Sie dient der Behandlung von arthrose- oder unfallbedingt zerstörten Fingermittelgelenken. • Die CapFlex PIP besteht aus einer Vielzahl von Komponenten und Zubehörteilen. • Die Auswahl der Implantate und die Vorgehensweise bei deren Einsatz liegt in der Verantwortung des Anwenders (behandelnder Arzt bzw. operierender Chirurg) und basiert 10 V. 1.1 Gebrauchsanweisung Endoprothesensystem CapFlex PIP auf der Beurteilung der jeweiligen anatomischen und funktionalen Ansprüche des Patienten. • Gebrüder Martin ist nicht verantwortlich für Komplikationen durch falsche Indikation, falsche Implantatauswahl, falsche Kombination von Systemkomponenten und Operationstechnik sowie Grenzen der Behandlungsmethode oder unzureichende Hygiene. • Alle Prothesenkomponenten, Instrumente und Zubehör müssen in einsatzbereitem Zustand verfügbar sein. • Die Implantate sind für den einmaligen Gebrauch und nicht zur Wiederverwendung konzipiert. Sofern nicht ausdrücklich vermerkt, sind die während der Implantation einzusetzenden Instrumente wiederverwendbar. • Das Produkt muss indikationsgemäß angewendet werden. • Bitte entnehmen Sie Details zur intraoperativen Vorgehensweise der Produktbroschüre des Herstellers. • In der Patientenakte ist der Einsatz des Implantats durch Eintrag der Bezeichnung sowie der REF und LOT zu dokumentieren. Nur so kann die eindeutige Rückverfolgbarkeit gewährleistet werden. • Der Prothesenpass befindet sich in der Umverpackung. 3 Eingangskontrolle • Die Lieferung sofort nach Empfang auf Vollständigkeit und Unversehrtheit überprüfen. • Eventuelle Transportschäden unverzüglich melden. • Nach Anlieferung des Produkts die Originalverpackung und das Verpackungssiegel auf Unversehrtheit prüfen. Verpackungen mit aufgebrochenem Siegel sind von der Rücknahme ausgeschlossen. V. 1.1 11 Gebrauchsanweisung Endoprothesensystem CapFlex PIP • Die sterilverpackten Implantate können ohne weitere Vorbehandlung direkt aus ihrer Sterilverpackung appliziert werden. • Sterilimplantate mit (auch versehentlich) geöffneter oder beschädigter Sterilverpackung, sowie Implantate, die aus Verpackungen mit abgelaufenem Verfallsdatum entnommen wurden, müssen als unsteril angesehen werden. • Jede Verpackung und das Produkt sind mit einer Chargenbezeichnung (LOT) gekennzeichnet. Bitte geben Sie im Fall einer Reklamation immer diese LOT und die Referenznummer (REF) an. 4 Anwender Die Regeln der ärztlichen Kunst, der Stand der Wissenschaft und die Inhalte einschlägiger wissenschaftlicher Veröffentlichungen der medizinischen Autoren müssen dem Anwender bekannt sein. Der Anwender muss Operationstechnik, Implantatsortiment sowie Instrumentarium und Zubehör kennen und beherrschen. Der Anwender trägt die Verantwortung für: • Sachgemäße Durchführung des chirurgischen Eingriffs • Zusammenstellung der Implantatkomponenten und deren sachgemäße Implantation • Vermeidung bzw. Reduzierung allgemeiner Risiken bei chirurgischen Eingriffen 12 V. 1.1 Gebrauchsanweisung Endoprothesensystem CapFlex PIP 5 Indikationen Schmerzhafte Zerstörung des proximalen InterphalangealGelenks im Rahmen von: • einer primär degenerativen, sekundären oder posttraumatischen Osteoarthrose, • einem entzündlichen rheumatischen Grundleiden mit geringer Entzündungsaktivität und guten Knochenverhältnissen. 6 Kontraindikationen WARNUNG Bei Nichtbeachtung der Kontraindikationen besteht die Möglichkeit einer schweren oder sogar tödlichen Verletzung des Patienten! • Knöcherne Missbildungen, Achsfehlstellungen oder andere Knochenverhältnisse, die den Einbau eines künstlichen Gelenks ausschließen • Zu erwartende Überbelastung des Gelenkimplantats • Seitliche Instabilität von mehr als 30° • Nicht intakte Streck- und Beugesehnen • Fingermittelgelenke ohne minimale Restbeweglichkeit • Radiologisch nicht intakte Gelenklinie, insbesondere fehlende Epikondylen • Radiologisch signifikante Zystenbildung • Radiologisch fortgeschrittene Protrusion des Gelenks • Patienten mit unzureichendem Knochenvolumen oder unzureichender Knochenqualität V. 1.1 13 Gebrauchsanweisung Endoprothesensystem CapFlex PIP • Manifeste Infektionen • Verdacht auf Materialempfindlichkeit oder Allergien gegen die verwendeten Implantatwerkstoffe • Autoimmunerkrankungen • Durchblutungsstörungen, Systemerkrankungen und Stoffwechselstörungen • Unzureichendes quantitatives bzw. qualitatives Hart- und Weichgewebe • Knochentumore im Bereich der Implantatverankerung • Begleiterkrankungen, z. B. degenerative Krankheitsprozesse mit negativem Einfluss auf die Funktion des Gelenkimplantats • Verwendung in Bereichen, die nicht angemessenen Kraftoder Gewichtsbelastungen ausgesetzt sind • Patienten, die aufgrund ihrer mentalen, neurologischen oder physischen Verfassung nicht bereit oder in der Lage sind, die Anweisungen für die postoperative Handhabung zu befolgen • Patienten mit schlechtem Allgemeinzustand im Sinne einer allgemeinen Kontraindikation (z. B. Bestrahlungspatienten) • Osteoporose oder Osteomalazie und andere schwerwiegende Schädigungen der Knochenstrukturen, die einer stabilen Verankerung der Implantatkomponenten entgegenstehen • Manifester Medikamenten-, Drogen- und / oder Alkoholmissbrauch • Im Wachstum befindliche Patienten mit offenen Epiphysen 14 V. 1.1 Gebrauchsanweisung Endoprothesensystem CapFlex PIP 7 Mögliche Nebenwirkungen und Komplikationen • Abnehmen der Knochendichte und / oder Knochennekrosen aufgrund von Belastung • Luxationen und Subluxationen • Früh- und Spätinfektionen, Oberflächen- und / oder Tiefeninfektionen • Nerven- und Gefäßverletzungen • Nervenschäden durch chirurgisches Trauma • Gefäßveränderungen • Hämatome und Wundheilungsstörungen • Periartikuläre Verkalkungen • Eingeschränkte Gelenkfunktion und Beweglichkeit • Eingeschränkte Gelenkbelastung und Gelenkschmerzen • Allergische Reaktion oder Empfindlichkeit gegen das Implantatmaterial • Wachstumseinschränkungen • Schmerzen, Beschwerden, anormale Empfindungen oder Tastbarkeit aufgrund des Implantats • Unzulängliche Heilung • Gewebeverfärbungen aufgrund von Kontakt mit Implantatmaterial • Unzureichende Knochenbildung oder verzögerte Knochenheilung, Osteoporose, Osteolyse, Osteomyelitis, gehemmte Revaskularisierung oder Infektionen, Knochennekrose können zum Lockern, Verbiegen, Reißen oder Brechen des Endoprothesensystems führen. • Nicht angemessene Kraft- oder Gewichtsbelastung kann zu Verschleiß, Bruch, Verbiegen, Lockerung oder Migration des Endoprothesensystems führen. V. 1.1 15 Gebrauchsanweisung Endoprothesensystem CapFlex PIP • Fragmente und mikroskopisch kleine Partikel können sich vom Endoprothesensystem lösen und sich vom Implantationssitus wegbewegen. Auch nach der Entfernung eines Metallimplantats kann es vorkommen, dass solche Metallpartikel im Körper verbleiben. Die langfristigen Wirkungen solcher Partikel sind derzeit nicht bekannt. Bei jedem chirurgischen Eingriff sind Komplikationen möglich, wie z. B. Infektionen, Nervenverletzungen und Schmerzen, die nicht auf das Implantat zurückzuführen sind. 8 Warnhinweise und Vorsichtsmaßnahmen WARNUNG Bei Nichtbeachtung der Warnhinweise besteht die Möglichkeit einer schweren oder sogar tödlichen Verletzung des Patienten! • Vor jedem Gebrauch das Produkt auf lockere, verbogene, gebrochene, rissige oder abgenutzte Komponenten untersuchen. • Sicherstellen, dass die Produkte weder verbogen noch anderweitig beschädigt sind. • Das Produkt niemals in beschädigtem Zustand verwenden. • Schadhafte Produkte aussondern. • Das Produkt nicht verwenden, wenn der Verdacht besteht, dass es durch Herabfallen oder falsche Handhabung beschädigt wurde. • Das Produkt darf nicht verändert oder an die anatomischen Verhältnisse ankonturiert werden (kein Verbiegen bzw. Verformen). • Aseptische Operationsbedingungen sind Voraussetzung. 16 V. 1.1 Gebrauchsanweisung Endoprothesensystem CapFlex PIP • Implantate, die einmal entfernt wurden, dürfen nicht wieder eingesetzt werden. Auch wenn das Implantat intakt scheint, können kleine Defekte und innere Schwachstellen vorhanden sein, die zum Versagen oder Brechen des Implantats führen können. • Wie bei jedem Implantat besteht die Gefahr einer Verschmutzung durch Fremdkörper und Grobpartikel wie z. B. Handschuhtalkum, Fussel von Abdecktüchern, Reinigungsmittel und sonstige Oberflächenverunreinigungen. Das Implantat daher so wenig wie möglich berühren. Ein Implantat, das mit Körperflüssigkeiten in Kontakt gekommen ist, darf nicht wieder verwendet werden! • Aus Metallimplantaten freigesetztes Chrom oder Kobalt kann allergische Reaktionen auslösen. Diese äußern sich unter anderem durch Ekzeme, Wundheilungsstörungen, Urtikaria oder persistierende Schwellung. • Endoprothesensysteme von Gebrüder Martin dürfen nicht mit Systemen anderer Hersteller kombiniert werden. Werden Instrumente verwendet, die nicht spezifisch für den Gebrauch in Verbindung mit den Implantaten vorgesehen sind, kann es zu einem Versagen des Endoprothesensystems kommen. V. 1.1 17 Gebrauchsanweisung Endoprothesensystem CapFlex PIP HINWEIS Mögliche Fehlinterpretation der Untersuchung! Das Endoprothesensystem kann sowohl bei der Computertomografie (CT) als auch bei der Magnet-Resonanz-Tomografie (MRT) störende Artefakte verursachen. WARNUNG Gefahr der Verbrennung bzw. unbeabsichtigten Bewegung des Implantats beim Einsatz von MRT! Aufgrund der Weiterentwicklung und weiter steigenden Energiedichte der MRT-Geräte ist ein negativer Einfluss auf Implantate zukünftig nicht auszuschließen. Im Zweifelsfall ist eine MRT-Untersuchung vor dem Ausschluss einer möglichen Patientenschädigung nicht zulässig. • Bei der MRT besteht die Gefahr des Erhitzens und der Bewegung von möglicherweise auf das Magnetfeld reagierenden Implantaten. • Stahlimplantate nach ISO 5832-12 werden als para- oder unmagnetisch eingestuft. • Titan ist vollständig unmagnetisch. 