Indeed, a smaller incision implies fewer risks in terms of intra- and post-operative complications and speedier visual recovery (Tsuneoka et al 2002). After a number of attempts with a range of different sources of energy and thanks to the introduction of the so-called cold ultrasounds, microincision techniques were eventually developed which could be carried out with both bimanual or microcoaxial procedures. The decrease in the size of the incision on one hand and the evolution of technology enabling us to use a greater and greater vacuum with ever decreasing energy on the other have increased the risk of insufficient irrigation. Therefore, despite the sophisticated fluid control systems incorporated in all modern phacoemulsifiers, there has been a need to increase the irrigation flow. La riduzione delle dimensioni dell'incisione, insieme all'evoluzione tecnologica che permette di utilizzare un vuoto sempre maggiore con un consumo energetico in costante diminuzione, hanno aumentato il rischio di irrigazione insufficiente. Di conseguenza, nonostante la presenza di sofisticati sistemi di controllo dei fluidi in tutti i facoemulsificatori moderni, sussisteva la necessità di un flusso di irrigazione più consistente. This has been made possible by working both on the design and on the localization of the openings, but above all the increase of the irrigating flow has been obtained increasing the bottle height. The use of plus or minus sophisticated forced irrigation systems has also been taken into account. It has now been proven beyond any doubt in many important papers that also after small incision cataract extraction there is an increase in retinal thickness and an alteration of the blood acqueous barrier, which can persist for many months after surgery (Ursell 1999, Miyake 2000, El-Harazi 1999, Lobo 2004, Schauersberger 2000, Luo 2004, Tognetto & Ravalico 2005, Liu 2002). There are many possible explanations for this, such as the fototoxicity of the microscope light, the amount of energy used etc. However a very important cause of this seems to be the anterior chamber pressure variation, which determines an external and an internal deformation of the anterior segment. This in turn causes the retraction of the iris-lenticular diaphragm and the elongation of the zonular fibers and the ciliary process, all of which determine a continuous movement of the ciliary body. Therefore, the inevitable questions which arise are, which pressures are reached in the anterior chamber during the various phases of surgery, which are the pressure fluctuations and with what speed do these fluctuations occur? This study was carried out to measure the anterior chamber fluctuations during phacoemulsification. Tuttavia, una causa molto rilevante sembra essere la variazione della pressione nella camera anteriore, che determina una deformazione all'interno e all'esterno del segmento anteriore. Questa a sua volta provoca la retrazione del diaframma iridolenticolare e l'allungamento delle fibre zonulari e del processo ciliare, le quali determinano un movimento continuo del corpo ciliare. Year 1 ı Number 3 ı 2015 139 Analisi delle variazioni della pressione intraoculare durante la facoemulsificazione: uno studio in vivo MATERIALS AND METHODS Continuous intraocular pressure measurement was performed using a special microsensor located in the anterior chamber in three groups of 20 cataract patients who underwent phacoemulsification and IOL implantation. In group 1 a standard coaxial phaco procedure (SCP), in group 2 an ultra-small incision coaxial phaco (MCP) and in group 3 bimanual micro incision cataract surgery (BIM) were performed. To measure the intraocular tension in vivo we adapted the Codman Microsensor ICP Transducer (Codman & Shurtleff, Inc. Raynham, MA,USA cod.82-6605).This instrument has been developed in neurology and neurosurgery to monitor intracranial pressure directly. Its use is indicated in both subdural and intraparenchymal pressure monitoring applications. The instrument consists of a catheter with a microminiature silicon strain gauge type sensor mounted at one end and an electrical connector at the other end. The transducer is interfaced with a control Unit Monitor where it is possible to read