Orsola Malpighi [email protected] Riassunto Abstract E’ noto come l’uso degli antiretrovirali possa arrecare danno al metabolismo osseo, e come i regimi contenenti l’associazione Tenofovir/Emtricitabina (TDF/FTC) + un inibitore della proteasi rafforzato farmacocineticamente da Ritonavir (PI/r) siano quelli più soggetti ad attivare l’unità di rimodellamento osseo. L’avvento della nuova classe degli inibitori dell’integrasi (INSTI), ha permesso di costruire regimi a minor impatto sulla BMD senza utilizzare TDF/FTC o i PI/r, sia nei soggetti naïve sia in quelli experienced. Con Raltegravir prima, Elvitegravir dopo e il recente sviluppo di Tenofovir Alafenamide (TAF) e la sua coformulazione con Elvitegravir, Cobicistat e Emtricitabina in E/C/F/TAF, sono stati disegnati diversi studi di confronto, sia su soggetti naïve, sia su quelli experienced. Questi studi hanno dimostrato come la performance dei regimi contenenti INSTI sia quella significativamente migliore sia a livello del rachide lombare, sia del collo femorale. I dati più recenti riguardano E/C/F/TAF (102, 104, 111 e 106 sui naïve e 109, 112 e NCT02071082 sui experienced). I regimi a base di INSTI si configurano al momento fra i più sicuri per il metabolismo osseo. It has been observed that the use of antiretrovirals can lead to damage to bone metabolism, and that regimens containing the combination Tenofovir/Emtricitabine (TDF/FTC) + a protease inhibitor pharmacokinetically reinforced by Ritonavir (PI/r) are those most subject to activating complete bone remodelling. The advent of the new class of integrase inhibitors (INSTIs), has made it possible to construct regimens with a lower impact on BMD without using TDF/FTC or PI/r, in both naïve as well as experienced subjects. Starting with Raltegravir, then Elvitegravir and the recent development of Tenofovir Alafenamide (TAF) and its co-formulation with Elvitegravir, Cobicistat and Emtricitabine in E/C/F/TAF, several comparative studies in both naïve and experienced subjects have been designed, which have shown that the performance of regimens containing INSTI is significantly better, both at the level of the lumbar spine and the femoral neck. The most recent data considered E/C/F/TAF (102, 104, 111 and 106 in naïve subjects, and 109, 112 and NCT02071082 in experienced subjects). The INSTI-based regimens were configured at the time to be amongst the safest possible for bone metabolism. La perdita di BMD correla con un aumento del rischio di fratture spontanee, alle quali corrisponde una riduzione dell’aspettativa di vita media del soggetto. Trattare l’infezione da HIV è pertanto utile anche ai fini della salvaguardia del metabolismo osseo. Purtroppo, anche i regimi antiretrovirali, in diversa misura, possono creare un danno all’unità di rimodellamento osseo (1): con i regimi tradizionali questo danno è stato quantificato in un 2-6% di riduzione della BMD nelle prime 48-96 settimane di terapia antiretrovirale (2). I regimi a più alto rischio di danneggiamento sono quelli costituiti dall’associazione Tenofovir/Emtricitabina (TDF/FTC) + un inibitore della proteasi rafforzato farmacocineticamente da Ritonavir (PI/r). Because of osteoclast activation and osteoblast inhibition by HIV, people with HIV infections have a lower average BMD when compared to their peers of the same gender who are not infected (1). The loss of BMD correlates with an increased risk of spontaneous fractures, which corresponds to a reduction in the life expectancy of the average person. To treat HIV infections therefore, it is useful to preserve bone metabolism. Unfortunately, anti-retroviral regimens can also, to varying degrees, cause damage to the start of bone remodelling (1): with the traditional regimes, this damage was estimated as a 2.6% reduction in BMD in the first 48-96 weeks of anti-retroviral therapy (2). The regimens at the highest risk of damage are those comprising the combination of Tenofovir/Emtricitabine (TDF/ FTC) + a protease inhibitor pharmacokinetically reinforced by CURE ~ 2015 11 Articolo Originale/Original Article La classe degli inibitori dell’integrasi (INSTI) ha in sé la potenzialità di dare una sicurezza maggiore su questo particolare aspetto, per via dell’azione