OSSIGENOTERPIA NORMOBARICA PER USO TOPICO: UNA SERIE CLINICA DI CASI Dipartimento di chirurgia, Madeyski Paolo, Babbo Andrea, Bartelloni Antonio Corezzola Antonio, Servizio di Terapia Antalgica Amato Giuseppe, Casa di Cura Rizzola, San Donà di Piave (VE) ESTRATTO I problemi vascolari legati a deficit arterioso e all’insufficienza venosa a carico degli arti inferiori rappresentano la causa principale delle lesioni trofiche degli arti inferiori. Tale patologia rappresenta un grosso problema medico ma anche sociosanitario. Si stima che in Italia tale patologia colpisca l’1% della popolazione e il 5% della popolazione over 65 anni.. La spesa sanitaria è stimata superiore al miliardo d’euro l’anno. L’ossigenoterapia è un elemento importante per la guarigione delle ferite. L’ossigenoterapia iperbarica nel trattamento delle ferite croniche ha evidenziato che l’iperossigenazione della ferita aumenta la sua granulazione, la formazione del tessuto, ed accelera la diminuzione dei diametri della ferita e la chiusura secondaria. Tuttavia la base fisiologica di questa modalità rimane in gran parte sconosciuta. Va rilevato ad ogni modo che l’ossigenoterapia iperbarica presenta dei rischi derivanti dalla tossicità dell’ossigeno, delle controindicazioni generali e dei costi non trascurabili.. L’ossigenoterapia per uso topico pur rappresentando un metodo poco studiato per la cura delle ferite rileva indiscutibili vantaggi, quali: Un basso costo, Mancanza di tossicità sistemica Facilità di ricevere il trattamento domiciliare e tutto ciò rende possibile ottenere benefici dell’ossigenoterapia per molti pazienti. Per più di un anno un’equipe composta da 4 chirurghi e da un anestesista specialista in terapia antalgica hanno trattato 72 pazienti che presentavano ulcere di varia origine agli arti inferiori con ossigenoterapia per uso topico, con un periodo di cura variabile da 1 a 8 mesi; i dati qui presentati sono un’analisi retrospettiva dei risultati acquisiti usando ossigeno topico su ferite complesse. risultati: 51 ferite sono guarite durante la cura, in 2 pazienti le ferite si sono ripresentate dopo la guarigione e in 2 pazienti sono comparse lesioni sull’arto controlaterale a distanza di tempo, in 13 ferite l’ossigeno per uso topico non ha avuto effetto e due di questi pazienti hanno richiesto l’amputazione dell’arto: infine 8 pazienti hanno mostrato miglioramenti soggettivi e oggettivi pur non arrivando alla guarigione Non ci sono stati effetti collaterali attribuibili all’ossigeno topico, in quanto il decesso di 2 pazienti durante e dopo la terapia è stato determinato da cause assolutamente non collegate. Due pazienti inoltre hanno abbandonato le cure durante la terapia. conclusioni: In tutta questa serie di casi l’ossigeno per uso topico, non ha avuto effetti negativi ed ha evidenziato positive indicazioni nella guarigione delle ferite. INTRODUZIONE INTRODUZIONE Le ulcere trofiche degli arti inferiori rappresentano una delle patologie che più frequentemente si riscontrano nei pazienti anziani. Vari studi evidenziano una frequenza variabile tra lo 0.4% e il 4-5% tra gli ultrasessantenni; la cronicità della malattia comporta, secondo gli autori inglesi, costi elevati valutabili tra le 200 e le 4000 sterline (pari a 300-6000 euro) per il trattamento di un solo malato per 4 mesi. A tali cifre vanno ovviamente aggiunti i costi familiari e sociali. I tempi necessari per ottenere la guarigione di tali lesioni sono molto lunghi e variabili secondo la malattia di base: la durata media di un ulcera varia è di circa 26 settimane con un range variabile tra le 4 settimane e i 30 anni e in molti casi non è possibile ottenere la completa chiusura della lesione. Nel 46% dei pazienti il decorso supera le 26 settimane e nel 15% i due anni. Le sedi più colpite sono in ordine di frequenza le facce mediale, laterale, anteriore della caviglia, i piedi e la superficie posteriore della caviglia . La storia evolutiva di tali lesioni è caratterizzata da decorso cronico ed ingravescente,da facili recidive,da una prognosi quo ad valetudinem severa con pesanti ripercussioni sulla vita lavorativa di tutti i giorni e sulla vita di relazione con pesanti costi sociali. Per ulcera s’intende una perdita di sostanza legata a modificazioni emodinamiche, emoreologiche e coagulative:nella sua genesi è fondamentale l’impegno del microcircolo che può essere primitivo o secondario e che determina una compromissione del trofismo tissutale. Le ulcere rappresentano spesso un epifenomeno di numerose patologie che hanno alla base un apporto ematico insufficiente con conseguente ipossia ed infezione(Bimonte). Comunque indipendentemente dalla patologia di base, la ossimetria-transcutanea ha evidenziato che a livello delle lesioni vasculopatiche la pO2 arriva a valori di 5-10 mm di Hg che sono incompatibili con la vita delle cellule.Inoltre i leucociti proliferano e svolgono la loro attività fagocitica a pO2 di 30-40 mm di Hg ed è evidente che una situazione di ipossia facilita la scarsa attività dei leucociti con pericolo di infezioni. Anche la sintesi del collageno a partenza dai fibroblasti non può prescindere dall’ossigeno: solo in presenza di ossigeno infatti si può formare un collagene ottimale partendo da una tripletta di amoniacidi idrossilati. In condizioni di ipossia avremo dunque un collagene immaturo e poco stabile con inevitabili problemi di cicatrizzazione (Bi-monte,Oriani,Petrolati,Marrone). Quindi indipendentemente dalle cause che hanno provocato la formazione dell'ulcera,traumi,turbe del circolo sia arterioso che venoso,per ottenere la riepitelizzazione sono necessari: a)un ottimale apporto di Ossigeno, b)una completa detersione della piaga, c)una stimolazione cellulare, d)una buona perfusione ematica. Allo scopo di raggiungere queste quattro condizioni vengono proposte ed utilizzate numerose terapie ma tutti noi sappiamo quanto i risultati siano molto spesso deludenti per il medico ed il paziente: i tempi necessari per ottenere la chiusura della lesione sono sempre molto lunghi e in molti casi non è possibile ottenere la completa guarigione della lesione. Una terapia che si è molto affermata in questi ultimi anni è la cosiddetta ossigenoterapia in camera iperbarica che è basata sulla respirazione di ossigeno ad alta pressione in apposite camere dette iperbariche. Essa agisce con due meccanismi (Oriani, Marrone): a) aumento della quota di O2 disciolto che si sostituisce funzionalmente all'ossigeno legato alla Hb quando questo sia in difetto per mancanza della stessa (anemia) o per incapacità funzionale (intossicazione da CO), b) ripristino della diffusione dell'O2 dai capillari alle cellule laddove questa è impedita o per diminuzione della perfusione ematica o per ispessimento dei mezzi di transito. Il fine ultimo è dunque quello di portare ossigeno in quantità adeguate alle necessità metaboliche dei tessuti. Tale terapia si è certamente rivelata efficace in molte sindromi ma purtroppo la sua pratica presenta