HZ-3518 1 INTENDED USE For the qualitative screening of serum IgG antibodies to Toxocara using an Enzyme-Linked Immunosorbent Assay (ELISA) technique. For In Vitro Diagnostic Use 2 SUMMARY Toxocariasis is the infection caused by roundworm of the genus Toxocara (usually T. canis, rarely T. cati) and is acquired by ingesting soil contaminated with embryonated eggs from an animal’s feces. These eggs become embryonated after 2 to 5 weeks after being passed by the animal. Thus, human infection does not occur by 6 contact with fresh feces. In addition, infected humans cannot pass the infection to other humans. The disease manifests itself as visceral larval migrans (VLM) and ocular larval migrans (OLM). Signs and symptoms of VLM may vary from an asymptomatic state with mild eosinophilia to a severe and potentially fatal disorder.Patients with OLM also vary widely in presentation, from acute lesions in the eye to asymptomatic infections. Toxocara larva migrans is believed to be the second most common helminth infection in developed 1,2 countries. There is no definitive method to diagnose Toxocara infections, thus true sensitivity and specificity of serologic tests cannot be accurately determined. The diagnosis is further complicated by the fact that the antibody response varies depending on worm burden and location. However, numerous studies have shown that immunoassays using a purified excretory antigen from the larval stage, as in this ELISA, have shown 1-6 dramatically improved sensitivities and specificities when compared to assays using crude antigens. 3 PRINCIPLE OF PROCEDURE The micro test wells are coated with an excretory/secretory antigen from the Toxocara larvae. During the first incubation with the diluted patients’ sera, any antibodies which are reactive with the antigen will bind to the coated wells. After washing to remove the rest of the sample, the Enzyme Conjugate is added. If antibodies have been bound to the wells, the Enzyme Conjugate will then bind to these antibodies. After another series of washes, a chromogen (tetramethylbenzidine or TMB) is added. If the Enzyme Conjugate is present, the peroxidase will catalyze a reaction that consumes the peroxide and turns the chromogen from clear to blue. Addition of the Stop Solution ends the reaction and turns the blue color to a bright yellow color. The reaction may then be read visually or with an ELISA reader. 4 REAGENTS Item Description Test Strips Microwells containing Toxocara antigens – 96 test wells in a test strip holder. Enzyme Conjugate One (1) bottle containing 11 ml of Protein A conjugated to peroxidase. Positive Control One (1) vial containing 1 ml of diluted positive rabbit serum. Negative Control One (1) vial containing 1 ml of diluted negative human serum. Chromogen One (1) bottle containing 11 ml of the chromogen tetramethylbenzidine (TMB). Wash Concentrate (20X) One (1) bottle containing 25 ml of concentrated buffer and surfactant. Dilution Buffer Two (2) bottles containing 30 ml of buffered protein solution. Stop Solution One (1) bottle containing 11 ml of 0.73 M phosphoric acid. Wash concentrate may show crystallization upon storage at 2 – 8 ºC. Crystallization will disappear after dilution to working strength. Do not use serum that may have supported microbial growth, or is cloudy due to high lipid content. Samples high in lipids should be clarified before use. Treat all sera as if capable of being infectious. Negative control has been tested and found negative for Hepatitis B surface antigen and for the antibody to HIV by required test methods. This product should be used under appropriate safety conditions that would be used for any potentially infectious agent. Do not add azides to the samples or any of the reagents. 