Questo è avvenuto principalmente attraverso l’introduzione di legislazioni che hanno forzato l’industria farmaceutica a saggiare nuovi farmaci per la qualità, sicurezza ed efficacia usando tecnologie farmaceutiche, esperimenti in vitro, modelli animali in viso, e trials clinici controllati . Arrivando al generale consenso, regolatorio ed industriale, che l’approccio valutativo farmacotossicologico e le linee guida sviluppate da oltre 50 anni, in particolar modo per le nuove entità chimiche, rappresentino un distillato della conoscenze scientifiche acquisite in questo campo. Immunogenicità/Immunotossicità Và controllato la risposta anticorpale per capire se si sviluppano anticorpi anti-MAB neutralizzanti o cross reattivi I primi pur preoccupando perché spesso riducono efficacia e alterano la cinetica non sono predittivi della risposta nell’uomo I secondi sono per molto pericolosi perché possono interagire con proteine endogene e causare effetti tossici. Questi effetti possono essere predetti saggiando la proteina omologa nella specie appropriata ( ad esempio l’anticorpo murino saggiato nel topo, la proteina “cynomolgus” saggiata nella scimmia) L’uso di animali transgenici, può essere un alternativa accettabile agli studi si sicurezza standard, in particolare quando è difficile definire la specie rilevante Evoluzione dei MAB TGN1412 TGN 1412 a first universal T cell growth factor !!! 13 marzo 2006: 6 giovani volontari sani trattati con TGN1412 manifestano una gravissima patologia associata ad un improvviso e rapido rilascio di citochine proinfiammatorie. Studio pilota in una scimmia Cynomolgus (singola dose TGN 1112, 2,5 mg/kg) livelli non misurabili di IFN γ, IL- 5 e IL-6, nella seconda scimmia IFN γ, IL- 6 e IL-10 non mostravano variazioni (non è specificato il limite di determinazione del metodo) Trascurata l’effetto sulla liberazione di citochine? Il dossier Non clinico è stato valutato sia dalla MHRA (UK) che dal PEI (Germania),ed entrambi non hanno sollevato obiezioni. Il fatto che il trial sia stato condotto in UK è dovuto alla maggior velocità della MHRA che ha emesso il suo assessment 15 gg prima del PEI.