numerosi svantaggi e precisamente: l'esistenza di pochi centri, problemi di trasporto per i pazienti, incapacità di molti pazienti a sopportare la terapia per motivi psicologici o per altre patologie, costi elevati. Rifacendosi alla bibliografia esistente (Van, Padberg, Williams, Fisher, Kaufman, Ignacio, Hammarlund,)e nel tentativo di ovviare a tali problematiche, abbiamo studiato e messo a punto una piccola camera, la camera distrettuale normobarica di Madeyski Tale apparecchiatura, così chiamata per la "somiglianza" con la camera total body, è nata allo scopo di poter sottoporre all’ossigenoterapia solo l'arto interessato mantenendo lo stesso in un’atmosfera d’ossigeno in elevata concentrazione monitorizzando inoltre la pressione, l'umidità della miscela gassosa e se necessario la concentrazione dell'ossigeno all'interno della camera stessa. La camera è costituita da un contenitore in Petg trasparente termoformato a forma di parallelepipedo, della dimensioni di cm 70 per 30 per 35, diviso in due parti, la superiore e l’inferiore, scorrevoli lungo un braccio che possono essere chiuse tramite un apposito fermo; l'arto viene introdotto da un lato attraverso un'apertura circolare all'interno della quale possiamo applicare delle guarnizioni circolari intercambiabili, con foro di diametro variabile, di materiale validato per uso medicale allo scopo di dare un tenuta ermetica a tutto il dispositivo. Sulla superficie superiore della camera troviamo un igrometro per misurare l'umidità all'interno della camera.. Da notare infine che nella camera esiste una valvola ad acqua che ha lo scopo di far uscire il gas quando la pressione all'interno della camera supera quella prefissata dall'ope-ratore: questo perché pressioni elevate comprometterebbero il circolo già di per se deficitario. L'ossigeno umidificato, introdotto tramite un ugello posto all'estremità opposta dell'apertura suddetta, viene insufflato sulla lesione tramite un condotto flessibile e orientabile dotato di una punta con diffusore del gas e posto a 5 cm di distanza dalla lesione stessa. A livello della lesione si ottiene così una concentrazione di O2 del 95% mentre l’ossigeno disciolto nel sangue non si modifica a meno che non si faccia respirare il malato con una mascherina d’ossigeno. Numerosi sono i vantaggi di tale dispositivo e precisamene: a) il basso costo con conseguente elevato numero di macchine, b) le dimensioni e il peso ridotti e conseguente facilità di trasporto della camera anche al domicilio dei pazienti, c) nessuna controindicazione locale e sistemica, d) ottima compliance dai parte dei pazienti. In effetti l'uso di tale camera sia presso la nostra casa di cura, sia presso altri centri ospedalieri nell’anno 2003 e nella versione domiciliare con la quale abbiamo eseguito e seguito le terapia sui casi che sono oggetto del presente lavoro si è rivelato molto soddisfacente. a) b) c) d) e) Con questo studio ci proponiamo: di determinare in quali condizioni può essere impiegato l’ossigeno per uso topico, di suggerire delle indicazioni precise pur nel rispetto della decisionalità di ogni medico, di standardizzare la metodica di tale terapia, di valutarne l’efficacia anche confrontandola con altre terapie, di spiegarne, se possibile, il meccanismo di azione. 2. PATOLOGIE TRATTATE 2.1 materiali e metodi 2.1.1 selezione dei pazienti Tutti i pazienti sono stati seguiti da equipe multidisciplinare composta: da tre chirurghi generali con indirizzi vascolari, da un chirurgo plastico da un anestesista specialista in terapia antalgica che fanno parte del Comparto di Chirurgia Generale della Casa di Cura Rizzola accreditata dal SSN a San Donà di Piave ( ULS N 10 Veneto Orientale I pazienti selezionati dopo una valutazione per la loro eleggibilità a tale terapia, hanno eseguito l’ossigenoterapia per via topica per un periodo variabile da mesi 1 a mesi 9 dal gennaio 2003, quando questa metodica è stata standardizzata nel nostro Istituto con proseguimento di terapia a domicilio. Non ci sono stati criteri specifici predeterminati d’esclusione od inclusione, né alcuna controindicazione per l’utilizzo dell’ossigeno per via topica salvo il fatto di principio che si trattassero di lesioni trofiche. Tutti i soggetti selezionati erano a conoscenza del tipo di terapia, della possibilità o meno della sua efficacia, dei costi che tale terapia comporta e si erano impegnati a farsi seguire regolarmente con le cadenze da noi designate. La selezione non ha incluso piccole ferite acute che potevano guarire normalmente senza terapia specifica o con presidi tradizionali. I soggetti in esame presentavano ferite che generalmente non erano guarite con i metodi tradizionali, o vi era un alto rischio di problemi generali legati a possibilità chirurgiche o a patologie che controindicavano l’iperbarismo. Per questi pazienti, l’ossigeno topico è stato considerato come alternativa ai metodi di cura tradizionali. 2.1.2 L’OSSIGENOTERAPIA L’ossigeno per via topica è stato somministrato direttamente sulle ferite con il dispositivo da noi brevettato e del quale si allega la descrizione e le foto. Tale dispositivo chiamato Camera iperbarica distrettuale di Madeyski solo per analogia alla Camera Iperbarica Tradizionale lavora in normobarismo. È stato somministrato ossigeno al 100% ad 1 atmosfera di pressione per un tempo di 60 minuti due volte al giorno sette giorni sui sette sia in ambiente ospedaliero che domicialiare Tale terapia è stata ripetuta fino a che la ferita non ha dimostrato segni di guarigione, o con la cicatrizzazione chirurgica, o fino a che risultava evidente che il trattamento non stava avendo successo (evidenziato da una aumento della ferita, o da nessun progresso dopo settimane di trattamento). Questo trattamento con ossigeno, non è stato affiancato dai metodi tradizionali. Le medicazioni erano eseguite solo da detersione del materiale essudato con garza imbevuta da soluzione fisiologica prima di ogni seduta di ossigeno . A Carico del paziente vi era il costo del noleggio del dispositivo 2.1.3 DOCUMENTAZIONE DELLO STATO DELLE FERITE Fotografie sono state fatte prima, durante ed alla fine della terapia con una camera digitale, e un programma appositamente studiato ha permesso di comparare le modifiche riguardanti la superficie, i diametri e la circonferenza, oltre a valutare il tipo di superficie e la granulazione della superficie. Le ferite sono state considerate guarite quando completamente coperte con epitelio. 2.1.4 ANALISI DEI PARAMETRI I dati dei pazienti sono stati archiviati per singola patologia, data della presenza della ferita prima di iniziare l’ossigenoterapia, cause della ferita, localizzazione e dimensione, durata del trattamento, e risultato finale. 