completamente diversa sulle dinamiche cellulari. Per questo motivo, non appena il primo INSTI (Raltegravir) si è reso disponibile, sono comparsi i primi studi che prevedevano al loro interno lo studio del metabolismo osseo attraverso assorbimetria a raggi X a doppia energia (DXA). Differenzieremo in questa review dapprima i risultati degli studi sui soggetti naïve, per poi passare a considerare l’effetto dell’utilizzo degli INSTI negli studi di switch. Nello studio PROGRESS, in aperto, di 96 settimane, sono stati confrontati un regime NRTI-sparing basato su Lopinavir/r 400/100 BID + Raltegravir 400 BID vs la terapia HAART standard, basata su Lopinavir/r 400/100 BID + TDF/FTC in 206 soggetti con HIV naïve (3). Nello studio RADAR 85 pazienti con HIV naïve sono stati randomizzati ad iniziare una terapia con un regime basato su Raltegravir (n = 42) o su TDF/FTC (n = 43), entrambi insieme a Darunavir/r (4). In entrambi gli studi si è dimostrato un vantaggio significativo in termini di variazione di BMD a favore del gruppo di pazienti che ricevevano il regime NRTI-sparing, che emerge però da un dato sensitometrico raccolto utilizzando esclusivamente una DXA total body (che analizza cioè l’intero calcio dell’organismo), che non può essere utilizzato ai fini di diagnosi di osteoporosi. Nello studio NEAT (in cui 805 pazienti con HIV naïve sono stati randomizzati in 78 centri clinici in 15 paesi a ricevere un regime Darunavir/r 800/100 QD + Raltegravir BID o TDF/FTC + Darunavir/r 800/100 QD) sono state invece eseguite le DXA sia della colonna lombare sia del collo femorale; i risultati dimostrano chiaramente come la performance del braccio NRTI-sparing sia superiore a quella del braccio tradizionale in entrambi i due siti in esame (5). Nel sottostudio dell’ACTG5257 condotto sul metabolismo osseo (A5260s), è stata analizzata la variazione di BMD con DXA a livello vertebrale, femorale e total body a 96 settimane in soggetti naïve arruolati ad assumere TDF/FTC insieme ad Atazanavir/r, o Darunavir/r o Raltegravir. Le variazioni medie percentuali della BMD sono risultate superiori nel braccio relativo ai due PI/r rispetto al braccio RAL (femore: 3.7% vs -2.4%, p=0.005; colonna -3.8% v -1.8%, p<0.001). A5260s ha dimostrato come sia possibile costruire ab initio un regime rispettoso per il metabolismo osseo pur includendo TDF/FTC, a patto che il terzo farmaco non sia un PI/r, bensì un inibitore dell’integrasi (6). Con la successiva disponibilità di un ulteriore INSTI, Elvitegravir, è stato possibile verificare anche l’effetto di Elvitegravir (potenziato da Cobicistat) associato a TDF/FTC (E/C/F/TDF) sui soggetti naïve, sempre utilizzando come confronto un regime classico comprendente TDF/FTC associato ad Atazanavir/r. Nello studio 103 sono stati 353 i soggetti che hanno assunto E/C/F/ TDF per 144 settimane. La performance del regime con E/C/F/TDF sulla BMD rispetto al regime tradizionale Atazanavir/r + TDF/FTC è risultata nettamente migliore, sia a livello del rachide (-1.43% vs 12 Ritonavir (PI/r). The class of integrase inhibitors (INSTI) has within it the potential to provide enhanced safety on this aspect, because its action on cellular dynamics is completely different. For this reason, as soon as the first INSTI (Raltegravir) was made available, the first studies appeared which included the study of bone metabolism via duel energy X-ray absorptiometry (DXA). This review first delineates the results of studies on naïve subjects, and then moves on to a consideration of the effects of using INSTI in switch studies. In the open-label, 96-week PROGRESS study, they were compared to an NRTI-sparing regimen based on Lopinavir/r 400/100 twice daily + Raltegravir 400 twice daily vs HAART standard, based on Lopinavir/r 400/100 twice daily + TDF/FTC in 206 naïve patients with HIV (3). In the RADAR study, 85 -naïve patients with HIV were randomly assigned to start treatment with a regimen based on Raltegravir (n = 42) or TDF/FTC (n = 43), both together with Darunavir/r (4). In both studies, a significant advantage was demonstrated, in terms of a change in BMD in favour of the group of patients receiving the NRTI-sparing regimen; however, what emerged from sensitometer data collected was that only