DEVICE DESCRIPTION The Toronto Transseptal Wire delivers Radiofrequency power in a monopolar mode between its distal electrode and a commercially available external Disposable Indifferent (Dispersive) Patch (DIP) Electrode, which is in compliance with the current ISO 60601-2-2 and/or ANSI/AAMI HF18 standards. The Toronto Transseptal Wire is connected to the BMC Radiofrequency Puncture Generator via the BMC Connector Cable (RFP-102). Detailed information concerning the BMC Radiofrequency Puncture Generator is contained in a separate manual that accompanies the Generator (entitled “BMC Radiofrequency Puncture Generator Instructions for Use”). Dimensions for different models of the Toronto Transseptal Wire are described on the device label. The insulation on the body of the Toronto Transseptal Wire facilitates smooth advancement of the device in conjunction with providing electrical insulation. The floppy distal portion of the Toronto Transseptal Wire has a J-curve and the active tip is rounded to be atraumatic to cardiac tissue unless RF energy is applied. II. INDICATIONS FOR USE The Toronto Transseptal Wire is indicated for creation of an atrial septal defect in the heart. III. CONTRAINDICATIONS The Toronto Transseptal Wire is not recommended for use with any conditions that do not require the creation of an atrial septal defect. IV. WARNINGS Only physicians with a thorough understanding of angiography and percutaneous interventional procedures should use this device. It is recommended that physicians avail themselves of pre-clinical training, a review of pertinent literature and other appropriate education before attempting new interventional procedures. The Toronto Transseptal Wire is supplied STERILE using an ethylene oxide process. Do not use if the package is damaged. Laboratory staff and patients can undergo significant x-ray exposure during Radiofrequency puncture procedures due to the continuous usage of fluoroscopic imaging. This exposure can result in acute radiation injury Page 1 of 6 as well as increased risk for somatic and genetic effects. Therefore, adequate measures must be taken to minimize this exposure. The Toronto Transseptal Wire is intended for single patient use only. Do not attempt to sterilize and reuse the Toronto Transseptal Wire. Reuse can cause the patient injury and/or the communication of infectious disease(s) from one patient to another. Failure to do so may result in patient complications. The Toronto Transseptal Wire must be used with the BMC Connector Cable (RFP-102). Attempts to use it with other connector cables can result in electrocution of the patient and/or operator. Do not attempt to deliver Radiofrequency energy with an initial power setting of greater than 10 Watts. The initial attempt should be made with a setting of 10 Watts. In subsequent attempts, the power setting can be increased, if necessary. The active tip of the Toronto Transseptal Wire is fragile. Be careful not to damage the tip while handling the Toronto Transseptal Wire. If the tip becomes damaged, discard the Toronto Transseptal Wire immediately. If the active tip of the Toronto Transseptal Wire becomes bent at any time during its use, dispose of the Toronto Transseptal Wire immediately. Do not attempt to straighten the active tip. The Toronto Transseptal Wire is not intended for use with neonatal patients (less than one month of age). Do not attempt to treat neonatal patients with the Toronto Transseptal Wire. PRECAUTIONS Do not attempt to use the Toronto Transseptal Wire or ancillary equipment before thoroughly reading the accompanying Instructions for Use. Radiofrequency puncture procedures should be performed only by physicians thoroughly trained in the techniques of Radiofrequency powered puncture in a facility fully equipped for transseptal procedures. The sterile packaging and the Toronto Transseptal Wire should be visually inspected prior to use to detect any compromise. Ensure that the packaging has not been damaged. Do not use the equipment if the packaging has been compromised. Do not use the Toronto Transseptal Wire after the USE BY date indicated on the label. The Toronto Transseptal Wire is intended for use with only those devices listed in section 0, Equipment Required. Read and follow the manufacturer’s instructions for use of the Disposable Indifferent (Dispersive) Patch (DIP) electrode. Always use DIP electrodes that meet or exceed ISO 60601-2-2 and AAMI/ANSI HF18 requirements. Placement of the dispersive electrode on the thigh could be associated with higher impedance. In order to prevent the risk of ignition, ensure that flammable materials are not present in the room during Radiofrequency power application. Take precautions to limit the effects that the electromagnetic interference (EMI) produced by the