2.1.2 STATISTICHE L’analisi di questi casi viene riassunta da grafici che mostrano le varie percentuali di guarigione e di miglioramenti dei vari parametri considerati. RISULTATI Per un anno (il periodo preso in esame per questo studio) l’equipe composta dai chirurghi generali, dal chirurgo plastico e dall’anestesista hanno trattato a secondo le loro competenze ma sempre collegialmente 72 pazienti con ossigenoterapia per via topica, con cure di durata da 1 a 9 mesi. Non ci sono state complicazioni associate all’ossigenoterapia topica, Tale terapia è stata usata per trattare ferite postchirurgiche, ulcere da decubiti, ulcere flebostatiche, ulcere neuropatiche del piede, ulcere del piede diabetico, ed ulcere indotte da traumi acuti e ulcere iatrogene e farmacologiche. Le lesioni erano localizzate nelle estremità inferiori. DI PAZIENTI 10 2 4 53 8 2 5 3.1 RISULTATI SUI PAZIENTI Delle 72 ferite, 51 (il 70%) sono guarite durante il trattamento con ossigenoterapia per via topica. Delle rimanenti 21 ferite, 8 sono guarite dopo un intervento chirurgico di trapianto associato a successiva ossigenoterpia, 13 non sono guarite; di queste 3 pazienti sono morti durante il trattamento e 3 pazienti hanno abbandonato la cura. Se includiamo le guarigioni postoperatorie la percentuale di ferite completamente guarite con trattamento d’ossigenoterapia per via topica sale al 81%. La percentuale di guarigioni s’incrementa al 86% se non vengono compresi nel numero totale i pazienti che hanno abbandonato la cura, in quanto lo stato finale delle loro ferite non è stata determinata e quelle deceduti per cause non correlate alla malattia di base. I pazienti che hanno interrotto la terapia non sono inclusi in alcun’analisi elencata nelle successive tabelle. 3.2 PAZIENTI CON FERITE SOTTOPOSTE A TRAPIANTO Otto pazienti sono stati sottoposti a trapianto di cute dopo trattamento con ossigenoterapia. Su tutti e otto i pazienti vi è stato un ottimo attecchimento dell’innesto cutaneo. Solamente su due pazienti, a distanza di due mesi, si è manifestata un’iniziale recidiva dell’ulcera che però è stata efficacemente contrastata dall’utilizzo dell’ossigenoterapia. L’alta percentuale di successo dell’intervento d’innesto cutaneo, su pazienti con ulcere distrofiche, deve essere imputata all’eccellente qualità del tessuto di granulazione sul quale è stato posizionato l’innesto e all’attenuazione dei fattori causali dell’ulcera (infezione, stasi, etc.). 3.3 Terapia del dolore nelle vasculopatie Certamente il sintomo dolore è presente nelle malattie vascolari in genere e nelle lesioni trofiche in particolare rappresentando anzi il sintomo che più spesso porta il paziente dal medico. La terapia di questo sintomo rientra in un approccio globale al problema della patologia ulcerosa che deve mirare innanzitutto ad eliminare tutti i fattori di rischio (fumo, ipercolesterolemia, miglioramento del microcircolo e del ritorno venoso, ecc) al fine di creare a livello della lesione un ambiente ideale per la cicatrizzazione (asepsi, buona per- fusione ed ossigenazione, ecc). Il ruolo dell’antalgologo in generale sarà quello di impostare una terapia del dolore razionale non solo ricorrendo ai vari farmaci, ad azione periferica e/o centrale,ma anche consigliandone un uso corretto sia in termini di dosaggio che di modalità di somministrazione. Più specificamente il terapista del dolore potrà intervenire anche con metodiche più invasive quali blocchi anestetici singoli o continui allo scopo di agire sul microcircolo in maniera selettiva e sullo spasmo che sempre è presente in quei tessuti coinvolti nelle varie sindromi dolorose. Nella nostra esperienza nel corso dell’anno 2003 siamo intervenuti 12 volte con blocchi anestetici , mentre in altri 18 casi si è ricorso a dosaggi valutati caso per caso di farmaci ad azione antalgica. A-guariti solo con ossigenoterapia(51) 70% B- non guariti con l’ossigenoterapia (13) 19% C- pazienti che hanno abbandonato la cura (3) 4% PAZIENTI CON FERITE NON GUARITE D- morti durante il trattamento per ragioni non Due pazienti con ferita non guarita, hanno richiesto l’amputazione dell’arto.4% Questi pazienti sono relazionabili con la terapia(3) stati catalogati nel nostro protocollo tra quelli che non hanno beneficiato della guarigione E- guariti con l’ossigenoterapia combinata con con l’ossigenoterapia; l’amputazione ha permesso la guarigione e la remissione della sintomatologia in entrambi innesto chirurgica (8) 9 %. i pazienti. Sommando i dati di A+E la percentuale di guarigione si attesta sul 81% La morte di 3 pazienti durante la terapia, non ha relazione con l’ossigenoterapia, e nel nostro protocollo sono catalogati come non guariti. Tutti 3 i pazienti erano diabetici e cardiopatici e avevano gravi problemi respiratori. Sono deceduti per complicanze cardiache 3.2 DURATA DEL TRATTAMENTO E RISPOSTA DELLE FERITE La durata del trattamento è variata da mesi 1 a mesi 9. Questo significa che il tempo di terapia è stato da 60 ore 540 ore. La durata del trattamento è stata mediamente da 1 a 4 mesi con due soli casi di 5 mesi, uno di 7 mesi e uno di 9 mesi.. La durata del trattamento per le ferite che sono guarite 30 a 120 giorni Inizialmente le dimensioni delle ferite variavano da 0.29 a 92.2 cm² con una media di 13,5 21,4 cm². Per le ferite che sono guarite la dimensione iniziale era da 8.111,1 cm², e per le ferite che non sono guarite la media iniziale era 25.3 ± 32 cm². Tra le ferite che non sono guarite il decremento medio nelle dimensioni è stato 6.5 ± 15.9 cm² e la percentuale media di decremento nelle dimensioni è stata da 71 ± 63.3%. Quattro ferite sono aumentate di dimensioni durante il trattamento. Il paziente che è in terapia da 9 mesi è stato incluso ma razionalmente per le dimensioni e le cause che concorrono alla sua genesi e al suo perdurare non offre possibilità di guarigione anche se sono apprezzabili miglioramenti soggettivi e obiettivi. 3.3 TEMPO DI PRESENZA DELLA FERITA Al momento dell’inizio della terapia con ossigeno per via topica, la genesi della ferita datava approssimativamente da giorno 15 (postraumatica) a 3 anni con un tempo medio di 10 mesi ( escluso un caso che era presente da 12 anni) I pazienti con ferite acute hanno avuto il 100% di guarigioni, comparate con l’93% di guarigione presenti nei trattamenti con durata inferiore ai 4 mesi e con il 80% di guarigioni nei trattamenti con durata superiore ai 4 mesi. Con le ferite croniche la terapia chirurgica è stata usata in combinazione con l’ossigenoterapia topica nel 80% dei pazienti prima della ossigenoterapia e nel 12% dopo e in combinazione con la ossigenoterapia e sono guariti. 