Alle coppie, laddove uno od entrambi i partner erano HIV-positivi, si sconsigliava la gravidanza. Con l’avvento della terapia antiretrovirale e alla luce di nuovi studi si è avviato un cambiamento di tendenza con la possibilità di offrire assistenza riproduttiva anche alle coppie siero discordanti. Until the mid-nineties, before the introduction of HAART, the life expectancy of patients with HIV/AIDS was short. Le coppie con un partner infetto avevano un alto rischio di infettare il partner sano e le donne HIV-positive di infettare i figli durante la gravidanza o il parto. Conseguentemente a queste coppie si consigliava l’uso del condom. Però Semprini (Milano) aveva aperto una nuova speranza nelle coppie siero discordanti, in quanto per ridurre il rischio di trasmissione sessuale, durante i tentativi di concepimento, iniziò già nel 1987 a proporre l’inseminazione intrauterina (IUI) alle donne HIV-negative con il seme “lavato” dei loro partner HIV-positivi. Per oltre un decennio Semprini rimase l’unico clinico a fornire assistenza riproduttiva a queste coppie ricevendo molte critiche dai colleghi del momento. Nel 1996 l’introduzione della HAART condusse ad un drammatico aumento dell’aspettativa di vita AIDS- free e della qualità della vita delle persone HIV-positive. Conseguentemente venne riconsiderata la riproduzione e l’assistenza riproduttiva per queste coppie. Da allora il “metodo Semprini” è stato adottato e riesaminato da molti gruppi a livello europeo. Allo stesso modo anche il nostro gruppo multidisciplinare iniziò nel 2002 a proporre l’assistenza riproduttiva mediante lo sperm washing e l’IUI nelle coppie siero discordanti con maschio HIV-positivo e partner femminile HIV- negativa . INTRODUCTION Up until 1996, as a result of the short life expectancy of patients with HIV/AIDS, pregnancy was discouraged for couples where one or both partners were HIV-positive. For couples with one infected partner, the risk of infection was high for the healthy partner and for HIV-positive women to infect their children during pregnancy or childbirth. As a consequence these couples were advised to use condoms. However Semprini (Milan) had opened up new hope to serodiscordant couples, inasmuch to reduce the risk of sexual transmission during conception attempts, in 1987 he began to propose intrauterine insemination (IUI) to HIV-negative women with their HIV-positive partner’s “washed” semen. For over a decade Semprini was the only clinician to provide reproductive assistance to these couples while receiving a much criticism from his contemporaries. The introduction of HAART in 1996 led to a dramatic increase in the expectancy of an AIDS- free life and in the quality of life of people with HIV. Consequently reproduction and reproductive assistance for these couples was reconsidered. Since then the “Semprini method” has been adopted and reviewed by many groups throughout Europe. Likewise our multidisciplinary group started in 2002 to offer reproductive assistance through sperm washing and IUI in serodiscordant couples with a HIV-positive male and a HIVnegative female partner. Da gennaio 2002 ad ottobre 2009, 150 coppie sierodiscordanti sono state inserite nel progetto di Procreazione Assistita Sicura (PAS). Le coppie venivano sottoposte, dopo una valutazione psicosociale ai fini di una genitorialità responsabile, ad uno studio di fertilità, condotto su entrambi i partner. Successivamente si procedeva a raccolta e lavaggio del seme e alla determinazione della “virospermia” su una aliquota del campione, che in attesa di risposta della determinazione dell’HIV-RNA quantitativo, veniva utilizzato fresco o congelato. La partner femminile veniva quindi sottoposta a stimolazione ovarica e successiva induzione dell’ovulazione e quindi ad inseminazione intrauterina con il concentrato di spermatozoi scongelati del proprio partner. Nelle nostre coppie l’86% dei soggetti di sesso maschile aveva fatto uso di sostanze stupefacenti e.v. come fattore di rischio per l’acquisizione dell’infezione ed il 70% erano coinfetti per HCV. Anche nelle partner femminili nel 15% dei casi si segnalava una