BEIA Rubella IgM Mab Quant 22292 Edizione 20-04-2013 Solo per uso professionale TECHNOGENETICS SRL N° 00501 Pag. 1 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 INTRODUZIONE Il kit BEIA Rubella Mab IgM Quant è un test immunoenzimatico qualitativo/quantitativo (ELISA) per la determinazione nel siero o nel plasma degli anticorpi di classe IgM specifici verso il virus della rosolia. SIGNIFICATO CLINICO La rosolia è causata da un virus RNA classificato come rubeovirus appartenente alla famiglia dei togavirus. L'infezione puo' essere asintomatica o provocare un modesto esantema. Se contratta durante la gravidanza causa la "rosolia congenita" che può provocare seri danni al feto. La maggior parte dei casi di rosolia congenita si verifica come conseguenza di un’infezione materna primaria, anche se sono segnalati casi estremamente rari di rosolia congenita in nati da madri con pregressa immunità, quindi in conseguenza di reinfezioni. Come conseguenza di una infezione materna al I trimestre si può avere aborto, morte fetale, infezione placentare, ma può anche non verificarsi alcuna infezione del feto. La placenta costituisce una valida barriera all’infezione fetale dalla 12ª alla 28ª settimana di gestazione, mentre la protezione è meno valida al I e III trimestre (in particolare nell’ultimo mese di gravidanza) e di conseguenza il virus può causare un’infezione placentare persistente anche senza infezione fetale. La sindrome della rosolia congenita (SRC) non è una malattia statica. E' stato dimostrato che ¾ dei bambini infetti sono asintomatici alla nascita e solo successivamente presenteranno delle sequele. La diagnosi si basa su indagini sierologiche e colturali dal momento che la sintomatologia clinica non permette di porre diagnosi di certezza. La ricerca di anticorpi (IgG ed IgM) può essere fatta in tempi molto rapidi mediante latex-agglutinazione, emoagglutinazione passiva, immunofluorescenza o metodi immunoenzimatici. La presenza di IgG indica uno stato di immunità. La presenza di IgM a titoli elevati o moderati permette di porre diagnosi d’infezione recente, tuttavia a basso titolo le IgM possono essere presenti anche in casi di reinfezione. La diagnosi in gravidanza risulta molto facilitata se è noto lo stato immunitario pregravidico; Pag. 2 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 se questo dato non è disponibile è comunque opportuno avere un dato all’inizio della gravidanza. Non esiste alcuna terapia specifica dell’infezione in atto, né alcuna possibilità di prevenire la trasmissione materno-fetale del virus nel caso di infezione in gravidanza. Tentativi di trattamento della SRC con vari farmaci (interferon, amantidina, isoprinosina) non hanno mostrato un’effettiva utilità. La vaccinazione per la rosolia costituisce l’unica strategia efficace per la prevenzione della rosolia congenita. La determinazione delle IgG e IgM specifiche rappresenta un test di I livello. Nel caso di positività alle IgM / IgG, in alcuni casi è necessario procedere ad ulteriori indagini eseguendo test di II livello quali il test dell'Avidità. PRINCIPIO DEL METODO Il test BEIA Rubella IgM Mab Quant è un dosaggio ELISA basato sulla tecnica “a cattura”. I campioni da esaminare vengono incubati nei micropozzetti sensibilizzati con anticorpi monoclonali anti-IgM umane. Le IgM del campione vengono catturate dagli anticorpi presenti sulla fase solida. Dopo eliminazione mediante lavaggio dei componenti del siero non legati alla fase solida, la presenza di IgM anti-Rubella viene rilevata attraverso il legame con un complesso costituito da antigene Rubella - anticorpo monoclonale anti-E1 di topo coniugato con perossidasi. Dopo allontanamento del complesso non legato viene aggiunto il Substrato-TMB che, per azione dell’enzima presente sulla fase solida, sviluppa una colorazione azzurra. L’aggiunta di una soluzione di acido solforico 1N blocca la reazione e provoca il viraggio del colore