BEIA HSV 1 IgG 20921 Edizione 20/04/2013 Solo per uso professionale TECHNOGENETICS SRL N° 00525 Pag. 1 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 INTRODUZIONE Il kit BEIA HSV 1 IgG è un test immunoenzimatico qualitativo (ELISA) per la determinazione nel siero o nel plasma degli anticorpi di classe IgG specifici verso l’Herpes Simplex Virus di tipo 1 (HSV 1). SIGNIFICATO CLINICO Il virus dell'Herpes simplex umano (HSV) appartiene alla famiglia delle Herpesviridae, sottofamiglia delle Alphaherpesviridae. La particella virale è di tipo icosaedrico e misura 180-200 nm di diametro. Il virione completo è costituito da un nucleo cilindrico proteico o core, attorno al quale si avvolge il genoma virale costituito da una molecola lineare a doppia elica di DNA; esso è rivestito da un capside e da un pericapside da cui protrudono proiezioni lunghe 8-10 nm costituite da glicoproteine. Sulla base di differenze antigeniche tra queste glicoproteine vengono distinti due differenti sierotipi: HSV 1 e HSV 2 La trasmissione del HSV richiede un contatto diretto tra un soggetto che elimina virus ed un individuo suscettibile. La penetrazione del virus avviene a livello delle mucose o attraverso lesioni della cute. Per HSV di tipo 1 la localizzazione più frequente è l'orofaringe, per HSV di tipo 2 la mucosa genitale. L'infezione primaria da HSV induce una risposta immunitaria umorale specifica caratterizzata da sintesi di IgM seguite da IgG ed IgA. La presenza ed il titolo di IgG e IgM HSV-specifiche possono essere determinati con tutta una serie di reazioni sierologiche che vanno da quelle più tradizionali, come la reazione di fissazione del complemento, la reazione di neutralizzazione dell'infettività e l'immunofluorescenza, a quelle più recenti, come i saggi immunoenzimatici. Qualora le eventuali IgM HSV-specifiche presenti nel siero vengano evidenziate è necessario tenere conto della possibile interferenza di alcuni fattori, tra i quali ad esempio la competizione da parte delle IgG HSV-specifiche che in questo modo possono rendersi eventualmente responsabili di risultati falsi negativi per IgM. Al contrario, una possibile causa di risultati falsi positivi per IgM è Pag. 2 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 rappresentata dalla presenza nel siero di fattore reumatoide (IgM anti-IgG). L'impiego di un test "a cattura" permette di risolvere entrambe queste insidie. La dimostrazione di IgM HSV specifiche nel siero del soggetto depone per una infezione in atto; tuttavia, data la possibile comparsa di IgM anche nel corso di infezioni ricorrenti, qualora sia necessario valutare un paziente per un'infezione primaria, tranne che si tratti di un neonato, è necessario procedere anche ad una titolazione delle IgG HSV-specifiche su due campioni di siero raccolti in tempi opportunamente distanziati (non meno di due settimane). Nel caso di un'infezione primaria, accanto alla positività per IgM sarà rilevabile una differenza significativa nel titolo anticorpale; al contrario qualora il soggetto sia affetto da un episodio ricorrente e conseguente a riattivazione, il titolo di IgG HSV-specifiche nei due campioni di siero sarà aumentato solo di poco, in maniera non significativa, o per nulla. PRINCIPIO DEL METODO Il test BEIA HSV 1 IgG è un dosaggio immunoenzimatico in micropozzetti. I micropozzetti di polistirene sono rivestiti con antigene HSV 1. I campioni da esaminare vengono incubati nei micropozzetti; le IgG del campione si legano formando un complesso antigene-anticorpo. Dopo eliminazione mediante lavaggio dei componenti del siero non legati alla fase solida, la presenza di IgG anti-HSV 1 viene rilevata attraverso il legame con un anticorpo monoclonale anti-IgG umane coniugato con perossidasi. Dopo allontanamento del coniugato enzimatico non legato viene aggiunto il substrato-TMB che, per azione dell’enzima presente sulla fase solida, sviluppa una colorazione azzurra. L’aggiunta di una soluzione di acido solforico 