storia di abuso di sostanze e di sieropositività per HCV. Per quanto riguarda le caratteristiche socio-culturali il 75% delle donne rispetto al 16% dei maschi possedeva un diploma di scuola media superiore o una laurea. Queste coppie tipicamente conducevano una relazione stabile, avevano un impiego fisso ed un livello socio-economico medioalto. Si è osservato un drop out del 18% dopo il primo colloquio. Nel 50% dei casi circa le coppie necessitavano dell’applicazione di tecniche di riproduzione di livello superiore (FIVET/ICSI + ET) per infertilità di uno od entrambi i partner. Negli ultimi anni relativamente al periodo di osservazione descritto si è osservato un incremento delle coppie straniere per lo più africane. OUR EXPERIENCE Data and characteristics of the pairs observed in the first period of activity. From January 2002 to October 2009, 150 serodiscordant couples were included in the project by Procreazione Assistita Sicura (PAS) (Safe Assisted Reproduction). Couples were submitted after a psychosocial assessment with the purpose of responsible parenthood and a fertility study, conducted on both partners. Successively we proceeded to collect and wash the sperm and to determine the “virospermia” on a portion of the sample, which pending the response of the determination of quantitative HIV-RNA, was used fresh or frozen. The female partner was then subjected to ovarian stimulation and subsequent ovulation induction and afterwards intrauterine insemination with their partner’s thawed sperm concentrate. In our couples, 86% of males had used intravenous drugs as a risk factor for acquiring the infection and 70% were co-infected by HCV. Even in female partners there was a reported a history of substance abuse and seropositivity for HCV in 15% of cases. As for the socio-cultural characteristics, 75% of women compared with 16% of males had a high school diploma or a degree. These couples typically conducted a stable relationship, had a steady job and a medium-high socio-economic level. There was an 18% drop out after the first interview. In 50% of cases the pairs required the application of a higher level of reproduction techniques (IVF/ICSI + ET) for the infertility of one or both partners. In the latter years relative to the period of observation described, an increase of foreign couples, who were African for the most part, was observed. Il tasso di gravidanza per ciclo di IUI è risultato essere del 15,9%, mentre il tasso di gravidanza per coppia del 44,8% Queste percentuali sono sovrapponibili a quelle ottenute in altre 5 casistiche segnalate in letteratura (Semprini et al., 1992; Marina (A) et al., 1998; Bujan (A) et al., 2004; GillingSmith et al., 2000; Weigel et al., 2001). I dati che riportano questi studi mostrano tassi di gravidanza dopo IUI variabili dal 14,8 al 30,6% per ciclo o dal 31,3% al 66,1% per coppia. Results 138 cycles of IUI were carried out in 49 women (96 with fresh semen and 42 with frozen semen). 22 children were born and there were four abortions. The pregnancy rate per cycle of IUI was found to be 15.9%, while the pregnancy rate per couple was 44.8% These percentages are similar to those obtained in five other case studies reported in the literature (Semprini et al., 1992; Marina (A) et al., 1998; Bujan (A) et al., 2004; GillingSmith et al., 2000; Weigel et al., 2001). The data related by these studies show pregnancy rates after IUI variable from 14.8 to 30.6% per cycle or from 31.3% to 66.1% per couple. Le caratteristiche e le sfide della nostra attività oggi . Dopo il 2009 abbiamo cominciato ad offrire assistenza riproduttiva anche alle coppie siero discordanti con partner femminile sieropositiva e sieroconcordanti in accordo con le correnti linee guida. Successivamente, grazie anche al supporto dei progressi scientifici nell’ambito della prevenzione dell’infezione, negli anni più recenti abbiamo iniziato ad assistere le coppie che optano per un concepimento naturale. Abbiamo pensato quindi di incoraggiare le coppie a scegliere una alternativa più sicura nel rispetto della loro scelta riproduttiva, offrendo il metodo del “concepimento naturale