WHAT IS THE PULSED LIGHT ? La luce pulsata intensa (IPL) è una terapia non invasiva e non ablativa che utilizza impulsi di luce visibile ad alta intensità. E’ meno invasiva della luce laser e agisce a minore profondità, senza danneggiare gli strati più superficiali della cute e con risultati assai più soddisfacenti. L’apparecchio IPL MED è un apparecchio medico-estetico di luce pulsata intensa, controllato da un microcomputer che può lavorare emettendo singoli impulsi, oppure impulsi ripetuti (fino a 3), di cui è possibile regolarne la densità di energia (Fluenza=J/cm2). La combinazione appropriata di questi parametri permette di calibrare opportunamente la luce pulsata in funzione delle caratteristiche del pelo consentendo un’eliminazione progressiva e definitiva e la dotazione di filtri consente di ampliare il trattamento su lesioni vascolari e dermatologico (acne, per esempio). The Intense Pulsed Light (IPL) is a non-invasive and non-ablative therapy which employs pulses of visible light at a high intensity. It is less invasive than the laser light and it works into lesser depth, without damaging the most superficial layers of the skin and producing much more satisfactory results. The IPL MED is an Intense Pulsed Light equipment intended for a medical-aesthetic use, which is controlled by a microcomputer that can be operated both in single or repeated pulses (up to 3). The operator can adjust the energy density (Fluency=J/cm2). The right combination of these parameters allows to properly calibrate the pulsed light according to the hair characteristics, and therefore to obtain a progressive and permanent elimination of the hairs.The different filters are studied to extend the treatment to other skin problems, such as vascular and dermatological damages. (acne, for example). A COSA SERVE ? La luce pulsata viene utilizzata in medicina estetica per migliorare l'aspetto della pelle e curarla in caso di lesioni vascolari, macchie, couperose, acne rosacea, vene varicose, rughe, lentiggini e soprattutto rimozione di peli indesiderati. La luce pulsata può anche essere usata, in diverse sessioni di trattamento, per curare l'acne e le smagliature cutanee (strie rubare). Gli effetti della Luce Pulsata Intensa sono spesso superiori a quelli ottenuti con il laser, sia per quanto riguarda la fotoepilazione che il fotoringiovanimento cutaneo, con ottimi risultati anche nella cura del melasma, delle macchie cutanee e dell’ invecchiamento cutaneo e con netto miglioramento progressivo delle fini rugosità, della lassità cutanea, dei pori dilatati. WHAT IS IT USED FOR ? The pulsed light is used in aesthetic medicine to improve the appearance of the skin and to treat it in case of vascular injuries, spots, couperose, acne rosacea, varicose veins, wrinkles, freckles and above all removal of unwanted hairs. Pulsed light can also be used, in different sessions, to treat acne and stretch marks (striae atrophicae). The effects of the Pulsed Light are often better than those obtained with the laser, both for photo-epilation and for skin photo-rejuvenation, with very good results also in the treatment of melasma, of skin spots and of skin ageing process and with net progressive enhancement of fine wrinkles, skin relaxation, and enlarged pores. I PELI SUPERFLUI E L’EPILAZIONE DEFINITIVA Il pelo ha un’esistenza “effimera”. Passa attraverso un periodo di crescita cui segue uno di riposo. Si parla di ciclo pilare, che si divide in tre fasi: • Fase anagen o di crescita: un nuovo pelo è generato dal follicolo che affonda nel derma e diventa molto attivo (sintesi di cheratina e melanina). • Fase catagen: è una fase di regressione. L’attività di sintesi cessa. Il bulbo si stacca dalla papilla e resta collegato tramite un cordone epiteliale formato dalla guaina epiteliale esterna. Questa fase dura circa 2 – 3 settimane. • Fase telogen o fase di riposo (fase non visibile del pelo): durante questa fase non si produce nessun pelo. La sua durata è variabile e dipende da molti fattori (sito anatomico, fattori esterni e fattori interni). Al termine della fase telogen il pelo viene espulso prima della successiva fase anagen . SUPERFLUOUS HAIRS AND PERMANENT HAIR REMOVAL The hair has an “ephemeral” life. This phase lasts about 2 - 3 weeks. • Telogen phase or rest phase (non-visible phase of the hair) : during this phase no hairs are produced. Its length is variable and depends from many factors (anatomical site, outside factors and inside factors). At the end of the telogen phase, the hair is expelled before the next anagen phase. Per questo è necessario ripetere più volte il trattamento di fotoepilazione: per ogni seduta si vanno a colpire i peli che si trovano in quel momento in fase anagen (20-30% dei peli presenti). not possible that all the follicles of the same area are simultaneously in anagen phase. LA FOTOEPILAZIONE I peli sono suscettibili agli effetti indotti dalla energia luminosa solo durante