Marasini 1 1 G.Gaslini Institute, Genova, 2 Di Venere and Sarcone Hospital, Bari, ITALY Objectives: to review the prenatal features of foramen ovale in foetuses with Transposition of the Great Arteries (TGA), focusing on those who needed of an urgent balloon atrial septostomy (BAS) for a restrictive interatrial communication at birth. Background: as already reported, prenatal diagnosis of TGA reduced neonatal mortality and morbidity. However, also neonates with prenatally diagnosed TGA may die immediately after birth because of the inadequate mixing at the level of the atrial septum. Therefore it is extremely important to assess the prenatal appearance of the atrial septum for having the chance to orchestrate the delivery conditions and achieve the optimal result from preoperative management and intraoperative conduct. Method: the cardiology department registry at the Gaslini Institute was retrospectively searched for all patients born between January 2000 and March 2013 with a prenatal diagnosis of TGA. Foetuses with ventricular septal defect were excluded from the study. Clinical data assessed were gestational age respectively, at the time of last fetal evaluation and at birth, delivery conditions, weight and oxygen saturation at birth. Prenatal echocardiographic data assessed were foramen ovale and septum primum (SP) appearance, ductal size and pulmonary veins flow pattern. Septum primum was considered hypermobile if oscillates between both atria, redundant if it herniates more than 50% beyond the plane of the septum secundum across the left atrium, restrictive if we noticed a hypo mobile, thick and fixed membrane with a small orifice. All foetuses were delivered in our Institution. For those who showed a non restrictive foramen ovale, a cardiologist was always in charge of immediate evaluation at the time of delivery in the maternity unit and both echocardiography and catheterization equipment were immediately available. For those foetuses with TGA and a restrictive foramen ovale, delivery was scheduled straight in the cardiac unit for performing the BAS in the shortest time as possible. A postnatal echocardiogram was always performed to confirm the diagnosis and to evaluate the interatrial communication. Neonates with a restrictive foramen ovale associated to a severe hypoxemia (oxygen saturation of 60% or less) underwent an urgent BAS (within 30 minutes after delivery). Results: a total of 38 foetuses had a diagnosis of TGA confirmed postnatally. The last fetal echocardiogram was performed at a mean gestational age of 36±1,07 weeks. All patients were delivered via caesarean section. The mean gestational age at birth was 38±1,3 weeks (range 35 to 39 weeks) and the mean birth-weight 3033 ± 67 g. Eleven patients required urgent BAS. Five of them were delivered in the cardiac unit because of the prenatal evaluation of a restrictive atrial septum, confirmed after birth. The others showed a redundant and sometimes also hypermobile atrial septum at the last fetal echocardiogram and were delivered in the maternal unit, but at birth required an urgent BAS as well. We did not find any significant abnormal flow pattern in the pulmonary veins or a particularly small ductus arteriosus in these foetuses compared to the rest of the study population. Conclusions: even if nowadays an accurate diagnosis of TGA is possible by fetal echocardiogram, the antenatal evaluation of the atrial septum for restriction is still challenging. Based on our experience, not only foetuses with a restrictive atrial septum before birth but also those who showed a redundant septum primum bulging towards the left atrium, are to be considered at risk of an urgent BAS for postnatal hypoxemia. La diagnosi prenatale di agenesia del Dotto Venoso è rara. Dal punto di vista anatomico ed ecografico quest’anomalia è caratterizzata da due pattern principali: il primo in cui la vena ombelicale drena direttamente nel sistema portale e il secondo in cui bypassa il fegato e drena direttamente nel cuore, o in vena cava inferiore o in vena iliaca /renale. Obiettivo di questo studio è valutare l’outcome feto-neonatale nei casi di agenesia del Dotto Venoso. Materiali e Metodi. Studio osservazionale eseguito presso il Centro di Medicina Fetale, “ Di Venere”-Bari