1N blocca la reazione e provoca il viraggio del colore dall’azzurro al giallo. La densità ottica letta a 450/620 è direttamente proporzionale alla quantità di IgG specifiche anti-HSV 1presenti nel campione in esame. Non utilizzare i reattivi oltre la data di scadenza riportata sull’etichetta. REAGENTI INTERCAMBIABILI Il Diluente Campioni BEIA System, il Tampone di Lavaggio, il SubstratoTMB e la Soluzione Stoppante sono intercambiabili tra i diversi kit della linea BEIA. MATERIALE E STRUMENTI RICHIESTI, MA NON FORNITI Lettore di colorimetria per micropiastre con filtro a 450/620 nm. Apparecchiatura manuale o automatica per il lavaggio di micropiastre. Micropipette con puntali monouso da 10, 100, 200, 500, 1000 µL. Provette monouso e normale vetreria da laboratorio. Acqua distillata o deionizzata. Pag. 5 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 AVVERTENZE E PRECAUZIONI Prima dell'uso portare tutti i reattivi a temperatura ambiente (18-25°C). Prelevare solo la quantità necessaria per l'analisi. Evitare l'essiccamento dei micropozzetti. Usare esclusivamente vetreria accuratamente pulita. Non utilizzare uno stesso puntale per pipettare reagenti diversi. Cambiare il puntale tra un campione e l’altro. Qualora il Substrato-TMB evidenziasse un viraggio da rosa ad azzurro, il reagente non deve essere utilizzato. Se il dosaggio viene ripetuto, preparare una nuova diluizione del campione. Si raccomanda di inserire in ogni corsa analitica dei campioni di controllo al fine di validare i risultati ottenuti, secondo criteri e procedure che ciascun utilizzatore deve definire. Procedere periodicamente alla manutenzione e taratura delle pipette e del lettore. AUTOMAZIONE Il test può essere eseguito con sistemi automatici per kit ELISA, nel qual caso Vi invitiamo a consultare il manuale specifico dello strumento per una corretta esecuzione della corsa analitica. PRELIEVO E PREPARAZIONE DEI CAMPIONI Il dosaggio può essere eseguito su siero o plasma (Eparina-EDTA). Se il dosaggio è eseguito entro 5 giorni dal prelievo, i campioni possono essere conservati a 2-8 °C. In caso contrario, aliquotare i campioni e congelarli a -20°C. Evitare ripetuti scongelamenti e congelamenti. Si sconsiglia l’uso di campioni lipemici, torbidi ed emolizzati. I campioni devono essere opportunamente diluiti con il Diluente Campioni BEIA System prima del dosaggio. Vedi esecuzione del test. Pag. 6 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 PREPARAZIONE DEI REAGENTI Tampone di Lavaggio Diluire il Tampone di Lavaggio (20x) 1:20 con acqua distillata o deionizzata prima dell'uso. Dopo ricostituzione il tampone è stabile 15 giorni se conservato a 2-8°C e comunque non oltre la data di scadenza riportata sull'etichetta originaria. Nel Tampone di Lavaggio concentrato si possono formare dei precipitati cristallini che si solubilizzano portando il reagente a 37°C prima della diluizione. Si raccomanda di diluire la quantità di tampone necessaria per eseguire i lavaggi dell'analisi in corso. CICLO DI LAVAGGIO Si consiglia di procedere come segue sia nel caso in cui si utilizzi un lavatore automatico di micropiastre sia nel caso in cui si operi manualmente. 1. Svuotare completamente i pozzetti. 2. Riempirli con 300 µL di tampone di lavaggio. 3. Svuotare i pozzetti. 4. Ripetere i punti 2 e 3 per ulteriori 3 cicli. 5. Asciugare con carta assorbente la superficie esterna dei pozzetti al termine del lavaggio. ESECUZIONE DEL TEST 1. 2. Portare i reagenti a temperatura ambiente (almeno 30-60 minuti a 18-25°C). Diluire (1:81) i campioni in esame con il Diluente Campioni BEIA System (esempio 10 µL di siero e 800 µL di Diluente Campioni BEIA System). Agitare su Vortex. Prelevare il numero di strip necessario per l'analisi in funzione del numero di campioni in esame, del Controllo Negativo, del Calibratore Cut-off e del Controlo Positivo (vedi il paragrafo “Schema del dosaggio”). Richiudere accuratamente la busta. Pag. 7 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 Dispensare nei rispettivi pozzetti 100 L di ciascun siero in esame prediluito, del Controllo Negativo, del Calibratore Cutoff e del Controlo Positivo. 