18 V. 1.1 Gebrauchsanweisung Endoprothesensystem CapFlex PIP 9 Aufbau und Beschreibung des Systems Prothesen Die CapFlex PIP besteht aus einer proximalen und einer distalen Komponente. Alle Prothesenkomponenten sind in unterschiedlichen Größen verfügbar: Beschreibung REF CapFlex proximal, S 26-100-01-04 CapFlex proximal, M 26-100-02-04 CapFlex proximal, L 26-100-03-04 CapFlex distal, S, Höhe 2,1 mm 26-100-11-04 CapFlex distal, S, Höhe 3,0 mm 26-100-12-04 CapFlex distal, S, Höhe 4,4 mm 26-100-13-04 CapFlex distal, M, Höhe 2,1 mm 26-100-21-04 CapFlex distal, M, Höhe 3,0 mm 26-100-22-04 CapFlex distal, M, Höhe 4,4 mm 26-100-23-04 CapFlex distal, L, Höhe 2,1 mm 26-100-31-04 CapFlex distal, L, Höhe 3,0 mm 26-100-32-04 CapFlex distal, L, Höhe 4,4 mm 26-100-33-04 Die proximale Komponente besteht aus Kobalt-Chrom (ISO 5832-12 / ASTM F1537) mit einer Beschichtung aus Reintitan (ISO/FDIS 13179-1). Die distale Komponente besteht ebenfalls aus Kobalt-Chrom mit einer Reintitanbeschichtung und verfügt über eine Gelenkfläche aus ultrahochmolekulargewichtigem Polyethylen (ISO 5834-2 / ASTM F648). Die proximale Komponente ist so klein wie möglich zu wählen, um Weichteilirritationen zu verhindern. V. 1.1 19 Gebrauchsanweisung Endoprothesensystem CapFlex PIP Die distale Komponente ist so groß wie möglich zu wählen, um dem Einsinken des Implantats vorzubeugen. Um den Bandapparat individuell auf die biomechanisch korrekte Spannung einstellen zu können, wird die distale Komponente in drei unterschiedlichen Höhen angeboten (die Dicke des Polyethylen-Einsatzes variiert). Kombinationsmöglichkeiten der Komponenten: CapFlex PIP proximal S proximal M proximal L distal S Höhe 2,1 mm Höhe 3,0 mm Höhe 4,4 mm distal M Höhe 2,1 mm Höhe 3,0 mm Höhe 4,4 mm distal L Höhe 2,1 mm Höhe 3,0 mm Höhe 4,4 mm Alle Prothesen sind ausschließlich für den einmaligen Gebrauch bestimmt. 20 V. 1.1 Gebrauchsanweisung Endoprothesensystem CapFlex PIP Probeprothesen Die Probeprothesen sind aus Titan (ISO 5832-3 / ASTM F136) gefertigt und dienen zur Prüfung des richtigen Sitzes der Prothese und zur Bestimmung der später zu verwendenden definitiven Prothese. Für eine vereinfachte Handhabung und den Ausschluss einer eventuell möglichen Verwechslungsgefahr sind alle Probeprothesen größenabhängig farbcodiert: Größe Farbcode S Gelb M Blau L Rosa Instrumente Für die Implantation der CapFlex PIP stehen spezielle Instrumente zur Verfügung, die ebenfalls größenabhängig mit Farbringen codiert sind. Zur Unterscheidung der proximalen und distalen Instrumente sind die Silikonhandgriffe unterschiedlich gekennzeichnet: Instrument Farbe Handgriff Kennzeichen Proximal Schwarz P Distal Grau D Auf der Kappe am Handgriff ist das Kennzeichen P für proximale oder D für distale Instrumente angebracht. V. 1.1 21 Gebrauchsanweisung Endoprothesensystem CapFlex PIP 10 Aufbereitung, Reinigung, Desinfektion und Sterilisation • Die Prothesenkomponenten werden einzeln in einer gammasterilisierten, entsprechend gekennzeichneten Verpackung ausgeliefert. • Die Prothesenkomponenten sind nach einem validierten Verfahren gammasterilisiert (Gammastrahlen mit mind. 25 kGy (2,5 Mrad)). • Die Prothesenkomponenten müssen in der Originalverpackung an einem geeigneten Ort gelagert werden und dürfen erst unmittelbar vor Gebrauch aus der Schutzverpackung entnommen werden. • Einmal aus der Verpackung entnommene, kontaminierte und / oder benutzte Implantatkomponenten dürfen nicht wieder verwendet werden. Sie müssen verworfen werden und dürfen nicht in das System zur Wiederaufbereitung eingebracht werden. Oberflächenbeschichtete Implantate, die mit Blut oder anderen Körperflüssigkeiten verunreinigt waren, dürfen unter keinen Umständen erneut sterilisiert werden. • Die Instrumente und Probeimplantate des Endoprothesensystems sind für eine maschinelle Aufbereitung / thermische Desinfektion geeignet. Sie können mit den Programmabläufen aufbereitet werden, die für chirurgisches Instrumentarium freigegeben sind. Für die Reinigung sind die Angaben der Hersteller der Reinigungsautomaten sowie der Hersteller der Behandlungsmittel zu beachten. Der Prozess inklusive der Beladung muss die ausreichende Entfernung von Rückständen gewährleisten. • Die erneute Sterilisation der Prothesenkomponenten ist nicht zulässig. 22 V. 1.1 Gebrauchsanweisung Endoprothesensystem CapFlex PIP WARNUNG Infektionsgefahr durch unsterile Handhabung! Unsachgemäße Sterilisation sowie unsterile Handhabung des Endoprothesensystems kann zu schweren gesundheitlichen Risiken für den Patienten führen. Die Sterilisation hat nach einem validierten Dampfsterilisationsverfahren, z. B. in einem Sterilisator gemäß EN 285:2009 und ANSI/AAMI ST 79, und validiert gemäß ISO 17665-1:2006, zu erfolgen. ANSI/AAMI ST79 empfiehlt für Dampfsterilisationszyklen mit dynamischer Entlüftung eine Mindeststerilisationszeit (Zykluszeit) von 4 Minuten bei 132 °C. Bitte die Hinweise in der Gebrauchsanweisung des Dampfsterilisators befolgen. HINWEIS Die Verantwortung für die Reinigung, Desinfektion und Sterilisation von Instrumenten und Zubehör liegt beim Betreiber / Anwender. It is used to treat proximal interphalangeal joints destroyed by arthrosis or an accident. • The CapFlex PIP consists of multiple components and attachments. • Selection of the implants and the procedure for applying them is the responsibility of the user (attending physician or surgeon) and to be based on the patient's anatomical and functional requirements. • Gebrüder Martin shall not be responsible for complications caused by wrong indications, wrong implant selection, incorrect combination of system components and surgical V. 1.1 27 Instructions for Use Endoprosthesis system CapFlex PIP technique, limitations of the treatment used, or lack of hygiene. • All prosthesis components, instruments and accessories must be available and ready for use. • The implants are intended for single use and must not be reused. If not explicitly stated otherwise, the instruments used during the implantation procedure may be reused. • The product must be used according to indication. • Please see the manufacturer's product brochures for details of the intraoperative procedure. • The use of the implant is documented in the patient file with its item description, REF and LOT. Only thus can clear traceability be guaranteed. The prosthesis passport can be found in the secondary packaging. 3 User’s Inspection • Immediately upon receipt, the goods must be checked for completeness and potential damage in transit. • Notice of any such damage must be given immediately. • Upon receipt of the product, the original packaging and its seal must be checked for integrity. Packages with a broken seal cannot be taken back. • Implants that are packed in sterile packing can be taken out of their packing and applied immediately without further pre-treatment. • Sterile implants whose sterile packaging has been opened or damaged for any reason, as well as implants taken from expired packages, must be considered non-sterile. • Each product and its packaging are marked with a specific lot number (LOT). Please always indicate this LOT together with the reference number (REF) in case of complaint. 28 V. 1.1 Instructions for Use Endoprosthesis system CapFlex PIP 4 Users Users must have adequate knowledge of the rules of medical practice, as well as the state of the art and the contents of pertinent scientific articles published by medical authors. The user must be thoroughly familiar with the surgical technique as well as with the set of implants and the instruments and accessories used. The user bears the responsibility for • the proper performance of the surgical procedure • selecting the right implant components and implanting them properly • avoiding or minimizing the general risks posed by surgical interventions 5 Indications Painful destruction of the proximal interphalangeal joint caused by: • primarily degenerative, secondary or posttraumatic osteoarthrosis, • underlying inflammatory rheumatic disease with low inflammatory activity and good bone conditions. V. 1.1 29 Instructions for Use Endoprosthesis system CapFlex PIP 6 Contraindications WARNING Non-observance of the contraindications poses a danger of serious or even lethal patient injury! • Osseous deformities, axial malalignment or other bone conditions that prevent the fixation of an artificial joint. • Anticipated overstraining of the joint implant • Lateral instability of more than 30° • Non-intact extensor and flexor tendons • Proximal interphalangeal joints without minimal residual mobility • Radiologically non-intact joint line, especially absent epicondyles • Radiologically significant cyst formation • Radiologically advanced joint protrusion • Patients with insufficient bone volume or quality • Obvious infections • Suspected sensitivity or allergies to the implant materials used • Autoimmune diseases • Circulatory problems, systemic diseases and metabolic disorders • Insufficient or inadequate hard and soft tissue • Bone tumors located in the implant base region • Concomitant diseases, e.g. degenerative disease processes with negative influence on the function of the joint implant 30 V. 1.1 Instructions for Use Endoprosthesis system CapFlex PIP • Regions exposed to inadequately high forces or excessive weight loads • Patients unwilling or unable to follow instructions during the postoperative phase due to their mental, neurological or physical condition • Patients in a poor condition of health in the sense of a general contraindication (e.g. irradiation patients) • Osteoporosis or osteomalacia or other severe structural bone damage that prevents the stable fixation of implant components • Obvious drug or alcohol abuse • Patients still growing, with open epiphyses 7 Potential Side Effects and Complications • Reduced bone density and/or bone necroses due to mechanical stress • Dislocations and partial dislocations • Early and late infections, surface and/or deep infections • Nerve and vascular injuries • Nerve damage due to surgical trauma • Vascular changes • Hematomas and slow wound healing • Periarticular calcification • Reduced joint function and mobility • Reduced joint load-bearing capacity; joint pain • Allergic response or sensitivity to the implant material • Growth restriction V. 1.1 31 Instructions for Use Endoprosthesis system CapFlex PIP • Pain, discomfort, anomalous sensations caused by the implant; implant palpability • Poor healing • Tissue discoloration due to contact with the implant material • Insufficient bone formation or delayed bone healing, osteoporosis, osteolysis, osteomyelitis, inhibited revascularization, infections or bone necrosis can result in loosening, bending, tearing or breaking of the implant systems. • Inappropriate force or weight loading can lead to wear, breaking, bending, loosening or migration of the endoprosthesis systems. • Fragments and microscopically small particles can detach from the endoprosthetic system and drift away from the implantation site. Metal particles may remain in the patient’s body even after removal of the implant. The longterm effects of such particles are presently unknown. Complications such as infections, nerve damage and pain that are unrelated to the implant can occur in any surgical procedure. 8 Warnings and Safety Instructions WARNING Failure to observe these warnings could result in severe or even lethal injury to the patient! • Before every use, the product must be examined for loose, bent, broken, cracked or worn components. • Make sure that the products are not bent or otherwise damaged. 32 V. 1.1 Instructions for Use Endoprosthesis system CapFlex PIP • Never use the product if it is damaged. • Faulty products must be discarded! • Never use the product when there is suspected damage due to dropping or incorrect handling. • The product must not be altered or adapted to the anatomical situation (no bending or distortion). • Operating conditions must be aseptic. • Once removed, implants may not be reused. Even if the implant seems intact, it may still feature small defects and internal weak points, which may lead to failure or fracture of the implant. • As with any implant, there is a risk of contamination by foreign matter and coarse particles, such as glove talc, lint from drapes, cleaning agents or other surface impurities. The implant should therefore be touched as little as possible. Once an implant has come into contact with body fluids, it must not be reused. • Chromium or cobalt released from metal implants can cause allergic reactions. They can manifest, among other things, as eczemata, wound healing dysfunction, urticaria or persistent swelling. • Endoprosthetic systems by Gebrüder Martin must not be combined with systems by other manufacturers. Use of instruments that are not specifically intended for use in connection with the implants can lead to failure of the endoprosthesis system. V. 1.1 33 Instructions for Use Endoprosthesis system CapFlex PIP NOTICE Potential misinterpretation of examination results! The endoprosthesis system can cause disturbing artifacts in both computer tomography (CT) and magnet resonance imaging (MRI). WARNING Danger of burns or accidental implant movement when using MRIs! Due to the further development and increasingly higher energy density of MRI systems, an adverse effect on implants cannot be ruled out in the future. Therefore, MRIs are not permitted unless potential patient injury can definitely be ruled out. • Due to the magnetic field, MRIs pose a danger of heating up or dislocating potentially susceptible implants. • Steel implants acc. to ISO 5832-12 are classified as paramagnetic or non-magnetic. • Titanium is completely non-magnetic. 34 V. 1.1 Instructions for Use Endoprosthesis system CapFlex PIP 9 Design and Description of the System Prostheses The CapFlex PIP consists of one proximal and one distal component. The distal component is also made of cobalt-chromium with a pure-titanium coating, and it has a joint surface made of ultra-high molecular weight polyethylene (ISO 5834-2/ASTM F648). V. 1.1 35 Instructions for Use Endoprosthesis system CapFlex PIP The proximal component must be as small as possible to prevent soft tissue irritation. The distal component should be as large as possible to prevent the implant from sinking in. The distal component is offered in three different heights (the thickness of the polyethylene insert varies) to allow individual adjustment of the ligamentous apparatus to the correct biomechanical tension. For more comfortable use and to avoid any danger of mix-ups, all test prostheses are color-coded according to their size: Size Color Code S Yellow M Blue L Pink Instruments There are special instruments available for implanting the CapFlex PIP that are also color-coded according to their size. V. 1.1 37 Instructions for Use Endoprosthesis system CapFlex PIP 10 Processing, Cleaning, Disinfection and Sterilization • The prosthesis components are delivered separately in gamma-sterilized packagings labeled accordingly. • The prosthesis components have been gamma-sterilized in accordance with a validated procedure (gamma rays of at least 25 kGy (2.5 mrad)). • The prosthesis components must be stored in their original packing and in a suitable location and may be removed from their protective packing only immediately before use. • Implant components taken out of their packing that have been contaminated and/or used must not be re-used. Such components must be discarded and may not be fed into the reprocessing system. Surface-coated implants that have been contaminated with blood or other bodily fluids may not be re-sterilized under any circumstances. • The instruments and test prostheses of the endoprosthesis system can be reprocessed by machine/disinfected thermally. They can be processed with the same programs that have been released for surgical instruments. As regards cleaning, be sure to follow the instructions provided by the manufacturer of your washer-disinfector(s) as well as those provided by the manufacturers of the cleaners and disinfectants used. The process (including loading) must guarantee sufficient removal of residues. • Re-sterilization of prosthesis components is not allowed. 38 V. 1.1 Instructions for Use Endoprosthesis system CapFlex PIP WARNING Infection Risk due to non-sterile handling! Improper sterilization and non-sterile handling of the endoprosthesis system can entail severe health risks for the patient. Sterilization must be carried out according to a validated steam sterilization process, for example in a sterilizer complying with EN 285:2009 and ANSI/AAMI ST 79, validated in accordance with ISO 17665-1:2006. ANSI/AAMI ST 79 recommends a minimum cycle time of 4 minutes at 132°C (270°F) for dynamic air removal steam sterilization cycles. Please follow the instructions of the user manual of your steam sterilizer. NOTICE The operator/user is responsible for cleaning, disinfecting and sterilizing the instruments and accessories. Be sure to observe your national/local regulations, including potential restrictions. Use appropriate and approved sterile packaging (e.g. in accordance with EN 868, ISO 11607) for sterilization, subsequent transportation and storage. V. 1.1 39 Instructions for Use Endoprosthesis system CapFlex PIP 11 Prior to Implantation • All implant components of the CapFlex PIP have to be available and ready for use. • Highly aseptic operating conditions • Implantation instruments, including special joint implant instruments from Gebrüder Martin for joint position preparation, are available and in technically perfect condition. • The surgeon and surgical team are familiar with the information on the surgical technique and the implant range and instruments, and this information is fully available on site. • In the event of an unclear preoperative situation with regard to the implants and the joint bed, information has been sought from the manufacturer. • The procedure has been explained to the patient and their consent to the following has been documented in writing in the patient record: — The function of the artificial replacement joint is significantly inferior to that of a natural joint. — The artificial replacement joint can only achieve a relative improvement over the preoperative state. — The artificial replacement joint can become loose through overloading, overstraining, wear or infection. — The life of the implant depends on the loads to which the joint is subjected. — The patient should avoid overstraining caused by their work and leisure activities. — In the event of loosening, revision surgery may be required. 40 V. 1.1 Instructions for Use Endoprosthesis system CapFlex PIP — Revision surgery may not be able to restore acceptable joint function. — The patient agrees to undergo regular medical follow-up examinations. Preparation of the implant bed • Do not remove any bone or soft tissue structures that could be used for fixation. • The appropriate trial implants are used for checking the implant bed. • It must be ensured that the implant position enables free joint mobility; dislocations and partial dislocations must be ruled out. • The implant surfaces must not be damaged under any circumstances. • Before the wound is closed, the correct position of the implants should be checked under X-ray control. 