la fase attiva anagen del loro ciclo di crescita che di norma rappresenta circa 1/3 dei peli presenti sul corpo. Per questa motivazione, la luce pulsata è efficace nel determinare la caduta del pelo, solo quando penetra ed attraversa il follicolo pilifero in questa determinata fase. I peli, in altri stadi di crescita (telogen oppure catagen), raggiungeranno la fase sensibile in diversi momenti futuri. A distanza di un mese altri peli si troveranno in fase anagen e sarà quindi possibile ripetere il trattamento. Per una scomparsa definitiva dell’80% circa dei peli sulla zona trattata, saranno necessarie circa 4/6 sedute realizzate a cadenza mensile. Per questo motivo, sono necessari trattamenti multipli. La fotoepilazione è un processo basato sulla fotoemolisi selettiva grazie al quale la luce è fortemente assorbita dalla melanina contenuta nel pelo, che conseguentemente raggiunge alte temperature. Il calore accumulato è trasmesso alle strutture vitali del follicolo e per un processo di termocoagulazione si interrompe il ciclo del pelo, producendo così effetti di lunga durata: • la luce viene fortemente assorbita dalla melanina del pelo • il fusto si scalda • per trasmissione termica si ha coagulazione delle strutture riproduttive del follicolo • pelo e follicolo sono danneggiati e il ciclo di ricrescita si interrompe. THE PHOTO-EPILATION The hairs are sensitive to the induced effects of the bright energy only during the active anagen phase of their cycle of growth, that generally represents about 1/3 of the present hairs on the body. For this reason, the pulsed light effectively determines the fall of the hair, only when it penetrates and go through the follicle in this specific phase.The hairs, in other growth phases (telogen or catagen), will reach the sensitive phase in different future moments. After one month from the first treatment, other hairs will be in anagen phase and it will be therefore possible to repeat the treatment. For a definitive removal of around 80% of the hairs on the zone treated, it will be necessary to carry out about 4/6 sessions, one per month. For this reason, various treatments are necessary. Photo-epilation is a process based on the selective photo-hemolysis thanks to which the light is deeply absorbed by the melanin contained in the hair, that consequently reaches high temperatures. The heat accumulated is transmitted to the vital structures of the follicle and thanks to a process of thermal coagulation the cycle of the hair is interrupted, thus producing long-lasting effects: • the light is strongly absorbed by the hair melanin • the shaft is heated • for thermal transmission, the reproductive structures of the follicle are coagulated • hair and follicle are damaged and the cycle of re-growth is interrupted. La conseguenza pratica del trattamento con luce pulsata è una riduzione duratura del numero di peli ed un rallentamento globale della ricrescita, tanto più evidente ed efficace con i peli più grossi e scuri che costituiscono il maggior problema estetico. The practical result of the pulsed light treatment is a lasting reduction in the number of hairs and a global slowing of the re-growth, which is more clear and effective with the thickest and darkest hairs that constitute the major aesthetic problem. 25 IPL LUCE PULSATA La selettività di assorbimento permette di conseguire questo risultato senza danni all'epidermide. Poichè la luce pulsata è efficace solamente nella fase di crescita del pelo, sono necessarie più sedute per ottenere il risultato ottimale. Un ciclo variabile da 5 a 10 trattamenti, a seconda della zona interessata e dalle caratteristiche individuali, assicura un risultato esteticamente valido. Ad un ciclo iniziale di trattamenti, intervallati da uno o più mesi, seguiranno delle sedute di mantenimento. Zone come l'inguine e le ascelle, che presentano peli grossi e scuri, rispondono più rapidamente di altre. Prima di sottoporre la pelle al flash di luce pulsata ad alta intensità, si provvede all'applicazione di un apposito gel che agisce da protettore della cute. Si procede quindi all'invio degli impulsi di luce, emessi dal manipolo ad intervalli regolari. La dimensione dei manipoli, molto più grande rispetto al laser, permette di trattare velocemente ampie superfici cutanee. Si possono trattare aree anche estese (come ad esempio le gambe), ma anche circoscritte come il labbro superiore o la zona del mento. I risultati sono molto più veloci quanto maggiore è il contrasto tra pelle chiara e pelo scuro. A distanza di circa un mese, cresceranno i peli i cui bulbi non si trovavano nella fase attiva all'epoca del trattamento precedente, pronti ora per essere colpiti dalla luce pulsata. Il metodo è molto tollerato: anche utilizzando alte potenze, il paziente può avvertire solo una sensazione come di "leggero colpo di elastico" sulla pelle. È altamente consigliato di non esporsi al sole immediatamente prima o dopo le