3.4 LOCALIZZAZIONE DELLA FERITA La sede considerata delle ferite che sono state elegibili del nostro trattamento e poste in osservazione è stata circoscritta agli arti inferiori e più precisamente alla gamba. Tale localizzazione è stata presa in considerazione in relazione al dispositivo che abbiamo brevettato e che è stato pensato per tale sede in considerazione della frequenza di sede delle lesioni trofiche ( vascolari e diabetiche) Per le ferite croniche abbiamo notato una risposta migliore alla ossigenoterapia nelle lesioni localizzate nella faccia mediale della gamba ( in genere ulcere flebostatiche) e in quelle del piede ( piede diabetico). Risultati meno confortanti sono stati quelli con localizzazione nella faccia laterale della gamba forse in relazione alla loro genesi arteriosa. Tra le ferite croniche, una buona risposta all’ossigenoterapia si è avuta nei pazienti con ulcere flebostatiche e ulcere diabetiche con una percentuale di guarigione rispettivamente del 85% e90% . D’altra parte dobbiamo notare che le ulcere in soggetti arteriopatici la guarigione è stata più lenta e in modo particolare il beneficio soggettivo è stato meno evidente specie per quanto riguarda il sintomo dolore. La figura 6 è importante in quanto dimostra come la presenza di più eventi morbosi abbia un rilievo significativo sulla risposta e questo è chiaro confrontando le ulcere acute che hanno una risposta in percentuale maggiore : generalmente le ferite meno sensibili a guarire con l’ossigeno per via topica sono state le ferite post-chirurgiche e post traumatiche alle estremità inferiori, seguite dalle ulcere flebostatiche e da quelle diabetiche. FIG. 3 AREA DELLA FERITA Le ferite trattate con ossigeno per via topica sono diminuite nelle dimensioni; in quest’analisi 51 delle 72 ferite sono diventate più piccole con il solo trattamento d’ossigenoterapia. I 7 pazienti non inclusi in quest’analisi sono i tre che hanno abbandonato la terapia,i tre deceduti e ed il paziente che aveva una ferita molto ampia. La media delle dimensioni delle ferite non guarite è stata sostanzialmente più grande di quelle che sono guarite Si deduce che la dimensione è un’importante variabile nel determinare se e quando usare questa terapia, alternativa ai metodi tradizionali. Le ferite guarite di dimensione media di 10 cm2 si sono cicatrizzate in media in 70gg., quelle non guarite di dimensione media di 30 cm2, si sono ridotte del 30% con una durata media della terapia di 120 gg. CONCLUSIONI Questa è stata un’analisi sull’efficacia terapeutica dell’ossigeno per via topica per le ferite croniche o con scarse possibilità di una completa guarigione localizzate agli arti inferiori. Abbiamo considerato varie di lesioni trofiche con diverse eziologie per valutare quali lesioni rispondessero più efficacemente al trattamento, e premesso che, pur avendo rilevato come risposta negativa, solo un aspetto soggettivo dato da fastidio al trattamento, rileviamo che non vi è stato alcun effetto collaterale con l’ossigenoterapia.Anzi in genere la compliance dei pazienti è stata ottima, mentre i risultati oggettivi sono discussi nella relazione. Quest’analisi pur se relativamente limitata dimostra che la maggior parte delle ferite croniche e tutte le ferite acute localizzate agli arti inferiori diminuiscono di dimensione durante la terapia, e dimostra che l’ossigeno per via topica è utile supporto per la guarigione. E’ presumibile che anche le ferite localizzate agli arti superiori e le lesioni da decubito possano avere beneficio da tale terapia La completa guarigione è stata acquisita in 59/72 ferite, ma la chirurgia complementare è stata necessaria per 8 ferite croniche per completare la chiusura, mentre in tutti casi la pulizia chirurgica ha sempre preceduto la ossigenoterapia. Per le estremità inferiori l’ossigeno topico può essere di aiuto alla chirurgia, come nella terapia iperbarica per la preparazione all’innesto od al trapianto e noi lo abbiamo visto in tutti i casi nei quali tale metodica è stata applicata ( 8 casi su 72). Tutto ciò necessita di indicazioni standardizzate e precise pur nella libertà decisionale del medico nel determinare dei limiti specifici alla terapia. Il medico curante ha un’ampia discrezione nel prescrivere questa tecnica come alternativa a quell’iperbarica ma deve attenersi ai principi di efficacia dimostrata.. Il raggiungimento del giusto equilibrio tra l’uso dell’ossigeno topico per la completa guarigione con l’uso dello stesso come terapia coadiuvante al trattamento chirurgico è importante per ottimizzare gli effetti ed i costi di questa procedura. Ottenere questo tipo di informazioni aiuterà ad affinare la selezione dei pazienti e migliorare il tasso di guarigione delle ferite Comparando l'ossigeno topico con gli altri metodi di cura delle ferite, riteniamo importante considerare il tempo di guarigione . E' difficile determinare la frequenza di guarigioni delle ferite usando l'ossigeno iperbarico e le opinioni nei lavori pubblicati è controversa . Negli ultimi dieci anni, sono stati fatte tre prove con controlli casuali e solo uno di questi studi riporta la completa guarigione come dato di fatto. Nello studio statistico controllato a doppio cieco con placebo da Bouachour e. comparando l'efficacia della camera iperbarica con la terapia standard per le ferite da schiacciamento, essi riportano un 94,4 % di tasso di guarigione in pazienti trattati nella camera iperbarica (100 % d’O2 a 2.5 atm) contro un 55,5 % di tasso di guarigione di pazienti che avevano ricevuto il placebo (21 % O2 a 1.1 atm.). E' importante notare come questi studi valutino solo i casi acuti. Un maggiore chiarezza sui risultati può essere ottenuta comparando il tasso di guarigione delle ferite al “Wound Care Centre “ diretta dal “ National Healing Corporation (NHC). Controllando i dati di 6 mesi appare un tasso globale di guarigione delle piaghe pari a 91 % su 16 settimane che include, ma non è limitato al solo uso dell’ossigenoterapia iperbarica. Il periodo di sedici settimane è frequentemente usato come punto di riferimento per valutare un piano terapeutico. Lo studio da noi fatto ha un valore non solo osservazionale ma anche prospettico. E ci promettiamo in un prossimo futuro uno studio comparativo tra l’efficacia dell’ossigenoterapia normobarica distrettuale e quella iperbarica nelle camera tradizionale sui trattamenti delle lesioni trofiche degli arti inferiori. E non solo sull’efficacia ma anche sui costi e sull’impatto sociosanitario ed economico. Il nostro tempo medio di trattamento con solo ossigeno topico è stato di 12 settimane con un tasso di guarigione del 68 %. Se includiamo quei pazienti trattati con metodologie chirurgiche aggiuntive ( trapianto o innesto) il nostro tasso di guarigione sale al 80 %. Facendo una selezione più accurata dei pazienti ( come indicazione) pensiamo di raggiungere tassi di guarigione migliori con la ossigenoterapia topica distrettuale. Tale miglioramento sarà raggiunto con un accorgimento che abbiamo potuto osservare nel corso di questo anno; la sola ossigenoterapia raggiunge migliori risultati se la ferita è medicata giornalmente da personale competente . Tenendo presente il limitato numero di pazienti che possono usufruire della camera iperbarica tradizionale e il largo spettro di controindicazioni per questa terapia combinata con una limitata disponibilità di centri, determina che molti pazienti non possono ricevere questo trattamento. Recentemente il dispositivo per ossigeno topico domiciliare ha permesso ad un numero certamente maggiore di pazienti di usufruire di questa terapia seppure