in real time the pressure variations (Fig. 1). The diameter of the tip is 1.2 mm, the outer diameter of the catheter is 0.7 mm. The functional pressure range is from -50 mmHg to +250 mmHg (-6666 Pa to +33333 Pa). The microsensor was located inside the anterior chamber at the beginning of surgery through a side port incision like an anterior chamber maintainer and it was left in this position for the whole of the surgery (Fig. 2). All the steps of the surgery performed through the operating microscope and the control unit monitor of intraocular pressure variation were recorded using two syncronized video cameras. In MATERIALI E METODI La misurazione della pressione intraoculare in continuo è stata condotta utilizzando un microsensore speciale, localizzato nella camera anteriore, in tre gruppi di 20 pazienti affetti da cataratta e sottoposti alla facoemulsificazione e all'impianto di lenti intraoculari. Nel Gruppo 1 è stata condotta una procedura standard di faço coassiale (SCP), nel gruppo 2 una faço microcoassiale (MCP) e nel gruppo 3 una chirurgia bimanuale della cataratta (BIM). Per misurare la tensione intraoculare in vivo abbiamo utilizzato un Codman Microsensor ICP Transducer adattato allo scopo (Codman & Shurtleff, Inc. Raynham, MA, Stati Uniti, cod.82-6605). Questo strumento è stato sviluppato per applicazioni neurologiche e neurochirurgiche di monitoraggio diretto della pressione intracranica. Il suo uso è indicato nelle applicazioni di monitoraggio della pressione subdurale e intraparenchimale. Lo strumento è composto da un catetere con un sensore in silicio microminiaturizzato di tipo estensimetrico montato ad una estremità e un connettore elettrico all'altra estremità. Il trasduttore è interfacciato con un'Unità Monitor di controllo, che permette di leggere in tempo reale le variazioni di pressione (Fig. 1). Il diametro della punta è 1,2 mm, il diametro esterno del catetere è 0,7 mm. Il campo di pressione di esercizio è compreso tra -50 mmHg e +250 mmHg (da -6666 Pa a +33333 Pa). Il microsensore è stato collocato come un mantenitore all'interno della camera anteriore all'inizio dell'intervento chirurgico attraverso un'incisione laterale ("side port") ed è stato lasciato in questa posizione per tutta la durata dell'intervento (Fig. 2). Results were expressed as the minimum, maximum and average intraocular pressure values during the following surgical phases: filling of the anterior chamber with OVD before caspulorhexis, phacoemulsification, automated cortical remnant removal and IOL implantation (Fig. 3). Surgery time and time of intraocular pressure exceeding 60 mmHg were also recorded. Exclusion criteria were: corneal pathologies, glaucoma, uveitis, pseudoexfoliation syndrome, diabetes mellitus. Informed consent was obtained. Surgical technique All patients received the same preand post-operative treatment. Sodium diclofenac 0.1% and ofloxacin 0.3% eyedrops were instilled four times both the day before and the morning of surgery. The pupil was dilated with topical 0.5% tropicamide and 10% phenylephrine. All 60 patients received topical anaesthesia and in 11 patients a subtenonian injection of 1 ml of lidocaine 2% was necessary in order to avoid excessive movement and pain (Jonas 2006). I risultati sono stati espressi come valori minimi, massimi e medi della pressione intraoculare durante le seguenti fasi chirurgiche: riempimento della camera anteriore con OVD prima della capsuloressi, facoemulsificazione, rimozione automatizzata dei residui corticali e impianto della lente intraoculare (Fig. 3). Sono stati registrati anche il tempo di intervento e il tempo durante il quale la pressione intraoculare ha oltrepassato i 60 mmHg. I criteri di esclusione sono stati: patologie della cornea, glaucoma, uveite, sindrome da pseudoesfoliazione, diabete mellito. È stato ottenuto il consenso informato. Tecnica chirurgica Tutti i pazienti hanno ricevuto lo stesso trattamento pre e post-operatorio. Diclofenac sodico 0,1% e ofloxacina collirio 0,3% sono stati instillati quattro volte, sia il giorno precedente sia la mattina dell'intervento. This incision was at least 1 mm in length in order to guarantee good sensor stability after being positioned in the anterior chamber. In the Standard