dall’azzurro al giallo. La densità ottica letta a 450/620 è direttamente proporzionale alla quantità di IgM specifiche presenti nel campione in esame. Nel kit BEIA RUBELLA IgM Mab Quant la concentrazione degli anticorpi di classe IgM viene calcolata mediante l'utilizzo di una curva di calibrazione ed espressa in Unità Arbitrarie/mL. REAGENTI INTERCAMBIABILI Il Diluente Campioni BEIA System, il Tampone di Lavaggio, il SubstratoTMB e la Soluzione Stoppante sono intercambiabili tra i diversi kit della linea BEIA. Pag. 5 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 MATERIALE E STRUMENTI RICHIESTI, MA NON FORNITI Lettore di colorimetria per micropiastre con filtro a 450/620 nm. Apparecchiatura manuale o automatica per il lavaggio di micropiastre. Micropipette con puntali monouso da 10, 100, 1000 µL. Provette monouso e normale vetreria da laboratorio. Acqua distillata o deionizzata. AVVERTENZE E PRECAUZIONI Prima dell'uso portare tutti i reattivi a temperatura ambiente (18-25°C). Prelevare solo la quantità necessaria per l'analisi. Evitare l'essiccamento dei pozzetti. Usare esclusivamente vetreria accuratamente pulita. Non utilizzare uno stesso puntale per pipettare reagenti diversi. Cambiare il puntale tra un campione e l’altro. Qualora il Substrato-TMB evidenziasse un viraggio da rosa ad azzurro, il reagente non deve essere utilizzato. Se il dosaggio viene ripetuto, preparare una nuova diluizione del campione. Si raccomanda di inserire in ogni corsa analitica dei campioni di controllo al fine di validare i risultati ottenuti, secondo criteri e procedure che ciascun utilizzatore deve definire. Procedere periodicamente alla manutenzione e taratura delle pipette e del lettore. AUTOMAZIONE Il test può essere eseguito con sistemi automatici per kit ELISA, nel qual caso Vi invitiamo a consultare il manuale specifico dello strumento per una corretta esecuzione della corsa analitica. Pag. 6 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 PRELIEVO E PREPARAZIONE DEI CAMPIONI Il dosaggio può essere eseguito su siero o plasma (Eparina o EDTA). Se il dosaggio è eseguito entro 5 giorni dal prelievo, i campioni possono essere conservati a 2-8 °C. In caso contrario aliquotare i campioni e congelarli a -20°C. Evitare ripetuti scongelamenti e congelamenti. Si sconsiglia l’uso di campioni lipemici, torbidi ed emolizzati. I campioni devono essere diluiti 1:81 con il Diluente Campioni BEIA System prima del dosaggio PREPARAZIONE DEI REAGENTI Tampone di Lavaggio Diluire il Tampone di Lavaggio(20x) 1:20 con acqua distillata o deionizzata prima dell'uso. Dopo ricostituzione il tampone è stabile 15 giorni se conservato a 2-8°C e comunque non oltre la data di scadenza riportata sull'etichetta originaria. Nel Tampone di lavaggio concentrato si possono formare dei precipitati cristallini che si solubilizzano portando il reagente a 37°C prima della diluizione. Si raccomanda di diluire la quantità di tampone necessaria per eseguire i lavaggi dell'analisi in corso. Complesso Antigene Rubella- Mab-HRP Ricostituire ogni flaconcino di Antigene liofilo con 3 mL di Diluente Complesso. Per una corretta ricostituzione, si consiglia di aggiungere il volume prescritto di Diluente Complesso, richiudere il flaconcino, lasciarlo 15 minuti a temperatura ambiente ed agitarlo delicatamente, evitando la formazione di schiuma, fino a completa dissoluzione del liofilo. Diluire il Coniugato 1:61 con la soluzione di Antigene Rubella (e.g. aggiungere 50 µl di Coniugato al flaconcino ricostituito, contenente 3 ml di antigene Rubella). Preparare esclusivamente la miscela Ag-Mab-Coniugato necessaria per eseguire l’analisi in corso. Qualora si rendesse necessario utilizzare più di un flaconcino monodose, si raccomanda di miscelarne i contenuti prima dell’uso. L'immunocomplesso deve essere preparato 60 minuti prima del suo utilizzo. Pag. 7 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 CICLO DI LAVAGGIO Si consiglia di procedere come segue sia nel caso in cui si utilizzi un lavatore automatico di micropiastre sia nel caso in cui si operi manualmente. 