There was no history of epigastric pain but known cholelithiasis. B-mode imaging revealed large (> 25 mm) presumably bile stones in the liver hilum (duodenal bulb) surrounded by a mass lesion (> 10 x 50 mm). Retention of gastric contents was documented. Localised aerobilia was seen subdiaphragmatically. The B-mode ultrasound findings were suggestive of an inflammatory or neoplastic mass in the liver hilum. Contrast enhanced ultrasound demonstrated arterial enhancement of the whole mass lesion, excluding an abscess. Enhancement and portal venous wash out was indicative of a malignant neoplasia. Endoscopy revealed the suspected bile stones within the bulb of duodenum but intubation into the distal duodenum was not possible. Endoscopic ultrasound confirmed the findings and revealed the stiff nature of the mass lesion tissue. Biopsies demonstrated underlying gallbladder malignancy. Surgery was declined. Her treatment was conservative, with the option of a stone extraction and stent placement offered. Discussion The Bouveret syndrome was first described in 1896 by a French physician, Léon Bouveret (1850-1929), who reported two cases of gastric outlet obstruction due to gall stones - “Revue Medicale” [(3)]. The typical Bouveret syndrome is characterized by a proximal small bowel gallstone ileus with gastric outlet obstruction, due to impaction of large gallstone(s) within the duodenal bulb or proximal duodenum [(4)]. Sometimes the pylorus or antrum can be affected also. Bilioenteric fistula is often caused secondary to previous cholecystitis [(5)]. Owing to its uncommon and unpredictable presentation, Bouveret's syndrome can pose a diagnostic and therapeutic challenge for clinicians [(6)]. Dietrich Medizinische Klinik 2, Caritas-Krankenhaus Uhlandstr. 7 97980 Bad Mergentheim Tel:+49 7931 58 2201 Email: [email protected] Caso clinico: riportiamo il caso di una anziana donna di 76 anni presentatasi per un episodio di ematemesi nel contesto di un quadro di nausea a vomito insorti da circa una settimana e non responsivi agli inibitori di pompa protonica. Non era presente una storia di dolore epigastrico mentre era nota una colelitiasi. L’ecografia B-mode ha evidenziato una formazione verosimilmente litiasica piuttosto ampia (>25 mm) all’ilo epatico (bulbo duodenale) circondata da una lesione formante massa (>10x50 mm). E’ stata documentata inoltre gastrectasia ed aerobilia localizzata in sede sottodiaframmatica. Le immagini in B-mode erano indicative di una massa ilare di natura neoplastica o infiammatoria. La ecografia con mezzo di contrasto (CEUS) ha mostrato un arricchimento contrastografico arterioso dell’intera massa escludendo quindi l’ipotesi di ascesso. Sia l’arricchimento che il successivo washout portale e tardivo erano suggestivi di una neoplasia maligna. L’endoscopia ha confermato la formazione litiasica a livello del bulbo duodenale ma l’intubazione del duodeno distale non è stata possibile. L’ecoendoscopia ha confermato i rilievi e ha rilevato la natura rigida della massa. Le biopsie hanno dimostrato una neoplasia della colecisti. La chirurgia è stata rifiutata dalla Paziente. Il trattamento è stato conservativo e come opzioni sono state proposte l’estrazione della formazione litiasica e il posizionamento di uno stent. Discussione: La sindrome di Bouveret è stata descritta per la prima volta nel 1896 dal medico francese Léon Bouveret (1850-1929), che ha riportato due casi di ostruzione dell’outlet gastrico (piloro o bulbo duodenale) causati da calcoli della colecisti sulla “Revue Medicale” [(3)]. La forma classica della sindrome di Bouveret è caratterizzata da un ileo biliare con ostruzione dell’outlet gastrico causato dalla presenza di un ampio calcolo colecistico (o da più calcoli) nel bulbo duodenale o nel duodeno prossimale [(4)]. Raramente possono essere interessati