I: CITTADINI IN TEMPORANEO SOGGIORNO (es. turisti):
Non devono essere iscritti al Servizio Sanitario Nazionale
A) Provenienti da Paesi C.E.E.
Il cittadino di un Paese della C.E.E. (ed i suoi familiari) che
soggiorna temporaneamente in Italia (ad es. per turismo)
NON DEVE ESSERE ISCRITTO AL S.S.N. ma può beneficiare delle
prestazioni sanitarie, in forma diretta, previste dalla legislazione
italiana, compresa l’assistenza medico-generica.
A tale scopo, occorre che lo stesso presenti alla A.S.L. competente per
territorio il Modello Comunitario o Certificato Sostitutivo Provvisorio della T.E.
A.M. o la T.E.A.M. previsti e comprovanti il diritto a ricevere dette prestazioni
e nei quali sia indicata la durata del periodo durante il quale dette prestazioni
possono essere erogate.
DAL I° GENNAIO 2005:
in applicazione della nuova normativa C.E.
E. (o UE) che ha introdotto il principio
dell’ACCESSO DIRETTO ai prestatori di cure
(eliminando così l’obbligo di presentare,
preventivamente, l’attestato di diritto
all’Istituzione del luogo di dimora - A.S.L.per ottenere in cambio il “Carnet della
salute” necessario per accedere ai prestatori
di cure) e all’introduzione della nuova
ricetta del S.S.N., i cittadini C.E.E., in temporaneo soggiorno, possono recarsi
DIRETTAMENTE dal medico convenzionato con il SSN (o ai servizi di Guardia
medica, turistica, 118, ecc.) presentando il Modello Comunitario o la T.E.A.M.
(Tessera Sanitaria Europea) o il Certificato sostitutivo provvisorio della T.E.A.M.
il quale utilizzerà la nuova ricetta per TUTTE LE PRESCRIZIONI DELLE
PRESTAZIONI SANITARIE.
La ricetta andrà compilata secondo le istruzioni impartite dal Ministero della
Salute e verrà trasmessa all’Ufficio della A.S.L. competente (Ufficio Ass.
Estero/Stranieri – Chiavari) compilata e tariffata dai vari servizi (visite
ambulatoriali, visite specialistiche, farmacie, lab. Analisi, ecc.) che provvederà
al recupero degli oneri dovuti dalle Istituzioni Estere, in base alle norme
Comunitarie, tramite il Ministero della Salute (fermo restando che rimane a
carico del cittadino la quota di partecipazione alla spesa, se dovuta).
L’accesso ai Laboratori o Istituti privati convenzionati accreditati è consentito
SE in possesso della ricetta del SSN; in caso contrario, l’onere sarà a totale
carico dell’assistito (come per i cittadini italiani).
N.B.: Sarebbe auspicabile che il medico, oltre la ricetta, potesse trasmettere
alla A.S.L. anche una fotocopia della T.E.A.M. o del Certificato sostitutivo onde
evitare possibili errate interpretazioni dei dati riportati che comporterebbero
mancato addebito delle prestazioni alle Istituzioni estere e, di conseguenza, un
minor introito con conseguente danno economico.

