Scale di valutazione per l’arto superiore nelle neuropatie Dott.ssa Laura Roncari Dott.ssa Giulia Berto Introduzione Nella revisione della letteratura da noi condotta ci siamo focalizzate sulla ricerca di scale di valutazione che prendessero in considerazione nello specifico la funzionalità della mano, più che l’arto superiore in generale. Questo criterio di ricerca è stato indirizzato dalla pratica clinica che ci mostra come la disabilità maggiore sia data dall’alterazione di sensibilità e forza a livello distale. Pertanto come parole chiave nella nostra ricerca sono state utilizzate: HAND, NEUROPATHY, ASSESSMENT, CMT, CIDP E DIABETES. Metodologia • La ricerca è stata condotta nello spazio temporale tra il 2008 al 2014 • Sono stati suddivisi gli articoli trovati per tipo di patologia • In tutto sono stati valutati 25 articoli. PAZIENTI ASSESSMENT 20 (10 DM-10 sani) Tactile sensation (SemmesWeinstein monofilament exam), Hand function (Jebsen-Taylor, Purdue Pegboard Evaluation), QoL (Modified Norfolk Quality of Life), kinematic and kinetic evaluation 10 PZ+10 CNT Sensory evaluation (SWMT), motor evaluation (maximum hand grip strength was evaluated using a biometrics hand dynamometer and wireless datalog system, maximum pinch evaluation was conducted using a biometrics pinch dynamometer and wireless datalog system), kinematic evaluation 159 PZ + 95 CNT Dexterity (pinch-holding up activity test), sensory (semmes-weinstein monofilament (SWM), static two-point discrimination (S2PD) and moving 2PD (M2PD)), 144 Objective evaluations, including the Purdue pegboard test, electrophysiological testing in sensory amplitude of the median nerve, and self-administrated measurements, including the michigan hand outcomes questionnaire (MHQ) and diabetes39 (D-39) 90 PZ Temperature,pain and vibration (Medoc TSA 2001 device); Cognitive function (Number connection Test); dexterity (Jebsen-Taylor and function test, Motor performance series) Risultati Andando ad analizzare le scale di valutazione prese in considerazione nei diversi studi abbiamo concluso che: • La neuropatia in cui sono presenti un maggior numero di studi dedicati alla funzionalità della mano è la CMT • In questa patologia vengono generalmente valutate: la destrezza, la forza, la sensibilità e la disabilità • Le scale di valutazione e gli strumenti maggiormente utilizzati sono: Destrezza: SOLLERMANN HAND FUNCTION TEST e NHPT Forza: MRC, DINAMOMETRO Sensibilità: SEMMES-WEINSTEIN MONOFILAMENT EXAM (per la sensibilità superficiale), DIAPASON (per la propriocezione) Disabilità: non esiste una scala di valutazione univoca per valutare la disabilità negli studi esaminati Inoltre due studi valutano anche il p-ROM e l’ a-ROM delle articolazioni IF delle dita secondo la descrizione di Kapandji e determinano l’entità delle dita in griffe con una scala a 4 punti. Per quanto riguarda le altre patologie prese in considerazione (DM e CIDP) gli studi presenti in letteratura oltre ad essere di numero esiguo non presentano un’omogeneità nelle scale di valutazione utilizzate MRC scale Conclusioni • Attualmente solo le neuropatie ereditarie presentano un numero significativo di studi che analizzano la disabilità legata ai disturbi dell’arto superiore. • Sono presenti delle scale di valutazione per la destrezza e la forza che sembrano essere adatte alla valutazione di questi pazienti, ma non sono state validate per le neuropatie. • Alcune delle scale o strumenti utilizzati negli studi (SWMF, SHFT) presentano dei tempi di esecuzione non adatti alla pratica clinica ambulatoriale. • Potrebbe essere utile, prendendo spunto da questi risultati, creare una scala di valutazione di facile applicabilità e con tempi di valutazione non superiore ai 5 minuti. • Manca una vera e propria scala validata che analizzi la disabilità di questi pazienti legata all’interessamento dell’arto superiore.
