Presentarsi in Consolato il giorno/ora dell’appuntamento muniti della seguente documentazione:
On the day of your appointment please submit the following documentation:
2 fotografie identiche e conforme alle norme ICAO (vedi photograph guidelines)
2 identical passport photographs that comply with ICAO standards (see photograph guidelines)
Formulario/Modulo richiesta MOD1 compilato correttamente.
A signed and completed application form MOD1
Per coloro che hanno figli minori di 18 anni – Far compilare e firmare all’altro genitore l’apposita sezione contenuta
nel MOD1 oppure presentare il Modulo Assenso Altro Genitore . In entrambi i casi, sara’ necessario presentare
fotocopia di un documento di riconoscimento dell’altro genitore con foto e firma(e.g. passaporto o carta identita’)

Individuals with children under the age of 18 must ask the other parent of said children to complete the bottom section of
the application form (MOD1) or provide a Consent form . In both cases, it’s mandatory to submit a photocopy of his/her
photo ID including the signature (e.g. passport or ID Card) Please read carefully pag. 2
Il passaporto precedente o, se si tratta del primo passaporto, un documento d'identità italiano (carta di identità) o
straniero (passaporto), munito di foto e firma. Se il passaporto è stato smarrito è necessario presentare denuncia presso le
competenti Autorità australiane e compilare il formulario di Denuncia Smarrimento/Furto Documenti
Your previous passport or, if this is your first passport, another form of identification (ID Card) or foreign passport,
showing your photograph and your signature. If your previous passport has been lost/stolen, you must also provide an
Australian Police Report and fill in the Denuncia Smarrimento/Furto Documenti form.

Il passaporto verrà rilasciato solo dopo l’avvenuto pagamento. Si accettano ESCLUSIVAMENTE le seguenti forme di
pagamento:

AVVERTENZE / IMPORTANTE
Se il richiedente ha figli minori ha SEMPRE bisogno dell’ASSENSO dell’altro genitore
anche per il rilascio del proprio passaporto (Art. 3 Legge 1185/1967).
In presenza di figli minori l’assenso è SEMPRE necessario, anche se il richiedente
non è sposato, è divorziato o separato.
Se l’altro genitore è di cittadinanza NON Comunitaria e NON è quindi in possesso di
un passaporto europeo in corso di validità, dovrà recarsi in Consolato con il/la
richiedente - il giorno dell’appuntamento - poiché il modulo dovrà essere firmato
alla presenza di un funzionario del Consolato previa esibizione di un documento
d'identità.
Si avverte che se il richiedente ha figli minori, il giorno dell’appuntamento verrà
ricevuto soltanto se sarà munito/a dell’ assenso ed in presenza dell’altro genitore,
se quest’ultimo non è cittadino comunitario.
Diversamente si avverte che dovrà fissare un altro appuntamento on line e
ripresentarsi con la documentazione mancante.
IMPORTANT NOTICE
If the applicant has children under the age of 18, consent of the other parent is
ALWAYS required, even for the issue of his/her own passport.
Written consent of the other parent is obligatory even if the applicant is unmarried,
separated or divorced.
If the other parent is NOT an EU national, and therefore, does not have a
valid European passport, the consent form must be signed in the presence of a
Consular Officer at the Consulate on the day of the appointment, and submitted
with photo identification.
Please be advised that if the applicant has children under the age of 18, he/she will
only be seen if in possession of consent of the other parent. In the case where the
other parent is not an EU national, this parent must be present at the appointment.
Please note, that if the above is not adhered to, you will need to make another
appointment online and return to the Consulate to submit the missing
documentation.