PRESTAZIONI DI PRONTO SOCCORSO NON SEGUITE DA RICOVERO
OSPEDALIERO
Il cittadino presenterà alla Struttura di P.S. il Modello
Comunitario o il Certificato sostitutivo provvisorio della T.E.A.M.
o la T.E.A.M. attestanti il diritto all’assistenza sanitaria e la
relativa copertura degli oneri dovuti.
PRESTAZIONI DI RICOVERO OSPEDALIERO
Il cittadino presenterà alla Struttura Ospedaliera il Modello Comunitario o il
Certificato sostitutivo provvisorio della T.E.A.M. o la T.E.A.M. attestanti il
diritto all’assistenza sanitaria e la relativa copertura degli oneri dovuti.
Se il cittadino C.E.E. è sprovvisto dei Modelli Comunitari
previsti:
•
in caso di prestazioni di Pronto Soccorso non seguite da
ricovero:
il paziente e/o i parenti dovranno far pervenire, durante il
tempo necessario all’erogazione della prestazione, l’attestato necessario
alla copertura degli oneri dovuti per la successiva richiesta di rimborso
alla Cassa Mutua Estera di appartenenza da parte dell’Uff. Ass. Estero/
Stranieri di Chiavari.
Se il paziente e/o i parenti non riescono a far pervenire detto attestato
con la tempistica sopra-indicata, dovrà essere attivata la procedura di
pagamento delle prestazioni DIRETTAMENTE in Pronto Soccorso, per
l’intero ammontare della prestazione.
•
in caso di ricovero ospedaliero:
se il paziente e/o i parenti non riescono a far pervenire l’attestato
necessario alla copertura degli oneri dovuti, dovrà essere attivata la
procedura di pagamento delle prestazioni, tramite l’Ufficio Spedalità di
Lavagna, per l’intero ammontare del D.R.G.
Il cittadino C.E.E., al suo ritorno nel Paese d’origine, potrà, eventualmente,
formulare richiesta di rimborso delle spese sopportate alla propria Cassa Mutua
di appartenenza.
B) Provenienti da Paesi Extra-C.E.E.
NON DEVONO ESSERE ISCRITTI AL S.S.N.
1) Il cittadino di un Paese Extra-C.E.E. (ed i suoi familiari), sempre in
temporaneo soggiorno, con il quale L’ITALIA HA STIPULATO CONVENZIONE O
ACCORDO BILATERALE (ARGENTINA, AUSTRALIA, BRASILE, CROAZIA,PRINCIPATO
DI MONACO, REPUBBLICA DI SAN MARINO, TUNISIA, VATICANO), che si presenta

con Modello similare C.E.E. (diverso e specifico per ogni Paese) può ugualmente
beneficiare delle prestazioni sanitarie, come già descritto per il cittadino C.E.
E., compresa l’assistenza medico-generica.
2) Il cittadino di un Paese Extra-C.E.E. (ed i suoi familiari) sempre in
temporaneo soggiorno, con il quale L’ITALIA NON HA STIPULATO ACCORDO O
CONVENZIONE BILATERALE e che non può, quindi, esibire alcun Modello ha il
diritto ad usufruire di TUTTE le prestazioni, comprese quelle ospedaliere
urgenti e/o di elezione previo pagamento delle relative tariffe secondo quanto
previsto per il cittadino C.E.E. sprovvisto di Modello Comunitario.
II: CITTADINI REGOLARMENTE SOGGIORNANTI:
Sono considerati regolarmente soggiornanti
i cittadini stranieri muniti di passaporto
vidimato con regolare visto di ingresso e
permesso, o carta, di soggiorno in corso di
validità.
A) DEVONO ESSERE OBBLIGATORIAMENTE ISCRITTI AL S.S.N.
I cittadini stranieri regolarmente soggiornanti, provenienti sia da Paesi C.E.E.
che da Paesi Extra-C.E.E., per essere iscritti al S.S.N. devono necessariamente
presentare:
PERMESSO – O CARTA – DI SOGGIORNO IN CORSO DI VALIDITA’ O IN ATTESA DI
RINNOVO, rilasciati per i seguenti motivi:
•
lavoro (subordinato o autonomo) (Tit. III° “Disciplina del lavoro” del T.U.);
•
iscrizione nelle liste di collocamento;
•
ricongiungimento familiare (D. Lgs. 286/98);
•
asilo politico e asilo umanitario;
•
richiesta di asilo politico e umanitario;
•
stato di gravidanza e puerperio (D. Lgs. 286/98);
•
adozione o affidamento (L. n°184/83);
•
acquisto cittadinanza italiana (L. n° 21/92);
•
motivi di salute (malattia o infortunio).
L’assistenza spetta altresì ai familiari a carico regolarmente
soggiornanti e viene assicurata, fin dalla nascita, ai minori
di stranieri iscritti al S.S.N. (D.L. 402/82 convertito nella
Legge n° 627/82).
L’iscrizione al S.S.N. si effettua presso l’Azienda A.S.L. di
residenza presentando i seguenti documenti in originale:
a) Permesso – o Carta – di soggiorno (o ricevuta di richiesta rinnovo)
b) Codice fiscale
c) Dichiarazione sostitutiva di certificazione di residenza (o ricevuta di
richiesta residenza)