a Total Body DXA was used (that is, analysis of the entire body), and this cannot be used for the purposes of diagnosing osteoporosis. In the NEAT study (in which 805 HIV patients naïve were randomised at 78 clinical centres in 15 countries to receive a regimen of Darunavir/r 800/100 four times daily + Raltegravir twice daily or TDF/FTC + Darunavir/r 800/100 four times daily), a DXA was performed of both the lumbar spine and femoral neck. The results clearly showed that performance in the NRTIsparing arm were higher than that in both traditional arms at the two sites concerned (5). In the sub-study of ACTG5257, carried out on bone metabolism (A5260s), DXA was used to analyse variations of BMD at the vertebral, femoral and total body level at week 96 in naïve subjects enrolled to take TDF/FTC together with atazanir/r or Darunavir/r or Raltegravir. The resultant mean percentage variations in BMD were higher in the arms related to the two PI/r compared to the RAL arm (femur: 3.7% vs -2.4%, p=0.005; spine -3.8% v -1.8%, p < 0.001). A5260s have shown that it is possible to construct, ab initio, a regimen that respects bone metabolism whilst including TDF/FTC, as long as the third drug is not a PI/r, but an integrase inhibitor (6). With the subsequent availability of a further INSTI, Elvitegravir, it was also possible to verify the effect of Elvitegravir (potentiated by Cobicistat) combined with TDF/FTC (E/C/F/TDF) on naïve subjects, always using a classic regime including TDF/FTC combined with Atazanavir/r as a comparison. In study 103, 353 subjects received E/C/F/TDF for 144 weeks. Più recentemente è stato iniziato lo studio WAVES, con analoghi bracci ma esclusivamente su una popolazione femminile, per analizzare il dato in questo specifico setting. I primi risultati dello studio a 48 settimane sono stati presentati oralmente da K. Squires allo IAS 2015 (#MOLBPE08) ma al momento non sono stati ancora pubblicati. Lo sviluppo di Tenofovir Alafenamide (TAF) e la sua coformulazione con Elvitegravir, Cobicistat e Emtricitabina in E/C/F/TAF, è stata creata per valutare se le differenti concentrazioni plasmatiche e cellulari rispetto a TDF si traducessero in un effetto positivo sulla tollerabilità clinica. Pertanto sono stati disegnati appositi studi di confronto, sempre su soggetti naïve, fra E/C/F/TDF e E/C/F/TAF (0102, 104 e 111), che hanno dimostrato come la performance di E/C/F/TAF sulla BMD sia significativamente migliore in entrambe le componenti ossee esaminate, vale a dire quella trabecolare (rachide lombare) e quella corticale (collo femorale) (8,9) [Figura 1 e 2]. Verosimilmente, pertanto, vi è una attivazione molto minore da parte di E/C/F/TAF sull’unità di rimodellamento osseo rispetto ad altriregimi, specialmente per quanto riguarda l’osso corticale, laddove raggiunge le performance migliori non solo rispetto a E/C/F/TDF, ma anche a tutti gli altri regimi antiretrovirali. Un dato ulteriore, molto prezioso dal punto di vista concettuale, è quello che viene dallo studio 106 su 50 adolescenti naïve [Figura 3], vale a dire su soggetti in età che precede il picco di massa ossea, quando ancora l’attività osteoblastica eccede quella osteoclastica: l’effetto di E/C/F/TAF si avvicina molto all’andamento fisiologico compared to the traditional regimen Atazanavir/r + TDF/FTC was significantly improved, both at the level of the spine (-1.43% vs. -3.68%) and the femur (-2.83 % vs. -3.77%) (7). Most recently the WAVES study began, with similar arms but exclusively on a female population, to analyse the data in this specific setting. The first results of the study at week 48 were presented orally by K. Squires at IAS 2015 (# MOLBPE08), but have not as yet been published. The development of Tenofovir Alafenamide (TAF) and its coformulation with Elvitegravir, Cobicistat and Emtricitabine in E/C/F/TAF, was created to assess whether the different plasma and cellular concentrations relative to TDF were translated into a positive effect on clinical tolerability. Therefore, specific studies were designed to compare, always on naïve subjects, E/C/F/TDF and E/C/F/TAF (0102, 104 and 111), which have shown that the performance of E/C/F/TAF on BMD was significantly better in both bone components