modificata. L’unica limitazione attualmente presente è data dal costo, seppur modesto, che al momento è a carico del paziente. Si spera che il SSN riconoscendo tale terapia come efficace possa contribuire tenendo presente in modo particolare il rilevante risparmio che ne deriverebbe al SSN stesso. I vantaggi dell'ossigeno topico consistono anche nella possibilità per i pazienti di ricevere il trattamento in casa propria associata al fatto che non ci sono rischi o controindicazioni conseguenie all'uso del dispositivo. Tuttavia tale terapia soffre della stessa sfiducia dovuta all'empirismo, come per la camera iperbarica, nel determinare quale sia il migliore utilizzo. I nostri dati rappresentano uno dei primi lavori osservazionali su una serie di pazienti dove si è usata la terapia dell'ossigeno topico per la cura delle ferite. Fino a quando il meccanismo dell'ossigenoterapia non sarà meglio compreso, siamo costretti a fidarci delle analisi critiche delle nostre esperienze attraverso revisioni di serie di casi come questi, e valutazioni cliniche di statistiche per ottimizzare l'uso di questa nuova opportunità. NORMOBARIC OXYGEN THERAPY FOR LOCAL USE: A CLINICAL SERIES OF CASES Surgery Department : Madeyski Paolo, Babbo Andrea, Bartelloni Antonio, Corezzola Antonio, Analgesic Therapy Service Amato Giuseppe, Casa di Cura Rizzola, San Donà di Piave (VE) EXTRACT The vascular problems tied to arterial deficit and venous insufficiency at the expense of the lower limbs represent the main cause of trophic injuries to the lower limbs. This pathology represents a great medical problem but also a health and social one. It is estimated that in Italy this pathology affects 1% of the population and 5% of the population over 65 years of age. The health cost is estimated to be greater than one billion euros a year. Oxygen therapy is an important element in the healing of injuries. Hyperbaric oxygen therapy in the treatment of chronic injuries has shown that hyperoxygenation of the injury increases its granulation, the formation of tissue, and accelerates the reduction in the diameters of injuries and secondary closure. Nevertheless the physiological basis of this method remains largely unknown. It has been revealed that in any case hyperbaric oxygen presents risks deriving from the toxicity of the oxygen, general side effects and costs that cannot be neglected. Oxygen therapy for local use, although representing a method which has not been well studied in the treatment of injuries does reveal indisputable advantages such as: Low cost Lack of systemic toxicity Facility of receiving treatment at home and all this makes it possible to obtain benefits from oxygen therapy for a lot of patients. For more than a year a team composed of 4 surgeons and an anesthesiologist specialized in analgesic therapy treated 72 patients with ulcers of various origins in the lower limbs with oxygen therapy for local use, with a period of treatment ranging between 1 and 8 months; the data presented here is a retrospective analysis of results acquired using topical oxygen on complex injuries. Results: 51 injuries were cured during the treatment, in 2 patients the injuries represented themselves after healing and in 2 patients injuries appeared to the contralateral limb after some time, in 13 injuries oxygen for local use had no effect and two of these patients requested amputation of the limb: finally 8 patients showed subjective and objective improvement though not being healed. There were no collateral effects attributable to the topical oxygen, since the death of 2 patients during and after the therapy was determined by causes totally unrelated. In addition two patients abandoned treatment during the therapy. conclusions: In all this series of cases the oxygen for local use did not have negative effects and showed positive indications in the healing of the injuries. INTRODUCTION INTRODUCTION Trophic ulcers of the lower limbs represent one of the pathologies most frequently found in elderly patients. Various studies show a variable frequency between 0.4% and 4-5% among over sixty year-olds; the chronicity of the illness involves high costs, according to English authors, evaluated at between 200 and 4000 pounds (equal to 300-6000 euros) for the treatment of just one sick person for 4 months. To these costs obviously the costs of family and companions must be added. The time necessary to reach healing of these injuries is very long and variable depending on the basic illness: the average length of an ulcer varies and is about 26 weeks with a range varying between 4 weeks and 30 years and in many cases it is not possible to obtain complete closure of the injury. In 46% of patients the course exceeds 26 weeks and in 15% two years. The places most affected are, in order of frequency, the medial faces, side, front of the ankle, the feet and the rear surfaces of the ankle. The evolutive history of such injuries is characterized by the chronic and worsening course, with easy recurrence, with a severe quo ad valetudinem prognosis. with heavy repercussions on everyday working life and on family life with heavy social costs. Ulcer means a loss of substance connected to hemodynamic, hemorheologic and coagulative modifications: in its origin the commitment of the microcirculation is fundamental, which may be primitive or secondary and determines an impairment of the tissue trophism. The ulcers often represent an epiphenomenon of numerous pathologies which have an insufficient hematic supply with consequent hypoxia and infection (Bimonte). However independently of the basic pathology, the transcutaneous oximetry has shown that at the level of vasculohepatic injuries p02 reaches values of 5 – 10 mm of Hg which are incompatible with the life of the cells. In addition the leukocytes proliferate and carry out their phagocytic activity at p02 with 30-40mm of Hg and it is evident that a situation of hypoxia facilitates the scarce activity of the leukocytes with a risk of infection. Even the synthesis of collagen starting from the fibroblasts cannot take away the importance of oxygen; only in the presence of oxygen in fact can an optimal collagen form starting with a treble of hydroxilated amoniacids. In conditions of hypoxia we shall thus have an immature, unstable collagen with inevitable healing problems (Bimonte, Oriani, Petrolati, Marrone). Therefore independently of the causes that have led to the formation of the ulcer, traumas, circulation disorders both arterial and venous, in order to obtain re-epithelialization, the following are necessary: a) An excellent oxygen supply; b) Complete cleansing of the sores; c) Good blood perfusion. In order to attain these four conditions numerous therapies are proposed and used but we all know how results are very often disappointing for the doctor and the patient: the time necessary to obtain closure of the injury is often very long and in many cases it is not possible to obtain