coaxial phaco group (SCP) the operation was performed through a self-sealing 2.75 mm clear corneal tunnel (Alcon Ophthalmic Knife 2.75 mm - Alcon Laboratories FortWorth TX USA) placed in the supero-temporal quadrant in the right eye and in the supero-nasal quadrant di lidocaina al 2% al fine di prevenire movimenti e dolori eccessivi (Jonas 2006). La prima operazione è stata un'incisione laterale sulla c.d. "side port" da 1,5 mm, eseguita nel quadrante temporale inferiore. Questa incisione doveva misurare almeno 1 mm in lunghezza per garantire una buona stabilità del sensore dopo il posizionamento nella camera anteriore. Nel Gruppo faço coassiale standard (SCP) l'operazione è stata condotta attraverso un tunnel corneale trasparente autosigillante da 2,75 mm (Alcon Ophthalmic Knife 2,75 mm - Alcon Laboratories, FortWorth, TX, Stati Uniti) collocato nel quadrante superotemporale dell'occhio destro e nel quadrante supero-nasale dell'occhio sinistro (Fig. 4a). La capsuloressi è stata condotta con pinze di dimensioni adeguate. Dopo l'idrodissezione, la lente è stata facoemulsificata con una punta da 19 G (OPOR 30 19 L - A.M.O. Inc. Santa Ana, CA, Stati Uniti) e una sleeve standard (OPO SL 19 - A.M.O. Inc. Santa Ana, CA, Stati Uniti), utilizzando una tecnica standard "stop and chop" (dopo la scultura centrale e la frattura del nucleo, i due eminuclei sono stati frammentati e facoaspirati). Nel secondo gruppo è stata condotta una faço microcoassiale (MCP) attraverso un tunnel da 2,2 mm (bisturi Sharpoint 2,2 mm - Sharpoint Surgical Specialities Corporation Reading, PA, Stati Uniti) (Fig. 4b) con una punta da 20 G (OPOR 30 20 L - A.M.O. Inc. Santa Ana, CA, Stati Uniti) con un apposito sleeve in silicone (OPO SL 20 - A.M.O. Inc. Santa Ana, CA, Stati Uniti), eseguendo la stessa tecnica utilizzata nel gruppo faço coassiale standard. Capsulorhexis was done using suitable forceps. After hydrodissection, the lens was phacoemulsified with a 19 gauge tip (OPOR 30 19 L - A.M.O. Inc. Santa Ana CA USA) and a standard sleeve (OPO SL 19 - A.M.O. Inc. Santa Ana CA USA), using a standard “stop and chop” technique (after a central sculpture the nucleus was cracked and the two heminuclei were chopped and phacoaspirated). In the second group an ultrasmall incision coaxial phaco (MCP) was performed through a 2.2 mm tunnel (Sharpoint 2.2 mm knife - Sharpoint Surgical Specialities Corporation Reading PA USA) (Fig. 4b) using a 20 gauge tip (OPOR 30 20 L - A.M.O. Inc. Santa Ana CA USA) with a dedicated silicone sleeve (OPO SL 20 - A.M.O. Inc. Santa Ana CA USA) performing the same technique used in the standard coaxial phaco group. Bimanual microincision cataract surgery (BIM) was performed through two stab incisions of 1.1 mm. One incision was done in the superotemporal quadrant and one in the supero-nasal. A trapezoidal tunnel was created using a diamond blade (Janach Srl-Como Italia mod. J 29816), giving an internal incision of 1.1 mm and an external one of 1.4 mm (Fig. 4c). We used a trapezoidal tunnel rather than a rectangular one since the latter renders tool manipulation in the anterior chamber very difficult. This can lead to corneal wrinkling that may impair vision during surgery. A continuous circular capsulorhexis was performed using either a needle or forceps designed to be inserted through a one mm incision (Janach Srl - Como Italy mod. J 3137.1). Then a sleeveless phaco chop was performed using a 19 gauge phaco tip and a 19 gauge open ended irrigating La chirurgia bimanuale della cataratta a microincisione (BIM) è stata condotta attraverso due incisioni a tutto spessore da 1,1 mm: una nel quadrante superotemporale e un'altra nel supero-nasale. È stata condotta una capsuloressi circolare continua utilizzando un ago e pinze adatte per l'inserimento attraverso un'incisione da 1 mm (Janach Srl - Como, Italia, mod. J 3137.1). Successivamente è stato eseguito un faço-chop utilizzando una punta faço da 19 G e un chopper irrigante da 19 G (Janach Srl, Como, Italia, mod. J 2234.26), utilizzando la stessa tecnica descritta per il gruppo faço coassiale standard. In ogni paziente è stato utilizzato Healon GV Ialuronato di Sodio 14mg/ml (AMO Advanced Medical Optics Inc. Santa Ana CA USA) per riempire la camera