Case report A description of two rare D phenotypes Adelina Ornella Perrone, Giovanna Ferrari, Ilda Ghiglione, Clemente Mazzei Servizio di Immunoematologia e Medicina Trasfusionale, ASL 1 Imperiese, Imperia Introduction Introduzione For some time now our Immunotransfusion Service has been systematically investigating discrepant or weak results found during Rhesus factor determinations1. Here we report two interesting cases detected in 2002 among 12,000 tests to determine blood group. Da tempo il nostro SIT approfondisce sistematicamente i risultati discrepanti o deboli nella determinazione del fattore Rh1. Comunichiamo due casi interessanti rilevati nel 2002, su 12.000 determinazioni di gruppo sanguigno. Case 1 (discrepant reaction) I caso (reazione discrepante) SL, a 29-year old parturient of Columbian origin. Blood group O. Direct and indirect antiglobulin tests, negative. Determination of RhD factor: - direct reaction on glass slide, with monoclonal IgM anti-D sera (Laboratoires Eurobio, Les Ulis Cedex, France), cell lines derived from the fusion of an EBV line and a human-murine heteromyeloma line (clone CAZ): result equivocal (±) after 2 minutes; - direct reaction on glass slide, with incomplete human polyclonal anti-D sera (Ditta DiaMed Italia Srl, Vedano al Lambro, Milan, Italy): result positive (1+) after 2 minutes; - direct reaction in DiaMed gel-test, with monoclonal antiD sera, cell lines LHM59/20 (LDM3), 175-2; and with polyclonal anti-D sera: result positive (4+); - partial RhD typing, with ID Partial RhD Typing – DiaMed sera (Figure 1) and with D-Screen sera (Laboratori Diagast, EuraSante, Loos Cedex, France) (Figures 2 and 3): pattern corresponding to RhDVa. SL, anni 29, partoriente, di origine colombiana, gruppo sanguigno O, test diretto ed indiretto all'antiglobulina, negativi. Determinazione del fattore RhD: - reazione diretta su vetrino, con sieri anti-D monoclonali di classe IgM (Laboratoires Eurobio, Les Ulis Cedex, France), linee cellulari derivanti dalla fusione della linea EBV con un eteromieloma umano- murino (clone CAZ): risultato dubbio (±) dopo 2 minuti, - reazione diretta su vetrino, con sieri anti-D policlonali umani incompleti (Ditta DiaMed italiana Srl, Vedano al Lambro, Milano): risultato positivo (1+) dopo 2 minuti - reazione diretta in gel-test DiaMed, con sieri anti-D monoclonali, linee cellulari LHM59/20 (LDM3), 175-2; e con sieri anti-D policlonali: risultato positivo (4+), - tipizzazione RhD parziale, con i sieri ID Partial RhD Typing-DiaMed (Figura 1) e con i sieri D-SCREEN (Laboratori Diagast, EuraSante, Loos Cedex, France) (Figure 2 e 3): pattern corrispondente a RhDVa. Conclusione: fenotipo: CcDVaee Il caso è stato inviato, tramite i laboratori DiaMed di Cressier sur Morat (Svizzera), all'International Blood Group Reference Laboratory di Bristol, diretto dal prof. The case was sent, via DiaMed's laboratories in Cressier-sur-Morat (Switzerland), to the International Blood Group Reference Laboratory in Bristol, directed by Prof. Anstee, where the sample was analysed with an extensive panel of anti-D sera and was confirmed to belong to the DVa category, with a probable genotype of Cdes/cDwe. The red blood cells were positive for the low frequency w D antigen (D Wiel or Rh23), VS+ ( es or Rh20), and had abnormal expression of C antigen. Specific molecular investigations were not carried out on the sample since the molecular bases of the DVa phenotype are already known2-7. Molecular analyses are reserved for samples which cannot be classified by other methods or whose classification remains uncertain. The RHD category Va haplotype originates from a hybrid RH(DBlood Transf 2003; 3: 284-88 Figure 2 - Partial RhD typing with D-Screen-IgG class sera- Patient SL estensivo di sieri anti-D e confermato come appartenente alla categoria DVa , probabile genotipo: Cdes/cDwe. Le emazie sono risultate positive per l'antigene Dw a bassa frequenza (D Wiel o Rh23), VS+ ( es o Rh20), e presentavano un'espressione anomala dell'antigene C. Il campione non è stato sottoposto ad indagine molecolare specifica, in quanto le basi molecolari del fenotipo DVa sono note2-7. L'analisi molecolare è riservata ai campioni che non possono essere classificati con altri metodi o di incerta classificazione. L'aplotipo RHD di categoria Va origina da un ibrido RH(D-CE-D) formatosi durante la meiosi tra i geni RHD ed RHCE. L'esone 5'dell'RHD è sostituito, del tutto o in parte, dall'esone 5 del gene RHCE. Di norma gli epitopi D1 e D5 sono assenti e sui globuli rossi sono presenti circa 9.400 siti antigenici per cellula. Alla nostra proposita abbiamo consegnato un referto di gruppo sanguigno in cui il fattore Rh è stato definito: "Variante RhD positivo di categoria Va , per la quale si consiglia la profilassi anti-D post partum o postamniocentesi e la somministrazione di sangue Rh negativo in caso di terapia trasfusionale. Da