complete healing of the injury. One therapy that has been highly acclaimed in recent years is the so-called oxygen therapy in the hyperbaric chamber which is based on breathing oxygen at high pressure in specific hyperbaric chambers. They act with two mechanisms (Oriani, Marrone): a) Increase in the rate of dissolved 02 which functionally substitutes oxygen connected to Hb when there is a defect through lack of the same (anemia) or for functional incapacity (intoxication of 02); b) Recovery of the distribution of 02 from the capillaries to the cells where this is prevented either for reduction of blood perfusion or for thickening of the means of transit. The last aim is that of taking oxygen in suitable quantities to the metabolic needs of the tissues. This therapy has certainly proved to be effective in many syndromes, however its practice presents numerous disadvantages which more specifically are: There are few centres; Transport problems for the patients Incapacity of many patients to bear the therapy for psychological reasons or for other pathologies High costs. Returning to the existing bibliography (Van, Padberg, Williams, Fisher, Kaufman, Ignacio, Hammerlund) and in the attempt to get round such problems, we have studied and set up a small chamber, Madeyski’s normobaric local chamber. This equipment, so called for the “similarity” to the total body chamber, was born with the aim of being able to submit to oxygen therapy only the limb concerned keeping this in an atmosphere of high concentration oxygen while monitoring the pressure, the humidity of the gassy mix and if necessary the concentration of oxygen inside the chamber itself. The chamber consists of a transparent Petg container thermoformed in a parallelepiped form, measuring 70 cm by 30 by 35, divided in two parts, the upper and the lower, running along an arm which can be closed by means of a special lock; the limb is introduced from one side through a circular opening inside which we can apply circular interchangeable seals, with a hole of differing diameters, with material convalidated for medical use in order to give a hermetic hold to the whole device. On the upper surface of the chamber we find a hygrometer to measure the humidity inside the chamber. Finally it should be noted that inside the chamber there is a water valve to let the gas out when the pressure inside the chamber exceeds that prefixed by the operator: this is because high pressures would compromise the circulation which is already in deficit. The humidified oxygen introduced through a flexible tube which can be orientated and fitted with a tip with a gas diffuser and placed 5 cm from the injury itself. At the level of the injury a concentration of 02 of 95% is thus obtained while the dissolved oxygen in the blood is not modified unless the patient is made to breathe with an oxygen mask. There are numerous advantages with this device, namely: a) The low cost and consequent high number of machines; b) The reduced size and weight and consequent ease of transport of the chamber even to the home of the patient; c) No local and systemic side effects; d) Excellent compliance on the part of the patients. In effect the use of this chamber both in our care home and in hospitals in the year 2003 and in the domestic version with which we have carried out and followed the therapy on cases which are the object of this current work and have proved to be very satisfying. With this study we propose: a) To determine in what conditions oxygen for local use can be used; b) To suggest precise indications with respect for the decision making of each doctor; c) To standardize the method of each therapy; d) To evaluate the effectiveness even comparing it with other therapies; e) To explain, if possible, the mechanism of action. 2. PATHOLOGIES TREATED 2.1 Materials and methods 2.1.1 selection of patients All the patients were followed by multi-disciplinary teams composed of: by three general surgeons in the vascular field, by a plastic surgeon by an anesthetist specialized in analgesic therapy, part of the Department of General Surgery of the Casa di Cura Rizzola, recognized by the National Health Service, in San Donà di Piave (ULS N10 Veneto Orientale). The patients selected from an evaluation of their eligibility for such therapy, underwent oxygen therapy locally applied for a period varying from 1 to 9 months from January 2003, when this method was standardized in our institute followed by therapy at home. There were no specific pre-determined criteria of exclusion or inclusion, nor other side effects for the use of oxygen applied locally save for the principle that it concerns trophic injuries. All subjects selected were aware of the type of therapy, of the possibility or otherwise of its effectiveness, of the costs that such therapy entails and they undertook to be followed regularly at a pace designed by us. The selection did not include small acute injuries that could heal normally without specific therapy or with traditional remedies. The subjects under examination presented injuries that generally were not healed by traditional methods or there was another risk of general problems connected to surgical possibilities or pathologies that indicated against Hyperbarism. For these patients topical oxygen was considered as an alternative to methods of traditional treatment. 2.1.2 OXYGEN THERAPY Oxygen by local means was administered directly on the injury with the device patented by us and for which we attach the descriptions and the photos. This device, called Madeyski’s local (topical) hyperbaric chamber only for the analogy with the traditional hyperbaric chamber works in normobarism. 100% oxygen was administered to 1 atmospheric pressure for a period of 60 minutes twice a day for seven days out of seven both in a hospital and at home. This therapy was repeated until the injury showed signs of healing either with surgical healing or until it was evident that the treatment was having no success (shown by a worsening of the injury, or by no progress after weeks of treatment). This treatment with oxygen was not supported by traditional methods. The medication was carried out only by cleansing of the material exudated with a gauze soaked in saline solution before each oxygen sitting. The cost of the hire of the device was borne by the patient. 2.1.3 DOCUMENTATION OF THE STATE OF THE INJURY Photographs were taken before, during and at the end of the therapy with a digital camera, and a carefully studied programme enabled comparison with the modifications regarding the surface, the diameters and the circumference besides evaluating the type of surface and the granulation of the surface. The injuries were considered healed when completely covered with epithelium. 2.1.4 ANALYSIS OF PARAMETERS The patients’ data has been filed by single pathology, date of presence of the injury before beginning oxygen therapy, causes of injury, location and dimension, duration of treatment and final result. 