Generator may have on the performance of other equipment. Check the compatibility and safety of combinations of other physiological monitoring and electrical apparatus to be used on the patient in addition to the Generator. Adequate filtering must be used to allow continuous monitoring of the surface electrocardiogram (ECG) during Radiofrequency power applications. Careful manipulation of the Toronto Transseptal Wire must be performed to avoid vessel trauma. Wire advancement should be done under fluoroscopic guidance. If resistance is encountered, DO NOT use excessive force to advance or withdraw the Toronto Transseptal Wire. Do not attempt to deliver Radiofrequency energy until the tip of the Toronto Transseptal Wire is confirmed to be in good contact with the target tissue. Do not bend the Toronto Transseptal Wire. Excessive bending or kinking of the wire shaft may damage the integrity of the Toronto Transseptal Wire and may cause patient injury. Care must be taken when handling the Toronto Transseptal Wire. It is recommended not to exceed five (5) Radiofrequency power applications per Toronto Transseptal Wire. The Generator is capable of delivering significant electrical power. Patient or operator injury can result from improper handling of the Toronto Transseptal Wire and/or DIP electrode, particularly when operating the device. During power delivery, the patient should not be allowed to come in contact with ground metal surfaces. Apparent low power output or failure of the equipment to function properly at normal settings may indicate faulty application of the DIP electrode, failure to an electrical lead, or poor tissue contact at the active tip. Check for obvious equipment defects or misapplication. Attempt to better position the active tip of the Toronto Transseptal Wire against the target tissue. Only increase the power if low power output persists. VI. ADVERSE EVENTS Adverse events that may occur while creating an atrial septal defect include: Tamponade Vascular thrombosis Vessel perforation Sustained arrhythmias Vessel spasm Arteriovenous fistula Hemorrhage Thromboembolic episodes Hematoma Myocardial Infarction Pain and Tenderness Atrial Flutter Sepsis/Infection Puncture of the myocardium Atrial Fibrillation Ventricular Tachycardia Allergic reaction to contrast medium VII. EQUIPMENT REQUIRED Radiofrequency transseptal procedures should be performed in a specialized clinical setting equipped with a fluoroscopy unit, radiographic table, physiologic recorder, emergency equipment and instrumentation for gaining vascular access. Ancillary materials required to perform this procedure include: BMC Radiofrequency Puncture Generator BMC RFP-102 Cable Transseptal sheath/dilator set, such as the Baylis Medical Company TorFlex Transseptal Guiding Sheath. Disposable Indifferent (dispersive) Patch (DIP) electrode meeting ISO 60601-2-2 and/or AAMI/ANSI requirements for electrosurgical electrodes. VIII. INSPECTION PRIOR TO USE Prior to performing the procedure, the individual components including the BMC Radiofrequency Puncture Generator, Toronto Transseptal Wire, and the BMC Connector Cable (RFP-102) should be carefully examined for damage or defects, as should all equipment used in the procedure. Do not use defective equipment. Do not reuse the Toronto Transseptal Wire. IX. DIRECTIONS FOR USE All instructions for equipment required should be carefully read, understood, and followed. Failure to do so may result in complications. Different models of the Toronto Transseptal Wire have different diameters. Only use dilators compatible with the size of the specific Toronto Transseptal Wire model. The Toronto Transseptal Wire is supplied sterile. Use aseptic technique when opening the package and handling the product in the sterile field. A Transseptal Sheath and Dilator is usually inserted through the right femoral vein and is then advanced over a guidewire to be positioned into the superior vena cava (SVC) under fluoroscopic guidance. Insert the Toronto Transseptal Wire through the sheath/dilator set. With the Toronto Transseptal Wire inside the dilator, position the wire/sheath/dilator assembly in the right atrium against the fossa ovalis under fluoroscopic guidance using standard technique. Connect the Toronto Transseptal Wire to the BMC Connector Cable (RFP102) ensuring that the Toronto Transseptal Wire is inserted properly into the connector. Make sure that the Connector Cable is plugged into the appropriate slot on the BMC Radiofrequency Puncture Generator. Be sure to carefully follow the Instructions for Use provided with the Generator and Connector Cable. Advance the Toronto Transseptal Wire so that the active tip is engaging the septum at the fossa ovalis, but still within the dilator. Once appropriate positioning has been