guidato” che consiste nella combinazione di una Pre-exposure profilassi (PrEP) alla donna, con rapporti mirati esclusivamente nel periodo ovulatorio, dopo aver escluso infezioni genitali e dopo aver effettuato uno studio di fertilità in entrambi i partner. E’ richiesta inoltre una viral load nel partner HIV-positivo indeterminabile stabilmente da almeno 6 mesi. In questo modo si riduce ulteriormente il rischio residuo di trasmissione durante i tentativi di concepimento spontaneo. Dopo 6 tentativi senza successo la coppia viene rivalutata. E’ ARRIVATO IL MOMENTO DEL CONCEPIMENTO NATURALE NELL’EPOCA DELLA SOPPRESSIONE VIROLOGICA COMPLETA? Una serie di studi (2-8) hanno gettato le basi per un cambiamento di tendenza, dalla riproduzione assistita alla riproduzione naturale, che si è consolidato successivamente dopo i risultati dello studio di Cohen del 2011 (1), pietra miliare nell’ambito della conoscenza sulla prevenzione dell’infezione, inducendo una trasformazione nell’atteggiamento dei clinici, sulla base di questi risultati. La HAART ha determinato la svolta della storia naturale dell’infezione da HIV e la viral load (VL) è stata il fattore chiave implicato nella trasmissione dell’infezione (Castilla et al., 2005; Del Romero, Castilla, Hernando, Rodriguez, &Garcia, 2010; Melo et al., 2008; Quinn et al., 2000) Nel 2008 la “Swiss Federal Commission for HIV/AIDS” dichiarò : ‘‘an HIV infected individual without an additional sexually transmitted disease and on antiretroviral therapy with completely suppressed viraemia is sexually non infectious” (Vernazza, Hirschel, Bernasconi, & Flepp, 2008) Il concetto di “treatment as prevention” (TASp) si sta sempre più diffondendo, grazie allo studio di Cohen ma non solo, con diminuzione dell’incidenza di HIV direttamente proporzionale alla copertura della HAART e l’efficienza della PrEp è stata ormai confermata in numerosi trial randomizzati. Vernazza per primo presentò nel 2007 i dati su 21 donne in preexposure prophylaxis con tenofovir al momento dei tentativi di concepimento: 15 su 21 (71%) rimasero gravide (con un numero massimo di 10 tentativi per coppia). Tale coorte, recentemente aggiornata con 46 donne (Vernazza et al., 2011), non ha documentato infezioni in 12 mesi di PreP ed il 75% dei casi ha raggiunto la gravidanza in 12 mesi. Peraltro anche i dati di Barreiro pubblicati su Human Reprod nel 2007 erano giunti alla stessa conclusione di mancanza di infezione in 62 coppie siero discordanti osservate con partner in ART e VL <500 copie HIV-RNA/ml. Sono 40.000.000 le persone infette, 15.000.000 delle quali in HAART, con 2.000.000 di nuove infezioni/anno/nel mondo. We thought then to encourage couples to choose a safer alternative while respecting their reproductive choice, offering the method of “natural guided conception” which is the combination of a Pre-exposure prophylaxis (PrEP) to the woman, with relations aimed exclusively during the ovulatory period, after excluding genital infections and after making a study of fertility in both partners. A consistently undetectable viral load from at least 6 months is also required in the HIVpositive partners. This will further reduce the residual risk of transmission during spontaneous conception attempts. After six unsuccessful attempts the couple is reassessed. HAS THE TIME ARRIVED FOR NATURAL CONCEPTION IN THE AGE OF COMPLETE VIROLOGICAL SUPPRESSION? A series of studies (2-8) laid the foundations for a change in trend, from assisted reproduction to natural reproduction, which were then consolidated after the results of the study by Cohen in 2011 (1), a milestone in the field of knowledge on infection prevention, inspiring a transformation in the attitude of clinicians, based on these results. HAART has determined the turning point in the natural history of HIV infection and the viral load (VL) has been the key factor involved in the transmission of infection (Castilla et al., 2005; Del Romero Castilla, Hernando Rodriguez, & Garcia, 2010; Melo et al., 2008; Quinn et al., 2000) In 2008, the “Swiss Federal Commission for HIV/AIDS,” declared: ‘’an HIV infected individual without