1. Svuotare completamente i pozzetti. 2. Riempirli con 300 µL di tampone di lavaggio. 3. Svuotare i pozzetti. 4. Ripetere i punti 2 e 3 per ulteriori 3 cicli. 5. Asciugare con carta assorbente la superficie esterna dei pozzetti al termine del lavaggio. ESECUZIONE DEL TEST Portare i reagenti a temperatura ambiente (almeno 30-60 minuti a 18-25°C). 1. Diluire (1:81) i campioni in esame con il Diluente Campioni BEIA System (esempio 10 µL di siero e 800 µL di Diluente Campioni BEIA System). Agitare su Vortex. 2. Prelevare il numero di strip necessario per l'analisi in funzione del numero di campioni in esame e dei Calibratori (vedi “Schema del dosaggio”). Richiudere accuratamente la busta. 3. Quantitativo: Dispensare nei rispettivi pozzetti 100 L di ciascun siero in esame prediluito e dei Calibratori. Qualitativo : Dispensare nei rispettivi pozzetti 100 L di ciascun siero in esame prediluito, del Calibratore 0 (0 UA/mL), del Calibratore 1 (10 UA/mL) e del Calibratore 4 (100 UA/mL) 4. Incubare la piastra a temperatura ambiente (18-25°C) per 60 minuti. 5. Eseguire la procedura di lavaggio dei pozzetti seguendo il procedimento raccomandato (vedi Ciclo di lavaggio). 6. Dispensare in ciascun pozzetto 100 µL del complesso Ag-MabConiugato (vedi preparazione dei reagenti). 7. Incubare la piastra a temperatura ambiente (18-25°C) per 60 minuti. 8. Eseguire la procedura di lavaggio dei pozzetti seguendo il procedimento raccomandato (vedi Ciclo di lavaggio). 9. Dispensare in ciascun pozzetto 100 µL di Substrato-TMB. Pag. 8 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 10. Incubare la piastra a temperatura ambiente (18-25°C) per 30 minuti. 11. Bloccare la reazione cromogenica dispensando in ciascun pozzetto 100 µL di Soluzione Stoppante. 12. Leggere la densità ottica (DO) in bicromatismo a 450/620 nm. CALCOLO ED INTERPRETAZIONE DEI RISULTATI VALIDAZIONE DELLA SEDUTA ANALITICA Le condizioni che consentono l’accettazione dei risultati sono : il rapporto tra la DO del Calibratore 4 (100 UA/mL) e la DO del Calibratore 1 (10 UA/mL) dev’essere > 3.0. il rapporto tra la DO del Calibratore 0 (0 UA/mL) e la DO del Calibratore 1 (10 UA/mL) dev’essere < 0.4. Qualora la seduta dia risultati non conformi alle condizioni sopra elencate, essa non è validabile ed i risultati relativi ai campioni non possono essere refertati senza analisi critica documentata. La non conformità va qualificata e trattata secondo le procedure di Assicurazione Qualità proprie del laboratorio. Si consiglia di instaurare ed attuare procedure di Controllo Qualità interne al laboratorio, i cui risultati concorrano alla validazione delle sedute ed alla gestione di eventuali non conformità. CALCOLO QUANTITATIVO DEI RISULTATI Calcolare la media dei valori di densità ottica per ogni calibratore e campione (i duplicati dovrebbero presentare un CV minore del 20%). Elaborare la curva di calibrazione riportando la media delle densità ottiche di ogni calibratore sull’asse delle ordinate e le corrispondenti concentrazioni sull’asse delle ascisse. Disegnare la migliore curva che passi attraverso i punti della calibrazione. Per calcolare la concentrazione di IgM anti-Rubella di ogni campione riportare il valore della densità ottica sull’asse delle ordinate e collegare con una linea retta alla curva di calibrazione. Al punto di intersezione, collegare con una linea verticale all’asse delle ascisse e leggere il corrispondente valore di IgM anti-Rubella. Qualora si disponesse di un sistema di calcolo automatico si consiglia di utilizzare una delle seguenti funzioni matematiche: spline cubica, iperbolica e lineare punto a punto. Non è di per sé e univocamente indice di uno stato infettivo; - una decisione medica non può essere basata sul risultato di un solo test, ma va fondata sull’insieme dei dati