achieved, Radiofrequency power can be delivered via the BMC Radiofrequency Puncture Generator to the distal tip. This results in puncture of the targeted cardiac tissue. Please refer to the Generator Instructions For Use for the correct operation of the generator. Apply firm pressure to the Toronto Transseptal Wire during the application of Radiofrequency energy to successfully advance it through the tissue. A power setting of 10 Watts has been experimentally determined to be sufficient for successful puncture. NOTE: Use the lowest appropriate power setting to achieve the desired effect. Do not use an initial power setting greater than 10 Watts. Radiofrequency power delivery can be terminated by pressing the RF ON/OFF button on the Generator if the timer has not expired. Entry into the left atrium can be confirmed by monitoring the Toronto Transseptal Wire under fluoroscopy. If puncture is not successful after five (5) Radiofrequency power applications, it is advised that the user utilize an alternate method for the procedure. Once the initial puncture is successfully completed, the Toronto Transseptal Wire should be mechanically advanced without any Radiofrequency power. CLEANING AND STERILIZATION INSTRUCTIONS The Toronto Transseptal Wire is intended for single use only. Do not clean or re-sterilize the Toronto Transseptal Wire. XI. TROUBLESHOOTING The following table is provided to assist the user in diagnosing potential problems. PROBLEM Generator Error Messages COMMENTS In order to successfully perforate tissue using Radiofrequency energy, all devices must be properly connected and in good working order. Wire breaks or kinks. Breaks and kinks in the Toronto Transseptal Wire are a potential cause of patient injury. TROUBLESHOOTING Ensure that all connections are made: - Toronto Transseptal Wire to Connector Cable - Connector Cable to Generator - Generator to power outlet - Generator to grounding pad Visually inspect the Toronto Transseptal Wire or Cable for damage. Immediately discard any damaged equipment. If problem persists discontinue use. For error messages encountered while attempting Radiofrequency puncture, refer to the Instructions for Use document that accompanies the Generator. Discard immediately. XII. LIMITED WARRANTY – DISPOSABLES AND ACCESSORIES Baylis Medical Company Inc. (BMC) warrants its Disposable and Accessory products against defects in materials and workmanship. BMC warrants that sterile products will remain sterile for a period of time as shown on the label as long as the original package remains intact. Under this Limited Warranty, if any covered product is proved to be defective in materials or workmanship, BMC will replace or repair, in its absolute and sole discretion, any such product, less any charges to BMC for transportation and labor costs incidental to inspection, removal or restocking of product. The length of the warranty is: (i) for the Disposable products, the shelf life of the product, and (ii) for the Accessory products, 90 days from shipment date. This limited warranty applies only to new original factory delivered products that have been used for their normal and intended uses. BMC’s Limited Warranty shall not apply to BMC products which have been resterilized, repaired, altered, or modified in any way and shall not apply to BMC products which have been improperly stored or improperly cleaned, installed, operated or maintained contrary to BMC’s instructions. È raccomandato ai medici di avvalersi di una formazione pre-clinica, una revisione della documentazione pertinente e altre conoscenze adeguate prima di provare nuove procedure di intervento. Il Toronto Transseptal Wire è fornito STERILE grazie ad un procedimento all'ossidio di etilene. Non usare se la confezione è dannegiata. Il personale di laboratorio ed i pazienti possono esser esposti ad una quantità importante di raggi X durante le procedure di punture a radiofrequenza a causa dell'uso continuato dell' imaging fluoroscopico. Questa esposizione può provocare ferite da radiazione acute nonché un aumento del rischio di effetti somatici e genetici. È quindi necessario prendere le dovute precauzioni per limitare al massimo l'esposizione. Il Toronto Transseptal Wire è inteso per uso singolo per un solo paziente. l Toronto Transseptal Wire non dev'essere né risterilizzato né riusato. Il riutilizzo potrebbe causare ferite al paziente e/o la trasmissione di malattie infettive da un paziente all'altro. La loro inosservanza può provocare complicanze nei pazienti. Il Toronto Transseptal Wire dev'essere usato con il BMC Connector Cable (RFP-102). Usarlo con altri cavi connettori potrebbe causare l'elettrocuzione del paziente e/o dell'operatore. DMR TSW 3.3 V-7 05-Jul-2013 IV. Non superare una potenza di 10 Watts durante la prima applicazione di energia