e) Dichiarazione datore di lavoro o contratto stipulato e/o ultima busta paga

effettuata presso l’I.N.P.S.
f) SE familiari di lav.re dipendente: dichiarazione datore di lavoro o busta
paga con voce “Familiare a carico/coniuge/figli”, compilata
g) SE familiari di lav.re autonomo: copia Mod.730/Unico con voce
“Familiare a carico/coniuge/figli”, presente.
- Nuovi nati:
a) Codice fiscale
b) Atto di nascita
c) Ricevuta di richiesta inserimento “nuovo nato” nel documento di
soggiorno di un genitore.
- SE MINORI in adozione/affido:
a) Permesso – o Carta – di soggiorno (o ricevuta di richiesta)
b) Codice fiscale
c) Documentazione Tribunale dei Minori riguardante il provvedimento di
adozione/affidamento (o autocertificazione di un genitore adottivo/
affidatario)
d) Documento, in corso di validità, genitore/i adottivo/i e/o affidatario
d) Autocertificazione di residenza (o richiesta di) compilata dal genitore
adott./affid.
L’iscrizione al S.S.N. dà diritto a ricevere tutte le prestazioni comunque
garantite al cittadino italiano, gratuitamente o con pagamento del ticket, ad
es.:

L’ISCRIZIONE HA VALIDITA’ TEMPORALE LIMITATA ALLA SCADENZA DEL
PERMESSO - O CARTA - DI SOGGIORNO - (rinnovabile, se verrà rinnovato il
documento di soggiorno).
B) CITTADINI REGOLARMENTE SOGGIORNANTI
(Residenti o non residenti ma non fiscalmente contribuenti)
POSSONO ISCRIVERSI, VOLONTARIAMENTE, AL S. S. N.
I cittadini stranieri provenienti sia da Paesi C.E.E. che da Paesi Extra-C.E.E.,
per iscriversi volontariamente al S.S.N., devono presentare:
PERMESSO – O CARTA – DI SOGGIORNO, IN CORSO DI VALIDITA’ E COMUNQUE
SUPERIORE A MESI TRE, (eccezioni: per studenti e persone alla pari anche con
permesso – o carta – inferiore a mesi tre) RILASCIATI PER MOTIVI NON
RIENTRANTI NELLE CATEGORIE PRECEDENTEMENTE INDICATE PER I CITTADINI
REGOLARMENTE SOGGIORNANTI ad es.:
- studio
- residenza elettiva
- attività sportiva professionistica
- religiosi (per motivi di studi religiosi)
- affari
- e tutte le altre categorie che possono essere individuate per
esclusione a quanto precisato in materia di iscrizione obbligatoria.
MODALITA’ PER ISCRIZIONE VOLONTARIA
Per l’iscrizione al S.S.N. deve essere corrisposto dal cittadino, a titolo di
partecipazione alla spesa, UN CONTRIBUTO ANNUALE - NON FRAZIONABILE E
NON RETROATTIVO - DI IMPORTO PERCENTUALE SUL REDDITO COMPLESSIVO
CONSEGUITO NELL’ANNO PRECEDENTE
•
in Italia e all’Estero nella misura del 7,50% fino a € 20.658,28 (L.
40.000.000)
•
sulla quota eccedente e fino al limite di € 51.645,69 (L. 100.000.000 ) è
dovuto un ulteriore contributo aggiuntivo del 4%.
•
L’ammontare del contributo non può, in ogni caso, essere inferiore
all’importo di
€ 387.34 (L. 750.000).
In particolare: l’importo minimo è di € 387,34 (L. 750.000), come sopra
specificato, ed E’ VALIDO ANCHE PER I FAMILIARI A CARICO;
- per motivi di studio l’importo è di € 149.77 (L. 290.000) SE detti
studenti non beneficiano di redditi diversi da borse di studio o da sussidi
economici erogati da Enti pubblici italiani (lo devono autocertificare).
IL CONTRIBUTO NON E’ VALIDO PER I FAMILIARI A CARICO;
- per persone alla pari l’importo è di € 219,49 (L. 425.000 ).
IL CONTRIBUTO NON E’ VALIDO PER I FAMILIARI A CARICO.