clinici disponibili. Dati clinici discrepanti vanno risolti prima di prendere la decisione, eventualmente con il conforto di test di conferma o di II livello; - risultati compresi entro la zona grigia vanno chiariti: con la ripetizione del test sullo stesso campione; con la ripetizione del test su un prelievo successivo; con il confronto con un test di riferimento. PRESTAZIONI DEL KIT Avvertenza: i dati presentati sotto questa voce non rappresentano le specifiche di funzionamento del kit, ma costituiscono evidenza sperimentale di come il kit funzioni entro tali specifiche nel modo previsto dal fabbricante. Pag. 11 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 SENSIBILITÀ Sono stati testati 15 sieri di pazienti con diagnosi accertata di infezione primaria da Rubella e 14 sieri prelevati entro i primi due mesi a pazienti vaccinati. I sieri sono stati confrontati verso un kit immunoenzimatico presente sul mercato con prestazioni accettabili (EIA Rubella IgM). 29 sieri furono trovati positivi con il kit EIA Rubella IgM, mentre 28 risultarono positivi per la presenza di IgM specifiche anti-Rubella con il kit BEIA Rubella IgM Mab Quant. La sensibilità risulta essere del 97%. SPECIFICITÀ Sono stati testati 103 sieri da donne in gravidanza, soggetti con immunità pregressa e non immuni, con infezioni acute da CMV, EBV, Parvovirus B19, con anticorpi anti-nucleo, anticorpi antitessuto e FR positivo. I sieri testati sono stati confrontati verso un kit immunoenzimatico presente sul mercato con prestazioni accettabili (EIA Rubella IgM). 103 sieri furono trovati negativi con il kit EIA Rubella IgM, mentre 100 sieri risultarono negativi per la presenza di IgM specifiche anti-Rubella con il kit BEIA Rubella IgM Mab Quant. La specificità risulta essere del 97%. LIMITE DI RILEVAZIONE Prove condotte in replicato sul Calibratore 0 indicano il limite di rilevazione in 0.4 UA/mL. SPECIFICITÀ ANALITICA Non è stata rilevata alcuna interferenza in sieri contenenti fino a 10 mg/mL di emoglobina, fino a 1 mg/mL di bilirubina e fino a 30 mg/mL di trioleina. L’uso di campioni lipemici, torbidi o emolizzati viene comunque sconsigliato. RIPETIBILITÀ La variabilità intra-saggio ed inter-saggio è stata determinata valutando un campione negativo con concentrazione 3 UA/mL e due campioni positivi a due concentrazioni 15 UA/mL e 80 UA/mL. I campioni sono stati replicati 6 volte per seduta in ciascuna di 12 sedute diverse. Sono stati ottenuti coefficienti di variazione inferiori al 15% per il campione negativo ed inferiori al 6% per i due campioni positivi. LIMITAZIONI Contaminazione batterica o ripetuti scongelamenti e congelamenti dei campioni possono alterare i livelli rilevabili di IgM specifiche. Pag. 12 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 PRINCIPI GENERALI DI SICUREZZA Tenere in ordine il laboratorio ed effettuare con cura le operazioni previste per l'esecuzione del test. Non mangiare, bere, fumare o usare cosmetici nell’area di laboratorio designata. Non pipettare soluzioni o campioni con la bocca. Evitare il contatto diretto con i reattivi ed i campioni indossando guanti monouso e camici. Lavare accuratamente le mani al termine del dosaggio. Pulire immediatamente con opportuni detergenti le superfici di lavoro eventualmente contaminate. Raccogliere il materiale solido o liquido utilizzato per il dosaggio in appositi contenitori ed effettuare lo smaltimento secondo le norme vigenti. Controllare periodicamente il grado di contaminazione dell'ambiente di lavoro e degli operatori. Reagenti contenenti siero I derivati da sangue umano inclusi nel kit sono stati analizzati e trovati negativi per HbsAg, anti-HCV ed anticorpi anti-HIV. Poiché nessun metodo può garantire che tali derivati non possano trasmettere epatite, AIDS o altre infezioni, si raccomanda di manipolare i reagenti con le opportune cautele. CLINICAL RELEVANCE Rubella is a viral illness caused by a RNA virus classified as