6 STORAGE CONDITIONS Reagents, strips and bottled components: Store between 2 – 8 ºC. Squeeze bottle containing diluted wash buffer may be stored at room temperature. 7 PREPARATION Wash Buffer Remove cap and add contents of bottle to 475 ml of reagent grade water. Place diluted wash buffer into a squeeze bottle with a narrow tip opening. Note: Washings consist of filling to the top of each well, shaking out the contents and refilling. Avoid generating bubbles in the wells during the washing steps. 8 COLLECTION AND PREPARATION OF SERUM Serological specimens should be collected under aseptic conditions. Coagulate blood and remove serum. Hemolysis is avoided through prompt separation of the serum from the clot. Serum should be stored at 2 – 8 °C if it is to be analyzed within a few days. Serum may be held for 3 to 6 months by storage at -20 °C or lower. Lipemic and strongly hemolytic serum should be avoided. Do not heat inactivate serum and avoid repeated freezing and thawing of samples Test samples: Make a 1:64 dilution of patients’ sera using the dilution buffer (e.g. 5 µL sera and 315 µL dilution buffer). 9 9.1 PROCEDURE Materials Provided Toxocara canis IgG ELISA Kit 9.2 9.3 Materials Required But Not Provided Pipettes Squeeze bottle for washing strips (narrow tip is recommended) Reagent grade water and graduated cylinder Tubes for sample dilution Absorbent paper Suggested Materials ELISA plate reader with a 450 nm and a 650 to 620 nm filter (optional if results are read visually) Hölzel Diagnostika GmbH, Hansaring 10, 50670 Köln, Germany 4 Toxocara canis IgG ELISA HZ-3518 9.4 Performance of Test 1. Break off number of wells needed (two for controls plus number of samples) and place in strip holder. 2. Add 100 µL (or two drops) of the negative control to well #1, 100 µL of the positive control to well #2 and 100 µL of the diluted (1:64) test samples to the remaining wells. Note: Negative and positive controls are supplied prediluted. Do not dilute further. 3. Incubate at room temperature (15 to 25 °C) for 10 minutes. 4. Shake out contents and wash 3 times with the diluted wash buffer. 5. Add 2 drops of Enzyme Conjugate to each well. 6. Incubate at room temperature for 5 minutes. 7. Shake out contents and wash 3 times with wash buffer. Slap plates against paper toweling to remove excess moisture. 8. Add 2 drops of the Chromogen to every well. 9. Incubate at room temperature for 5 minutes. 10. Add 2 drops of the Stop Solution and mix by tapping strip holder. 10 READING OF RESULTS Visually: Look at each well against a white background (e.g. paper towel) and record as clear or +, ++ or +++ reaction. ELISA Reader: Zero reader on air. Set for bichromatic readings at 450/650-620 nm. 11 TEST LIMITATIONS Serologic results are an aid in diagnosis but cannot be used as the sole method of diagnosis. 12 QUALITY CONTROL The use of controls allows validation of kit stability. The kit should not be used if any of the controls are out of range. Expected values for the controls are: Negative - 0.0 to 0.3 OD units Positive - 0.5 OD units and above 13 TROUBLESHOOTING Negative control has excessive color after development. Reason: inadequate washings. Correction: wash more vigorously. Remove excessive liquid from the wells by tapping against an absorbent towel. Do not allow test wells to dry out. Read all wells at 450/650-620 nm. Positive - Absorbance reading equal to or greater than 0.3 OD units. Negative - Absorbance reading less than 0.3 OD units. A negative OD reading indicates that the patient has no detectable level of antibodies. This may be due to lack of infection or poor immune response by the patient. 