Timeline of the black box warning A first official warning was issued in 2005 by the US Food and Drug Administration (FDA)19 regarding the increased mortality risk associated with use of antipsychotics in older people with dementia. This warning was based on the results of a pooled analysis20 of 17 randomized controlled trials, which reported a 1.7 times increased risk of mortality with atypical antipsychotic use in older adults with dementia when compared with placebo. The primary aim of this study wa evaluate the association between any antipsychotic prescription and CVEs in Italian elderly; second, to compare the effect of typical and atypical tipsychotics on CVEs; and third, to investigate the effect of antipsychotics on CVEs in the subgroup of persons coprescribed with acetylcholinesterase hibitors (AChEIs). Administrative claims from community-dwelling dwelling people aged 65 to 94 years living in Northern Italy were analyzed using a trospective case-control control design, from 2003 to 2005. The primary outcome measure was a hospital discharge diagnosis of CVEs during dur 2005. Four age-, x-, and local health unit-matched matched control subjects were identified for each case. Antihypertensive drugs, anticoagulants, platelet plat inhibitors, antidiabet ugs, lipid-lowering drugs, and AChEI were used as covariates in conditional al lo logistic regression models testing the odds ratio (OR) for CVEs due to tipsychotics use. In conclusion, only typical antips tipsychotics were associated with an increased odd of CVEs, ut the association was duration dependent. Se il trattamento è inefficace, sospendere gradualmente nte il farmaco e prendere eventualmente in considerazione un diverso composto. Se il trattamento è efficace, continuare a trattare e monitorare il soggetto per un periodo di 1-3 1 mesi e poi, una volta che il soggetto sia asintomatico, tentare di sospendere gradualmente il farmaco. Gli alti tassi di risposta al placebo in tutte le sperimentazioni effettuate (mediamente attorno al 40%) ci ricordano infatti che siamo in presenza di sintomi per loro natura fluttuanti nel tempo po e che tendono a risolversi spontaneamente nel breve periodo. • Evitare di somministrare due o più antipsicotici contemporaneamente. Take home message Evitare l’uso concomitante di antipsicotici e benzodiazepine. Questa associazione andrebbe fortemente limitata, soprattutto alla luce della dichiarazione dell’EMEA che riporta l’uso concomitante di benzodiazepine e olanzapina tra i fattori predisponenti associati all’aumento di mortalità. Monitorare attentamente sicurezza ed efficacia dei antipsicotici e segnalare tempestivamente tutti gli effetti indesiderati. indesiderati Somministrare con estrema cautela gli antipsicotici a soggetti con fattori di rischio cardiovascolare dopo attenta valutazione dello stato clinico e con rivalutazione dei parametri vitali (e in particolare della pressio essione in clino e in ortostatismo) a distanza di una settimana dall’inizio della terapia.