- Il contributo si versa sul conto N° 606160 intestato alla Regione Liguria
indicando come causale: Iscrizione VOLONTARIA al S.S.N.
- L’iscrizione al S.S.N. si effettua presso l’Azienda A.S.L. del Comune in
cui il cittadino dimora presentando i seguenti documenti in originale:
a) Permesso – o carta – di soggiorno (o ricevuta di richiesta)
b) Codice fiscale
c) Ricevuta di avvenuto pagamento del contributo annuale
d) Documentazione comprovante l’eventuale iscrizione al corso di
studi, attività sportiva, ecc.
- L’iscrizione volontaria al S.S.N. dà diritto a ricevere dalla data di
iscrizione, gratuitamente o con pagamento del ticket, le stesse
prestazioni garantite a tutti i cittadini iscritti al S.S.N.
L’iscrizione ha scadenza ANNUALE (anno solare 1°gennaio/31 dicembre)
nonché limitata alla scadenza del permesso di soggiorno e, quindi,
eventualmente rinnovabile se è rinnovato il documento di soggiorno.
Il contributo annuale versato NON E’ FRAZIONABILE e, quindi, rimane valido
anche nel caso di più iscrizioni nel corso dell’anno solare di riferimento.
Al cittadino straniero, non iscrittosi volontariamente al S.S.N. pur avendone i
requisiti per farlo, vengono assicurate:
- tutte le prestazioni ospedaliere, ambulatoriali, di Pronto Soccorso e di
elezione con oneri a totale carico del paziente.
- In caso di eventuale dichiarazione di indigenza, le prestazioni saranno
erogate gratuitamente e gli oneri dovuti saranno richiesti al Ministero
dell’Interno, tramite la Prefettura, o alla Regione Liguria.
C) CITTADINI REGOLARMENTE SOGGIORNANTI
(C.E.E. ed EXTRA - C.E.E.) NON ISCRITTISI AL S.S.N.
NE’ OBBLIGATORIAMENTE (perché non sono obbligati ad iscriversi)
NE’ VOLONTARIAMENTE (perché non si sono iscritti volontariamente pur avendo
i requisiti per farlo) IN POSSESSO DI UN PERMESSO – O CARTA – DI SOGGIORNO
CON UNA VALIDITA’ TEMPORALE CHE NON SUPERA I MESI TRE (es. studenti e
persone alla pari).
Vengono loro assicurate tutte le prestazioni ospedaliere, ambulatoriali, di
Pronto Soccorso e di elezione con oneri a totale carico del paziente.
In caso di eventuale dichiarazione di indigenza, le prestazioni saranno erogate
gratuitamente e gli oneri dovuti saranno richiesti al Ministero dell’Interno,
tramite la Prefettura, o alla Regione Liguria.

D) CITTADINI STRANIERI (C.E.E. ed EXTRA-C.E.E.) IN POSSESSO DI
PERMESSO – O CARTA – DI SOGGIORNO RILASCIATI PER:
MOTIVI DI CURA, MOTIVI TURISTICI
NON DEVONO ESSERE ISCRITTI AL S.S.N.
NE’ OBBLIGATORIAMENTE - NE’ VOLONTARIAMENTE
Ad essi vengono comunque assicurate le prestazioni sanitarie ospedaliere, di
Pronto Soccorso, ambulatoriali, urgenti, essenziali e/o di elezione con oneri a
totale carico del paziente in quanto le motivazioni del rilascio del documento di
soggiorno escludono l’iscrizione al S.S.N., sia obbligatoria che volontaria.
Nel caso di permesso o carta di soggiorno per motivi turistici, se i cittadini CEE
sono in possesso della tessera TEAM in corso di validità, sono assicurate tutte le
prestazioni medicalmente necessarie (come previsto al punto 1 A), in forma
diretta, compresa l’assistenza medico-generica.
III: CITTADINI IRREGOLARMENTE SOGGIORNANTI
NON DEVONO ESSERE ISCRITTI AL S.S.N.
Provenienti da Paesi C.E.E. - solo nel caso la presente
normativa risulti più favorevole.
Provenienti da Paesi Extra - C.E.E. NON IN REGOLA CON LE
NORME RELATIVE ALL’INGRESSO E AL SOGGIORNO.
L’assistenza sanitaria a questo tipo di cittadini è
comunque assicurata presso le strutture pubbliche e private accreditate del
S.S.N. per le seguenti prestazioni:
1. Cure ambulatoriali ed ospedaliere, urgenti o comunque essenziali,
ancorché continuative, per malattia o infortunio.
In ordine alla tipologia di prestazioni previste dal terzo comma dell’art. 35
del T.U. si chiarisce che:
- per cure urgenti si intendono le cure che non possono essere differite
senza pericolo per la vita o danno per la salute della persona;
- per cure essenziali si intendono le prestazioni sanitarie, diagnostiche e
terapeutiche, relative a patologie non pericolose nell’immediato e nel
breve termine ma che nel tempo potrebbero determinare maggiore danno
alla salute o rischi per la vita ad es. complicanze, cronicizzazioni o
aggravamenti;
- è stato, altresì, affermato dalla Legge il principio della continuità delle
cure urgenti ed essenziali, nel senso di assicurare all’infermo il ciclo
terapeutico e riabilitativo completo riguardo alla possibile risoluzione
dell’evento morboso;
2. Interventi di medicina preventiva e prestazioni di cura ad essi correlate
a salvaguardia della salute individuale e collettiva e, in particolare, relativi
a:

tutela della gravidanza e della maternità;
tutela della salute del minore;
vaccinazioni e interventi di prevenzione collettiva;
interventi di profilassi internazionale;
profilassi, diagnosi e cura delle malattie infettive;
interventi di prevenzione, cura e riabilitazione degli stati di
tossicodipendenza.
L’art. 35 del T.U., pur affermando che di norma non esiste il principio della
gratuità delle prestazioni erogate dal S.S.N. ai cittadini non iscritti, prevede,
nel comma 4, che le prestazioni sopra elencate siano erogate senza oneri a
carico degli stranieri irregolarmente presenti qualora privi di risorse
economiche sufficienti (art. 43 – comma 4 – del Regolamento di attuazione)
fatte salve le quote di partecipazione alla spesa a parità di condizioni con il
cittadino italiano.
L’esonero della quota di partecipazione è esteso allo straniero per le
prestazioni di primo livello, le urgenze, le prestazioni correlate allo stato di
gravidanza, quelle correlate a patologie esenti, a gravi stati invalidanti e quelle
esenti in ragione dell’età dell’assistito (come per i cittadini italiani).
Al momento della prima erogazione dell’assistenza sanitaria, la prescrizione e
la registrazione delle prestazioni a carico di uno straniero temporaneamente
presente viene effettuata mediante l’attribuzione di un Codice identificativo
regionale preceduto dalla sigla STP (Straniero Temporaneamente Presente)
come indicato nell’art. 43 – comma 3 – del Regolamento di attuazione, che ha
validità semestrale ed è rinnovabile in caso di permanenza dello straniero sul
territorio nazionale. Per il rilascio di tale Codice viene richiesto,
preventivamente, il passaporto o un documento equivalente.
Tale Codice identificativo è costituito da 16 caratteri e precisamente:
- la sigla STP (3 caratteri);
- il Codice ISTAT della Regione Liguria – 070 – e quello della struttura
erogante – 104 – per la A.S.L. 4 – “Chiavarese” (in totale 6 caratteri);
- la codifica personale di 7 caratteri, come numero progressivo attribuito al
momento del rilascio.
Il Codice STP è unico a livello regionale ed è riconosciuto su tutto il territorio
nazionale.Deve essere utilizzato, ai fini del rimborso, sia per la rendicontazione
delle prestazioni erogate dalle strutture pubbliche o accreditate del S.S.N. sia
per la prescrizione, su ricettario Regionale, dei farmaci erogabili, a parità di
condizione di partecipazione alla spesa con i cittadini italiani.
Con l’attribuzione del Codice viene consegnato all’interessato un modello
cartaceo con Codice STP con l’avvertenza di custodirlo e presentarlo alle
strutture sanitarie per tutto il periodo di permanenza sul territorio nazionale.
Qualora il soggetto si presenti sprovvisto del Modello presso una struttura
diversa, questa provvederà ad assegnare il proprio Codice STP ed a rilasciare il
Modello cartaceo.
Insieme al modello cartaceo con Codice STP, viene rilasciato un modello