12 Disposal Be sure to observe all national/local regulations and guidelines when disposing of the packaging material and potentially infectious items (e.g. removed metal components). Salvo que se especifique expresamente lo contrario, los instrumentos que deban utilizarse durante la implantación son reutilizables. • El producto deberá utilizarse conforme a las indicaciones. • Consulte los detalles sobre el procedimiento intraoperatorio en la documentación del producto del fabricante. • El uso del implante se documenta en la ficha del paciente registrando la designación, el número REF y la designación LOT. La documentación de la prótesis se encuentra en el embalaje exterior. 3 Control de entrada • En el momento del suministro deberá comprobar que éste esté completo e íntegro. • Posibles daños de transporte deberán notificarse sin dilación. • Después de la recepción del producto deberá controlarse que el embalaje original y el precinto del mismo no estén dañados.Los embalajes que tengan el precinto roto no podrán retornarse. 44 V. 1.1 Instrucciones de uso Sistema de endoprótesis CapFlex PIP • Los implantes embalados de forma estéril pueden emplearse directamente y sin tratamiento previo tras extraerlos del embalaje. • Los implantes estériles que presenten un envase estéril abierto (también involuntariamente) o dañado, así como aquellos que hayan sido extraídos de envases cuya fecha de caducidad haya sido rebasada, deberán considerarse no estériles. • Cada embalaje está identificado con una designación del lote (LOT). En el caso de una reclamación, rogamos que siempre nos indiquen esta LOT junto con el número de referencia (REF). 4 Usuarios El usuario deberá conocer las reglas de la práctica médica, así como el estado de la ciencia y los contenidos de publicaciones científicas pertinentes de autores médicos sobre la intervención. El usuario debe conocer y dominar la técnica quirúrgica, el surtido de implantes, así como el instrumental y los accesorios. El usuario es responsable de: • realizar la intervención quirúrgica conforme a las reglas del arte; • de la combinación de los componentes del implante y de su implantación conforme a las prescripciones; • la prevención o la reducción de los riesgos generales inherentes a las intervenciones quirúrgicas. V. 1.1 45 Instrucciones de uso Sistema de endoprótesis CapFlex PIP 5 Indicaciones Destrucción dolorosa de la articulación interfalángica proximal asociada a: • Artrosis primaria degenerativa, artrosis secundaria o artrosis postraumática. • Enfermedad inflamatoria reumática subyacente con reducida actividad inflamatoria y buenas condiciones óseas. 6 Contraindicaciones ADVERTENCIA ¡En caso de no tener en cuenta las contraindicaciones existe la posibilidad de una lesión grave o incluso mortal para el paciente! • Malformaciones óseas, malposiciones axiales u otras condiciones óseas que descarten la incorporación de una articulación artificial. • Previsión de una sobrecarga del implante de la articulación. • Inestabilidad lateral superior a 30° • Tendones extensores y flexores no intactos • Articulaciones interfalángicas proximales sin movilidad residual mínima • Línea articular no intacta observada radiológicamente y, en especial, falta de epicóndilos • Formación significativa de quistes observada radiológicamente • Protrusión avanzada de la articulación observada radiológicamente • Pacientes con volumen óseo o calidad ósea insuficientes 46 V. 1.1 Instrucciones de uso Sistema de endoprótesis CapFlex PIP • Infecciones manifiestas • Sospecha de sensibilidad o alergias contra los materiales utilizados en la fabricación de los implantes • Enfermedades autoinmunes • Trastornos circulatorios, enfermedades sistémicas y anomalías metabólicas • Insuficiencia cuantitativa o cualitativa de tejidos compactos o blandos • Tumores óseos en la zona de anclaje del implante • Enfermedades concomitantes, como p. ej. procesos patológicos degenerativos que influyan negativamente sobre la función del implante articular • Utilización en sectores que no están expuestos a cargas de fuerza o de peso adecuados • Pacientes que, debido a su estado mental, neurológico o físico, no estén dispuestos o no sean capaces de seguir las indicaciones para el manejo postoperatorio • Pacientes con estado general deficiente en el sentido de una contraindicación generalizada (p. ej. pacientes sometidos a radioterapia) • Osteoporosis u osteomalacosis y otros deterioros graves de las estructuras óseas, que evitan un anclaje estable de los componentes del implante • Abuso manifiesto de medicamentos, drogas y/o alcohol • Pacientes en edad de crecimiento con epífisis abierta V. 1.1 47 Instrucciones de uso Sistema de endoprótesis CapFlex PIP 7 Posibles efectos secundarios y complicaciones • Reducción de la densidad ósea y/o osteonecrosis debido a cargas. • Luxaciones y subluxaciones. • Infecciones primarias y secundarias, infecciones superficiales y/o profundas. • Traumatismos nerviosos y vasculares. • Lesiones nerviosas por traumatismos quirúrgicos. • Modificaciones vasculares. • Hematomas y alteraciones en la cicatrización de heridas. • Calcificaciones periarticulares. • Función articular y motilidad limitada. • Carga articular limitada y dolores articulares. • Reacción alérgica o sensibilidad frente al material del implante. • Limitación del crecimiento. • Dolores, molestias, sensaciones o palpabilidad anómala debidos al implante. • Curación insuficiente. • Alteraciones cromáticas del tejido debido al contacto con el material del implante. • La formación ósea insuficiente o la curación ósea retrasada, la osteoporosis, la osteólisis, la osteomielitis, la revascularización inhibida, las infecciones y la necrosis ósea pueden provocar el aflojamiento, la deformación, la fisura o la rotura del sistema de endoprótesis. • Una carga de fuerza o de peso desproporcionada puede provocar el desgaste, la rotura, la deformación, el aflojamiento o la migración del sistema de endoprótesis. 48 V. 1.1 Instrucciones de uso Sistema de endoprótesis CapFlex PIP • Es posible que se desprendan fragmentos y pequeñas partículas microscópicas del sistema de endoprótesis y que se desplacen fuera del sitio del implante. Esta es la razón por la que pueden permanecer partículas metálicas dentro del cuerpo incluso después de haber eliminado el implante metálico. Hasta el momento se desconocen los efectos a largo plazo de estas partículas. En toda intervención quirúrgica cabe la posibilidad de que se produzcan complicaciones, como, p. ej., infecciones, lesiones en los nervios y dolor, que no son atribuibles al implante. 8 Avisos y medidas de precaución ADVERTENCIA ¡Si no se tienen en cuenta estas advertencias, pueden producirse daños graves o incluso la muerte del paciente! • • Antes de su utilización, asegúrese de que los componentes no estén sueltos, torcidos, rotos, agrietados ni gastados. Asegúrese de que los productos no estén doblados ni sufran ningún otro tipo de daño. • Bajo ningún concepto, no utilice el producto si está dañado. • Los productos dañados deben eliminarse. • No debe utilizarse el producto si se sospecha la presencia de daños por una caída o una manipulación incorrecta. • El producto no se debe modificar ni contornear según las condiciones anatómicas del paciente (no doblar ni deformar). La intervención quirúrgica debe realizarse en condiciones asépticas. • V. 1.1 49 Instrucciones de uso Sistema de endoprótesis CapFlex PIP • Los implantes que se hayan extraído no deben volverse a emplear. Aunque el implante parezca estar intacto, es posible que presente pequeños defectos y puntos débiles internos que podrían comportar el fallo o la rotura del implante. • Como en todo implante, existe el peligro de ensuciamiento por cuerpos extraños y partículas de mayor tamaño como, por ejemplo, talco para guantes, pelusas de paños quirúrgicos, productos de limpieza y otros tipos de impurezas superficiales. Por ese motivo, el implante debe tocarse lo menos posible. No debe reutilizarse un implante que haya estado en contacto con líquidos corporales. • El cromo o el cobalto desprendido de los implantes metálicos puede causar reacciones alérgicas que se manifiestan, entre otras formas, mediante eccemas, trastornos en la cicatrización, urticaria o hinchazón persistente. • Los sistemas de endoprótesis de Gebrüder Martin no deben combinarse con sistemas de otros fabricantes. Si se utilizan instrumentos que no están previstos específicamente para su uso en combinación con los implantes, puede producirse un fallo del sistema de endoprótesis. 50 V. 1.1 Instrucciones de uso Sistema de endoprótesis CapFlex PIP INDICACIÓN ¡Posible interpretación errónea de la exploración! El sistema de endoprótesis puede causar artefactos perturbadores tanto en la tomografía computarizada (TC) como en la tomografía por resonancia magnética (TRM). ADVERTENCIA ¡Peligro de quemaduras o desplazamiento involuntario del implante en el caso de utilizar la TRM! Debido al desarrollo ulterior y a la densidad energética creciente de los equipos para la TRM, en el futuro no puede descartarse una influencia negativa sobre los implantes. Una TRM sólo será admisible en el caso de que pueda excluirse definitivamente cualquier lesión del paciente. • En la TRM existe el peligro de calentamiento y de desplazamiento de los implantes por influencia del campo magnético generado. • Los implantes de acero conforme a la norma ISO 5832-12 están catalogados como paramagnéticos o antimagnéticos. • El titanio es totalmente antimagnético. V. 1.1 51 Instrucciones de uso Sistema de endoprótesis CapFlex PIP 9 Diseño y funcionamiento del sistema Prótesis CapFlex PIP consta de un componente proximal y uno distal. El componente proximal debe ser del menor tamaño posible para evitar la irritación de los tejidos blandos. 