In adults, the “first and worst” headache raises concerns for aneurismal; subarachnoid hemorrhage. In children, this clinical pattern is most commonly due to febrile illness related to an upper respiratory tract infection-secondary headache. Secondary headaches 5 temporal patterns CAUSES OF HEADACHE ACUTE IN CHILDREN Secondary headaches 5 temporal patterns ACUTE--RECURRENT ACUTE Acute-recurrent headaches imply pyap pattern of attacks of head pain separated by symptom-free intervals. Primaryy headache syndromes y such as migraine g or tension-type headache usually cause this pattern. Recurrent headaches could be attributed to epileptic p p syndromes y ((eg, g, benign g occipital p epilepsy), p p y), substance abuse, or recurrent trauma. Secondary headaches 5 temporal patterns CHRONIC PROGRESSIVE Chronic p progressive g headache represents p the most ominous of the temporal patterns and implies ag gradually y increasing g frequency q y and severityy of headache. The pathological correlate is increasing intracranial pressure. Causes off thi C this pattern tt include i l d pseudotumor d t cerebri, brain tumor, hydrocephalus, chronic meningitis, i iti brain b i abscess, b or subdural bd l collections ll ti Secondary headaches 5 temporal patterns DIFFERENTIAL DIAGNOSIS FOR CHRONIC PROGRESSIVE HEADACHE Secondary headaches 5 temporal patterns CHRONIC PROGRESSIVE HEADACHE KEY POINTS Secondary headaches 5 temporal patterns CHRONIC NONPROGRESSIVE ((CHD)) Chronic nonprogressive or chronic daily headache (CDH) represents a pattern of frequent or constant headache. CDH is generally defined as 4 months history of 15 headaches/month with headaches lasting g 4 hours. Affected p patients have normal neurological g examinations. There are usually interwoven psychological p y g factors and g great anxiety y about underlying organic causes. Secondary headaches 5 temporal patterns MIXED Mixed headache pattern represents the superimposition i iti off acute-recurrent t t headache (usually migraine) upon a chronic daily background pattern, and therefore, represents a variant of CDH. Parto eutocico. Peso alla nascita 3.800 gr. Buon adattamento alla vita extrauterina Anam. fisiologica: vaccinazioni secondo legge Anam legge, nella norma l’acquisizione delle principali tappe dello sviluppo psicomotorio psicomotorio. Acute headache Case Report • APR: due episodi p di convulsione febbrile (rispettivamente a 18 mesi ed a 2 anni) • APP: giunge alla nostra osservazione inviato dal pediatra di famiglia per riferita sintomatologia cefalalgica, insorta da circa un n mese mese, che non beneficia di terapia ed a sede temporo-parietale dx Acute headache Case Report Esame neurologico: nei limiti di norma per età Esame obiettivo: nei limiti, eccezion fatta per la presenza alla palpazione del cranio di una tumefazione di consistenza duro elastica della grandezza di circa 3x3 cm in sede parietale i t l d dx non mobile bil ed d adesa d aii piani i i sottostanti la cute Acute headache Case Report Eseguita E it Rx R C Cranio: i area di ridotta id tt iintensità t ità ossea tecale rotondeggiante diametro di 20 mm Acute headache Case Report Eseguita RNM encefalo con e senza mdc + TC. In sede parietale dx al vertice,, posteriormente p è apprezzabile una lesione osteolitica che erode quasi completamente il tavolato cranico di morfologia ovalare di mm 22 x 16 circa. Acute headache Case Report La lesione è dotata di intenso enhacement ed infiltra i tessuti molli pericranici profondamente f d t non dissociabile dalla dura madre che sembra in parte essere coinvolta, ipertintensa e ispessita. La neoformazione impronta le circonvoluzioni cerebrali sottostanti nel cui contesto non si apprezzano pp fenomeni edemigeni. Acute headache Case Report Acute headache Case Report Federico viene sottoposto ad: • Intervento NCH: asportazione lesione di consistenza molle e colore giallo-rosso • E Esame istologico: i t l i i un contesto in t t riccamente i t infiltrato di granulociti eosinofili, presente densa proliferazione di cellule di Langerhans (Cd1a,S(Cd1a,S 100:+). La lesione è preda di fenomeni necrotico-emorragici e mostra una frazione di crescita it parii a circa i 2 2-3 3 mitosi it i XHPF • Diagnosi: Istiocitosi a cellule di Langerhans Acute headache Case Report Istiocitosi: gruppo di disordini proliferativi g sul p piano clinico,, estremamente eterogeneo caratterizzato da infiltrazione e accumulo di cellule nel sistema reticolo-endoteliale reticolo endoteliale nei tessuti interessati Acute headache Case Report Federico, esegue stadiazione dell’istiocitosi: • Esame ecotomografico dell’addome: nella norma • Rx rachide cervicale, dorsale, arti superiori e inferiori: non lesioni ossee a focolaio • RX torace per coste, lomboscrale: non lesioni ossee a f focolaio l i pleuro parenchimali • Rx torace: non alterazioni pleuro-parenchimali evolutive in atto, normali gli ili, il mediastino, il cuore ed i grossi vasi …...GRANULOMA EOSINOFILO ISOLATO Conclusioni • Raccogliere accuratamente la storia del pz e del suo disturbo, secondo una terminologia appropriata per età. età • Dedicare tempo e pazienza alla visita, all’esame neurologico ed obiettivo obiettivo. • Saper riconoscere i segnali di allarme e/o esplorare la natura t del d l sintomo i t ““cefalea”(es. f l ”( di diario i che h d documenti ti frequenza, intensità, durata, grado di disabilità, presenza di sintomi associati, uso di farmaci) • Conoscere i pattern temporali Ciò costituisce un requisito necessario non solo per una una corretta diagnosi g di una cefalea secondaria ma anche per un suo adeguato trattamento.