4. Incubare la piastra a temperatura ambiente (18-25°C) per 30 minuti. 5. Eseguire la procedura di lavaggio dei pozzetti seguendo il procedimento raccomandato (vedi Ciclo di lavaggio). 6. Dispensare in ciascun pozzetto 100 µL di Coniugato. 7. Incubare la piastra a temperatura ambiente (18-25°C) per 30 minuti. 8. Eseguire la procedura di lavaggio dei pozzetti seguendo il procedimento raccomandato (vedi Ciclo di lavaggio). 9. Dispensare in ciascun pozzetto 100 µL di Substrato-TMB. 10. Incubare la piastra a temperatura ambiente (18-25°C) per 15 minuti. 11. Bloccare la reazione cromogenica dispensando in ciascun pozzetto 100 µL di Soluzione Stoppante. 12. Leggere la densità ottica (DO) in bicromatismo a 450/620 nm. 3. CALCOLO ED INTERPRETAZIONE DEI RISULTATI VALIDAZIONE DELLA SEDUTA ANALITICA Le condizioni che consentono l’accettazione dei risultati sono : il rapporto tra la DO del Controllo Positivo e la DO del Calibratore Cut-off deve essere essere > 1.5 il rapporto tra la DO del Controllo Negativo e la DO del Calibratore Cut-off deve essere < 0.6 Qualora la seduta dia risultati non conformi alle condizioni sopra elencate, essa non è validabile ed i risultati relativi ai campioni non possono essere refertati senza analisi critica documentata. La non conformità va qualificata e trattata secondo le procedure di Assicurazione Qualità proprie del laboratorio. Si consiglia di instaurare ed attuare procedure di Controllo Qualità interne al laboratorio, i cui risultati concorrano alla validazione delle sedute ed alla gestione di eventuali non conformità. INTERPRETAZIONE > 1.1 Il campione è da considerare POSITIVO per la presenza di anticorpi di classe IgG anti-HSV 1 0.9 Il campione è da considerare NEGATIVO per la presenza di anticorpi di classe IgG anti-HSV 1 0.9 1.1 ZONA GRIGIA: Il campione è da considerare DUBBIO per la presenza di anticorpi di classe IgG anti-HSV 1 Va tenuto presente che: - il test indica solo la presenza o assenza di anticorpi specifici di classe IgG. Non è di per sé e univocamente indice di uno stato infettivo; Pag. 9 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 - una decisione medica non può essere basata sul risultato di un solo test, ma va fondata sull’insieme dei dati clinici disponibili. Dati clinici discrepanti vanno risolti prima di prendere la decisione, eventualmente col conforto di test di conferma o di II livello; - risultati compresi entro la zona grigia vanno chiariti: con la ripetizione del test sullo stesso campione; con la ripetizione del test su un prelievo successivo; col confronto con un test di riferimento. PRESTAZIONI DEL KIT Avvertenza: i dati presentati sotto questa voce non rappresentano le specifiche di funzionamento del kit, ma costituiscono evidenza sperimentale di come il kit funzioni entro tali specifiche nel modo previsto dal fabbricante. SENSIBILITÀ E SPECIFICITÀ Sensibilità e specificità sono state valutate confrontando le prestazioni del kit BEIA HSV 1 IgG nei confronti di un kit già immesso sul mercato con prestazioni accettabili (EIA HSV 1 IgG). L’uso di campioni lipemici, torbidi o emolizzati viene comunque sconsigliato. Pag. 10 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 RIPETIBILITÀ La variabilità intra-saggio ed inter-saggio è stata determinata valutando un campione negativo con I.A. < 0.5 e due campioni positivi rispettivamente con I.A. di 1.2 e 2.7. I campioni sono stati replicati 6 volte per seduta in ciascuna di 12 sedute diverse. Sono stati ottenuti coefficienti di variazione inferiori al 20% per il campione negativo ed inferiori al 10% per i due campioni positivi. LIMITAZIONI Contaminazione batterica o ripetuti scongelamenti e congelamenti dei campioni possono alterare i livelli rilevabili di IgG specifiche. PRINCIPI GENERALI DI SICUREZZA Tenere in ordine il laboratorio