Tale rischio era particolarmente elevato negli individui a più basso peso neonatale (probabilità 6 volte maggiore di DM II e 18 volte maggiore di sindrome metabolica)” Vita intrauterina: interazione tra feto e mamma Teoria del “development origins of health and disease”: Il feto nella situazione di iponutrizione, per sopravvivere all’ambiente ostile deve adeguare il suo metabolismo (teoria del gene risparmiatore) Questi aggiustamenti diventano sfavorevoli dopo la nascita, quando l’apporto di nutrienti diventa sufficiente o eccessivo, a causa dell’incapacità di adattarsi al nuovo ambiente e di regolare l’eccessivo intake calorico. L’accumulo di massa grassa comporta obesità viscerale fin dall’infanzia con maggior rischio di presentare malattie correlate (ipertensione, obesità, diabete mellito di tipo 2, ipertensione) Età critiche per lo sviluppo dell’obesità Vita intrauterina : interazione tra feto e mamma - si differenziano i centri ipotalamici responsabili della regolazione dell’appetito, i centri della fame e della sazietà; - alla fine della gravidanza si ha l’incremento del numero degli adipociti che prosegue nei primi mesi di vita, oltre avremo solo un incremento di volume delle cellule adipose. Il Comitato per la Nutrizione dell’Accademia Americana di Pediatria (AAP) ha definito come dieta ottimale per il neonato di basso peso quella che determina una crescita simile a quella del feto durante il terzo trimestre di vita intrauterina, senza sottoporre a stress i sistemi metabolici ed escretori. OBIETTIVI NUTRIZIONALI • Replicare il tasso di crescita intrauterina a parità di EG • Replicare la composizione corporea FATTORI INTERFERENTI • immaturità metabolica e gastrointestinale • immaturità funzioni immunitarie • patologie tipiche • farmaci Alimentazione neonato pretermine Cosa? un difficile compromesso tra “nutrizione fetale” (aminoacidi e glucosio max, lipidi min.) e nutrizione postnatale (lipidi e glucosio max, proteine min.). Proteine • Turnover proteico = elevata richiesta! Non soddisfatta da sintesi endogena (1.0-1.5g/ 30Kcal/ kg/die per prevenire il catabolismo) ma almeno 2g/ 60Kcal /kg/die e dal 4°giorno di vita 3.7-4g/ 120 Kcal/kg/die per assicurare la crescita! Alimentazione neonato pretermine Come ? • Minimal Enteral Feeding minore durata della NP, ridotto ittero, minori residui gastrici, crescita più rapida e minore NEC • latte materno/latte umano di banca fortificato con miscele di sieroproteine idrolizzate (- allergizzanti e + assimilabili), maltodestrine (+ assimilabili, + calorie), sali minerali ed oligoelementi, vitamine • formule per prematuri (+ calorie, + ferro. + sodio, + zinco) con sieroproteine > caseina, EFAe PUFA, nucleotidi • nutrizione parenterale equilibrata (totale o parziale) • adeguata supplementazione di minerali, oligoelementi e vitamine • attento monitoraggio parametri nutrizionali e di crescita Nutrizione del prematuro: Tassi di crescita tra 15 e 20 g/kg/die MEF (Minimal Enteral Feeding), da iniziare al più presto, cmq entro 5 gg: 10-20 ml/kg/die di latte umano per 2-3 gg, quindi proseguire con incremento non superiore a 20 ml/kg/die FEF (Full Enteral Feeding): 150 - 160 ml/kg/die di latte* * se Latte Umano, si inizia fortificazione a volumi di 80-100 ml/kg/die fino alle 40 settimane di EGc FORMULE PER NEONATI PRETERMINE Latte materno non più modello biochimico quantitativo, ma modello qualitativo per il suo contenuto in nutrienti essenziali per il pretermine. If formula-fed, such infants should receive special postdischarge formula with high contents of protein, minerals and trace elements as well as LCPUFA supply, at least until a 40 wks PA, but possibly until about 52 wks PA…” ESPGHAN in JPGN 2006;42:569-603 PDF (formule post-dimissione) Utilizzate dalla metà anni 90 Livelli di nutrienti intermedi tra formula per pretermine (F 0) e formula per nati a termine (F 1) Principali differenze vs F1: • Maggiore apporto proteico • Lievemente maggiore apporto energetico • Maggiore apporto di Ca, P, Zn e altri micronutrienti Carver et al (Pediatrics 2001;107:683–9): Miglioramento della crescita ( P, L, CC) a 6 mesi EC, in particolare in lattanti di PN < 1250 gr Koo WWK et al. (Am J Clin Nutr 2006;84:1357–64): RDB confronto tra PDF e F1 utilizzate per 12 mesi dopo la dimissione in 89 prematuri (EG 24–34 wks, PN 630–1620 g): Migliore crescita, massa ossea e composizione corporea in F1 vs PDF. Criticità degli studi: nessuna informazione sull’apporto totale di energia e nutrienti, in mancanza di diario alimentare ed informazioni sull’intake di altri alimenti. Karger 2007 “… results from larger studies suggest that preterm infants, particularly males and very low birth weight infants, grow better when fed a nutrient-enriched formula following discharge until 6 months’ corrected age or beyond. JPGN 2007;45:S195-203 Cochrane Database of Systematic Review 2005 There is some evidence that post-hospital growth is greater for preterm infants fed calorie and protein-enriched formula milk compared to standard formulas There is no evidence of better neurodevelopmental outcomes There is no evidence of better bone mineralization (DEXA) There are no data on long term effects (growth, cognitive outcomes, metabolic and cardio-vascular health) In conclusione, i reali benefici dell’uso prolungato (> 6 mesi) di formule arricchite in nutrienti rimane incerto o comunque non compiutamente definito, nonostante i teorici vantaggi. [email protected] Abstract Postnatal growth restriction and failure to thrive still remain a major problem in Extremely Low Birth Weight (ELBW) infants . The goal for the nutritional care of these infants is to achieve rate of growth similar to those of the fetus in utero at the equivalent gestational age. Human milk fortified remains the best food for all these preterms. Two groups of preterm of weight 580-1250 g and gestational age 23-32 wk, were fed with different protein intake in the human/maternal milk fortified ( 3,5 g Kg-1 per day and 4,8 g Kg-1 per day in the control and intervention group respectively).The feeding tolerance, intrahospital growth, neurological outcome and anthropometric data until 12 months of corrected age, were evaluated. The protein supplemented group (PSG) showed an intrahospital highter growth rate ( mostly in head circumference, p 0,02, and length growth, p 0,04) only in the preterms with 580-980 g and 23-30 wk. In the same preterms, Griffith Development Mental Score at 3 and 12 months corrected age showed higher score than in the control group in the Performance (p 0,04) and Hearing/Language (p 0,03) items. The auxological evaluation in the postdischarge period showed in the PSG group mean z-score values for length higher than those in the control group at 9 (p 0,04) months of corrected age. [email protected] METHODS: A total of 407 PLBWIs discharged from the newborn departments of ten hospitals in Guangzhou City and Foshan City in Guangdong Province, China were chosen for this study. According to feeding pattern, they were assigned to three groups: PDF-fed (n=258), BM-fed (n=58) and TF-fed (n=91). Their body weight, length and head circumference were measured at 3 months after birth, and the increase rates of growth indices relative to baseline values (at birth) were calculated and compared. RESULTS: At 3 months after birth, the PDF-fed group had significantly greater body weight, length and head circumference than the BM-fed and TF-fed groups (P<0.05). The increase rates of body weight and length were significantly higher in the PDF-fed group than in the BM-fed and TF-fed groups (P<0.05). CONCLUSIONS: Compared with those fed with BM and TF after discharge, the PDF-fed PLBWIs have higher increase rates of body weight and length and