anche il piloro o l’antro. Una fistola bilio-enterica è spesso causata da una precedente colecistite [(5)]. A causa della sua presentazione inusuale e imprevedibile la sindrome di Bouveret può essere una sfida diagnostica e terapeutica per il clinico [(6)]. BOUVERET SYNDROME SINDROME DI BOUVERET Ultrasound, computed tomography and endoscopy have been diagnostic in our case. CT did not appreciate the underlying pathology (gallbladder carcinoma), whereas contrast enhanced ultrasound revealed typical features. Risk Factors for Bouveret's syndrome include age over 70 years, female gender, gallstones larger than 2.5 cm and post surgical altered GI anatomy [(7)]. Common symptoms include vomiting (87 %), abdominal pain (71 %), haematemesis (15 %), recent weight loss (14 %) and anorexia (13 %) [(8)]. Bouveret's syndrome results in complications such as duodenal perforation, hematemesis (Mallory-Weiss tear), distal oesophageal rupture (Boerhaave's syndrome) and gastric bezoar [(8)] amongst others. In patients with gastric outlet obstruction primary or secondary malignancy of the duodenum and surrounding structures needs to be considered, including ductal adenocarcinoma of the pancreas, cholangiocarcinoma, lymphoma, carcinoid, metastases and, as shown in our patient, advanced gallbladder carcinoma. L’ecografia, la tomografia computerizzata (TC) e l’endoscopia sono state diagnostiche nel nostro caso. La TC non ha individuato la patologia sottostante (il carcinoma della colecisti) mentre la CEUS ne ha rivelate le caratteristiche tipiche. I fattori di rischio per lo sviluppo della sindrome di Bouveret includono un’età superiore ai 70 anni, il genere femminile, i calcoli biliari di dimensioni superiori ai 2.5 cm e un’anatomia gastrointestinale alterata da un precedente intervento chirurgico [(7)]. I sintomi più comuni includono il vomito (87%), il dolore addominale (71%), l’ematemesi (15%), una recente perdita di peso (14%) e l’anoressia (13%) [(8)]. Tra le possibili complicanze della sindrome di Bouveret vi sono la perforazione duodenale, l’ematemesi (lesioni di MalloryWeiss), la rottura esofagea distale (sindrome di Boerhaave) e i bezoari gastrici [(8)]. In Pazienti con un’ostruzione dell’outlet gastrico la presenza di una neoplasia primitiva o secondaria del duodeno o delle strutture adiacenti deve essere presa in considerazione, in particolare l’adenocarcinoma duttale del pancreas, il colangiocarcinoma, i linfomi, i carcinoidi, le metastasi e, come nel caso della nostra Paziente, le neoplasie colecistiche avanzate. Abdominal X-ray can show the so-called “Rigler's triad” of pneumobilia, small bowel obstruction and gall stones, in 30 – 35 % cases [(9)]. Abdominal ultrasound is most helpful in the hands of experienced examiners, but review of the literature revealed positive diagnosis in 60 % of cases. The sonographic findings include a fluid filled distended stomach, features suggestive of cholecystitis, ectopic gallstones and pneumobilia. Abdomen CT with contrast has a published sensitivity of 93 %, specificity of 100 % and diagnostic accuracy of 99 % [(10)] but missed the neoplastic nature of the disease in our case. MRCP can differentiate between gall stones and surrounding bile (CT cannot in 15 – 25 % cases) [(11)]. Endoscopy is both diagnostic and therapeutic. The common findings are gall stone causing obstruction (69 %), gastric outlet obstruction without cause identified (31 %) whilst the fistula is visualized in only 13 % of cases [(8)]. Treatment options include endoscopic, open surgical and laparoscopic approaches. Endoscopic techniques include endoscopic extraction, endoscopic laser lithotripsy, extracorporeal shockwave lithotripsy and intracorporeal electrohydraulic lithotripsy [(4)]. The first successful endoscopic extraction was described in 1985 by Bedogni et al [(12)]. Endoscopic management often requires the use of different sized and shaped snares, grasping forceps, retrieval baskets and nets, biliary balloons, and sometimes even a side-viewing endoscope. Hence it can be technically challenging, timeconsuming and the success rates reported in case series have been less than 10 % [(13)]. L’ecografia dell’addome, se nelle mani di un ecografista esperto, è la metodica più utile ma una review della letteratura ha dimostrato una diagnosi corretta solo nel 60% dei casi. I rilievi ecografici includono la presenza di gastrectasia, aspetti suggestivi di colecistite, la presenza di un’ectopia della colecisti e l’aerobilia. La TC dell’addome con contrasto ha in letteratura una sensibilità del 93%, una specificità del 100% e una accuratezza diagnostica del 99% [(10)] ma ha mancato la diagnosi di neoplasia nel nostro caso. La colangiografia mediante risonanza magnetica può differenziare il calcolo dalla bile (differenziazione impossibile alla TC nel 15-25% dei casi) [(11)]. L’endoscopia è sia diagnostica che terapeutica. I reperti più frequenti sono un calcolo causante l’ostruzione (69%), una ostruzione dell’outlet gastrico senza una causa evidente (31%) mentre una fistola è visualizzabile solo nel 13% dei casi [(8)]. Le opzioni di trattamento includono l’endoscopia, la chirurgia a cielo aperto e gli approcci laparoscopici. Le tecniche endoscopiche includono l’estrazione endoscopica, la litotrissia endoscopica laser, la litotrissia extracorporea con onde d’urto e la litotrissia elettroidraulica intracorporea [(4)]. La prima estrazione endoscopica coronata da successo è stata descritta nel 1985 da Bedogni e colleghi [(12)]. La gestione endoscopica necessita l’uso di anse, pinze da presa, cestelli e reti di recupero, palloncini per dilatazioni biliari e, alle volte, addirittura di endoscopi con possibilità di visione laterale. Essendo tecnicamente difficile e necessitando di tempi operativi lunghi il tasso di successo riportato nelle serie presenti in letteratura è di meno del 10% [(13)]. BOUVERET SYNDROME SINDROME DI BOUVERET Endoscopy could be considered as the first treatment in the elderly patient with significant comorbidity, combined with mechanical, electrohydraulic, or laser lithotripsy. Surgical options include simple stone extraction or definitive fistula closure in either 1 or 2 stages. The former is associated with a lower mortality rate and spontaneous fistula closure is reported, whilst the latter is thought to decrease the risk of subsequent complications [(14)]. Indications for open surgery are stone size greater than 2.5 cm, residual stones in GB, multiple stones in intestinal lumen, sepsis, perforation, stricture and failure of endoscopic approach [(13)]. Treatment choice should consider advanced age, comorbidity [(15)], underlying etiology and available equipment [(16)]. L’endoscopia combinata con tecniche di litotrissia laser, meccanica o elettoidraulica può essere considerata come primo approccio in Pazienti anziani con comorbidità significative. Le opzioni chirurgiche includono l’estrazione del solo calcolo o la chiusura definitiva della fistola in uno o due tempi. La prima opzione è associata ad una mortalità più bassa e vi sono casi in letteratura di chiusura spontanea della fistola mentre le ultime opzioni sono associate ad un ridotto numero di complicanze successive [(14)]. Le indicazioni ad una chirurgia a cielo aperto sono le dimensioni del calcolo oltre i 2.5 cm, la presenza di altri calcoli in colecisti, la presenza di multipli calcoli nel lume intestinale, la presenza di sepsi, perforazione, stenosi o il fallimento dell’approccio endoscopico [(13)]. La scelta del trattamento deve tenere in considerazione l’età, le comorbidità [(15)], l’eziologia sottostante e la strumentazione disponibile [(16)]. References 1.