rubeovirus and belonging to the togaviruses family. Infection can be asymptomatic or can cause a mild rash. Rubella, contracted during pregnancy causes «congenital Rubella» which poses a serious threat to the fetus. Most cases of congenital Rubella are due to a primary maternal Rubella infection even though rare cases have been reported in which infants with congenital Rubella syndrome were born to mothers who were reinfected during pregnancy. Rubella infection in the first three months of pregnancy can lead to miscarriages, stillbirths, placental infection but it is also possible that it doesn’t result in any birth defects. Maternal placenta is a valid barrier to infection from 12th to 28th week of pregnancy, while during the first three months infection can cross it; protection drops again in the third trimester, in particular during the last month, when Rubella virus can cause a persistent placental infection even though the fetus is not infected. Congenital Rubella syndrome (CRS) is not a static condition. It has been proved that the 25% of the infected babies appear normal at birth and have health problems later. Diagnosis can be formulated against serological and coltural evidence and cannot be based only on clinical symptoms suggestive of this infection. The determination of anti-Rubella virus antibodies can be easily and quickly performed by latex agglutination, passive hemoagglutination, immunofluorescence or enzyme immunoassays. The presence of IgG antibodies is indication of an immune condition. High or moderate titers of IgM antibodies suggest a recent infection although low IgM titers can also be found in cases of Rubella reinfection. Pag. 16 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 Diagnosis during pregnancy is easy if there is documented evidence of immunity states to Rubella before pregnancy; women who missed being tested prior to pregnancy should be tested early during pregnancy. There is no effective treatment for Rubella during pregnancy, nor there is an effective way to prevent the likelihood of the illness passed from the mother to the fetus. Any attempts of treatment with various drugs such as interferon, amantidine, isoprinosine have proven useless. Vaccination is the only effective way to prevent Rubella in susceptible women so that their future children will be protected from the congenital Rubella syndrome. PRINCIPLE OF THE ASSAY The BEIA Rubella IgM Mab Quant kit is a qualitative/quantitative enzyme-linked immunosorbent assay (ELISA) based on “capture method”. During the first incubation, IgM antibodies present in serum or plasma bind to the inner surface of the wells coated with the monoclonal anti-human IgM. After the first incubation the wells are washed to remove nonreactive serum components. During the second incubation a complex (Rubella antigensmonoclonal antibody anti-E1 labelled with HRP) binds only to the specific IgM anti-Rubella, bound in the anti-IgM-IgM complex present on the wells. After a second washing cycle, a Substrate-TMB solution is dispensed into the wells in order to detect specific antibodies during a subsequent incubation. The enzymatic reaction is then stopped by adding a Stop Solution which changes to yellow the blue colour developed into the wells. The amount of colour is directly proportional to the specific IgM anti-Rubella concentration in the patient samples. Since no International references were available, the BEIA Rubella IgM Mab Quant has been arbitrarly calibrated. Use a clean disposable pipette tip for each reagent. Use a clean disposable pipette tip for each sample. If the chromogenic substrate shows a change from pink to blue colour, discard the reagent. For repeated assays, each time provide for new diluted samples. Every analytical run should include control samples in order to validate the results, according to criteria and procedures established by each laboratory. Periodical maintenance and