ed effettuare con cura le operazioni previste per l'esecuzione del test. Non mangiare, bere, fumare o usare cosmetici nell’area di laboratorio designata. Non pipettare soluzioni o campioni con la bocca. Evitare il contatto diretto con i reattivi ed i campioni indossando guanti monouso e camici. Lavare accuratamente le mani al termine del dosaggio. Pulire immediatamente con opportuni detergenti le superfici di lavoro eventualmente contaminate. Raccogliere il materiale solido o liquido utilizzato per il dosaggio in appositi contenitori ed effettuare lo smaltimento secondo le norme vigenti. Controllare periodicamente il grado di contaminazione dell'ambiente di lavoro e degli operatori. Reagenti contenenti siero I derivati da sangue umano inclusi nel kit sono stati analizzati e trovati negativi per HbsAg, anti-HCV ed anticorpi anti-HIV. Poiché nessun metodo può garantire che tali derivati non possano trasmettere epatite, AIDS o altre infezioni, si raccomanda di manipolare i reagenti con le opportune cautele. CLINICAL RELEVANCE Human Herpes Simplex virus (HSV) belongs to the family of Herpesviridae, a subfamily of the Alphaherpesviridae. HSV is a icosahedral virus with a diameter of 180-200 nm. Virion consists of a cylindrical proteinic nucleus or core, and the viral genome made of a linear double-stranded DNA molecule is wrapped around it. A capsid and a pericapsid surround the core and from their surface glycoproteins project as spikes approximately 8-10 nm in length. According to the antigenic differences among the glycoproteins it's possibile to distinguish two different serotypes: HSV 1 and HSV2. HSV infection spreads from person to person by direct contact between an individual eliminating the virus and another one catching it. HSV penetrates through mucosas or skin lesions. HSV type 1 usually affects oropharynx, while HSV type 2 affects the genital area. Primary HSV infection induces a specific humoral immune response, characterized by the production of IgM followed by IgG and sometimes IgA. Specific IgG and IgM to HSV antibodies can be detected in serum and their titers can be determined by different serological assays, starting from the most traditional such as the complement fixation assay, the neutralization assay and the immunofluorescence assay to the recent ones like the enzyme immunoassays. The use of serological assays for the detection of HSV specific IgM is complicated by interfering factors like specific IgG which, competing with IgM, could be responsible for false negative results. On the contrary, false positive results in the detection of IgM antibodies can occur in presence of the rheumatoid factor in serum (IgM anti-IgG). A test that utilizes "Capture system" can elude the occurrence of both false results. Pag. 14 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 Although IgM specific HSV antibodies should indicate an active infection, detectable levels of IgM can also be found in recurrent infections. Therefore, whenever the diagnosis of a primary infection is needed, the patients, new-born excluded, should be tested for the presence of HSV specific IgG antibodies in two serum samples taken at least 2 weeks one from the other. In primary infections, positive results for the presence of IgM and a significant difference in the antibody titer of sprcific IgG antibodies will occur; on the contrary, reactivation of the virus due to recurrent infections will result in a low, not significant or not detectable increase of the HSV specific IgG antibody titer in both serum samples. PRINCIPLE OF THE ASSAY The BEIA HSV 1 IgG kit is a qualitative enzyme-linked immunosorbent assay (ELISA) for the detection of specific IgG antibodies to HSV 1, in human serum or plasma. During the first incubation, only anti-HSV 1 specific antibodies present in serum or plasma bind to the inner surface of the wells coated with the HSV 1 antigen. After the first incubation the wells are washed to remove non-reactive serum