2.1. 2 (5??) STATISTICS The analysis of these cases is summarized by graphs which show the various percentages of healing and improvement of the various parameters considered. RESULTS For a year (the period taken into examination for this study) the team composed of general surgeons, plastic surgeon and anesthetist treated 72 patients with oxygen therapy locally administered, in accordance with their competence but always collegially with treatment lasting from 1 to 9 months. There were no complications associated with the local oxygen therapy. This therapy was used to treat post-surgical injuries, decubitis ulcers, phlebostatic ulcers, neuropathic ulcers of the foot, diabetic ulcers of the foot and ulcers induced by acute trauma and iatrogenic and pharmacological ulcers. The injuries were localized in the lower extremities. Of the remaining 21 injuries, 8 were healed after a surgical transplant intervention associated with subsequent oxygen therapy, 13 were not healed; of these 3 patients died during the treatment and 3 patients abandoned the treatment. If we include post operation healing the percentage of injuries completely healed with oxygen therapy treatment by local means rises to 81%. The percentage of healing increases to 86% if the patients who abandoned treatment in the total number since the final state of their injuries was not determined and those deceased for causes not correlated with the basic illness are not included. The patients who interrupted the therapy are not included in any analysis listed in the following tables. 3.2 PATIENTS WITH INJURIES UNDERGOING TRANSPLANT Eight patients underwent skin transplant after treatment with oxygen therapy. In all eight patients the skin grafting had excellent results . Only with two patients, after two months, an initial recurrence of the ulcer manifested itself which, however, was effectively dealt with by oxygen therapy. The high percentage of success in the skin grafting operation on patients with dystrophic ulcers must be attributed to the excellent quality of the tissue of granulation on which the graft was positioned and the attenuation of the factors causing the ulcer (infection, stasis etc). 3.3 Pain therapy in vascular diseases Certainly the pain symptom is present in vascular disease in general and in trophic injuries in particular, representing, in fact, the symptom which most often takes the patient to the doctor. The therapy of this symptom falls within a global approach to the problem of ulcerous pathology which should aim above all at eliminating all the risk factors (smoking, hypercholesterolemia, improvement of micro-circulation and venous recovery etc) with the goal of creating at the place of the injury an ideal environment for healing (aseptis, good for fusion and oxygenation etc). The role of the “antalgologo” in general will be that of imposing a therapy of rational pain not just turning to various medicines, to peripheral and/or central action but also advising correct use both in terms of dosage and mode of administration. More specifically the pain therapist could also intervene with more invasive methods such as single or continuous anesthetic blocks with the aim of acting on the micro-circulation in a selective way and on the spasm which is always present in those tissues involved in the various painful syndromes. In our experience in the course of 2003 we intervened 12 times with anesthetic blocks, while in another 18 cases we turned to dosages of analgesic action medicine evaluated case by case. -----------------------------------------------------Table 2 – RESULTS ON THE PATIENTS % OF HEALING of 72 Patients treated with topical oxygen having completed the therapy during a period of 12 months of study. A - healed only with oxygen therapy (51) 70% B - not healed with oxygen therapy (13) 19% C - patients who abandoned the treatment (3) 4% D - deaths during treatment for reasons not related to the therapy (3) 4% E – healed with oxygen therapy combined with surgical skin grafting (8) 9% Summing up the data of A+E the healing percentage is 81% The duration of the treatment varied from 1 to 9 months. This signifies that the time of therapy was from 60 to 540 hours. The duration of the treatment was on average from 1 to 4 months with just two cases of 5 months, one of 7 months and one of 9 months. The duration of treatment for injuries that healed was from 30 to 120 days. Initially the sizes of injuries varied from 0.29 to 92.2 cm2 with an average of 13.5 +_ 21.4 cm. For the injuries that healed the initial size was 8.1 +_ 11.1 cm2 and for injuries that did not heal the initial average was 25.3+_ 32 cm2. Among the injuries that were not healed the average size was 6.5 +_ 15.9 cm2 and the average decrease in size was 71 +_ 63.3%+ Four injuries increased in size during the treatment. The patient who was under therapy for 9 months was included but rationally for the sizes and causes that relate to his genesis+ and his continuing do not offer the possibility of healing even if they are appreciable subjective and objective improvements. 3.3 TIME OF PRESENCE OF THE INJURY At the time of the start of the therapy with topical oxygen, the genesis of the injury dated approximately from day 15 (post traumatic) to 3 years with an average time of 10 months (excluding a case which was present for 12 years). The patients with acute injuries had 100% healing compared with 93% healing in cases with a duration of less than 4 months and with 80% healing in treatments lasting more than 4 months. With chronic injuries the surgical therapy was used in combination with topical oxygen therapy in 80% of patients before oxygen therapy and in 12% after and in combination with oxygen therapy and were healed. 3.4 LOCALIZATION OF THE INJURY The position considered of the injuries that were eligible for our treatment and placed under observation was limited to the lower limbs and more precisely to the leg. This localization was taken into consideration in relation to the device we patented and which was thought of for this position in consideration of the frequency of the position of trophic injuries (vascular and diabetic). For chronic injuries we noted a better response to oxygen therapy in injuries localized in the medial aspect of the leg (in general phlebostatic ulcers) and in the foot (diabetic foot). Less comforting results were those situated in the side aspect of the leg perhaps in relation to their arterial genesis. Among the chronic injuries a good response to oxygen therapy was in patients with phlebostatic ulcers and diabetic ulcers with a percentage of healing respectively of 85% and 90%. On the other hand we should note that with ulcers in arteriopath subjects healing was slower and in particular the subjective benefit was less evident especially as regards the pain symptom. Figure 6 is important since it shows how the presence of more morbid events has a significant effect