considerare Rh positivo come donatore". II caso (reazione debole) MC, anni 36, gravida al VI mese, gruppo sanguigno A, test diretto e indiretto all'antiglobulina, negativi. Determinazione del fattore RhD: - reazione diretta su vetrino, con sieri anti-D monoclonali Eurobio e con sieri policlonali DiaMed: negativa, - reazione diretta in gel-test DiaMed, con sieri anti-D mono e policlonali: negativa , 285 O Perrone et al. Exon 5' of the RHD is substituted, completely or in part, by exon 5 of the RHCE gene. The D1 and D5 epitopes are normally absent and there are about 9,400 antigenic sites on each red cell. We issued a blood group report in which the Rh factor was defined: "RhD variant positive, category Va. Anti-D prophylaxis is advised post-partum or post-amniocentesis. Administer Rh negative blood if transfusion therapy is needed. To be considered Rh positive as a donor". Case 2 (weak reaction) MC, a 36-year women in her 6th month of pregnancy. Blood group A. Direct and indirect antiglobulin tests, negative. Determination of RhD factor: - direct reaction on glass slide, with Eurobio monoclonal anti-D sera and with DiaMed polyclonal sera: negative; - direct reaction in DiaMed gel-test, with monoclonal and polyclonal anti-D sera: negative; - search for Du in gel-test with polyclonal sera: positive, reaction classified as 2+; - partial RhD typing, with ID Partial RhD Typing, DiaMed (Figure 4) and with DIAGAST D-screen (Figures 5 and 6): pattern with weak positivity, corresponding to weak RhD with very low expression. Conclusions: Rh phenotype: ccDu*Ee * Du expression very low The case was sent to the DiaMed laboratories at Cressier-sur-Morat, where the typing was repeated with 286 Figure 6 - Partial RhD typing with D-Screen-IgM class sera- Patient MC ricerca Du in gel-test con sieri policlonali: positiva, reazione classificata con 2+, - tipizzazione RhD parziale, con ID Partial RhD Typing, DiaMed (Figura 4) e con D-Screen Diagast (Figure 5 e 6): pattern con positività deboli, corrispondente a RhD debole a bassissima espressività. Conclusioni: fenotipo Rh: ccDu*Ee * Du a bassissima espressività. Il caso è stato inviato ai laboratori DiaMed di Cressier sur Morat, dove è stata ripetuta la tipizzazione con i sieri ID Partial e D-Screen. Il campione è stato inoltre testato con numerosi sieri anti-D ed è stato definito come "very very weak D". Si tratta del secondo caso da noi descritto, dato che nel 2001 avevamo già riscontrato1 in un'altra paziente un "Rh D debole di tipo 11". I fenotipi RhD deboli sono generalmente considerati - Blood Transf 2003; 3: 284-88 Two rare D phenotypes ID Partial and D-Screen sera. The sample was also tested with numerous other anti-D sera and was defined as "very very weak D". This is the second case that we have described, having already found1 another patient in 2001 with "Rh D weak, type 11". Individuals with the weak RhD phenotypes are normally considered positive as both donors and recipients and no cases of immunisation involving the more common types (Dweak 1, 2, 3) have been reported. The situation is not, however, so clear for some of the less frequent types, and particularly for those with very low expression. Reports of immunisation, together with molecular structure analyses, have demonstrated that some subjects with weak RhD are not exempt from immunisation and that some, besides have quantitative differences from normal RhD samples, also have mutations in the structure of the gene (qualitative differences)8, 9. It is, therefore, advisable to transfuse carriers of altered proteins with Rh negative products because these patients are potentially at risk of developing antibodies. Since our patient was a young woman and her blood sample contained a very low number of antigenic sites, our blood group report defined the Rh factor as "very very weak Du, to be considered as Rh negative as a recipient and positive as a blood donor". La situazione non è altrettanto chiara con alcuni tipi meno frequenti e, in particolare, con i tipi a bassissima espressività. Segnalazioni di eventi immunizzanti, unitamente all'analisi delle strutture molecolari, hanno dimostrato che anche per alcuni soggetti RhD deboli non è da escludere la possibilità di immunizzarsi e che alcuni, oltre a presentare differenze quantitative rispetto ai campioni RhD normali, hanno mutazioni nella struttura del gene (differenze qualitative)8, 9. È, quindi, opportuno indirizzare verso trasfusioni Rh negative i portatori di proteine alterate perché potenzialmente a rischio di sviluppare anticorpi. Nel nostro caso, poiché il campione presentava un numero di siti antigenici molto basso e si trattava di una giovane donna, le abbiamo consegnato un referto di gruppo sanguigno definendo il fattore Rh " Du a bassissima espressività, da considerarsi Rh negativo come ricevente e positivo come donatore di sangue". 287 O Perrone et al.