examined, namely that of trabecular bone (lumbar spine) and the cortical bone (femoral neck) (8.9) [Figures 1 and 2]. It is likely therefore, that there is a very minor activation by E/C/F/ AF on the unit of remodelled bone with respect to these regimens, especially with regard to the cortical bone, where the best performance was achieved, not only with regard to E/C/F/ TDF, but also to all other anti-retroviral regimens. Tutto questo sin qui argomentato vale per quanto riguarda l’utilità dell’impiego degli INSTI nei soggetti naïve. Resta, però, il capitolo più interessante da esaminare, vale a dire la performance degli INSTI sulla BMD negli studi di switch, per verificare se il cambio di classe nei soggetti virosoppressi possa portare ad un recupero del danno 14 when osteoblastic activity exceeds osteoclastic activity: the effect of E/C/F/TAF is very close to the physiological performance of BMD in this particular age group (12-17 years) (10). All this has so far argued about the utility of using INSTI in naïve subjects. However, the most interesting chapter remains to be explored, namely the performance of INSTIs on BMD CURE ~ 2015 Articolo Originale/Original Article osseo provocato dai regimi tradizionali. Come per i soggetti naïve, i primi studi relativi all’utilizzo degli INSTI nello switch hanno riguardato Raltegravir, il primo resosi disponibile. Nello studio monobraccio TROP sono stati arruolati pazienti con HIV virosoppressi osteopenici o osteoporotici in terapia con TDF/ FTC + PI/r per una media di 3 anni. In questi soggetti passare da TDF/FTC a Raltegravir si è dimostrata una strategia in grado di aumentare la BMD e di ridurre il turnover osseo sia a livello femorale sia vertebrale a 48 settimane (11). Nello studio KITE, in aperto, della durata di 48 settimane, 60 pazienti con HIV virosoppressi sono stati randomizzati a passare a Lopinavir/r 400/100 mg BID + Raltegravir 400 mg BID o a continuare la HAART standard, ma in questo caso l’andamento della BMD non ha mostrato differenze significative fra i due bracci (12). Nello studio SECOND-LINE oltre 500 pazienti con HIV in fallimento virologico con un regime basato su NNRTI + TDF/FTC sono passati ad uno basato su PI /r + Raltegravir o su PI/r + altro backbone. In questo studio il braccio di terapia basato su PI/r + inibitori dell’integrasi mostra una perdita di BMD minore rispetto all’altro braccio: verosimilmente la necessaria immissione in terapia di un PI/r (a sostituzione dell’NNRTI) annulla in parte l’effetto protettivo ottenibile a livello della BMD dall’utilizzo dell’INSTI (13). Per superare la problematica connessa al mantenimento di un PI/r nel regime, si è ritenuto utile intervenire con un meccanismo sostitutivo basato non più sul backbone, quanto sul PI/r, costruendo quindi regimi basati su un INSTI insieme a TDF/FTC, analogamente a quando descritto in precedenza per i soggetti naïve. Nello studio SPIRAL-LIP, lo switch alla combinazione tra un backbone costituito da due NRTI (comprendente TDF nel 65% dei casi) e Raltegravir in 39 pazienti con HIV virosoppressi, precedentemente trattati con backbone + PI/r ha dato un risultato positivo sul recupero della BMD a 48 settimane sia a livello femorale sia vertebrale nei confronti dei 35 pazienti che hanno invece continuato il regime tradizionale PI/r + backbone (14). Abbiamo precedentemente richiamato come lo sviluppo di Tenofovir Alafenamide (TAF) e la sua coformulazione con Elvitegravir, Cobicistat e Emtricitabina in E/C/F/TAF, sia stata creata per valutare se le differenti concentrazioni plasmatiche e cellulari rispetto a TDF si traducessero in un effetto positivo sulla tollerabilità clinica e di come gli studi sopra menzionati abbiamo dimostrato come questo sia vero sulla BMD dei soggetti naïve che iniziano questo regime. Si rendeva necessario a questo punto verificare se l’utilizzo di Elvitegravir con TAF (E/C/F/TAF), come nei soggetti naïve, portasse un beneficio in termini di BMD anche nei soggetti già in terapia antiretrovirale aviremici. Nello studio 109, 1436 pazienti virosoppressi con tre regimi diversi comprendenti TDF (E/C/F/TDF, EFV/TDF/FTC, o ATV/r /TDF/FTC) sono stati randomizzati 2:1 a continuare il loro regime o a passare a E/C/F/TAF. Il