e 'Sarcone'-Terlizzi e presso la UOC di Neonatologia del Policlinico di Bari. Lo studio ha interessato sia le gestanti sottoposte a screening delle aneuploidie a 11-13+6 settimane e ad esame di screening delle malformazioni fetali a 19-21 settimane che le gestanti inviate da altri ospedali per sospetta anomalia fetale. Per ciascun feto è stato studiato ecograficamente il decorso della vena ombelicale, il circolo porto-ombelicale e la presenza/ assenza del Dotto Venoso. Una dettagliata valutazione dell’anatomia fetale compreso lo studio del cuore mediante ecocardiografia fetale è stata eseguita nei casi in cui il Dotto Venoso era assente. In questi casi era offerta alla gestante anche la possibilità dello studio del cariotipo fetale. La diagnosi era confermata al reperto autoptico o dopo la nascita. Risultati. Sono stati individuati 19 casi di agenesia del Dotto Venoso: 8 casi in cui la vena ombelicale bypassava il fegato e 11 in cui la vena ombelicale drenava regolarmente nel circolo portale. Degli 8 casi in cui il fegato era bypassato, in 3 la vena ombelicale drenava direttamente nel cuore, in 3 direttamente in vena cava inferiore e in 2 in vena iliaca destra. Anomalie fetali (incluse le cromosomopatie ) erano associate in 3 degli 11 casi in cui la vena ombelicale drenava nel circolo portale ( 2 casi di trisomia 21 - con canale atrio-ventricolare completo in uno dei 2 - e 1 caso di persistenza della vena cava superiore sinistra associata a interruzione della vena cava inferiore con azygos continuation) e in 4 degli 8 casi in cui la vena ombelicale bypassava il fegato (1 caso di trisomia 18, 1 caso di ritorno venoso polmonare anomalo, 1 caso di modica steno-insufficienza della mitrale e 1 caso di persistenza della vena cava superiore sinistra). Inoltre in 4 degli 8 casi in cui la vena ombelicale bypassava il fegato era presente, al momento della diagnosi o si sviluppava durante il prosieguo della gravidanza, scompenso cardiaco fetale e si evidenziava ecograficamente una significativa alterazione dell’anatomia del circolo portale. Per quanto riguarda l’outcome, in 3 casi la gestante optò per l’interruzione della gravidanza perché erano associate altre anomalie fetali . In 1 caso il feto morì in utero per idrope fetale e in uno morì dopo la nascita per una complicanza successiva all’ intervento legato alla presenza di ritorni venosi anomali polmonari. In 2 casi sono attualmente presenti problematiche cliniche di natura epatica. Nei restanti 12 casi, i bambini sono vivi e non mostrano alcuna sintomatologia clinica. In 3 dei 12 casi il pattern ecografico-anatomico è caratterizzato dalla vena ombelicale bypassante il fegato e nei restanti 9 il fegato non è bypassato. Conclusioni. Dallo studio si evince che l’ associazione con altre anomalie è significativa in entrambi i pattern anatomici associati ad agenesia del Dotto Venoso. Inoltre nei casi in cui la vena ombelicale drena regolarmente nel circolo portale e il reperto è isolato, la prognosi è buona. Nei casi in cui la vena ombelicale bypassa il fegato, anche se il reperto è isolato, è significativo il rischio di scompenso cardiaco e di alterazione della funzionalità epatica. C3 DIAGNOSI ECOCARDIOGRAFICA ED OUTCOME DELLE BRADIARITMIE FETALI, ESPERIENZA DELL'AMBULATORIO DI ECOCARDIOGRAFIA FETALE DI PADOVA 12 ANNI DI N. Zabadneh 1, L. Campisi 2, E. Reffo 1, A. Cerutti 1, R. Milanesi 1 1 Cardiologia Pediatrica Dipartimento di Pediatria Salus Pueri, Padova, 2 Divisione di Reumatologia Azienda Ospedaliera, Padova, 3 Dipartimento per la Salute della Donna e del Bambino, Pediatria, Novara, ITALY Introduzione: le bradicardie e le tachiaritmie fetali ricorrono nel 2% delle gravidanze, con genesi congenita, o acquisita (per danno da infezione materna o per genesi autoimmune). Una percentuale maggiore di gravidanze, dal 10 al 20% viene riferita al cardiologo pediatra per sospetta aritmia quando, durante un’ecografia di routine, viene registrata un’anomalia del ritmo o della frequenza cardiaca fetale. Attualmente lo studio delle aritmie fetali si avvale dell’ecocardiografia. L’ecocardiografia fetale consente uno studio strutturale del cuore (morfologico e funzionale) e una valutazione del ritmo cardiaco mediante M-mode e Doppler Pulsato (PW); permette di riconoscere le