components. During the second incubation a monoclonal antibody anti-human IgG conjugated with horseradish peroxidase (HRP) is added. After a second washing cycle, a Substrate-TMB solution is dispensed into the wells in order to detect specific antibodies during a subsequent incubation. The enzymatic reaction is then stopped by adding a Stop Solution which changes to yellow the blue colour developed into the wells. Do not use reagents after the expiry date indicated on the label. INTERCHANGEABLE REAGENTS The Sample Diluent BEIA System, Wash Buffer, Substrate-TMB and Stop Solution are common to all BEIA kits. WARNINGS AND PRECAUTIONS Bring all reagents to room temperature (18-25°C) before use. Only take the quantity necessary for the assay. Avoid wells drying. Pag. 17 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 Use clean glassware. Use a clean disposable pipette tip for each reagent. Use a clean disposable pipette tip for each sample. If the Substrate-TMB shows a change from pink to blue colour, discard the reagent. For repeated assays, each time provide for new diluted samples. Every analytical run should include control samples in order to validate the results, according to criteria and procedures established by each laboratory. Periodical maintenance and calibration of pipettes and microplate reader are required. AUTOMATION The test can be performed on automated ELISA processor. Please refer to specific instrument manual for proper applications. SPECIMEN COLLECTION AND PREPARATION The test can be performed on serum or plasma (heparin – EDTA). Specimens may be stored refrigerated at 2-8 °C up to 5 days. For longer storage they should be aliquoted and stored frozen at – 20°C. Avoid repeated thawing and freezing. Do not use haemolyzed, lipemic or turbid specimens. All serum and plasma samples must be pre-diluted with Sample Diluent BEIA System prior to assay. See assay protocol. PREPARATION OF REAGENTS Wash Buffer (20 X) Dilute 1:20 the content of the Wash Buffer bottle with distilled or deionised water. The diluted Wash Buffer is stable 15 days when stored at 2-8 °C; in any case, do not use it after expiry date printed on the original label. If crystallization occurs in the concentrated Wash Buffer, warm the solution to 37 °C before diluting. Only dilute the quantity necessary for the assay. Pag. 18 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 WASHING CYCLES Both using a microplate washer and washing the plate manually, perform the following steps. 1. Empty the wells 2. Fill them with 300 µL of Wash Buffer. 3. Empty the wells 4. Repeat steps 2 and 3 for other 3 cycles 5. Dry the rim of the wells with blotting paper at the end of the washing cycles. ASSAY PROTOCOL Allow all reagents to reach room temperature leaving them at least 30-60 minutes at 18-25 °C. 1. Dilute the samples to be assayed (1 :81) with the Sample Diluent BEIA System (i.e. 10 µL of serum and 800 µL of Sample Diluent BEIA System). Shake on Vortex. 2. Select the number of strips required for the assay according to the number of samples to be assayed and the Negative Control, Cut-off Calibrator and Positive Control (see paragraph “Summary of protocol” below). Reseal the pouch. 3. Dispense 100 µL of Negative Control, Cut-off Calibrator, Positive Control and of the diluted samples. 4. Incubate the plate at room temperature (18-25 °C) for 30 minutes. 5. Perform a washing cycle following the suggested procedure (see Washing Cycles). 6. Dispense 100 µL of Conjugate in each well. 7. Incubate the plate at room temperature (18-25 °C) for 30 minutes. 8. Perform a washing cycle following the suggested procedure (see Washing Cycles). 9. Dispense 100 µL of Substrate-TMB in each well . 10. Incubate the plate at room temperature (18-25 °C) for 15 minutes. 