52 V. 1.1 Instrucciones de uso Sistema de endoprótesis CapFlex PIP El componente distal a seleccionar, en tanto, será del mayor tamaño posible para evitar el hundimiento del implante. Para poder adaptar de forma individual el aparato ligamentoso a la tensión biomecánica correcta, el componente distal está disponible en tres alturas distintas (el espesor del inserto de polietileno varía). V. 1.1 53 Instrucciones de uso Sistema de endoprótesis CapFlex PIP Prótesis de prueba Las prótesis de prueba están hechas de titanio (ISO 5832-3/ ASTM F136) y sirven para comprobar el asiento correcto de la prótesis y para determinar la prótesis definitiva que se utilizará posteriormente. Para permitir un manejo más sencillo y descartar cualquier riesgo de confusión, todas las prótesis de prueba cuentan con un código cromático en función del tamaño: Tamaño Código cromático S Amarillo M Azul L Rosa Instrumental Para la implantación de CapFlex PIP, se dispone de instrumentos especiales que también cuentan con anillos de colores distintos según su tamaño. Para diferenciar los instrumentos proximales y distales, los mangos de silicona están marcados de forma distinta: Tipo de instrumento Color del mango Código Proximal Negro P Distal Gris D En la tapa del mango se especifica una P para los instrumentos proximales o una D para los instrumentos distales. 54 V. 1.1 Instrucciones de uso Sistema de endoprótesis CapFlex PIP 10 Reprocesado, limpieza, desinfección y esterilización • Los componentes protésicos se suministran individualmente en un embalaje esterilizado con rayos gamma y con la identificación correspondiente. • Los componentes protésicos están esterilizados mediante un proceso validado de rayos gamma (rayos gamma con un mínimo de 25 kGy [2,5 Mrad]). • Los componentes protésicos deben almacenarse en su embalaje original en un lugar adecuado, y solo deben extraerse del embalaje de protección inmediatamente antes de su uso. • Los componentes del implante extraídos del embalaje, contaminados y/o utilizados no deben reutilizarse. Deben desecharse y no deben introducirse en el sistema para su reprocesamiento. Los implantes con superficie revestida que se hayan ensuciado con sangre u otros fluidos corporales no deben reesterilizarse bajo ningún concepto. • Los instrumentos e implantes de prueba del sistema de endoprótesis son aptos para el procesamiento mecánico/la desinfección térmica. Pueden ser reprocesados con las secuencias de programa homologadas para el instrumental quirúrgico. Para la limpieza deberán observarse las instrucciones del fabricante de la máquina de lavado, así como del fabricante de los productos de tratamiento utilizados. El proceso, incluyendo la carga, debe asegurar la eliminación suficiente de suciedad e incrustaciones. • No se permite la reesterilización de los componentes protésicos. V. 1.1 55 Instrucciones de uso Sistema de endoprótesis CapFlex PIP ADVERTENCIA ¡Peligro de infección por manipulación no estéril! Una esterilización inadecuada, así como un manejo no estéril del sistema de endoprótesis, pueden provocar riesgos graves para la salud del paciente. La esterilización deberá realizarse mediante un proceso de esterilización por vapor de agua validado, p. ej. en un esterilizador conforme a la norma EN 285:2009 y ANSI/AAMI ST 79, y validado conforme a la norma ISO 17665-1:2006. ANSI/AAMI ST79 recomienda un tiempo mínimo de 4 minutos a 132 °C para ciclos de esterilización por vapor con eliminación dinámica del aire. Rogamos que tenga en cuenta las indicaciones dadas en las instrucciones de uso de su esterilizador por vapor. INDICACIÓN La responsabilidad sobre la limpieza, desinfección y esterilización de los instrumentos y accesorios recae sobre el operador/usuario. En este sentido es prescriptivo que se observen las regulaciones nacionales y también las restricciones respectivas. Para la esterilización, el consiguiente transporte y el almacenamiento se tienen que utilizar los embalajes de esterilización correspondientes y aprobados (por ejemplo, conforme a las normas EN 868, ISO 11607). 56 V. 1.1 Instrucciones de uso Sistema de endoprótesis CapFlex PIP 11 • • • • • • Antes de la implantación Todos los componentes implantables del sistema CapFlex PIP están disponibles ya listos para su uso. La intervención quirúrgica debe realizarse en condiciones totalmente asépticas. Los instrumentos del implante de Gebrüder Martin, incluido el instrumental del implante de la articulación, para la preparación de la zona donde se colocará la articulación están en un estado técnico perfecto y listos para utilizarse. El cirujano y el equipo de quirófano conocen la información relativa a la técnica quirúrgica, a los tipos de implante y al instrumental necesario, y todo ello está disponible en el quirófano. Si se da una situación previa a la intervención confusa con relación al implante y la zona de colocación de la articulación, se solicita información al fabricante. Debe proporcionarse información clara al paciente sobre la intervención. En la ficha del paciente, debe constar por escrito su consentimiento respecto a la información siguiente: — Por regla general, la sustitución artificial de la articulación no tiene la misma funcionalidad que la articulación natural. — La sustitución artificial de la articulación solo puede mejorar relativamente el estado de la articulación antes de la intervención. — La sustitución artificial de la articulación puede aflojarse por sobrecarga, utilización excesiva, desgaste o infecciones. — La vida útil del implante depende de la carga articular. — El paciente debe evitar las sobrecargas debido al trabajo y a las actividades en el tiempo libre. V. 1.1 57 Instrucciones de uso Sistema de endoprótesis CapFlex PIP — En caso de aflojamiento, puede que sea necesaria una intervención de revisión. — En algunos casos, durante una revisión la función articular no puede recuperar su funcionamiento hasta un nivel aceptable. — El paciente declara estar dispuesto a someterse a controles médicos periódicos después de la intervención. Elle sert au traitement des articulations centrales des doigts détruites par l'arthrose ou suite à un accident. • Le CapFlex PIP comprend de nombreux composants et accessoires. • Le choix des implants ainsi que l'utilisation des ces derniers incombent à l'utilisateur (médecin traitant ou chirurgien) et reposent sur l'évaluation des exigences fonctionnelles et anatomiques du patient. 60 V. 1.1 Mode d’emploi Système endoprothétique CapFlex PIP • Gebrüder Martin ne sera pas responsable de complications résultant d’une indication erronée, du choix des mauvais implants, d’une mauvaise combinaison de composants du système et d’une technique opératoire non adaptée, ainsi que des limites de la méthode de traitement ou d’une hygiène insuffisante. • Tous les composants prothétiques, les instruments ainsi que les accessoires doivent être prêts à l'emploi. • Les implants sont prévus pour un usage unique et ne sont pas conçus pour être réutilisés. Sauf stipulation expresse contraire, les instruments nécessaires à l'implantation sont réutilisables. • Le produit devra être utilisé conformément aux indications. • Veuillez consulter les détails relatifs à la procédure intraopératoire dans la documentation relative au produit du fabricant. • Outre les REF et LOT, la désignation de l’implant utilisé est à consigner dans le dossier du patient. Ce n'est qu'ainsi qu'une traçabilité claire peut être garantie. La certificat de la prothèse se trouve dans l'emballage. 3 Contrôle de réception • Dès réception, vérifiez que la livraison est complète et en parfait état. • D’éventuels dommages de transport devront être signalés immédiatement. • Après la livraison du produit, contrôler que l’emballage original et le sceau de l’emballage sont intacts. Les emballages dont le sceau est endommagé ne seront pas repris. • Les implants conditionnés dans un emballage stérile peuvent être appliqués directement et ne nécessitent aucun pré-traitement supplémentaire. V. 1.1 61 Mode d’emploi Système endoprothétique CapFlex PIP • Les implants stériles dont l’emballage stérile est ouvert (même par inadvertance) ou endommagé, ainsi que ceux sortis d’emballages dont la date limite d’utilisation est expirée devront être considérés comme non stériles. • Chaque emballage et le produit portent un numéro de lot (LOT). En cas de réclamation, nous vous saurions gré de bien vouloir indiquer ce numéro LOT et la référence (REF). 4 Utilisateur L’utilisateur devra avoir connaissance des règles de l’art médical, de l’état actuel de la science et des contenus de publications scientifiques afférentes d’auteurs médicaux. L’utilisateur est tenu de connaître la technique opératoire, la gamme d’implants et les instruments et de les maîtriser. L’utilisateur est responsable : • De la réalisation conforme de l’intervention chirurgicale • De l’assemblage des composants d’implants et de leur implantation conforme • Et est tenu d’éviter ou de réduire les risques généraux liés aux interventions chirurgicales 62 V. 1.1 Mode d’emploi Système endoprothétique CapFlex PIP 5 Indications Destruction douloureuse de l'articulation interphalangienne proximale suite à : • une ostéoarthrose dégénérative primaire, secondaire ou post-traumatique. • une affection de base rhumatismale inflammatoire avec faible activité inflammatoire et de bonnes conditions osseuses. 