cartaceo di dichiarazione d’indigenza che dovrà essere compilata
dall’interessato, qualora privo di risorse economiche sufficienti, alla richiesta
di pagamento delle prestazioni erogategli, comprese le quote di partecipazione
alla spesa (ticket).
Gli oneri relativi alle prestazioni di cui al n°1) e cioè quelle ospedaliere, di
Pronto Soccorso, urgenti ed essenziali, erogate a favore di stranieri irregolari
che abbiano compilato la dichiarazione d’indigenza, sono a carico del
Ministero dell’Interno e dovranno essere chieste alla competente Prefettura
(Legge n° 6972 del 17/07/1890 e succ.ve mod.ni).
Gli oneri relativi alle prestazioni di cui al n° 2) e cioè quelle previste dall’art.
35 – comma 3 – del T.U. e del D. Lgs. 286/99 (tutela della gravidanza e
maternità, ecc.) erogate a favore di stranieri irregolari che abbiano compilato
la dichiarazione d’indigenza, comprese le eventuali quote di partecipazione
alla spesa rimaste insolute, sono a carico dell’Azienda A.S.L. del territorio ove
è ubicato il presidio che ha erogato la prestazione che provvederà a richiedere
semestralmente il rimborso all’Ente Regione.
I dati personali dello straniero irregolarmente presente sul territorio nazionale,
registrati dalla struttura che eroga la prestazione al momento
dell’assegnazione del Codice, non comportano alcun tipo di segnalazione
all’autorità di Pubblica Sicurezza, fatti salvi i casi in cui vi sia obbligo di referto
a parità di condizioni con il cittadino italiano.
La struttura sanitaria deve in ogni caso provvedere, anche in assenza di
documenti d’identità, alla registrazione delle generalità fornite dall’assistito
non solo perché il beneficiario non può, in linea di principio, rimanere anonimo
(per es. per l’accertamento di eventuali responsabilità degli operatori sanitari)
ma anche ai fini degli adempimenti previsti dall’art. 4 del Regolamento di
attuazione, in ordine alle comunicazioni, previo consenso dell’interessato,
salvo che sia impossibilitato a farlo, all’autorità consolare del suo Stato di
appartenenza e dalla rilevazione dei casi di malattie infettive e diffusive
soggette a notifica obbligatoria.

but he can get sanitary services, directly, provided by Italian law, included
medical general assistance. To do so, he has to show to Local Sanitary Firm
(ASL) the Community form or the substitutive temporary certificate of T.E.A.M.
or the T.E.A.M. that gives the right to receive those services and where it’s
written the period during which those services can be given.
On receipt of the form the Local Sanitary Firm (ASL), untill the date of
December 31 2004, gave the “Carnet della salute” (coupon for visit and for
prescriptions), with that the citizen had to go to a doctor of the National
Sanitary Service, to First Aid station (touristic, where present) to Emergency
Services, to get the required prescriptions.
From January 1 2005:
following the new EEC laws, based on the direct admittance to the ones who
give cures and on the introduction of the new prescription of the National
Sanitary Service, the EEC citizens, who stay temporary in Italy, can go directly
to a doctor of SSN (or to First Aid station, Emergency Services....) showing the
Community Form or the T.E.A.M. (European Sanitary Card) or the substitutive
temporary Certificate, and the doctor will utilize the new prescription for all
the prescriptions of sanitary performances for which it was used the “Carnet
della salute”. The prescription will be filled according to the instructions given
by Health Department and will be sent to the Foreigner Office of Local Sanitary
Firm in Chiavari, filled and with the rates by the different services (ambulatory
visits, specialistic visits, chemist, laboratory of analysis etc.) that will recover
the expenses due to Foreign Institutions, based on Community Laws, through
Health Department (we remind that the share of participation to the expense,
if due, is charged to the citizen).
The admittance to the laboratories or private Institutes is possible if in
possession of the SSN prescription; on the contrary, the expense will be charged
completely to the citizen.
It could be better that the doctor could send to ASL, as well as the