miglioramento della BMD nel braccio in E/C/F/TAF è stato statisticamente significativo [Fi- CURE ~ 2015 in switch studies, to check whether a change of class in virally supressed subjects might lead to the repair of bone damage caused by traditional regimes. As for naïve subjects, the first studies relating to the use of INSTIs in a switch concerned Raltegravir, the first made available. In the single arm TROP study, patients with virally supressed osteopenic or osteoporotic HIV were enrolled on treatment with TDF/FTC + PI/r for an average of 3 years. In these subjects, passage from TDF/FTC to Raltegravir was shown to be a strategy to increase BMD and decrease bone turnover at both a femoral and vertebral level at 48 weeks (11). In the KITE study, which was open-label and lasted 48 weeks, 60 patients with virally supressed HIV were randomized to switch to Lopinavir/r 400/100 mg twice daily + Raltegravir 400 mg twice daily, or to continue on the standard HAART, but in this case, the BMD trend showed no significant differences between the two arms (12). In the SECOND-LINE study, over 500 HIV patients in virological failure with a regimen based on NNRTI + TDF/FTC were switched to PI/r + Raltegravir or PI/r + another backbone. In this study the treatment arm, based on PI/r + integrase inhibitors showed a loss of BMD that was lower than the other arm: probably the necessary release from treatment for a PI/r (a substitution of the NNRTI) cancelled part the protective effect obtainable at the BMD level from using the INSTI (13). To overcome the problems associated with maintaining a PI/r in the regimen, it was considered useful to intervene with a substitute mechanism based not on the backbone, as with PI/r, but building regimes based on a INSTI with TDF/FTC, similar to those previously described for naïve subjects. In the SPIRAL-LIP study, a switch to the combination of a backbone consisting of two NRTIs (including TDF in 65% of cases) and Raltegravir in 39 patients with virally supressed HIV, previously treated with backbone + PI/r provided a positive result on BMD recovery at 48 weeks, at both the femoral and vertebral level for the 35 patients who continued the regimen instead of the traditional PI/r + backbone (14). We have previously mentioned the development of Tenofovir Alafenamide (TAF) and its co-formulation with Elvitegravir, Cobicistat and Emtricitabine in E/C/F/TAF, which was created to assess whether the different plasma and cellular concentrations compared to TDF translated into a positive effect on clinical tolerance, and how the aforementioned studies showed this to be true in respect of the BMD of naïve subjects who start this regimen. It becomes necessary at this point to verify whether the use of Elvitegravir with TAF (E/C/F/TAF), as in naïve subjects, brings any BMD benefit to aviral subjects who are already on antiretroviral therapy. Uno dei punti cruciali della terapia antiretrovirale è quello che riguarda l’approccio ai pazienti con insufficienza renale in terapia antiretrovirale. Lo studio 112 ha permesso di verificare la performance di E/C/F/TAF nei soggetti in terapia antiretrovirale 16 TDF/FTC) were randomized 2: 1 to continue with their regimen or switch to E/C/F/TAF. In conclusione, certamente molte sono le variabili in grado di condizionare l’andamento della BMD in corsi di terapia antiretrovirale e la tossicità ossea non può essere mai imputata singolarmente ad una singola molecola, configurandosi piuttosto come una tossicità di regime. Se consideriamo nel soggetto naïve una riduzione di circa il 2-6% di massa ossea entro le prime 48-96 settimane di terapia antiretrovirale con i regimi tradizionalmente in uso, possiamo affermare che E/C/F/ TAF si configura come un regime fra i più sicuri per il metabolismo CURE ~ 2015 in bone metabolism in this particular setting of subjects. The switch study NCT02071082 was performed on subjects coinfected with HIV/HBV, and allowed us to evaluate changes in bone turnover markers in the delicate setting of patients who switch from E/C/F/TAF, and documented a minor activation of unit bone remodelling by determining a marker for neoformation, another for resorption, and for the parahormone (17) [Figure 7].