calibration of pipettes and microplate reader are required. AUTOMATION The test can be performed on automated ELISA processor. Please refer to specific instrument manual for proper applications. SPECIMEN COLLECTION AND PREPARATION The test can be performed on serum or plasma (heparin – EDTA). Specimens may be stored refrigerated at 2-8 °C up to 5 days. For longer storage they should be aliquoted and stored frozen at – 20°C. Avoid repeated thawing and freezing. Do not use haemolyzed, lipemic or turbid specimens. All serum and plasma samples must be pre-diluted with Sample Diluent prior to assay. See assay protocol. PREPARATION OF REAGENTS Wash Buffer (20 X) Dilute 1:20 the content of the Wash Buffer bottle with distilled or deionised water. If crystallization occurs in the concentrated Wash Buffer, warm the solution to 37 °C before diluting. Only dilute the quantity necessary for the assay. Complex Antigens - Mabs anti-Rubella - HRP Reconstitute the lyophilized vial with 3 mL of Complex Diluent. It's recommended to add the volume of Complex Diluent into the vial, close the cap, wait 15' at room temperature and gently mix in order to avoid foaming. Add 50 L of Conjugate to the reconstitute vial and gently mix. The complex of Ag-Mab-Conjugate must be prepared 1 hour before being dispensed into wells. It's recommended to dilute only the quantity necessary for the analytical run. If the analytical run requires the reconstitution of more than one single-dose vial, it's recommended to mix contents of the vials before use. WASHING CYCLES Both using a microplate washer and washing the plate manually, perform the following steps. 1. Empty the wells 2. Fill them with 300 µL of Wash Buffer. 3. Empty the wells 4. Repeat steps 2 and 3 for other 3 cycles 5. Dry the rim of the wells with blotting paper at the end of the washing cycles. ASSAY PROTOCOL 1. 2. Allow all reagents to reach room temperature leaving them at least 30-60 minutes at 18-25 °C. Dilute the samples to be assayed (1 :81) with the Sample Diluent BEIA System (i.e. 10 µL of serum and 800 µL of Sample Diluent BEIA System). Shake on Vortex. Select the number of strips required for the assay according to the number of samples to be assayed and the Calibrators (see paragraph “Summary of protocol” below). Reseal the pouch. Qualitative: Dispense 100 µL Calibrator 0 (0 AU/mL), Calibrator 1 (10 AU/mL), Calibrator 4 (100 AU/mL) and of the diluted samples. Incubate the plate at room temperature (18-25 °C) for 60 minutes. Perform a washing cycle following the suggested procedure (see Washing Cycles). Dispense 100 µL of Complex Ag-Mab-Conjugate in each well. Incubate the plate at room temperature (18-25 °C) for 60 minutes. Perform a washing cycle following the suggested procedure (see Washing Cycles). Dispense 100 µL of Substrate-TMB in each well . Incubate the plate at room temperature (18-25 °C) for 30 minutes. Stop the chromogenic reaction dispensing 100 µL of Stop Solution into each well. Read the optical density (O.D.) in bi-chromatism at 450/620 nm. CALCULATION AND INTERPRETATION OF RESULTS RUN VALIDATION CRITERIA Criteria used to validate the obtained results are the following: The ratio of the OD for Calibrator 4 (100 AU/mL) to the OD for the Calibrator 1 (10 AU/mL) must be > 3,0 The ratio of the OD for Calibrator 0 (0 AU/mL) to the OD for the Calibrator 1 (10 AU/mL) must be < 0.4 If the above criteria are not met, the run is invalid and the results cannot be reported without a documented critical review of the data. The non conformity must be addressed and treated according to each laboratory’s Quality Assurance procedures. Each lab should establish and follow its own internal Quality Control procedures and use the QC results to validate runs and manage non conformities. Pag. 22 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 QUANTITATIVE CALCULATION OF RESULTS Calculate the average O.D. values for each set of duplicate