an additional sexually transmitted disease and on antiretroviral therapy with completely suppressed viraemia is not sexually infectious”(Vernazza, Hirschel, Bernasconi & Flepp, 2008) The concept of “treatment as prevention”(TasP) is becoming increasingly widespread, thanks to Cohen’s study among others, with a decrease in the incidence of HIV being directly proportional to the HAART coverage and the efficiency of PrEP which has by now been confirmed in a number of randomised trials. In 2007 Vernazza first introduced data on 21 women in Preexposure prophylaxis with tenofovir during the conception attempts: 15 in 21 (71%) remained pregnant (with a maximum of 10 attempts per couple). This cohort, recently updated with 46 women (Vernazza et al., 2011), has not documented infections in 12 months of PrEP and 75% of the cases achieved pregnancy within 12 months. However, the data from Barreiro published in Human Reprod in 2007 had come to the same conclusion in the absence of infection in 62 serodiscordant couples observed with partners in ART and VL <500 copies HIV-RNA/ml. 40 million people are infected, 15 million of them on HAART, with 2,000,000 new infections/year/in the world. In Italia invece la tecnica di SW +IUI, ideata da Semprini è stata applicata fin dal 1987 e in gran parte d’ Europa tale tecnica è diventata la routine. Le recenti linee guida dei CDC per la prima volta hanno raccomandato la PreP come una tra le diverse opzioni di concepimento sicuro per le coppie sierodiscordanti sebbene sia una raccomandazione basata su opinione di esperti e non su analisi specifiche in questa popolazione. Inoltre sempre i CDC, tenendo presente che il desiderio di concepire può superare la paura di trasmissione, raccomandano un counseling riproduttivo non coercitivo ma supportivo delle decisioni del paziente, avendo osservato che spesso non viene effettuato fino al momento del concepimento. Lo studio di S. Finocchario-Kessler infatti del 2010 documentò che su 181 donne solo il 31% aveva ricevuto un counseling riproduttivo e la maggior parte di queste aveva chiesto specificamente di poterne parlare al proprio medico (9). Quindi quali opzioni adottare allo stato attuale per una procreazione sicura ? Nella coppia discordante la ART riduce la viral load del partner positivo e quindi l’infettività e la PrEP evita una potenziale infezione del partner negativo. Quindi la terapia antiretrovirale può essere considerata una opzione aggiuntiva nella prevenzione, visto che ormai numerosi studi ne hanno dimostrato l’efficacia, quando vengono osservate determinate condizioni, come l’aderenza, l’assenza di infezioni del tratto genitale di entrambi i partner, l’assunzione mirata al periodo periconcezionale e i rapporti mirati al periodo ovulatorio oltre alla protezione dei rapporti al di fuori dei tentativi di concepimento (10). Le Linee Guida francesi sulle strategie riproduttive nelle coppie sirodiscordanti sono state aggiornate nel settembre 2013 ma gli esperti continuano a raccomandare PMA e IUI, sebbene ammettano che si possa proporre il TASp come strategia alternativa se la coppia è fertile, non ha infezioni genitali o altre MTS e la VL è stabilmente undetectable nel partner HIV +. Si afferma inoltre che le coppie dovrebbero limitare i rapporti ai giorni fertili. Al contrario la PreP non è raccomandata per la mancanza di studi su vasta scala. Lo studio di G.Mabileau (HIV – serodiscordant couples desiring a child: “treatment as prevention”, preexposure prophylaxis, or medically assisted procreation? pubblicato sull’American Journal of Obstetrics and Gynecology) di settembre 2015, ci offre una accurata analisi su: rischio residuo di trasmissione di HIV-1, costo e rapporto costo/efficacia delle diverse strategie di 84 States there are approximately 140,000 serodiscordant couples, half of which wish to conceive. From 1990 and until recently, the CDC recommended IVF or ICSI+ET to discordant couples with a male HIV+ partner, due to a case of seroconversion which occurred after sperm washing and intrauterine insemination (SW and IUI), occurring precisely in 1990. In Italy, however the technique