14.2 Interpretation of Results -Visual Compare results to the controls. A sample should be interpreted as positive if the degree of color development is significant and obvious. 15 EXPECTED RESULTS The number of individuals showing positive results can vary significantly between populations and geographic regions. If possible, each laboratory should establish an expected range for its patient population. 16 PERFORMANCE DATA Study #1 – Canadian Reference Center Compared HÖLZEL ELISA to another commercial ELISA. Found concordance of 84% (n=82). Stopplösung 1 Fläschchen, 11 ml 0.73 M Phosphorsäurelösung Symbol 2 VORSICHTSMAßNAHMEN Verwenden Sie die Lösungen nicht, wenn sich ein Niederschlag oder eine Trübung gebildet hat. Wenn das Waschkonzentrat bei 2 - 8 ºC gelagert wird, können sich Kristalle darin bilden. Beim Verdünnen auf die Arbeitskonzentration lösen sich die Kristalle auf. Serum, in dem möglicherweise Bakterien gewachsen sind oder das aufgrund eines hohen Lipidgehalts trüb erscheint, darf nicht verwendet werden. Aus Proben mit hohem Lipidgehalt muss vor der Verwendung das Lipid entfernt werden. Behandeln Sie alle Seren als infektiöses Material. Die Standards wurden getestet und mit den erforderlichen Testmethoden als negativ für Hepatitis-B-Oberflächenantigen und Antikörper gegen HIV befunden. Bei Verwendung dieses Produkts müssen die im Fall von infektiösem Material erforderlichen Sicherheitsvorkehrungen eingehalten werden. Fügen Sie den Proben oder den Reagenzien kein Azid zu. 3 LAGERBEDINGUNGEN Reagenzien, Teststreifen und Komponenten in Flaschen: Zwischen 2 °C - 8 °C lagern. Achten Sie darauf, dass sich beim Waschen keine Luftblasen in den W ells bilden. Proben können vor Testbeginn für 3 bis 5 Tage bei 2 °C bis 8 °C gelagert werden. Für eine längere Aufbewahrung (3 bis 6 Monaten) sollten die Proben eingefroren bei ≤ –20°C gelagert werden. Lipämische und stark hämolytische Proben sollen nicht verwendet werden. Die Proben dürfen nicht hitzeinaktiviert werden. Vermeiden Sie wiederholtes Auftauen und wieder Einfrieren der Proben. Brechen Sie die benötigte Anzahl Wells ab (zwei für die Kontrollen und eines für jede Probe) und setzen Sie sie in den Streifenhalter. 2. Geben Sie 100 µl negative Kontrolle in Well 1, 100 µl positive Kontrolle in Well 2 und 100 µl der verdünnten (1:64) Proben in die verbliebenen W ells. Negative und positive Kontrollen werden vorverdünnt geliefert. NICHT weiter verdünnen. 3. Bei Raumtemperatur(15 °C bis 25 ºC) 10 Minuten lang inkubieren. 4. Den Inhalt herausschütteln. 3-mal mit verdünntem Waschpuffer waschen. 5. 2 Tropfen Enzym-Konjugat zu jedem Well hinzufügen. 6. Bei Raumtemperatur(15 °C bis 25 °C) 5 Minuten lang inkubieren. 7. Den Inhalt herausschütteln. 3-mal mit verdünntem Waschpuffer waschen. Klopfen Sie nach dem letzten Waschschritt die Wells auf einem sauberen, saugfähigen Tuch aus, um überschüssigen W aschpuffer zu entfernen. 8. 2 Tropfen Chromogen zu jedem Well hinzufügen. 9. Bei Raumtemperatur(15 °C bis 25 °C) 5 Minuten lang inkubieren. 10. 2 Tropfen Stopplösung zu jedem Well hinzufügen. Messen Sie die Absorption in den W ells bei 450 nm mit einem Referenzfilter bei 650 - 620 nm 8 GRENZEN DES TESTVERFAHRENS Serologische Resultate sollen die Diagnose unterstützen, dürfen aber nicht als alleinige Methode zur Diagnosestellung eingesetzt werden. 