a radiofrequenza. La prova iniziale dev'essere fatta con un setting a 10 Watts. Nelle prove susseguenti, la potenza può venire aumentata, se necessario. La punta attiva del Toronto Transseptal Wire è fragile. Far ben attenzione a non dannegiare la punta mentre si manipola il Toronto Transseptal Wire. Se la punta si danneggia, cestinare il Toronto Transseptal Wire immediatamente. Se la punta attiva del Toronto Transseptal Wire si piega a qualsiasi momento durante l'uso, disfarsi immediatamente del Toronto Transseptal Wire. Non provare a raddrizzare la punta attiva. Il Toronto Transseptal Wire non è inteso per uso su neonati (meno di un mese dalla nascita). Non rattare pazienti neonati con il Toronto Transseptal Wire. PRECAUZIONI Non usare il Toronto Transseptal Wire o una delle sue attrezzature ausiliari prima di leggere attentamente le istruzioni per l'uso allegate. Le procedure di punture a radiofrequenza dovrebbero essere eseguite solo da medici completamente formati alle techniche di puntura alimentata a Radiofrequenza, in un ambiente pienamente attrezzato per procedure transettali. La confezione sterile e il Toronto Transseptal Wire devono essere esaminati prima dell'uso per rilevarne l'integrità. Assicurarsi che la confezione non sia stata danneggiata. Non utilizzare il dispositivo se la confezione non è integra. Non usare il Toronto Transseptal Wire dopo la data USARE PRIMA DEL indicata sull'etichetta. Il Toronto Transseptal Wire è inteso per l'uso solo con due dispositivi elencati nella sezione VII, Attrezzatura richiesta. Leggere le istruzioni prima dell'uso per l'elettrodo Disposable Indifferent (Dispersive) Patch (DIP). Utilizzare sempre elettrodi DIP che soddisfino o eccedano le norme ISO 60601-2-2 e AAMI/ANSI HF18. Il posizionamento dell'eletrodo dispersivo sulla coscia potrebbe essere associato ad un'impedenza più alta. Al fine di evitare rischi di ignizione, assicurarsi che materiali infiammabili non sono presenti della stanza durante l'applicazione di energia a Radiofrequenza. Prendere le dovute precauzioni per limitare gli effetti d'interferenza elettromagnetica (EMI) che il Generatore potrebbe avere sul funzionamento delle altre attrezzature. Controllare la compatibilità e la sicurezza delle combinazioni con altri apparati elettrici e di monotoraggio fisiologico che sono usati sul paziente oltre al Generatore. Per consentire il monitoraggio continuo dell'elettrocardiogramma di superficie, un filtraggio adeguato è necessario durante le applicazioni di energia a radiofrequenza. Per evitare traumi ai vasi sanguigni, è necessario un'attenta manipolazione del Toronto Transseptal Wire. L'inserimento ed il progresso del Wire devono essere effettuati sotto guida fluoroscopica. Se si dovesse incontrare resistenza, NON usare forza eccessiva per far avanzare o ritirare il Toronto Transseptal Wire. Non provare a liberare energia a Radiofrequenza finché non c'è conferma che la punta del Toronto Transseptal Wire sia a contatto nel modo giusto giusto con il tessuto mirato. Non piegare il Toronto Transseptal Wire. Una flessione o attorcigliamento eccessivo del Toronto Transseptal Wire e può causare ferite al paziente. Della cura è necessaria per la manipolazione del Toronto Transseptal Wire. È raccomandato non eccedere cinque (5) applicazioni di energia a Radiofrequenza per il Toronto Transseptal Wire. Il Generatore può liberare una quantità significativa di energia elettrica. Una manipolazione non accurata del Toronto Transseptal Wire e/o dell'elettrodo DIP, particolarmente durante l'operazione del dispositivo può quindi provocare ferite al paziente o all'operatore . Mentre l'energia viene liberata, il paziente di non dovrebbe essere a contatto con superfici metalliche collegate a terra. Un'erogazione apparentamente bassa di energia o un funzionamento difettoso dell'attrezzatura con le impostazioni normali possono indicare un'applicazione inadeguata dell'elettrodo DIP, un guasto ad un filo elettrico, o un contatto debole tra tessuto e punta attiva. Controllare l'attrezzatura per vedere se ci sono difetti evidenti o errori di applicazione. Provare a posizionare meglio la punta attiva del Toronto Transseptal Wire contro il tessuto mirato. Aumentare la potenza elettrica solamente se l'erogazione dell'energia continua ad essere debole. La Baylis Medical Company fa affidamento che il medico determini, valuti e comunichi individualmente ad ogni paziente tutti i rischi collegabili ad un intervento di puntura a Radiofrequenza. EVENTI AVVERSI Fra gli eventi avversi che possono occorrere mentre si crea un difetto settale atriale, si osserva: Tamponamento Trombosi vascolare Perforazione vasale Aritmia sostenuta Spasma vasale Fistola arterovenosa Emorragia Episodi tromboembolici Ematoma Infarto miocardico Dolore e sensibilità Flutter atriale