on the response and this is clear comparing acute ulcers which have a greater response percentage: generally the injuries less sensitive to healing with topical oxygen have been postsurgical and post-traumatic injuries to the lowest extremities, followed by phlebostatic and diabetic ulcers. FIG. 3 AREA OF THE INJURY ------------------------------AREA OF THE INJURY cm2 DAYS -----------------The injuries treated with topical oxygen diminished in size; in this analysis 51 out of 72 injuries became smaller just with oxygen therapy treatment. The 7 patients not included in this analysis are the three who abandoned the therapy, the three deceased and the patient who had a very large injury. The average in size of the injuries that were not healed was substantially greater than those which healed. It can be deduced that size is an important variable in determining if and when to use this therapy, an alternative to traditional methods. The injuries that healed with an average size of 10 cm2 were healed, on average, in 70 days, those that did not heal with an average size of 30 cm2 were reduced by 30% with average therapy duration of 120 days. CONCLUSIONS This has been an analysis of the therapeutic effectiveness of topical oxygen for chronic injuries with scarce possibility of complete healing localized in the lower limbs. We considered various trophic injuries with different etiologies to evaluate which injuries would respond more effectively to the treatment and, provided that, although having revealed as a negative response just one subjective aspect irritated by the treatment, we can reveal that there was no collateral effect with the oxygen therapy. In fact, in general, the compliance of the patients was excellent while the objective results are discussed in the report. This analysis, while being relatively limited shows that most chronic injuries and all acute injuries localized in the lower limbs diminish in size during therapy and show that oxygen topically applied is a useful support for healing. It is presumable that even injuries localized in the upper limbs and decubitis injuries can gain benefit from this therapy. Complete healing was achieved in 59/72 injuries but complementary surgery was necessary for 8 chronic injuries to complete closure while in all cases surgical cleansing always preceded oxygen therapy. For the lower extremities topical oxygen can be of help to surgery as in hyperbaric therapy for the preparation of grafting or for transplant and we have seen this in all cases in which this method was applied (8 cases out of 72). All this necessitates standardized and precise indications also in the freedom of decision making by the doctor in determining the specific limits of the therapy. The doctor providing treatment has ample discretion in prescribing this technique as an alternative to hyperbaric but must adhere to the principles of effectiveness demonstrated. Reaching the right equilibrium between the use of topical oxygen for complete healing with the use of the same as adjuvant therapy to surgical treatment is important to optimize the effects and costs of this procedure. Obtaining this type of information will help to evaluate the selection of patients and to improve the rate of healing of injuries. Comparing topical oxygen with other methods of treatment of injuries we maintain the importance of considering the time of healing. It is difficult to determine the frequency of healing of injuries using hyperbaric oxygen and the opinions of works published is controversial. In the last ten years three tests with random checks have been made and just one of these studies has reported complete healing as a fact. In the statistical study controlled with the outcome unknown with Bouachour’s placebo and comparing the effectiveness of the hyperbaric chamber with standard therapy for injuries from crushing, it reports a 94.4% rate of healing in patients treated in the hyperbaric chamber(100% of O2 at 2.5 atm) against a 55.5% rate of healing in patients who received the placebo (21% O2 at 1.1atm). It is important to note how these studies evaluate only acute cases. Greater clarity on the results could be obtained comparing the rate of healing of injuries in the “Wound Care Centre” directed by the “National Healing Corporation (NHC)”. Checking the data of 6 months a global rate of healing of sores appears equal to 91% over 16 weeks which includes, but is not limited to just the use of hyperbaric oxygen therapy. The period of sixteen weeks is frequently used as a point of reference in evaluating a therapeutic plan. The study we have done has a value which is not just observational but also prospective. And we promise in the near future a comparative study between the effectiveness of local normobaric oxygen therapy and the traditional hyperbaric chamber for the treatment of trophic injuries in the lower limbs. And not just on the effectiveness but also on the costs and the social-health and economic impact. Our average time of treatment with just topical oxygen was 12 weeks with a rate of healing of 68%. If we include those patients treated with added surgical methods (transplant or grafting) our rate of healing rises to 80%. Making a more accurate selection of patients (as an indication) we think we can achieve better rates of healing with local topical oxygen therapy. This improvement will be reached with what we have learned in our observations over the course of this year; just oxygen therapy achieves better results if the injury is medicated daily by competent personnel. Keeping in mind the limited number of patients that can use the traditional hyperbaric chamber and the wide spectrum of side effects for this therapy combined with limited availability of centres, determines that many patients cannot receive this treatment. Recently the device for topical oxygen at home has enabled a greater number of patients to take advantage of this modified therapy. The only limitation at present is the cost, though modest, which at present is borne by the patient. It is hoped that the SSN recognizing this therapy as effective may contribute in particular taking into consideration the relevant saving to the SSN itself. The advantages of topical oxygen also lie in the possibility for patients to receive the treatment in their own homes together with the fact that there are no consequent risks or side effects in using the device. However this therapy suffers from the same mistrust due to empiricism, as with the hyperbaric chamber in determining which is the best use. Our data represents one of the first works of observation on a series of patients where topical oxygen therapy is used for treatment of injuries. Until the mechanism of oxygen therapy is not better understood, we are compelled to trust the critical analyses of our experiences through a revision of series of cases like these and clinical evaluations of statistics to optimize the use of this new opportunity.
