SUMMARY AND EXPLANATION OF THE TEST Apolipoprotein(a), Apo(a), is a glycoprotein linked by disulphide bridges to apolipoprotein B in the lipoprotein(a) (Lp(a)) particle. Apo(a) is formed by three different structural domains. One ofthe domains, called kringle 4, is present in multiple copies, the number of which varies and is genetically determined, giving rise to different sizes of Apo(a) and consequently Lp(a). Depending on the method used, six to 23 isoforms of Apo(a) ranging from about 300 to 900 kD have been identified (1,2,15,16). Most individuals have one or two Apo(a) isoforms, although in some subjects no Apo(a) band can be detected when analyzed in SDS-gel electrophoresis followed by immunoblotting (3). Recently, much interest has been focused on Lp(a) since there is a lot of evidence that circulating levels represents an independent risk factor for coronary vascular disease. The Lp(a) level has been found to be an inherited risk factor for ischaemic heart disease (4–8). High Lp(a) levels have been demonstrated in familial hypercholesterolemia and its measurement may be clinically useful for risk prediction in these patients (9,10). Results have also been published on Lp(a) as a strong indicator for cerebrovascular disease (11,12). Apo(a) is homologous to the protease zymogen plasminogen (13,14). Lp(a) inhibits plasminogen activation and recent studies have shown that Apo(a) and Lp(a) compete with plasminogen for binding to the plasminogen receptor. These properties of Apo(a) may explain the association of high Lp(a) concentrations with myocardial infarction. PRINCIPLE OF THE PROCEDURE Mercodia Apo(a) ELISA is a solid phase two-site enzyme immunoassay. It is based on the direct sandwich technique in which two monoclonal antibodies are directed against separate antigenic determinants on the Apo(a) molecule. During incubation Apo(a) in the sample react with peroxidase-conjugated anti-Apo(a) antibodies and anti-Apo(a) antibodies bound to microtitration well. A simple washing step removes unbound enzyme labeled antibody. The bound conjugate is detected by reaction with 3,3’,5,5’-tetramethylbenzidine. The reaction is stopped by adding acid to give a colorimetric endpoint that is read spectrophotometrically. 5 English INTENDED USE WARNINGS AND PRECAUTION • For in vitro diagnostic use. Not for internal or external use in humans or animals. • The content of this kit and their residues must not be allowed to come into contact with ruminating animals or swine. • The Stop Solution in this kit contains 0.5 M H2SO4. The source of material have been tested by immunoassay for hepatitis B surface antigen, antibodies for Hepatitis C virus and antibodies for HIV virus and found to be negative. Nevertheless, all recommended precautions for the handling of blood derivates should be observed. Please refer to HHS Publication no. For larger series of assays, use pooled reagents from packages bearing identical lot numbers. The expiry date for the complete kit is stated on the outer label. The recommended storage temperature is 2–8°C. Specimen may be stored for 1 week at 2-8°C. For longer periods store samples at -20°C. Avoid repeated freezing and thawing. Plasma Collect blood by venipuncture into tubes containing EDTA or heparin as anticoagulant, and separate the plasma fraction. Specimen may be stored for 1 week at 2-8°C. Wash 6 times with 700 µl per well using an automatic plate washer with overflow-wash function. Do not include soak step in washing procedure. Discard the wash solution, tap firmly several times against absorbent paper to remove excess liquid. Repeat 5 times. Put the plate on the shaker for 5 seconds to ensure mixing of Substrate and Stop Solution. 10 Measure the absorbance at 450 nm and evaluate. Read within 30 minutes. Note! To prevent contamination between the conjugate and substrate, separate pipettes are recommended. INTERNAL QUALITY CONTROL Internal plasma pools with low, intermediate and high Apo(a) concentration should routinely be assayed as samples, and results charted from day to day, it is good laboratory practice to record the following data for each assay: kit lot number, reconstitution dates of kit components: OD values for the blank, Calibrators and Controls. CALCULATIONS OF RESULTS Computerized calculations The concentration of Apo(a) is obtained by computerized data reduction of the absorbance for the Calibrators, except for Calibrator 0, versus the concentration using cubic spline regression. Multiply the concentration of the samples with the dilution factor (e.g. × 202) 9 English TEST PROCEDURE Manual calculation 1. Plot the absorbance values obtained for the Calibrators, except for Calibrator 0, against the Apo(a) concentration on a lin-lin paper and construct a calibrator curve. 