6 Contre-indications Un non-respect des contre-indications peut entraîner des blessures graves, voire mortelles, chez le patient ! • Malformations osseuses, déviations de l'axe ou autres troubles osseux interdisant toute insertion d'une articulation artificielle • Sollicitation trop importante prévisible de l'implant d'articulation • Instabilité latérale de plus de 30° • Tendons extenseurs et fléchisseurs non intacts • Articulations centrales des doigts sans mobilité résiduelle minimale • Ligne d'articulation non intacte sur un plan radiologique, en particulier, absence d'épicondyles • Formation de kystes significative sur un plan radiologique • Protusion avancée de l'articulation sur un plan radiologique • Patients présentant un volume osseux insuffisant ou une qualité osseuse insuffisante V. 1.1 63 Mode d’emploi Système endoprothétique CapFlex PIP • Infections manifestes • Soupçon de sensibilité ou allergies connues aux matériaux de l’implant • Maladies auto-immunes • Troubles de vascularisation, maladies systémiques et troubles du métabolisme • Tissu dur et mou en quantité et de qualité insuffisantes • Tumeurs osseuses dans la région d’ancrage de l’implant • Maladies concomitantes, comme p. ex. les processus dégénératifs exerçant une influence négative sur la fonction de l'implant d'articulation • Utilisation dans des régions soumises à des poids ou efforts excessifs • Patients qui, du fait de leur état mental, neurologique ou physique, ne sont pas prêts à ou ne sont pas en mesure de suivre les instructions données pendant le suivi postopératoire • Patients dans un mauvais état général au sens d’une contre-indication générale (par ex. patients subissant une radiothérapie) • Ostéoporose ou ostéomalacie et autres lésions graves des structures osseuses qui s’opposent à un ancrage stable des composants de l’implant • Abus manifeste de médicaments, de drogues et / ou d’alcool • Patients en pleine croissance présentant des épiphyses ouvertes 64 V. 1.1 Mode d’emploi Système endoprothétique CapFlex PIP 7 Effets secondaires et complications possibles • Diminution de la densité osseuse et / ou nécroses osseuses dues à des charges • Luxations et subluxations • Infections précoces et tardives, infections superficielles et / ou profondes • Lésions nerveuses et vasculaires • Lésions nerveuses dues à un traumatisme chirurgical • Modifications des vaisseaux • Hématomes et troubles de la cicatrisation • Calcifications péri-articulaires • Fonction articulaire et mobilité limitées • Charge articulaire restreinte et douleurs articulaires • Réaction allergique ou sensibilité aux matériaux de l’implant • Restriction de la croissance • Douleurs, inconfort, sensations anormales ou implant palpable • Cicatrisation insuffisante • Décoloration des tissus due au contact avec le matériau de l’implant • Formation osseuse insuffisante ou cicatrisation osseuse ralentie, ostéoporose, ostéolyse, ostéomyélite, revascularisation ralentie ou infections, nécrose osseuse pouvant entraîner un relâchement, une déformation, une fissure ou une cassure du système endoprothétique. • Des forces ou des poids inappropriés peuvent entraîner une usure, une rupture, une déformation, un relâchement ou une migration du système endoprothétique. V. 1.1 65 Mode d’emploi Système endoprothétique CapFlex PIP • Des fragments et particules microscopiques peuvent se détacher du système endoprothétique et migrer du site d'implantation. Même après avoir enlevé l’implant, ces particules peuvent encore rester dans le corps. Leurs effets à long terme ne sont pas connus à ce jour. Des complications sont possibles à chaque intervention chirurgicale, comme par ex. des infections, des lésions nerveuses ou des douleurs qui ne sont pas dues à l'implant. 8 Consignes de mise en garde et mesures de précaution Le non-respect des mises en garde peut entraîner des blessures graves, voire la mort, pour le patient ou l'utilisateur ! • Avant toute utilisation, vérifier que le produit ne comporte aucun composant disloqué, tordu, cassé, fissuré ou usé. • Vérifiez que le produit n'est ni tordu ni abîmé d'aucune autre façon. • N'utilisez jamais un produit abîmé. • Les produits endommagés doivent être éliminés ! • Ne pas utiliser le produit en cas de présomption de dommages dus à une chute ou une mauvaise manipulation. • Le produit ne doit subir aucune modification ou être adapté aux conditions anatomiques (aucune torsion ou déformation). Des conditions opératoires aseptiques conformes sont impératives. • 66 V. 1.1 Mode d’emploi Système endoprothétique CapFlex PIP • Les implants qui ont été enlevés ne doivent pas être réutilisés. Même si l'implant paraît être intact, il peut comporter de petits défauts et présenter des points faibles intérieurs qui peuvent causer une défaillance ou une rupture de l'implant. • Comme pour chaque implant, des corps étrangers et des particules grossières, tels que le talc des gants, les peluches de serviettes de recouvrement, les produits nettoyants et autres saletés superficielles, risquent de salir l'implant. C'est pourquoi il convient de toucher l'implant le moins possible. Un implant qui entre en contact avec des liquides corporels ne doit plus être utilisé. • Le chrome ou le cobalt dégagé des implants métalliques peut déclencher des réactions allergiques. Celles-ci se manifestent entre autres par de l'eczéma, des troubles de la cicatrisation, de l'urticaire ou un gonflement persistant. • Les systèmes endoprothétiques de Gebrüder Martin ne doivent pas être combinés avec des systèmes d'autres fabricants. L'utilisation d'instruments n’étant pas spécifiquement conçus pour un usage en liaison avec les implants peut provoquer une défaillance du système endoprothétique. V. 1.1 67 Mode d’emploi Système endoprothétique CapFlex PIP INDICATION Interprétations erronées possibles de l’examen ! Le système endoprothétique peut être à l'origine d'artefacts perturbateurs pendant la tomodensitométrie (TDM) et la imagerie par résonance magnétique (IRM). Risque de brûlure et / ou de mouvement intempestif de l’implant lors de l’utilisation de l'IRM ! En raison du perfectionnement et de la densité énergétique croissante des appareils IRM, il est impossible d’exclure à l’avenir toute influence négative sur les implants. En cas de doute, un examen IRM ne sera autorisé que si un dommage pour le patient est définitivement exclu. • Lors de l’IRM, le champ magnétique risque de provoquer un réchauffement et un déplacement des implants susceptibles de réagir à ce champ magnétique. • Les implants en acier conformes à ISO 5832-12 sont classifiés comme paramagnétiques ou non magnétiques. • Le titane n’est absolument pas magnétique. 68 V. 1.1 Mode d’emploi Système endoprothétique CapFlex PIP 9 Structure et mode de fonctionnement du système Prothèses Le CapFlex PIP comprend un composant proximal et un composant distal. Le composant proximal choisi doit être le plus petit possible afin d'éviter toute irritation des tissus mous. Le composant V. 1.1 69 Mode d’emploi Système endoprothétique CapFlex PIP distal choisi doit être le plus grand possible afin d'éviter tout affaissement de l'implant. Pour ajuster individuellement l'appareil ligamentaire à la bonne tension biomécanique, le composant distal est proposé dans trois hauteurs différentes (l'épaisseur de l'insert en polyéthylène varie). Afin de faciliter la manipulation et d'exclure toute éventuelle interversion, toutes les prothèses d'essai ont un code couleur qui diffère en fonction de leur taille: Taille Code couleur S Jaune M Bleu L Rose Instruments Des instruments spéciaux sont disponibles pour l'implantation de PIP de CapFlex ; ceux-ci sont également dotés de bagues de couleurs différentes en fonction de leur taille. V. 1.1 71 Mode d’emploi Système endoprothétique CapFlex PIP 10 Traitement, nettoyage, désinfection et stérilisation • Chacun des composants prothétiques est fourni dans un emballage stérilisé aux rayons gamma et identifié en conséquence. • Les composants prothétiques sont stérilisés aux rayons gamma selon un procédé validé (rayons gamma d'un min. de 25 kGy (2,5 Mrad)). • Les composants prothétiques doivent être stockés dans leur emballage d'origine dans un lieu approprié et ne doivent être extraits de leur emballage de protection que peu de temps avant leur utilisation. • Les composants d'implant déjà extraits de leur emballage, contaminés et / ou utilisés ne doivent pas être réutilisés. Ils doivent être jetés et ne doivent pas être introduits dans le système en vue d'un retraitement. Les implants à surface revêtue qui ont été contaminés par du sang ou d'autres liquides corporels ne doivent en aucun cas être restérilisés. • Les instruments et implants d'essai du système endoprothétique conviennent à un traitement / à une désinfection thermique en machine. Ils peuvent être traités avec les cycles de programmes homologués pour les instruments chirurgicaux. Il convient de respecter les informations données par les fabricants des automates de nettoyage, ainsi que celles des fabricants des produits de traitement. Le processus, chargement inclus, doit garantir l’enlèvement suffisant de résidus. • Il est interdit de stériliser à nouveau les composants prothétiques. 