sedute è per non incorrere in possibili effetti indesiderati come le iperpigmentazioni. Gli effetti collaterali sono pressoché nulli, occorre comunque una preparazione professionale specifica. L’effetto della luce pulsata consiste nel determinare una riduzione della dimensione del bulbo pilifero, contrastando la crescita dello stesso fino alla sua completa eliminazione. In caso di ricrescita del pelo colpito, questa consisterà in peli molto più sottili e di dimensioni ridotte, nonché di colore più chiaro rispetto al precedente. Prima di effettuare un qualsiasi trattamento di epilazione è importante identificare il fototipo (da I aV) ed il grado di pigmentazione della persona che si vuole trattare. Per ottenere il miglior risultato dal trattamento di epilazione è necessario effettuare tale trattamento durante la fase anagen del pelo. Durante questa fase il pelo è in crescita quindi, mediante il trattamento con IPL, andiamo a colpire il bulbo pilifero nel pieno della sua attività avendo un risultato più efficace e duraturo. Altra condizione necessaria per ottenere la maggior efficacia dal trattamento di epilazione mediante IPL è che la lunghezza del pelo sia compresa tra n.02 e n.03 mm. Una maggiore lunghezza del pelo comporterebbe una dispersione eccessiva dell’energia luminosa con il mancato raggiungimento del target (bulbo pilifero) a discapito dell’efficacia del trattamento. Inoltre una lunghezza eccessiva potrebbe causare una eventuale bruciatura del pelo stesso con conseguenze trascurabili dal punto di vista di chi si sottopone al trattamento, ma importanti a livello di funzionamento del sistema IPL. Questo perché la bruciatura dei peli superflui provoca un intaccamento del filtro (o quarzo) IPL con conseguente diminuzione della luce trasmessa al bulbo pilifero. 26 IPL INTENSE PULSED LIGHT The absorption selectivity allows to achieve this result without damages to the epidermis. Since the pulsed light is effective only during its growth phase, more sessions are necessary to obtain an important result. A variable cycle from 5 to 10 treatments, according to the interested area and to individual features, ensures a valid aesthetic result. After an initial cycle of treatments, around one per month, some sessions of maintenance will follow. Areas like the groin and the armpits, that present thick and dark hairs, can be removed more rapidly than the others. Before submitting the skin to the flash of the high intensity pulsed light, a dedicated gel will be put on the area to protect the skin. The light pulses are then sent by means of the probe, at regular intervals.The dimension of the probes, a lot larger with respect to the laser, allows to rapidly treat wide skin areas. It is possible to treat wider areas (like for example the legs), as well as smaller ones, like the upper lip or the chin. Results are much faster if the contrast between clear skin and dark hair is high. About a month later, the hairs whose bulbs were not within the active phase during the previous treatment, are now ready to be hit by the pulsed light . The method is highly tolerated: even when using high powers, the patient can just feel a sensation similar to a "light blow of rubber band" on the skin. It is extremely advised not to immediately expose oneself to the sun before or after the sessions in order not to provoke any possible unwanted effect like the hyperpigmentation. The side effects are nearly absent, yet it is necessary to have a specific professional training. The effect of pulsed light consists in determining a reduction in the dimension of the hair bulb, preventing its growth until its complete elimination. In case the hair hit re-grows, it will be much thinner, smaller and clearer in colour as opposed to the previous one. Before carrying out any depilation treatment,it is important to identify the skin phototype (from I toV) and the pigmentation degree of the person to treat. To obtain the best result from the treatment it is necessary to carry out such treatment during the anagen phase of the hair. During this phase the hair is growing, therefore by means of the IPL treatment, the hair bulb is hit during its major activity and the result is more effective and lasting. Another necessary condition to obtain the greatest effectiveness from the IPL treatment is that the length of the hair shall be included between 2 and 3 mms. A greater length of the hair would cause an excessive dispersion of the bright energy and the target (hair bulb) is not properly attained, decreasing the treatment effectiveness. Besides, an excessive hair length could cause a possible burning of the hair with negligible results for the person who is submitted to the treatment, but with important results as far as the IPL system is concerned. The burning of the superfluous hairs, namely, provokes a damaging of the IPL filter (or quartz) with consequent decreasing of the light transmitted to the hair bulb.