calibrators and samples (duplicates should have a CV < 20%). Create a calibration curve by plotting the mean of O.D. for each calibrator concentration on the ordinate against the IgM anti-Rubella concentration on the abscissa. Draw a best fit curve through the points of the graph. To determine the concentration of IgM antiRubella for each sample, first find the O.D. value on the ordinate and extend a horizontal line to the calibration curve. At the point of intersection, extend a vertical line to the abscissa and read the corrisponding IgM anti-Rubella concentration. If an automated processor is available it’s advise to select among the following curve fit types: cubic spline, hyperbolic and point-to-point straigth line. PERFORMANCE DATA Warning: Data below do not represent the performance specifications of the kit, but only experimental evidence that the performance of the kit is within such specifications, as intended by the manufacturer. SENSITIVITY 15 sera from patients with diagnosed Rubella primary infection and 14 sera collected from people vaccinated since 2 months were evaluated. 29 sera were found IgM positive with an established EIA commercial kit (EIA Rubella kit) with acceptable performances. 28 sera were found positive for the presence of IgM anti-Rubella with the BEIA Rubella IgM Mab Quant. The sensitivity is of 97%. SPECIFICITY The specificity was assessed by testing 103 specimens from a population expected to be negative for IgM to Rubella (pregnant women, subjects immune and not immune to Rubella, with acute infection caused by CMV, EBV, Parvovirus B19, with autoimmune diseases (ANA), anti-tissue and RF positive). The specificity was evaluated comparing the BEIA Rubella IgM Mab Quant results versus an established EIA commercial kit (EIA Rubella IgM) with acceptable performances. 103 specimens were found negative for the presence of IgM anti-Rubella with the EIA Rubella IgM Kit, while 100 were observed negative for the presence of IgM anti-Rubella with the BEIA Rubella IgM Mab Quant kit. The specificity is of 97.1%. DETECTION LIMIT Trials in replicate on the standard 0 lead to calculate the detection limit in 0.4 AU/mL. ANALYTICAL SPECIFICITY No interference has been observed in samples with hemoglobin up to 10 mg/mL, bilirubin up to 1 mg/mL, triolein up to 30 mg/mL. Lipemic, turbid or hemolyzed samples, however, should not be used. Pag. 25 - 28 BEIA Rubella IgM Mab Quant Ed. 20-04-2013 REPEATABILITY The within-run and run-to-run variability were determined using one negative sample with concentration of 3 AU/mL and two positive samples, containing different concentration : 15 AU/mL and 80 AU/mL. These samples were tested 6 times per single run in 12 different analytical runs. Coefficients of variation lower than 15% have been obtained for the negative sample and lower than 6% for the two positive samples. LIMITATIONS Bacterial contamination or repeated freeze-thaw cycles of specimens may affect the detectable levels of specific IgM GENERAL DIRECTIONS FOR A SAFE USE Keep the laboratory clean and tidy and strictly follow the procedural directions. Do not eat, drink, smoke or apply cosmetics in a clinical lab. Do not pipette solutions or samples by mouth. Wear disposable gloves and overalls when handling reagents and samples. Avoid direct contact. Wash hands thoroughly after assay. Thoroughly clean working area with proper detergent solutions. Collect solid and liquid waste material in proper containers and provide for disposal according to the local regulations. Periodically check contamination level of operators and rooms. Reagents containing serum All components of human origin have been tested and found negative for HBsAg, anti-HCV and anti-HIV antibodies. Since no known test can guarantee, however, that products derived from human blood will not transmit Hepatitis, AIDS or other infectious diseases, these reagents should be handled as hazardous material.