cardiopatie congenite complesse, per le quali è necessario intervenire alla nascita, l’identificazione di aritmie fetali e, ove possibile, il loro trattamento mediante somministrazione di farmaci alla madre. Scopo del nostro studio è valutare prevalenza delle bradiaritmie fetali diagnosticate mediante ecocardiografia, e l’outcome nei casi di diagnosi di blocco atrio-ventricolare (BAV). Materiali e metodi: sono state incluse le pazienti sottoposte a screening presso l’ambulatorio di ecocardiografia fetale per bradi aritmia dal 2001 al 2012, è stato analizzato in numero totale di ecografie eseguite, le diagnosi di bradi aritmie, e l’outcome nei casi con diagnosi di BAV. Risultati: sono state eseguite in totale 7569 ecocardiografie, di cui il 10.5% (n°796) per screening delle bradiaritmie fetali, su indicazione materna, SSA+ o altra malattia autoimmune e su indicazione fetale. In 93 esami (effettuati su 71 feti) sono state riscontrate: BAV in 24 casi (3%), extrasistolia isolata in 37 casi (4,6%), bradicardia sinusale in 10 casi (1.2%). Il restante 91.2% dei feti esaminati per ritmo, presentava un ritmo cardiaco nella norma. Nei 12 anni analizzati l’età gestazionale media alla diagnosi di BAV è stata 26 sg, minima 20 sg. Su un totale di 24 feti affetti da BAV, in 3 casi (12.5%) è stata operata interruzione volontaria di gravidanza, in un caso (4.1%) si è assistito a morte in utero a 21 sg, 19 bambini (79.1%) sono nati vivi, mentre 1 gravidanza è ancora in corso. Sui 19 nati vivi, 5 bambini (20% dei feti affetti) hanno necessitato di impianto di pace maker in epoca neonatale, nonostante ciò 2 di questi 5 pazienti sono deceduti in epoca neonatale. Dal 2009, 8 feti con BAV e madre SSA+ sono stati sottoposti a trattamento in utero con desametasone 4mg/die, plasmaferesi e Ig endovena ogni 15gg, 3 di questi (affetti da BAV tipo II) sono regrediti, mentre i restanti 5 hanno presentato stabilità del ritmo cardiaco, e non hanno sviluppato miocardiopatia. Conclusioni: il BAV, più spesso riscontrato nelle madri SSA+, viene diagnosticato nel 3% delle gravidanza riferite al centro di ecocardiografia fetale per screening ritmologico. Il periodo più a rischio per sviluppo di BAV nei feti di madre SSA+ è tra le 18 e le 26 SG, si conclude che è necessario un monitoraggio stretto nella suddetta epoca gestazionale al fine di identificare precocemente i casi di BAV. Dal 2009 la triplice terapia (desametsone, plasmaferesi ed immunoglobuline) sembra infatti aver migliorato la prognosi dei feti con diagnosi di BAV tipo II, ed impedito la progressione di miocardiopatia ai feti affetti da BAV III, quando la diagnosi è stata precoce. La prognosi del BAV isoimmune potrebbe essere migliorata dalla terapia materno fetale qualora la diagnosi precoce ne permettesse l’inizio tempestivo. Tuttavia occorreranno altri dati per confermare il successo della terapia. C4 INCIDENCE AND TYPES OF CONGENITAL HEART DEFECTS IN TWIN PREGNANCIES: A COMPARISON BETWEEN SPONTANEOUS AND MEDICALLY ASSISTED TWIN PREGNANCIES V. Martucci, M. Unolt, C. Putotto, A. Caiaro, L.M. In twin pregnancies (TP) two fetuses develop simultaneously in the womb. TP are riskier than single pregnancy. Moreover, congenital anomalies (CA) are more frequent in twins than in singletons. The incidence of TP varies on ethnic lines. Women with a family history of twins have a higher chance of conceiving twins. Other factors increasing the odds of TP include maternal age, fertility drugs, and use of assisted reproductive technologies (ART). Some studies suggested that ART is associated with an increased risk of CA. MATERIALS AND METHODS: We performed a retrospective study, including 146 women with TP that came in our Division of Pediatric Cardiology to perform fetal echocardiography (FE), from 01/01/2002 to 31/12/2012. The aim of the study was to evaluate the incidence of congenital heart defects (CHD) in TP. In particular, we compared the incidence of CHD in spontaneously conceived twins with that in twins conceived by ART. In all patients FE has been performed using Philips Sonos 5500 in the first period of the study and Philips HD 15 in the second period. We divided our cohort in two groups: group 1 – 63 medically assisted TP and group 2 – 83 spontaneous TP. RESULTS: Analysis of our cohort