9 QUALITÄTSKONTROLLE Die Verwendung von Kontrollen ermöglicht eine Überprüfung der Stabilität des Kits. Der Kit sollte nicht verwendet werden, wenn eine der Kontrollen außerhalb des entsprechenden Bereichs liegt. Die für die Kontrollen erwarteten W erte sind: Negativ: 0.0 bis 0.3 OD-Einheiten. Positiv: 0.5 OD-Einheiten und darüber. 10 FEHLERBEHEBUNG Die negative Kontrolle zeigt nach dem Entwickeln eine starke Farbreaktion. Ursache: ungenügendes W aschen. Abhilfemaßnahme: Intensiveres Waschen, Entfernen überschüssiger Flüssigkeit aus den W ells durch Ausklopfen auf einem saugfähigen Tuch. Lassen Sie die Wells nicht austrocknen. 11 INTERPRETATION DER ERGEBNISSE Interpretation der Ergebnisse:- ELISA Readers Stellen Sie den Nullpunkt des ELISA-Readers mit Luft auf Null, messen Sie die Absorption in den W ells bei 450/650 - 620 nm Positiv: Die Absorption ist höher als Oder gleichen Sie zu 0.3 OD-Einheiten. Negativ: Die Absorption ist geringer als 0.3 OD-Einheiten. Ein negativer OD-Wert zeigt an, dass bei dem Patienten kein Antikörper nachgewiesen werden kann. Dies kann auf das Nichtvorhandensein einer Infektion oder auf eine schwache Immunreaktion beim Patienten zurückzuführen sein. Interpretation der Ergebnisse: Visuell Vergleichen Sie die Kontrollen. Eine Probe sollte als positiv interpretiert werden, wenn eine Farbentwicklung deutlich erkennbar ist. Soltanto per l’uso diagnostico in vitro. 2 RIASSUNTO Toxocariasi é ina infezione causata da ascaridi del genere Toxocara (normalmente T. canis, raramente T. cati) che si contrae dall’ingestione di uova fecondate presenti in feci animali. Queste uova sviluppano embrioni 2-5 settimane dopo aver lasciato l’animale. Una infezione non occorre pertanto dal contatto con feci freschi. Inoltre l’infezione non passa dall’uomo all’uomo (6). La malattia si manifesta sia come stato di larvi migrans viscerali (VLM) sia come stato di larvi migrans oculari (OLM). I sintomi di VLM possono variare da stati asintomatici con una leggera eosinofilia a una condizione grave e potenzialmente fatale. Anche pazienti con OLM possono variare molto nella severità dei sintomi; da asintomatici fino al presentare acute lesioni dell’occhio. Le larve migranti di Toxocara sono al secondo posto delle infezione di elminti in paesi sviluppati (1,2). Non esiste alcun metodo definitivo e univoco per la diagnosi di una infezione di Toxocara. Pertanto la vera specificità e sensitività di test sierologici non può essere determinata. La diagnosi e anche complicata dal fatto che la risposta anticorporale immunologica dipende fortemente dal sito dell’infezione e dal numero di embrioni assunti. Nonostante queste limitazioni, numerosi studi mostrano che test immunologici che usano un antigene escreto purificato delle larve, come lo fa questo ELISA, portano ad una sensitività e specificità enormemente incremenata comparati a test basati su un antigene non purificato (1-6). 3 PRINCIPIO DEL TEST I micropozzetti sono ricoperti con l’antigene escreto dalle larve di Toxocara. Durante la prima incubazione con il siero diluito di patienti, gli anticorpi reattivi con l’antigene rimaranno legati ai pozzetti. Dopo il lavaggio necessario per rimuovere il restante siero, il coniugato enzimatico è aggiunto. Se si trovano anticorpi legati ai pozzetti, il coniugato enzimatico si legherà a sua volta a questi anticorpi. Dopo una ulteriore serie di lavaggi, un cromogeno (benzidine tetrametilico, TMB) è aggiunto. Se il coniugato enzimatico è presente, la perossidasi del coniugato catalizzerà la reazione che consuma perossido d’idrogeno e il cromogeno vira dall’incolore al blu. L’aggiunta della soluzione d’arresto termina la reazione enzimatica e il colore blu vira ad un giallo chiaro. La reazione può essere letta otticamente o con uno spettrometro ELISA. 