2. This corresponds to a sample concentration of 10 U/l when the sample is diluted 1/202. Recovery Recovery upon addition is 96–111 % (mean 102 %). Hook effect Samples with a concentration of up to 9600 U/l can be measured without giving falsely low results if they are pretreated and diluted 1/202 as described above. (Clinical concentration of plasminogen is below 2.1 g/l.) Apolipoprotein B has no measurable crossreaction. 14 14 Mercodia Apo(a) ELISA kit is calibrated against a highly purified, fully validated, commercial Lp(a) preparation. The concentration of Apo(a) is expressed in Units/l. It is not possible to express the concentration of Apo(a) in mass units as there are at least six different isoforms described with molecular weights varying from approximately 300 kD to 900 kD (1,2,15,16). Thus each patient sample will contain different proportions of the different isoforms. Therefore no exact conversion factor can be given between Units of Apo(a) and milligrams of Apo(a). 1 Unit of Apo(a) is approximately equal to 0.7 mg Lp(a) protein. ZUSAMMENFASSUNG UND ERKLÄRUNG DES TESTS DAS TESTPRINZIP Mercodia Apo(a) ELISA ist ein Festphasenimmunoassay. Er basiert auf der direkten Sandwichtechnik, bei der zwei monoklonale Antikörper gegen separate Antigene auf dem Apo(a)-Molekül gerichtet sind. Während der Inkubation reagiert das Apo(a) in der Lösung mit den Anti-Apo(a) Antikörpern im Peroxidase-konjugat und den Anti-Apo(a)-Antikörpern, welche auf der Mikrotiterplatte gebunden sind. Durch einfaches Waschen werden ungebundene, durch Enzym gekennzeichnete Antikörper entfernt. Das gebundene Konjugat wird durch 3,3’,5,5’Tetramethylbenzidin sichtbar gemacht. Durch Zugabe von Säure wird die Reaktion gestoppt. Der daduch erhaltene colorimetrischen Endpunkt wird spektrophotometrisch abgelesen. 17 Deutsch Apolipoprotein(a), Apo(a), ist ein Glykoprotein, das über Disulfidbrücken mit dem Apolipoprotein B eines Lipoprotein-Partikels (a) (Lp(a)) verbunden ist. Man hat herausgefunden, dass die Lp(a)-Konzentration zu den erblichen Risikofaktoren von ischämischen Herzkrankheiten (4–8) zählt. Hohe Lp(a)-Konzentrationen sind bei familiärer Hypercholesterinämie nachgewiesen worden, weswegen die Messung der Konzentration bei diesen Patienten zur Risikoeinschätzung klinisch sinnvoll sein könnte (9,10). Des Weiteren sind Ergebnisse veröffentlicht worden, die darauf hinweisen, dass Lp(a) ein starker Indikator für zerebrovaskuläre Erkrankungen darstellt (11,12). Apo(a) ist homolog zu der Protease Zymogen Plasminogen (13,14). Lp(a) hemmt die Aktivierung des Plasminogens, und neue Untersuchungen haben gezeigt, dass Apo(a) und Lp(a) mit dem Plasminogen um die Bindung des Plasminogen-Rezeptors konkurrieren. Récemment, la Lp(a) a suscité beaucoup d’intérêt. La Lp(a) inhibe l’activation du plasminogène et des études récentes ont montré que l’Apo(a) et la Lp(a) entrent en compétition avec le plasminogène au niveau du récepteur de ce dernier. Ces propriétés de l’Apo(a) pourraient expliquer l’association entre les concentrations élevées de Lp(a) et l’infarctus du myocarde. Uno dei domini, chiamato kringle 4, è presente in copie multiple, di numero variabile e determinate su base genetica, questo porta a dimensioni diverse di Apo(a) e di conseguenza di Lp(a). In relazione al metodo utilizzato, sono state identificate da 6 a 23 isoforme di Apo(a) che variano da 300 a 900 kD (1,2,15,16). La maggior parte degli individui ha una o due isoforme di Apo(a), anche se in alcuni soggetti non è possibile rilevare alcuna banda di Apo(a) quando si eseguono le analisi mediante SDS-gel elettroforesi seguita da immunoblotting (3). Recentemente è stato dedicato molto interesse alla Lp(a) sulla base di un gran numero di dati che indicano che il loro livello in circolo rappresenta un fattore di rischio indipendente per le malattie vascolari