show greater body weight and length at 3 months after birth. However, further study is needed to investigate the longterm effects. CONSIDERAZIONI CONCLUSIVE La cura dell’alimentazione del lattante costituisce uno dei compiti primari del pediatra Soltanto il pediatra possiede le conoscenze necessarie per operare una scelta consapevole: • I latti per lattanti NON SONO TUTTI UGUALI • I Lattanti NON SONO TUTTI UGUALI L’alimentazione infantile va tutelata e … soprattutto… non deve essere abbandonata alla pura legge del mercato DIVEZZAMENTO • Assenza di linee guida condivise, in presenza di idee contrastanti tra i neonatologi. • Approccio molto vario e scarsa raccomandazioni generali esistenti. aderenza alle poche • Evento cruciale come importante contributo ad ottimizzare la crescita e lo sviluppo neuro-motorio del prematuro che: ha spesso ritardo di crescita, ha accumulato gravi deficit nutrizionali e presenta elevati fabbisogni energetici e di nutrienti United Kingdom, Department of Health (Weaning and the Weaning Diet: Report on Health and Social Subjects No. 45. London: HerMajesty’s Stationery Office, 1994 ): ‘‘A reasonable compromise may need to be adopted such that weaning can be advised when the infant weighs at least 5 kg, has lost the extrusion reflex, and is able to eat from a spoon’’. Raccomandazioni vaghe, di buon senso, ma non basate su precise evidenze scientifiche. Fewtrell et al (Arch Dis Child Fetal Neonatal Ed 2003;88:F296–301): 2 trials dal 1993 al 1997, 1 condotto su 264 prematuri (<37 wks EG) e 1 su 228 con PN < 2000 gr: 98% divezzamento entro il 4° mese dopo il termine. Yee J et al. (J Am Diet Assoc 2001;101:A76): USA 67% e Cile 41%. Norris FJ et al.(Eur J Clin Nutr 2002;56:448–54): 253 prematuri di 34.4 wks (range 28–36). Conclusioni: epoca di divezzamento molto variabile e scarsa aderenza alle raccomandazioni disponibili, poiché il 49% lo aveva iniziato prima di quanto disposto dalle linee guida del Department of Health (media 17.1, ± SEM 0.23 wks). Se rapportate al termine di gravidanza, non aderenza pari al 95% (media 11.5, ± SEM 0.21 weeks) Crema di riso come primo alimento solido in 84.6% dei prematuri Recente studio svizzero su 354 nati vivi < 1250 gr: La crescita post-natale è significativamente associata e determina l’outcome neuroevolutivo. Lo studio sui prematuri VLBW, SGA e AGA, dimostra che il “post-natal catchdown growth”, cioè un peso a 2 anni < al 10° pc, comporta il rischio più elevato per ritardo mentale e motorio e per paralisi cerebrale in tutti i prematuri valutati ed influisce negativamente su lunghezza e circonferenza cranica. Benchè non ci sia piena evidenza di una relazione causale tra scarso recupero di crescita post-natale ed alterata integrità del SNC,un incremento di peso insufficiente deve rappresentare un campanello di allarme per il pediatra e per chi valuterà lo sviluppo neuropsicomotorio del bambino. “Evidenza che lo sviluppo di eczema entro il 12° mese nei pretermine è influenzato da precoce divezzamento. Sviluppo di eccessiva massa grassa a 7 aa in bambini divezzati entro le 15 settimane L’accelerato recupero di peso post-natale ha effetti positivi su intelligenza, ma anche negativi su incremento di morbidità nell’infanzia e nell’età adulta. Una crescita di recupero più lenta, pertanto, potrebbe rappresentare un giusto compromesso” “In conclusione: Al momento attuale, assenza di linee guida basate sull’evidenza sul divezzamento del prematuro Ottime ragioni per ritenere che l’epoca del divezzamento vada adattata al singolo bambino ex prematuro Finchè saranno disponibili nuovi dati, il criterio di prudenza ed il buon senso suggeriscono di evitare una precoce introduzione di cibi solidi e adattarne l’introduzione sulla base di: EG, sviluppo pre- e post-natale e stato nutrizionale.” Carne e glutine in media 5 e 7 wks dopo l’inizio del divezzamento!!!