of SW + IUI, conceived by Semprini has been applied since 1987 and this technique has become routine in much of Europe. For the first time, the recent CDC guidelines recommended PrEP as one among several options of safe conception for serodiscordant couples, although it is a recommendation based on expert opinion and not on specific analyses within this population. Keeping in mind that the desire to conceive can overcome the fear of transmission, the CDC also recommends reproductive counselling which is not coercive but supportive of the decisions of the patient, having observed that it is often not carried out until the moment of conception. The study by S. Finocchario-Kessler in 2010 in fact, documented that in over 181 women, only 31% had received reproductive counselling and most of these women had specifically asked to discuss it with their doctor (9). So what options should be adopted at present for safe procreation? In discordant couples ART reduces the viral load of the positive partner and therefore infectivity and PrEP avoids a potential infection of the negative partners. Therefore, antiretroviral therapy may be considered an additional option in the prevention, now that numerous studies have demonstrated its efficacy, when observed under certain conditions, such as adherence, the absence of genital tract infections of both partners, the assumption aimed at the periconceptional period and sexual relations targeted during ovulatory periods in addition to the use of protection during intercourse outside of the conception attempts (10). The French Guidelines on reproductive strategies in serodiscordant couples were updated in September 2013 but experts continue to recommend MAP and IUI, whilst they admit that they can propose TasP as an alternative strategy if the couple is fertile, has no genital infections or other STDs and the VL is consistently undetectable in the HIV+ partner. It also states that couples should restrict their relations to fertile days. On the other hand PrEP is not recommended due to the lack of large-scale studies. G. Mabileau’s study (HIV - serodiscordant couples desiring a child: “treatment as prevention”, preexposure prophylaxis, or medically assisted procreation? published in the American Journal of Obstetrics and Gynecology) in September 2015, pro- CURE ~ 2015 Articolo Originale/Original Article riproduzione nelle coppie sierodiscordanti a seconda delle varie opzioni adottate all’interno della coppia (le coppie sono fertili con maschio HIV + in HAART e VL <50 copie/ml e partner femminile HIV-neg ): • Rapporti sessuali non protetti TASp del partner positivo) (quindi esclusivamente • TASp del partner HIV+ e rapporti limitati ai giorni fertili • TASp con Prep della partner non infetta (TDF/FTC) • TASp + PreP limitata ai giorni fertili • PMA (procreazione medicalmente assistita) Disegno dello studio: l’analisi è basata su un modello di costo efficacia effettuato dalla prospettiva della società francese. I parametri di riferimento derivano dalla letteratura internazionale: 85% di probabilità di nati vivi con le diverse strategie; 0,0083%/ mese il rischio di trasmissione HIV con rapporti vaginali non protetti, 1% rischio MTCT, e 4,4% il rischio di difetti alla nascita correlati alla HAART quando la madre si infetta al concepimento. I rapporti nei giorni fertili e la PreP si stimava diminuissero il rischio di trasmissione di HIV del 80% e 67% rispettivamente e del 93% per la PreP limitata ai giorni fertili. I risultati hanno evidenziato che : • Il rischio di trasmiss. è più alto con il TASp e più basso con la PMA (rispettivamente 5,4 e 0 donne HIV infette/10000 gravidanze). • I rapporti nei giorni fertili sono più efficaci della PreP (0,9 vs 1,8) e si associano a costi inferiori. La PreP limitata nei giorni fertili è più efficace dei rapporti nei giorni fertili (0,3 vs 0,9) con un rapporto costo/effiicacia di 1.130.000€ /anno vita salvato. Il rapp costo/efficacia della PMA comparato alla PreP nei giorni fertili è 3.600.000€/anno vita salvato. Conclusioni: • i rapporti limitati ai giorni fertili si associano a basso rischio di trasmissione