti i Centri di Radioterapia, riduce significativamente il tasso di complicanze acute, rispetto alla precedente RT 2D, anche nei pazienti trattati con dosi tot. >70 Gy. TOX ACUTA RTOG (≤70 Gy) gastroenterica: G2=8-10%, G3=2% TOX ACUTA RTOG (≤70 Gy) urogenitale: G2=15-18%, G3=2-3% RADIOTERAPIA ESTERNA : Tossicità Tossicità acuta dopo 3D-CRT “dose escalation” (72 Gy 78 Gy) • Gastroenterica: G2=18-20%, G3=8-10% • Genitourinaria: G2=10-20%, G3=5-7% Con tecnica IMRT ed ipofrazionamento moderato (2.7 Gy/fraz. – 70.2 Gy in 28 frazioni) la tossicità acuta è risultata sovrapponibile al frazionamento standard (2 Gy – 76 Gy in 38 frazioni) [Studio randomizzato Fox Chase Cancer Center; Pollack A. et al, IJROBP 2006, 64:518-526] RADIOTERAPIA ESTERNA : Tossicità Tossicità tardiva (≥6 mesi post 3D-CRT dose escalation >70 Gy): GE(retto): ≥G2=6-17% GU: ≥G2=10% (4-13%) Stenosi uretrale=1.5% (4% dopo TURP) Tossicità tardiva dopo IMRT dose escalation ≤81 Gy: - TOX rettale GE: G2=1.5-3%, G3=1% IMRT + TOX vescicale GU: G2=11%, G3=5% DE:15% a 20 mesi, 47% a 60 mesi RADIOTERAPIA ESTERNA : Tossicità ridotta con IMRT e PBRT Rischio di tossicità GE e GU moderata e severa: aumenta sempre con la dose. IMRT e PBRT (Protonterapia):ridotta tossicità GE anche con dose-escalation rispetto a 3D-CRT Our meta-analysis of randomized trials, using a uniform toxicity scale, indicates that late toxicity rates increase with RT dose. Published series in which dose escalated RT is delivered using IMRT or PBRT have relatively short follow up but report lower late GI toxicity rates than those employing 3D-CRT to deliver similar doses. This analysis supports the role of IMRT or PBRT for the safe delivery of dose escalated RT, and these observations should be further evaluated through prospective studies.