11. Stop the chromogenic reaction dispensing 100 µL of Stop Solution into each well. 12. Read the optical density in bi-chromatism at 450/620 nm. Pag. 19 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 CALCULATION AND INTERPRETATION OF RESULTS RUN VALIDATION CRITERIA Criteria used to validate the obtained results are the following: The ratio of the OD for Positive Control to the OD for the Cutoff Calibrator must be > 1,5 The ratio of the OD for Negative Control the OD for the Cutoff Calibrator must be < 0,6 If the above criteria are not met, the run is invalid and the results cannot be reported without a documented critical review of the data. The non conformity must be addressed and treated according to each laboratory’s Quality Assurance procedures. Each lab should establish and follow its own internal Quality Control procedures and use the QC results to validate runs and manage non conformities. CALCULATION OF RESULTS In order to correlate the analytical results to the immunity of the sample results should be determined calculating the antibody index (A.I.) using the following formula: O.D. sample ________________________________________ Antibody Index (A I) = O.D. INTERPRETATION > 1.1 Sample should be considered POSITIVE for the presence of anti-HSV 1 IgG antibodies. < 0.9 Sample should be considered NEGATIVE for the presence of anti-HSV 1 IgG antibodies. 0.9 1.1 GREY ZONE: Sample should be considered BORDERLINE for the presence of anti-HSV 1 IgG antibodies. Please consider that: - the test only detects specific IgG antibodies. It cannot per se and univocally be referred to as evidence of infection; - a medical decision cannot be made on the basis of a single test result but should be grounded on all the available clinical data; discrepancies in clinical data should be clarified prior to final decisions, also, if necessary, by confirmatory or 2 nd level tests; - results within grey zone should be made clear: re-testing the same sample; testing a sample from another collection from the same patient; comparing the results with those of a reference test. Pag. 21 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 PERFORMANCE DATA Warning: Data below do not represent the performance specifications of the kit, but only experimental evidence that the performance of the kit is within such specifications, as intended by the manufacturer. SPECIFICITY AND SENSITIVITY The diagnostic sensitivity and specificity were evaluated comparing the BEIA HSV 1 IgG results versus an established EIA commercial kit (EIA HSV 1 IgG) with acceptable performances. Lipemic, turbid or hemolyzed samples, however, should not be used. REPEATABILITY The within-run and run-to-run variability were determined using one negative sample with A.I. of < 0.5 and two positive samples, containing value of Antibody Index (A.I) : 1.2 and 2.7. These samples were tested 6 times per single run in 12 different analytical runs. Coefficients of variation lower than 20% have been obtained for the negative sample and 10% for the positive samples. LIMITATIONS Bacterial contamination or repeated freeze-thaw cycles of specimens may affect the detectable levels of specific IgG. Pag. 22 - 24 BEIA HSV 1 IgG Ed. 20/04/2013 GENERAL DIRECTIONS FOR A SAFE USE Keep the laboratory clean and tidy and strictly follow the procedural directions. Do not eat, drink, smoke or apply cosmetics in a clinical lab. Do not pipette solutions or samples by mouth. Wear disposable gloves and overalls when handling reagents and samples. Avoid direct contact. Wash hands thoroughly after assay. Throughly clean working area with proper detergent solutions. Collect solid and liquid waste material in proper containers and provide for disposal according to the local regulations. Periodically check contamination level of operators and rooms. Reagents containing serum All components of human origin have been tested and found negative for HbsAg, anti-HCV and anti-HIV antibodies. Since no known test can guarantee, however, that products derived from human blood will not transmit Hepatitis, AIDS or other infectious diseases, these reagents should be handled as hazardous material.