coronariche. E’ stato scoperto che il livello di Lp(a) rappresenta un fattore di rischio ereditario per le malattie cardiache di natura ischemica (4–8). E’ stata dimostrata la presenza di livelli elevati di Lp(a) nella ipercolesterolemia familiare e la sua misurazione può essere utile dal punto di vista clinico per la previsione dei rischi in questi pazienti (9,10). Sono stati inoltre pubblicati i risultati di studi eseguiti sulla Lp(a) che indicano questa sostanza come un forte indicatore di malattie cerebrovascolari (11,12). Apo(a) è omologa alla proteasi di zimogeno plasminogeno (13,14). Lp(a) inibisce l’attivazione del plasminogeno e studi recenti hanno dimostrato che Apo(a) e Lp(a) competono con il plasminogeno per il legame al recettore plasminogeno. Queste propertà di Apo(a) possono spiegare l’associazione tra concentrazioni elevate di Lp(a) e infarto miocardico. Mercodia Apo(a) ELISA è un metodo di immunoanalisi enzimatica a due siti a fase solida. E’basata sulla tecnica diretta sandwich nella quale due anticorpi monoclonali vengono diretti contro determinanti antigenici separati sulla molecola Apo(a). Durante l’incubazione la Apo(a) presente nel campione reagisce con gli anticorpi anti-Apo(a) coniugati a perossidasi e gli anticorpi anti-Apo(a) legati al pozzetto per microtitolazione. Un semplice passaggio di lavaggio rimuove gli enzimi non legati gli anticorpi etichettati dall’enzima non legati. Il coniugato legato viene rilevato mediante reazione con 3,3’,5,5’-tetrametilbenzidina. Ripetere 5 volte. Evitare prolungate pause di immersione durante la procedura di lavaggio. 7 Aggiungere 200 µl di Substrate TMB 8 Incubare per 15 minuti 9 Aggiungere 50 µl di Stop Solution. Mettere la piastra sullo shaker per 5 secondi per assicurarsi di miscelare il Substrate TMB con la Stop Solution. 10 Misurare l’assorbanza 450 nm e valutare. Leggere entro 30 minuti. Dansk 75 76 AVSEDD ANVÄNDNING Mercodia Apo(a) ELISA är ett test för kvanitativ bestämning av humant apolipoprotein(a) i serum eller plasma. SAMMANFATTNING OCH FÖRKLARING AV TESTET Apolipoprotein(a), Apo(a), är ett glykoprotein som är bundet till apolipoprotein B i lipoprotein(a) (Lp(a))-partikeln genom disulfidbryggor. Apo(a) bildas av tre olika strukturella domäner. En av domänerna, kringla 4, finns i flera kopior och antalet varierar och bestäms genetiskt, vilket ger upphov till olika storlekar av Apo(a) och följdaktligen Lp(a) . Beroende på vilken metod som har använts har mellan 6 och 23 isoformer av Apo(a) på cirka 300 till 900 kD identifierats (1,2,15,16). De flesta människor har en eller två Apo(a)-isoformer, men hos vissa personer kan inget Apo(a)-band detekteras vid analys med SDS-gel-elektrofores som följs av immunoblotting (3). Den senaste tiden har Lp(a) fått stor uppmärksamhet eftersom det finns många bevis för att cirkulerande halter av Lp(a) utgör en obeoende riskfaktor för kardiovaskulär sjukdom. Lp(a)halter har visat sig vara en nedärvd riskfaktor för ischemisk hjärtsjukdom (4–8). Höga Lp(a)halter har påvisats vid familjär hyperkolesterolemi och det kan vara kliniskt betydelsefullt att kunna mäta dessa halter, för att förutsäga riskerna hos dessa patienter (9,10). Resultat har även publicerats som visar att Lp(a) är en stark indikator för cerebrovaskulär sjukdom (11,12). Apo(a) är homolog till proteasproenzymet plasminogen (13,14). Lp(a) hämmar plasminogenaktiveringen och nyligen genomförda studier har visat att Apo(a) och Lp(a) konkurrerar med plasminogenet om att binda till plasminogenreceptorn. Dessa egenskaper hos Apo(a) skulle kunna förklara sambandet mellan höga Lp(a)-koncentrationer och hjärtinfarkt. TESTPRINCIP Mercodia Apo(a) ELISA är en tvåsidig enzymimmunanalys med fast fas. Den baserar sig på ”sandwich”-teknik, med två monoklonala antikroppar som är riktade mot separata antigena determinanter på Apo(a)-molekylen. Under inkubation reagerar Apo(a) i provet med peroxidaskonjugerade Apo(a)-antikroppar och Apo(a)-antikroppar som är bundna till en mikrotiterbrunn. En enkel tvätt avlägsnar obunden enzymmärkt antikropp. Det bundna konjugatet detekteras genom reaktion med 3,3’,5,5’-tetrametylbensidin. Det går inte att uttrycka koncentrationen av Apo(a) i massenheter eftersom det finns minst sex olika isoformer med molekylvikter som varierar från cirka 300 kD till 900 kD (1,2,15,16).