anteriore prima di eseguire la capsuloressi e Healon Ialuronato di Sodio 10 mg/ml (AMO -Advanced Medical Optics Inc., Santa Ana, CA, Stati Uniti) per riempire nuovamente il sacco capsulare prima di impiantare la lente. La macchina è stata utilizzata con le stesse impostazioni in tutte le procedure. Durante entrambe le fasi di scultura e chopping, sono stati impostati i seguenti parametri: portata 24 cc/min, vuoto massimo 320 mmHg, vuoto nell'ambiente di stabilizzazione 250 mmHg e potenza 40%. J 2234.26), using the same technique described for the standard coaxial phaco group. In each patient Healon GV 14mg/ ml Sodium Hyaluronato (AMO -Advanced Medical Optics Inc. Santa Ana CA USA) was used to refill anterior chamber before performing capsulorexi, and Healon 10 mg/ml Sodium Hyaluronato (AMO -Advanced Medical Optics Inc. Santa Ana CA USA) to refill capsular bag before implanting the lens. The same machine setting was selected in all the procedures. During both the sculpture and the chop phase a flow-rate of 24 cc/min, a maximum vacuum of 320 mmHg, a case vacuum of 250 mmHg and a power of 40% were selected. The other parameters which are important to preselect when using ICE are reported in the table 1. An acrylic single piece 6mm foldable lens (Alcon SA60AT Acrysof SinglePiece - Alcon Laboratories FortWorth TX USA) was implanted using the Monarch injector (Alcon Laboratories FortWorth TX USA) in all patients. In the MICS. group, an enlargement to 2 mm of one of the two stab incisions was necessary to implant the lens. A 10.0 nylon monofilament single stitch (Ethilon Johnson & Johnson Inc.) was placed in all patients belonging to the first group. At the end of surgery 4 mg betamethasone were administered subconjunctivally. fondamentali da preselezionare quando si utilizza l'ICE sono riportati in tabella 1. Una sutura singola monofilamento in nylon da 10,0 (Ethilon Johnson & Johnson Inc.) è stata impiantata in tutti i pazienti appartenenti al primo gruppo. Al termine dell'intervento chirurgico sono stati somministrati 4 mg di betametasone per via sottocongiuntivale. Analisi statistica L'effetto delle tre procedure sulla tensione della camera anteriore è stata valutata tramite il test di omoschedasticità, che ha rivelato una differenza significativa nella variabilità (p = 0,0006). Dal momento che non sussistevano le condizioni necessarie per eseguire il test t di Student, i dati sono stati analizzati con un test non parametrico (test di Wilcoxon per dati indipendenti). Un valore di P inferiore a 0,05 è stato considerato significativo. Per quanto riguarda la durezza del nucleo, l'omogeneità dei tre gruppi di pazienti è stata valutata mediante il test di Fisher. Since the conditions necessary for the Student t-test were not satisfied, data were analysed using a non parametric test (Wilcoxon test for independent data). A P value inferior to 0.05 was considered significant. The homogeneity of the three groups of patients regarding the nucleus hardness was evaluated using the Fischer test. To evaluate the cataract hardness, the Oxford Clinical Cataract Classification and Grading System (Sparrow et al 1986) was used. RESULTS All operations were uneventful and were followed by a regular postoperative procedure. Surgery times measured from the start of the incision to the wound hydration and reformation of the anterior chamber, are shown in table 1. Even if in the bimanual group the procedure time is a little longer there is only a small statistically difference between the results obtained in the SCP and BIM group The average maximum tension reached during the refill of the anterior chamber with a viscoelastic substance was 36 ± 8 mmHg in the SCP group, 48 ± 6 in the MCP group and 51 ± 11 in the BIM group (SCP vs MCP - P = 0.29, SCP vs BIM - P = 0.12, MCP vs BIM - P = 0.026) (Fig. 5). Intraoperative ocular hypertension is not the only factor capable of determining intraocular complications. In fact during surgery we witness important and frequent tension variations which provoke anterior chamber fluctuations. Each time that the surgical tools are removed from the incisions, the tension drops in the space of a few seconds to around 0 mmHg. Pressure therefore ranges from 0 to 100 mmHg during surgery.