prescriptions, also a copy of T.E.A.M. or of the substitutive certificate to avoid
possible wrong interpretations of the data and the unsuccessful debit to foreign
Institutions of performances and, consequently, less income with following
economic damage.
FIRST AID SERVICES NOT FOLLOWED BY HOSPITALIZATION
The citizen will show to the First Aid Office the Community Form or the
substitutive temporary certificate of T.E.A.M. or the T.E.A.M. that shows the
right to the sanitary assistance and the relative cover of the due expenses.
You have to pay attention to the following way:
Urgent performances free from ticket payment
We will ask the total reimbursement to the national insurance of citizen’s
country of origin after signal from First Aid
Performances that you have to pay a ticket for
We will ask the reimbursement, always to the national insurance of citizen’s
country of origin, of the difference between the total performance and the
ticket paid by the citizen (that he has anyhow to pay).
PERFORMANCES OF HOSPITALIZATION
The citizen will show to the hospital the Community Form or the substitutive
temporary certificate of the T.E.A.M. or the T.E.A.M. that gives the right to
sanitary assistance and the relative cover of the due expenses.
The relative reimbursements of performance that have been given will be
asked to the national insurance of the country of origin always by the
competent office of ASL (Foreigner Office) on the basis of DRG (Diagnostic
Related Group), given at the moment of hospitalization.
If the citizen from EEC doesn’t have the Community Forms which are foreseen:
in case of First Aid performances, not followed by
hospitalization:
- the patient and /or his relatives will have to send to the Foreigner Office of
ASL in Chiavari, during the necessary time to give the performance, the
document, to cover the due expenses, for the following request of
reimbursement to the national insurance of the citizen’s country of origin.
- if the patient and/or his relatives aren’t able to send that document within
the time above, it will be activated the procedure for performances payment
directly at First Aid, for the total amount of the performance.
- in case of hospitalization:
if the patient and/or his relatives aren’t able to send the document
necessary to cover the due expenses, it will be activated the procedure for
performances payment through “Ufficio Spedalità” in Lavagna, for the total
amount of DRG.

Citizen from EEC, going back to his country of origin, can, if he wants, ask

Out of EEC countries ( Argentina, Australia, Brazil, Croatia, Monaco, Repubblica

in Italy, that Italy stipulated an agreement with, who has a form, similar to
the EEC one (different and specific for each country) can get sanitary
performances, included general medical assistance
And he should not be registered to the national sanitary system

Citizen from an extra EEC country (and his relatives) who stay always
temporary in Italy, and that Italy didn’t stipulated an agreement with, who
can’t, therefore, show any form
Should not be registered to the national sanitary system (SSN) but he can get
all performances, included those urgent in hospital, after the payment of the
expenses, according to the laws about citizens belonging to EEC without
Community form.

GIVE FISCAL CONTRIBUTION)

FOREIGN CITIZENS COMING FROM EEC AND OUT OF EEC COUNTRIES, IN
ORDER TO REGISTER THEMSELVES VOLUNTARILY TO SSN, HAVE TO SHOW:
Valid permission to stay, that lasts more than 3 months (except for students and
people au pair also with permission which lasts less than 3 months) given for
reasons, different from those shown for citizens who stay regularly in Italy for
example:
- study
- elective residence
- professional sport
- religion
- business
- and all other categories which can be individualized with
exclusion from those obligatory

CONDITIONS FOR VOLUNTARY REGISTRATION
The citizen, in order to register himself to the SSN, has to participate to the
expense, with an annual contribution not to divide and not retroactive,
with the following rates on the total income of the previous year
- in Italy and abroad 7,50% till € 20.658,28
- till the limit of € 51.645,69 you have to give a further
contribution of 4%
- the amount of contribution can’t be, in any case, less than
€ 387,34.
In particular:
- the minimum amount is € 387,34, as written above, and is
valid also for relatives in charge
- for reason of study the amount is € 149,77 if the students
haven’t any other income different from scholarship or
economic subsidies given by Italian public organizations (they
have to selfcertificate it). The contribution isn’t valid for
relatives in charge
- for people au pair the amount is € 219,49. The contribution
isn’t valid for relatives in charge
- You have to give the contribution on the account number
606160 of Regione Liguria writing voluntarily registration to
SSN
- The registration to the SSN is made at the Local Sanitary Firm
of the town municipality where the citizen lives, showing the
following documents in original:
a) permission to stay (or receipt of the request)
b) fiscal code
c) receipt of payment of the annual contribution
d) documents about the inscription to the studies, sport
activity etc.
- the voluntary inscription to the SSN gives the right to receive,
from the date of inscription, free or with ticket payment, the
same performances guaranteed to all the citizens registered
to the SSN.
- The inscription is annual (from January 1 to December 31) and
is limited to the expire of permission to stay and therefore
eventually to renew if it’s renewed the document of stay. The
annual contribution given isn’t to divide and so it remains
valid also in case of more inscriptions during the referring
solar year.
- To foreign citizen, who isn’t voluntarily registered to SSN,
even if he has got requisites to do that, are guaranteed:
- all performances in hospital, ambulatory, at First Aid with
payment completely charged to the patient. In case of

be given free and the amount will be asked to Ministero
dell’Interno, through Prefettura or to Regione Liguria.