nel partner HIV neg ini coppie fertili con infezione controllata nell’altro partner. • Il rischio è più basso con la PreP limitata nei giorni fertili o la PMA, ma queste strategie sono associate a rapporti costo/efficacia sfavorevoli con i costi correnti Per la maggior parte delle coppie sierodiscordanti lo SW+ IUI come opzione realistica e sicura di concepimento è precluso da barriere finanziarie e logistiche. L’uso della PrEP periconcezionale come aggiunta alla HAART soppressiva per il partner maschile HIV+, può essere l’intervento ideale per tali coppie impossibilitate ad accedere o che non desiderino sottoporsi a tecniche costose ed invasive . Sebbene siano necessari ulteriori studi osservazionali nel mondo reale per stabilire sia le attitudini verso l‘uso della PrEP tra i clinici e i pazienti, sia le barriere e CURE ~ 2015 vides us with an accurate analysis on the residual risk of the transmission of HIV-1, the cost and cost/effectiveness of the different reproduction strategies in serodiscordant couples depending on the various options adopted by the couple (couples are fertile with with HIV+ male on HAART and VL <50 copies/ml and the female partner is HIV-neg): • Unprotected intercourse (thus exclusively TasP of the positive partner) TasP of the HIV+ partner and relations limited to fertile days • TasP with PrEP of the uninfected partner (TDF/FTC) • TasP + PrEP limited to the fertile days • MAP (medically assisted procreation) • Study design: the analysis is based on a cost effectiveness model carried out from the perspective of the French company. The benchmarks are derived from international literature: 85% chance of live births with the different strategies; 0.0083%/ month risk of HIV transmission by unprotected vaginal intercourse, 1% MTCT risk and 4.4% risk of birth defects related to HAART when the mother becomes infected at conception. Intercourse during the fertile days and the PrEP was estimated to decrease the risk of HIV transmission by 80% and 67% respectively and 93% for PrEP limited to the fertile days. The results underlined that : • The risk of transmission is higher with TasP and lower with MAP (respectively 5.4 and 0 HIV infected women/10,000 pregnancies). • PrEP is most effective for intercourse during the fertile days (0.9 vs 1.8) and is associated with lower costs. Limited PrEP during the fertile days is more effective than intercourse during the fertile days (0.3 vs 0.9), with a cost/efficiency of € 1,130,000/year of life saved. The cost/effectiveness correlation of MAP compared to PrEP during the fertile days is € 3.6 million/year of life saved. Conclusions: • intercourse limited to the fertile days is associated with a low risk of HIV transmission to negative partners in fertile couples with controlled infection in the other partner. • The risk is lower with limited PrEP during fertile days or MAP, but these strategies are associated with an unfavourable cost/effectiveness with current costs. For the majority of serodiscordant couples, SW + IUI as a realistic and safe conception option is precluded by financial and logistical barriers. A systematic review and meta-analysis in 2013 (12) on the infectivity of HIV infection in heterosexual couples, analysing 51 studies, in 9 of which it was possible to compare the infectivity in ART to that of non-ART, could attest that ART reduces the HIV transmission by 91% in serodiscordant heterosexual couples [figure 3]. The study concludes that HAART reduces infectivity in serodiscordant couples as is proven in observational studies, however the lack of zero risk to the uninfected partner requires the application of counselling and other strategies for risk reduction. Therefore PrEP appears to be the most effective method today which supports ART to protect the high-risk uninfected partner in the context of serodiscordant heterosexual couples seeking parenthood. Sono stati effettuati 2113 cicli IUI (tasso di gravidanza per ciclo =16%, per donna=48%), la percentuale di nati vivi è 13% per ciclo IUI, 41% per donna, l’HIV status post-inseminazione è risultato negativo per tutte le donne considerate.
