Thorax 2008; 63:738-746 a 10% increase in weight is associated with 6-fold greater risk of developing OSA among persons initially free of OSA Peppard PE, Young T, Palta M, et al. Longitudinal study of moderate weight change and sleep disordered breathing. Wisconsin cohort (n=690) with a 4-year follow-up JAMA. 2000;284:3015-3021) 2968 men and women (62 years) Conclusion: Modest changes in weight were related to an increase or decrease in SDB, and this association was stronger in men than in women. Arch Intern Med. 2005;165:2408-2413 A Randomized Study on the Effect of Weight Loss on Obstructive Sleep Apnea Among Obese Patients With Type 2 Diabetes The Sleep AHEAD Study G.D. Foster et alt 264 participants with type2 diabetes , age of 61.2 (6.5) years, BMI of 36.7and an apnea-hypopnea index (AHI) of 23.2 (16.5) events per hour Vol 169(No. 17),SEP 28, 2009 The participants were randomly assigned to either a behavioral weight loss program (intensive lifestyle intervention [ILI]) or 3 group sessions related to effective diabetes management [DSE]. The ILI participants lost more weight at 1 year than did DSE participants (10.8 kg vs 0.6 kg; P.001). At 1 year, more than 3 times as many participants in the ILI group (13.6%) than in the DES group (3.5%) had total remission of their OSA, and the prevalenceof severe OSA among ILI participants was half that of the DSE more than twice as many ILI participants, compared with DSE participants, demonstrated improvement in their OSA category (eg, severe to moderate, moderate to mild, and mild to none) over1 year Lifestyle Intervention with Weight Reduction First-line Treatment in Mild Obstructive Sleep Apnea Henri P. I. Tuomilehto Methods :72 paz (BMI, 28–40) with mild OSA were recruited. The intervention group (n = 35) completed the VLCD program with supervised lifestyle modification, and the control group (n = 37) received routine lifestyle counseling. The apnea–hypopnea index (AHI) was the main objectively measured outcome variable. . Results: reduce body weight (–10.7 ± 6.5 kg; body mass index, –3.5 ± 2.1 There was a statistically significant difference in the mean change in AHI between the study groups (P = 0.017). Conclusions: VLCD combined with active lifestyle counseling resulting in marked weight reduction is a feasible and effective treatment for the majority of patients with mild OSA, and the achieved beneficial outcomes are maintained at 1-year follow-up. American Journal of Respiratory and Critical Care Medicine Vol 179. pp. 320-327, (2009) 2007 CHEST 2005 128:618-623 CHEST 2005 128:618-623 CONCLUSION: Morbidity obese men with OSAS have a reduced pharingeal crosssectional area. A weight reduction of about 15% of baseline body weignt may substantially increase the pharyngeal cross-sectional area and substantially improve the severity of OSAS in morbidly obese subjects with sleep apnea.