72 V. 1.1 Mode d’emploi Système endoprothétique CapFlex PIP Risque d’infection suite à une manipulation non stérile ! Toute stérilisation réalisée de manière non conforme ainsi que toute manipulation à l'état non stérile du système endoprothétique peut entraîner de graves risques sanitaires chez le patient. La stérilisation doit être effectuée selon un procédé de stérilisation à la vapeur validé, par exemple dans un stérilisateur conforme à EN 285:2009 et ANSI / AAMI ST 79, et validé conformément à ISO 17665-1:2006. La norme ANSI / AAMI ST79 recommande pour les cycles de stérilisation à la vapeur avec retrait de l’air dynamique un temps de stérilisation minimal (durée de cycle) de 4 minutes à 132 °C (270 °F). Veuillez respecter les consignes indiquées dans le mode d’emploi de votre autoclave. INDICATION L'exploitant / l'utilisateur est responsable du nettoyage, de la désinfection et de la stérilisation des instruments et accessoires. Les réglementations nationales, y compris les limitations respectives, doivent absolument être respectées. Pour la stérilisation, le transport et le stockage, utiliser les emballages de stérilisation homologués correspondants (p. ex. selon EN 868, ISO 11607). V. 1.1 73 Mode d’emploi Système endoprothétique CapFlex PIP 11 • • • • • • Avant l’implantation Tous les composants de l'implant du système CapFlex PIP sont prêts à l'emploi. Conditions opératoires aseptiques absolues conformes Les instruments de pose, notamment les instruments de pose articulaires spéciaux de Gebrüder Martinpour la préparation de la position de l'articulation doivent être dans un état technique irréprochable. Le chirurgien et l'équipe opératoire connaissent les informations concernant la technique opératoire, la gamme d'implants, l’instrumentation de pose et s'assurent que ces informations sont à leur entière disposition sur place. En cas de doute pour une situation préopératoire en ce qui concerne les implants et la rotule, les informations du fabricant serviront de référence. Le patient a été informé sur l'intervention et a donné par écrit son accord sur les informations suivantes (documentation dans son dossier) : — La prothèse articulaire artificielle a une fonction inférieure à celle d'une articulation naturelle. — L'articulation artificielle ne peut offrir qu'une amélioration relative par rapport à l'état préopératoire. — L'articulation artificielle peut se relâcher suite à une surcharge, une surfonction, une usure ou une infection. — La durée de vie de l'implant dépend de la charge soumise à l'articulation. — Le patient doit éviter toute surcharge, dans ses activités professionnelles et dans ses loisirs. — Un relâchement peut nécessiter une opération de révision. 74 V. 1.1 Mode d’emploi Système endoprothétique CapFlex PIP — Dans le cas d'une révision, la fonction de l'articulation peut ne pas être rétablie de manière acceptable. — Le patient s'engage à suivre des contrôles médicaux postopératoires réguliers. Préparation du lit de l'implant • Ne pas enlever de structure osseuse ou tissulaire qui pourraient servir à l'ancrage ! • Les échantillons d'implants sont destinés à vérifier la rotule correspondante. • Vous devez vérifier que la position de l'implant permet une mobilité parfaite de l'articulation et exclure toute luxation et subluxation. • Les surfaces des implants doivent être en parfait état. • Avant la fermeture de l'incision, vérifier la position correcte de l'implant, le cas échéant par le biais du contrôle du convertisseur d'images. 12 Mise au rebut Lors de l’élimination de matériau d’emballage et de matériel présentant un risque d’infection potentiel (par ex. après le retrait du métal), il convient de respecter les prescriptions et directives nationales en matière d’élimination des déchets. Essa è stata messa a punto per il trattamento delle articolazioni interfalangee affette da artrosi o irrimediabilmente danneggiate a seguito di un incidente. • CapFlex PIP è costituito da numerosi componenti e accessori. • La scelta dell'impianto e la procedura di impiego ricadono nella responsabilità dell'utente stesso (medico curante o chirurgo operante) e vengono determinati dall'utente previa valutazione delle esigenze anatomiche e funzionali specifiche del singolo paziente. V. 1.1 77 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP • Gebrüder Martin non è responsabile per le avvenute complicazioni causate da indicazione erronea, scelta sbagliata dell’impianto, combinazione errata di componenti del sistema e tecnica operatoria inadeguata, nonché per i limiti del metodo terapeutico o per l’igiene insufficiente. • Tutti i componenti della protesi, gli strumenti e gli accessori devono essere disponibili pronti all'uso. • Gli impianti sono monouso e non devono essere riutilizzati. Finché non espressamente specificato, gli strumenti da impiegare durante l'impianto sono invece riutilizzabili. • Il prodotto deve essere utilizzato conformemente alle indicazioni. • I dettagli sulla procedura intraoperatoria sono allegati nella documentazione del prodotto forniti dal costruttore. • L'uso dell'impianto viene documentato nella cartella clinica del paziente registrando la designazione, il REF e il LOT. Solo in questo modo è possibile garantire una rintracciabilità univoca. Il documento dell'impianto si trova nella confezione esterna. 3 Verifica alla consegna • Al momento della ricezione della merce consigliamo di verificarne immediatamente l’integrità e di assicurarsi che non abbia subito danni. • Comunicare subito eventuali danni da trasporto. • Al ricevimento del prodotto è necessario verificare che l’imballaggio originale e il relativo sigillo siano intatti. Le confezioni con sigillo rotto non saranno riprese in consegna. • Gli impianti imballati in maniera sterile possono essere applicati soltanto dopo averli prelevati direttamente dalla confezione sterile senza ulteriore pretrattamento. 78 V. 1.1 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP • Gli impianti sterili con imballaggio sterile aperto o danneggiato (anche inavvertitamente), nonché gli impianti rimossi da imballaggi con data di scadenza superata, devono essere considerati non sterili. • Ogni prodotto e il suo imballaggio sono contrassegnati da un numero specifico di lotto (LOT). In caso di reclamo indicare sempre questo LOT e il numero di riferimento (REF). 4 Utenti All'utente devono essere note le regole della pratica medica, gli aggiornamenti scientifici ed i contenuti rilevanti di pubblicazioni scientifiche ad opera di autori medici. L'utente deve conoscere ed avere padronanza della tecnica operatoria, dell’assortimento di impianti, nonché dello strumentario e degli accessori. L'utente è responsabile: • della corretta attuazione dell’intervento chirurgico • della combinazione dei componenti dell’impianto e del loro impianto conforme • della prevenzione o riduzione dei rischi generali inerenti agli interventi chirurgici 5 Indicazioni Danneggiamento irrimediabile e doloroso dell'articolazione interfalangea prossimale per: • Osteoartrosi primaria, secondaria o post-traumatica. • Disturbo reumatico infiammatorio con ridotta attività infiammatoria e buone condizioni ossee. V. 1.1 79 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP 6 Controindicazioni AVVERTENZA La mancata osservanza delle controindicazioni può comportare il rischio di lesioni gravi o addirittura letali per il paziente! • Malformazioni ossee, disallineamento assiale o altre condizioni ossee che escludono l'impiego di un'articolazione artificiale • Previsto sovraccarico dell'impianto articolare • Instabilità laterale di più di 30° • Tendini estensori e flessori non intatti • Articolazioni interfalangee prive di una mobilità residua minima • Linea articolare non intatta ad un esame radiologico, in particolare per la mancanza degli epicondili • Formazione significativa di cisti ad un esame radiologico • Protrusione avanzata dell'articolazione ad un esame radiologico • Pazienti dal volume osseo o dalla qualità ossea insufficienti • Infezioni manifeste • Sospetto di intolleranza o di allergie ai materiali utilizzati per l’impianto • Malattie autoimmuni • Disturbi circolatori, malattie sistemiche e disturbi del metabolismo • Tessuto duro o tessuto molle insufficienti per quantità o qualità 80 V. 1.1 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP • Tumori ossei nella zona di ancoraggio dell’impianto • Patologie collaterali, ad es. processi patologici degenerativi con effetti negativi sulla funzione dell'impianto articolare • Utilizzo in zone sottoposte a sollecitazioni inadeguate di forza o a peso eccessivo • Pazienti che, a causa del loro stato mentale, neurologico o fisico, non sono disposti o non sono in grado di seguire le indicazioni per l’assistenza postoperatoria • Pazienti in precarie condizioni generali di salute, nel senso di una controindicazione generale (p.es. pazienti sottoposti a radiazioni) • Osteoporosi od osteomalacia e altri danni gravi delle strutture ossee che sono di impedimento allo stabile ancoraggio dei componenti dell’impianto • Abuso manifesto di medicinali, sostanze stupefacenti e/o alcoliche • Pazienti in crescita con epifisi aperte 7 Possibili effetti collaterali e complicazioni • Perdita di densità e/o necrosi ossee dovute a carico • Lussazioni e sublussazioni • Infezioni precoci e tardive, infezioni superficiali e/o in profondità • Lesioni nervose e vascolari • Danni al sistema nervoso in seguito a trauma operatorio • Alterazioni vascolari • Ematomi e problemi di cicatrizzazione • Calcificazioni periarticolari • Funzioni articolari e mobilità limitate V. 1.1 81 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP • Carico articolare limitato e dolori articolari • Reazione allergica o ipersensibilità al materiale dell’impianto • Limitazioni della crescita • Dolori, disturbi e anomalie della sensibilità e del tatto a causa dell’impianto • Guarigione insufficiente • Colorazioni del tessuto in seguito a contatto con il materiale dell’impianto • L'insufficiente attività osteogenetica o il ritardo nella guarigione ossea, l'osteoporosi, l'osteolisi, l'osteomielite, la mancata rivascolarizzazione o le infezioni, le necrosi ossee possono provocare l'allentamento, la deformazione, lo strappo o la rottura del sistema di endoprotesi. • Una sollecitazione non adeguata in termini di forza o di peso può provocare l'usura, la rottura, la deformazione, l'allentamento o la migrazione del sistema di endoprotesi. • I frammenti o le piccole particelle visibili solamente al microscopio possono staccarsi dal sistema di endoprotesi e migrare dal sito dell'impianto. Anche dopo la rimozione di un impianto metallico può succedere che queste particelle metalliche rimangano nel corpo. Attualmente non sono noti gli effetti a lungo termine di queste particelle. In tutti gli interventi chirurgici possono verificarsi complicazioni, come per esempio infezioni, lesioni neurali e dolori, non riconducibili all'impianto. 