produced the following data: In group 1 the average maternal age (MA) at diagnosis was 37,09 years (range 27-50), while the average gestational age (GA) was 29,49 weeks (range 20-35). In this group, we found 55 twin pairs (87,3%) with normal fetal heart (NFH). In 8 pairs (12,7%) one of the two fetuses had CHD: 6 (9,5%) ventricular septal defect (VSD), 1 (1,6%) atrioventricular canal defect (AVC) in a fetus with multiple CA, and 1 (1,6%) double outlet right ventricle (DORV). To provide more specific data, we divided the group in four subgroups, depending on the ART used to conceive: • Subgroup A: 38 TP conceived following In Vitro Fertilization and Embryo Transfer (FIVET). In 34 twin pairs FE showed NFH (89,5%); in 3 pairs one of the twins (7,9%) had VSD, while in 1 pair (2,6%) one fetus (with multiple CA) had AVC. • Subgroup B: 21 TP after Intra Cytoplasmic Sperm Injection (ICSI). 18 twin pairs (85,7%) were diagnosed as having NFH, whereas in 3 pairs one of the two fetuses had VSD (14,3%). • Subgroup C: 3 TP conceived using heterologous ICSI fertilization. 2 twin pair (66,6%) had a NFH and one of the two fetuses of 1 pair (33,4%) had DORV. • Subgroup D: 1 TP resulting from Intrauterine Insemination (IUI), FE showed a NFH (100%). In group 2 the average MA at diagnosis was 32,18 years (range 20-44) and the average GA was 25,70 weeks (range 19-34). In this group, we found 73 twin pairs (87,9%) with NFH. In 10 pairs (12,1%) one of the two fetuses had CHD: 3 (3,6%) tetralogy of Fallot, 2 (2,4%) VSD, 2 (2,4%) biventricular hypertrophy (BVH), 2 (2,4%) pulmonary stenosis and 1 univentricular heart. CONCLUSIONS: Previous reports in literature demonstrated an increased rate of CA in twins, independently from the type of conception (spontaneous vs ART-related). As regards the association of CHD and medically assisted TP, in literature there are contrasting reports. Several studies described mostly septal defects in medically assisted TP. Accordingly with these data, among all fetuses with CHD, we found 6 cases of VSD. Concerning the group of spontaneous TP, our data confirmed the higher risk of CHD in monozygotic TP as compared to those dizygotic, since the two cases of BVH occurred both in spontaneous monozygotic twins pregnancy with twin-to-twin transfusion syndrome. C5 ASSISTED REPRODUCTIVE TECNOLOGIES (ART) AND INCIDENCE OF MAJOR CONGENITAL HEART DISEASES IN 13 YEARS OF FETAL ECHOCARDIOGRAPHY S. Costa 1, M. Cresci 2, U. Bottone 1, P. Festa 1, I. Monasterio, Ospedale del Cuore, Massa, 2 IFC-CNR Istituto di Fisologia Clinica, Pisa, ITALY Background: The use of assisted reproductive technology (ART) to treat infertility is increasing rapidly. Previous studies suggested that assisted reproduction technology (ART) may increase the risk of birth defects. However, the role of ART in aetiology of congenital defects, in particular on the risk of Congenital Heart Diseases (CHDs), remains a controversial question. Aim : to estimate the risk of major CHDs in foetuses conceived after ART in 13 years of our experience. Methods: We performed a retrospective analysis in 6529 women referred to fetal echocardiography at our institution.between January 2000- December 2013. ART pregnancies included in vitro fertilisation with Embryo Transfer (IVF-ET ) or Intracytoplasmatic Sperm Injection (ICSI). Results: We analyzed 137 ART pregnancies (87 IVF-ET and 50 ICSI) of whom 86 twin gestations (64%). An increase of the use of ART between 2006 and 2013 was observed when compared to the period 2000-2005 (2.5% vs 1%, respectively) in particular, an increased number of ICSI (41% vs 9%). The incidence of CHDs in all naturally conceived foetuses was 5.7% (368/6395) while in ART foetuses was 10.2% (14/137). In particular, the incidence further increased in the ART foetuses when data were restricted to the period between 2000 and 2005 (6/22; 27%) respect to the period 2006-2013 (8/115; 6.9%). ART pregnancy had a significantly higher risk of CHDs (odds ratio [OR] 3.02, 95% confidence interval [CI] 1.2 to 7.2, p = 0.01). Conversely, any association was found with other extracardiac malformations. Conclusion : The use of ART represents a risk factor for the incidence of CHDs and therefore remains an appropriate indication for fetal echocardiography.
