Lancet • • At present, only a small number of biomarkers have been found to have prognostic significance, these include certain chromosomal deletions1 In a Phase III trial, chromosomal aberrations were detected by FISH analysis of blood from patients with CLL treated with FC (n = 409) or R-FC (n = 408)2 • The prognosis for patients with 17p– was substantially poorer both in the FC and R-FC groups • Chemoimmunotherapy improved survival for patients in other cytogenetic groups (11q–, trisomy 12 and 13q–) compared with chemotherapy alone FC = fludarabine and cyclophosphamide; FISH = fluorescence in situ hybridization; R-FC = MabThera plus FC. 1. Younes, et al., JCO 2012 Considerazioni conclusive • La presenza di un bersaglio molecolare sensibile è un elemento necessario, ma talora non sufficiente per una risposta clinica ottimale • E’ poco probabile che “l’evento imatinib” possa ripetersi di frequente, perché è poco probabile che un singolo agente da solo possa curare un tumore • E’ più probabile che i farmaci diretti contro bersagli molecolari possano esprimere la loro massima efficacia in combinazione con altri farmaci (altri farmaci molecolari, chemio-immunoterapia, vaccini..), ma: • diventa essenziale identificare marker prognostici in grado di predire la risposta alle differenti terapie
