Ovim izjavljujem pod materijalnom i kaznenom odgovornošću da su gore navedeni podaci istiniti, te ih kao
takve vlastoručno potpisujem.
Suglasan sam da se moji osobni podaci (što uključuje i OIB) sukladno odredbama Zakona o zaštiti osobnih
podataka (dalje: Zakon) mogu prikupljati samo u svrhu ostvarivanja usluge internetskog bankarstva, da se
mogu dalje obrañivati samo u tu svrhu, (čl. 6. Zakona) i da se mogu čuvati za čitavo razdoblje korištenja
usluge odnosa (čl. 12. Zakona), odnosno i dulje ako je to posebnim zakonom propisano.
Potpisom Pristupnice za korištenje usluge internetskog bankarstva Korisnik izjavljuje da je upoznat sa Općim
uvjetima za korištenje usluge internetskog bankarstva te ostalim općim uvjetim poslovanja Banke koji se mogu
odnositi na korištenje usluge internetskog bankarstva.
Korisnik odgovara za svaku štetu koju nanese Banci nesavjesnom upotrebom ili zloporabom usluge internetskog
bankarstva. Korisnik odgovara za svaku štetu koju Banci nanese ovlašteni korisnik usluge internetskog
bankarstva bez obzira na pravni odnos samog Korisnika i ovlaštenika.Banka nije odgovorna za bilo kakvu štetu
koju pretpi Korisnik zbog radnji i/ili propusta ovlaštenika usluge internetskog bankarstva.
Sve nastale sporove koji proizlaze iz ovog Ugovora, rješavat će se sporazumno. Ukoliko se ne nañe sporazumno
rješenje, ugovara se nadležnost suda u Zagrebu.

Dichiaro altresì, che i miei dati personali (compreso lo OIB – codice anagrafico), ai sensi della Legge sulla tutela
dei dati personali (in seguito: Legge), possono essere raccolti solo ed esclusivamente ai fini del Servizio di
operazioni bancarie via Internet ed elaborati solo allo stesso fine (art. 6 della Legge) e possono essere custoditi
per l’intera durata di uso del servizio di rapporto (art. 12 della legge), ovvero anche più a lungo, qualora
prescritto da una legge speciale.
Firmando il Modulo di adesione per i servizi dell’Internet Banking, il Beneficiario dichiara di conoscere tutte le
Condizioni generali dell’utilizzo dei servizi dell’Internet banking e altre condizioni generali sulle attività della
banca riferite all’utilizzo dei servizi dell’Internet Banking.
Il Cliente sarà responsabile per tutti i danni cagionati alla Banca dall’utente autorizzato al Servizio di operazioni
bancarie via Internet, indipendentemente dal rapporto giuridico tra lo stesso Cliente e l’Autorizzato. La Banca
non sarà responsabile per qualsiasi danno patito dal Cliente per operazioni e/o omissioni dell’Autorizzato al
Servizio di operazioni bancarie via Internet.
Tutte le controversie derivanti dal presente Contratto dovranno essere risolte di comune accordo. Qualora ciò
non sia possibile la vertenza dovrà essere portata al competente Tribunale di Zagabria.

I also agree that my personal data (including my personal identification number –OIB) may be collected
pursuant to the Law on protection of personal data (hereinafter as the Law) only for the purpose of providing
the service of Internet banking service, and may be processed only to that purpose (article 6 of the Law) and
kept throughout the whole period of the use of such service (article 12 of the Law), or even longer if so
provided for by a particular regulation.
By signing the Application form for using the internet banking service, the User is acknowledging that he is
acquainted and is accepting the General terms for using the internet banking service and other general terms of
the Bank that could refer to internet banking service.
The User shall be held responsible for any damage caused to the Bank by his/her negligent use or misuse of the
Internet banking service.
The User shall be held liable for any damage that might be inflicted on the Bank by the authorized user of the
Internet banking service, without any bearing to the legal relationship between the User and the authorized
individual. The Bank shall not be held liable for any damage inflicted on the User due to the actions and/or the
omissions on the part of the individual authorized to use the Internet banking service.
All eventual disputes that might arise from this Agreement shall be resolved by mutual agreement. In case that
this is not possible, the parties agree to the jurisdiction of the competent court in Zagreb.