Sanitary performances in hospital, at First Aid, in ambulatory, urgent and
essential performances are guaranteed to them with payment completely
charged to the patient, because the document of stay has been given for
reasons that exclude the registration to SSN, both obligatory and voluntarily.
CITIZENS WHO STAY IRREGULARLY IN ITALY
should not be registered to SSN
(coming from EEC countries – only in case of more favourable laws)
coming from out of EEC countries who are not regular with laws about
admittance and stay in Italy
- Sanitary assistance is anyhow guaranteed to them, in public and private
organizations, recognized by SSN, for the following performances:
1) treatments in ambulatory and in hospital, urgent or anyhow essential,
also continuative, for illness or accident.
- urgent treatments are the ones that you can’t postpone
without danger for life or damage for one’s health;
- essential treatments are sanitary, diagnostic and therapeutic

performances, referring to not dangerous pathologies (in this
moment) but in the future could make damage to health or
risk for life, for example complications, aggravation.
- Law declares the principle of continuity of urgent and
essential cares, that is to guarantee to the patient the
complete therapeutic and rehabilitation treatments to solve
illness.
2) preventive medicine and treatments referring to it for individual and
collective health and in particular about:
a) pregnancy and motherhood care
b) young people health care
c) vaccine and collective prevention actions
d) international prophylaxis actions
e) prophylaxis, diagnosis and care about infectious disease
f) prevention, treatment and rehabilitation of drugs addicts
Art. 35 T.U., even if it says that normally the principle of free performances
given by SSN to the citizens who are not registered doesn’t exist, foresees that
the performances above are given without payment for foreigners irregularly
present when they haven’t enough money, except for rates of participation to
the expense at the same conditions of Italian citizen.
Participation to the amount is not due for foreigners for first level
performances, emergency, pregnancy, those referred to free pathology, to
serious matters of invalidation and those free for patient’s age (like Italian
citizens).
When the first performance of sanitary assistance is given, the
prescription and registration of performances charged to a foreigner, who’s
temporarily present, are made through attribution of an identifiable regional
code with the abbreviation STP (Straniero Temporaneamente Presente) as
written in art.43, c.3, Regolamento di attuazione, which is valid for 6 months
and it’s renewable if the foreigner remains in Italy. To have that code you’re
asked to show before the passport or an equivalent document.
This identifiable code is made by 16 letters:
The abbreviation STP (3 letters)
ISTAT code of Regione Liguria –070- and the one related to the structure
that gives the performance-104-for ASL 4 Chiavarese (total: 6 letters)
the personal code of 7 letters, as progressive number given at the release
STP code is unique at a regional level and it’s recognised everywhere in
the country.
It has to be used, in order to get the reimbursement, both for statement
about performances given by public and recognised by SSN and for drugs
prescription, on regional prescription-book, like the participation to the
amount with Italian citizens. The patient receives, with the code attribution, a
form with STP code to keep and show to sanitary structures for the whole
period in the country.
If the patient goes without that form to a different structure, this one

will give its own STP code and the form. Together with the form with STP code,
a form of declaration of indigence is given to the patient, who will have to fill
it, if he hasn’t got enough money, to pay the performances, included the
participation to the expense (ticket).
The expense referred to performances (art.1), that is in hospital, at First
Aid, urgent and essential performances, given to irregular foreigners, who
filled the declaration of indigence, will be asked to Ministero dell’Interno,
through the competent Prefettura.
The expense referred to performances (at n.2) (pregnancy, motherhood etc.)
given to irregular foreigners, who filled the declaration of indigence, included
the eventual participation to the amount not paid, will be paid by Azienda ASL
of the structure that gave the performance, which will ask the reimbursement
every 6 months to the Region.
Personal data about the foreigner irregularly present in the country, registered
by the structure that gives performance when the code is given, don’t
represent any kind of communication to Police, except when it’s obligatory to
do report, with the same conditions with Italian citizen.
Sanitary structure in any case has to register, also without identity documents,
patient’s data, not only because he can’t remain anonymous (for example to
verify eventual responsability of sanitary operators) but also to fulfil (as
mentioned in art.4 regolamento di attuazione) to communication, after
patient’s consent, except he can’t do it, to his own country Consulate to notice
case of infectious and diffusive disease, that have obligatory to be known.