Setticemia/Infezione Puntura del miocardo Fibrillazione atriale Tachicardia ventricolare Reazione allergica al mezzo di contrasto VI. ATTREZZATURA NECESSARIA Le procedure transettali a Radiofrequenza devono essere eseguite in un ambiente clinico specializzato attrezzato con un reparto di fluoroscopia, un tavolo radiografico, un registratore fisiologico, attrezzatura e strumentazione di emergenza per un acceso vascolare. I materiali ausiliari richiesti per eseguire questa procedura includono: Il BMC Radiofrequency Puncture Generator BMC RFP-102 Cable Combinazione di guaina/dilatatore, come la Baylis Medical Company TorFlex Transseptal Guiding Sheath. Elettrodo Disposable Indifferent (dispersive) Patch (DIP) che soddisfi le norme ISO 60601-2-2 e/o AAMI/ANSI richieste per gli elettrodi elettrochirurgici. VII. ISPEZIONE PRIMA DELL'USO Prima di eseguire la procedura, le componenti individuali, compreso il BMC Radiofrequency Puncture Generator, il Toronto Transseptal Wire e il BMC Connector Cable (RFP-102) devono essere esaminati attentamente per accertare eventuali danneggiamenti e difetti, come tutta l'attrezzatura usata nell'intervento. Non usare attrezzatura difettosa. Non riusare il Toronto Transseptal Wire. VIII. DIRETTIVE PER L'USO Tutte le istruzioni per le attrezzature necessarie devono essere lette attentamente, comprese e seguite. Non osservare queste direttive potrebbe risultare in complicazioni. I diversi modelli di Toronto Transseptal Wire sono di calibri diversi. Usare solo dilatatori compatibili con le dimensioni del modelo specifico del TorontoTransseptal Wire. Il Toronto Transseptal Wire è fornito sterile. Usare tecniche asettiche mentre si apre la confezione e si manipola il prodotto in un ambiente sterile. La guaina ed il dilatatore Transettali sono abitualmente inseriti nella vena femorale destra ed sono fatti progredire lungo un filo guida i nella vena cava superiore sotto guida fluoroscopica. Inserire il Toronto Transseptal Wire dentro la combinazione guaina/dilatatore. Con il Toronto Transseptal Wire dentro il dilatatore, posizionare l'insieme fili/guaina/dilatatore nell'atrio destro contro la fossa ovalis sotto guida fluoroscopica usando una tecnica standard. Connettere il Toronto Transseptal Wire al BMC Connector Cable (RFP102) assicurandosi che il Toronto Transseptal Wire sia inserito adeguatamente al connettore. Assicurarsi che il Cavo connettore sia ben inserito nello slot giusto sul BMC Radiofrequency Puncture Generator. Assicurarsi di seguire fedelmente le Istruzioni per l'uso fornite con il Generaore e il Cavo connettore. Far avanzare il Toronto Transseptal Wire in modo che la punta attiva si ingrani con il setto alla fossa ovalis, ma sempre dentro il dilatatore. Una volta il posizionamento adeguato raggiunto, dell'energia a Radiofrequenza può essere inviata alla punta attiva via il BMC Radiofrequency Puncture Generator. Questo provoca una puntura del tessuto cardiaco mirato. Riferirsi alle Istruzioni per l'uso del Generatore per un'operazione corretta del generatore. Applicare una pressione ferma sul Toronto Transseptal Wire durante l'applicazione di energia a Radiofrequenza per un progresso efficace attraverso il tessuto. È stato determinato durante la sperimentazione che una potenza di 10 Watt è sufficiente per una puntura riuscita. NOTA: Usare l'impostazione di potenza adeguata minima per raggiungere l'effetto desiderato. Non usare un'impostazione di potenza iniziale di più di dieci Watt. Se il cronometro d'applicazione è ancora in funzione, l'invio di energia a Radiofrequenza può essere interrotto premendo il tasto RF ON/OFF sul Generator. L'entrata nell'atrio sinistro può essere confermata monitorando il Toronto Transseptal Wire sotto fluoroscopia. Se la puntura non è riuscita dopo cinque (5) applicazioni di energia a Radiofrequenza, è consigliato all'utente di usare un metodo altenativo per l'intervento. DMR TSW 3.3 V-7 05-Jul-2013 Una volta la puntura iniziale completata con successo, il Toronto Transseptal Wire deve essere spinto in avanti meccanicamente con l'energia a Radiofrequenza disinserita. PROBLEMA COMMENTO Messaggi di errore del Generatore Per perforare con successo un tessuto usando energia a Radiofrequenza, tutti i dispositivi devono essere connessi correttamente e in buone condizioni di operazione. Il Wire si rompe o si attorciglia. Rutture e attorcigliamento nel Toronto Transseptal Wire possono causare eventuali ferite al paziente. PROCEDURE DI RICERCA GUASTI E SOLUZIONI Assicurarsi che tutte le connessioni sono state fatte: - Il Toronto Transseptal Wire al Cavo connettore - Il Cavo di Connessione al Generatore -Il Generatore all'uscita di potenza - Il Generatore alla piattaforma di messa a terra Ispezionare visivamente il Toronto Transseptal Wire o il Cavo per individuare danneggi.