82 V. 1.1 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP 8 Avvertenze e misure precauzionali AVVERTENZA La mancata osservanza delle avvertenze comporta la possibilità di causare una lesione grave, se non letale al paziente! • • Prima di ogni utilizzo, esaminare il prodotto per escludere eventuali allentamenti, distorsioni, rotture, spaccature o usure nei componenti. Accertarsi che i prodotti non siano piegati o comunque danneggiati. • Non utilizzare il prodotto se presenta danni. • Eliminare prodotti danneggiati! • Non utilizzare il prodotto se esiste il sospetto che sia danneggiato in seguito a caduta o ad un impiego errato. • Non modificare né profilare il prodotto sulle condizioni anatomiche (non piegarlo né deformarlo). Accertarsi che l'ambiente chirurgico sia asettico. Non riutilizzare impianti già rimossi. Anche se l'impianto sembra intatto, possono essere presenti piccoli difetti e imperfezioni interne che possono causare il rigetto o la rottura dell'impianto. • • • Come in ogni intervento di impianto, sussiste il rischio di contaminazione con corpi o particelle estranei, ad esempi talco dei guanti, sfilacciamenti di teli, detergenti ed altre impurità superficiali. Si raccomanda pertanto di toccare il meno possibile l'impianto. Un impianto che è venuto a contatto con fluidi corporei non può essere riutilizzato! • Il cromo o il cobalto liberati dagli impianti metallici possono scatenare reazioni allergiche. Queste possono manifestarsi tra gli altri con eczemi, disturbi della cicatrizzazione, orticaria o rigonfiamento persistente. V. 1.1 83 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP • Non combinare i sistemi di endoprotesi della Gebrüder Martin con sistemi di altri produttori. Se si utilizzano strumenti non previsti specificamente per l'impiego in connessione con gli impianti, si può verificare l'avaria del sistema di endoprotesi. NOTA Possibile interpretazione errata dei risultati dell’analisi! Il sistema di endoprotesi può causare artefatti disturbanti sia durante la tomografia assiale computerizzata (TAC) che durante la tomografia a risonanza magnetica (TRM). AVVERTENZA Pericolo di ustione o di movimento accidentale dell’impianto durante l’effettuazione di una TRM! A causa dell’ulteriore sviluppo e dell’aumento della densità di energia degli apparecchi TRM, non è possibile escludere in futuro ripercussioni negative sugli impianti. In caso di dubbio, non è consentito un esame con apparecchi TRM, al fine di evitare possibili danni al paziente. • Con gli apparecchi TRM sussiste infatti il pericolo del riscaldamento e dello spostamento all’interno del campo magnetico degli impianti, che possono reagire al campo magnetico. • Gli impianti in acciaio ai sensi della norma ISO 5832-12 vengono classificati come paramagnetici o non magnetici. • Il titanio è un materiale completamente non magnetico. 84 V. 1.1 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP 9 Struttura e funzionamento del sistema Protesi CapFlex PIP comprende un componente prossimale ed uno distale. Anche il componente prossimale è realizzato in cobalto-cromo e rivestito in titanio puro, e dispone di una faccia articolare in polietilene ad altissimo peso molecolare (ISO 5834-2/ASTM F648). Scegliere il componente prossimale in modo che sia il più piccolo possibile, per evitare irritazioni alle parti molli. V. 1.1 85 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP Scegliere invece il componente distale in modo che sia il più grande possibile, per prevenire l'affossamento dell'impianto. Per poter portare l'apparato legamentoso alla tensione corretta dal punto di vista biomeccanico a seconda del paziente, il componente distale è disponibile in tre diverse altezze (varia lo spessore dell'inserto in polietilene). Per semplificarne l'impiego e per evitare eventualmente di confonderle, tutte le protesi di prova sono dotate di un codice colore a seconda della misura: Dimensioni Codice colore S Giallo M Blu L Rosa Strumenti Per l'impianto del sistema CapFlex PIP sono disponibili appositi strumenti, anch'essi codificati con anelli colorati a seconda delle dimensioni. V. 1.1 87 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP 10 Trattamento, pulizia, disinfezione e sterilizzazione • I componenti della protesi vengono forniti singolarmente in una confezione sterilizzata ai raggi gamma, opportunamente contrassegnata. • I componenti della protesi sono stati sterilizzati ai raggi gamma secondo una procedura validata (raggi gamma di min. 25 kGy (2,5 Mrad)). • Custodire i componenti della protesi nella loro confezione originale in un luogo idoneo, ed estrarli dalla confezione protettiva subito prima dell'uso. • I componenti dell'impianto, una volta estratti dalla confezione, contaminati e/o utilizzati, non vanno riutilizzati, ma anzi vanno scartati e non devono essere immessi nel sistema per il trattamento. In nessun caso risterilizzare gli impianti dotati di rivestimento superficiale e contaminati con sangue o altri liquidi corporei. • Gli strumenti e gli impianti di prova del sistema di endoprotesi sono idonei per il trattamento meccanico/disinfezione termica. Essi possono essere sterilizzati con programmi autorizzati per gli strumenti chirurgici. Per la pulizia vanno osservate le disposizioni del fabbricante dei macchinari di pulizia automatica e del fabbricante del prodotto di trattamento (detergente, disinfettante). Il processo, compreso il corretto caricamento, deve garantire la sufficiente asportazione dei residui. • Non è consentito sterilizzare nuovamente i componenti della protesi. 88 V. 1.1 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP AVVERTENZA Pericolo di infezione a causa di uso non sterile! Una sterilizzazione non corretta, oppure un impiego non sterile del sistema di endoprotesi possono comportare gravi rischi per la salute del paziente. La sterilizzazione deve avvenire secondo un procedimento convalidato di sterilizzazione a vapore, p.es. in uno sterilizzatore conforme alla norma EN 285:2009 e ANSI/AAMI ST 79, e convalidato conformemente alla norma ISO 17665-1:2006. ANSI/AAMI ST79 raccomanda un tempo minimo di ciclo di 4 minuti a 132 °C per cicli di sterilizzazione a vapore con eliminazione dinamica dell’aria. Osservate le indicazioni del manuale d’istruzioni del vostro sterilizzatore a vapore! NOTA Il gestore/l'utente è responsabile della pulizia, della disinfezione e della sterilizzazione degli strumenti e degli accessori. Pertanto, è assolutamente necessario rispettare le normative nazionali, nonché le limitazioni esistenti. Per la sterilizzazione, il successivo trasporto e il magazzinaggio vanno applicati gli imballaggi sterili appositamente omologati (p. es. ai sensi della EN 868, ISO 11607). V. 1.1 89 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP 11 • • • • • • • Prima dell’impianto Tutti i componenti per l'impianto del sistema CapFlex PIP sono disponibili già pronti all'uso. Le condizioni operatorie sono altamente asettiche L'apposita strumentazione per l'intervento di impianto, inclusa quella specifica per impianto articolare Gebrüder Martinper la preparazione del sito articolare, sono disponibili e in condizioni tecnicamente ineccepibili. Il chirurgo e l’équipe operatoria devono conoscere le informazioni relative alla tecnica operatoria, l’assortimento di impianti e la strumentazione; tali informazioni devono inoltre essere interamente disponibili in loco. Devono essere note e rispettate le regole della scienza medica, lo stato dell'arte della stessa, nonché i contenuti delle pubblicazioni scientifiche degli autori medici. Se la situazione preoperatoria non è chiara o se nella regione da trattare sono già presenti degli impianti devono essere richieste informazioni al produttore. Il paziente è stato informato sull'intervento ed ha fornito il suo consenso scritto, documentato nella sua documentazione, rispetto a quanto segue: — La funzionalità della protesi articolare è fondamentalmente paragonabile a quella dell'articolazione naturale. — La protesi articolare artificiale può produrre soltanto un miglioramento relativo delle condizioni pre-operatorie. — La protesi articolare artificiale può allentarsi in conseguenza di sovraccarico, sovrasollecitazione, usura o infezioni. — La durata della protesi impiantata dipende dal carico articolare. — Il paziente deve evitare sovraccarichi da lavoro e attività del tempo libero. 90 V. 1.1 Istruzioni per l’uso Sistema di endoprotesi CapFlex PIP — In caso di allentamenti, può rendersi necessario un intervento di revisione. — Nel caso in cui si renda necessaria una revisione, potrebbe non essere possibile ripristinare in modo accettabile la funzionalità articolare. — Il paziente si dichiara disponibile a sottoporsi a regolari visite mediche di follow up. Preparazione del sito dell'impianto • • • • • Non rimuovere strutture ossee e tessuti molli che possono servire per l'ancoraggio! Sono stati scelti impianti di prova idonei per la verifica del cuscinetto. Occorre assicurare che la posizione dell'impianto consente libertà di movimento all'articolazione, escludendo la possibilità di lussazioni e sublussazioni. Evitare assolutamente di danneggiare le superfici degli impianti. Prima di suturare, accertarsi che gli impianti siano posizionati correttamente, eventualmente mediante un visore elettronico. 12 Smaltimento Per lo smaltimento del materiale di imballaggio e del materiale potenzialmente infettivo (p.es. componenti in metallo rimossi) occorre attenersi alla norme ed alla direttive nazionali/locali in materia di smaltimento.