4 REAGENTI Item Description Test Strips micropozzetti contenenti antigeni contro Toxocara – 96 pozzetti in un contenitore. Enzyme Conjugate Coniugato enzimatico: Un (1) flacone contenente 11 ml di una proteina A coniugata alla perossidasi. Positive Control Siero di controllo positivo: Una (1) fiala contenente 1 ml di siero di coniglio positivo. Negative Control Siero di controllo negativo: Una (1) fiala contenente 1 ml di siero umano diluito. Chromogen Cromogeno: Un (1) flacone contenente 11 ml del cromogeno TMB. Wash Concentrate (20X) Soluzione di lavaggio concentrato (20X): Un (1) flacone contenente 25 ml un concentrato di una soluzione tamponante e tensioattiva. Il concentrato di lavaggio può presentare cristalizzazzioni se mantenuto a temperature tra 2 °C – 8 °C. I cristalli si dissolveranno se diluito alla concentrazione di lavoro. Non utilizzare sieri contaminati microbiologicamente o che presentano torbidità per un elevato contenuto lipidico. Campioni iperlipidici devono essere chiariti prima dell’uso. Trattare ogni siero come possibilmente infettato. Controlli negativi sono stati analizzati e trovati negativi per l’antigene dell’epatite B e per l’anticorpo a HIV. Questo prodotto dovrebbe comunque essere trattato con le opportune precauzioni come ogni potenziale materiale infettivo. Non aggiungere acidi o uno dei reagenti ai campioni di siero. 6 MAGAZZINAGGIO Reagenti, pozzetti e flaconi: mantenere a 2 °C – 8 °C. La soluzione di lavaggio diluita può essere mantenuta a temperatura ambiente. 7 PREPARAZIONE Soluzione di lavaggio Aprire il tappo e aggiungere il contenuto del flacone a 475 ml di acqua pura. Versare la soluzione così diluita in una bottiglia a spruzzo con una stretta apertura. NOTA: il lavaggio si attua riempendo ogni pozzetto con la soluzione. Poi si vuota capovolgendo i pozzetti per riempirli nuovamente. 8 COLLEZIONE E PREPARAZIONE DEI SIERI Coagulare il sangue e rimuovere il siero. I campioni dovrebbero essere magazzinati ben chiusi fino a 3-5 giorni a 2 °C a 8 °C. Campioni magazzinati per un periodo più lungo (3-6 mesi) dovrebbero essere congelati a ≤ 20°C. Non riscaldare campioni inattivati ed evitare di congelare e scongelare ripetutamente i campioni. Campioni: Diluire i sieri dei pazienti con un rapporto 1:64 usando il tampone di diluizione (p.es. 5 µL siero e 315 µL tampone). 9 MATERIALI 9.1 Materiali e strumenti richiesti ma non contenuti nel kit Micropipette Bottiglia a spruzzo per i lavaggi Acqua pura da analisi e cilindro calibrato Tubetti per la diluizione dei campioni Carta assorbente Fotometro ELISA calibrato per microtiter-piastre con un filtro per 450 nm e uno da 650 a 620 nm (non necessario se i risultati sono determinati otticamente) Hölzel Diagnostika GmbH, Hansaring 10, 50670 Köln, Germany 12 Toxocara canis IgG ELISA HZ-3518 10 ATTUAZIONE DEL TEST 1. Staccare il numero di pozzetti richiesti (due per I controlli piu’ il numero dei campioni) e posizionare nell’aposito supporto. 2. Aggiungere 100 µL (o due gocce) del controllo negativo al pozzetto #1, 100 µL del controllo positivo al pozzetto #2 e 100 µL dei campioni diluiti (1:64) ai rimanenti pozzetti. NOTA: controlli positivi e negativi sono gia’ diluiti. Non diluire ulteriormente. 3. Lasciar incubare a temperatura ambiente (15-25°C) per 10 minuti. 4. Vuotare i pozzetti capovolgendoli con una leggera scossa e lavare tre volte con il tampone di lavaggio. 5. Aggiungere due gocce del coniugato enzimatico in ogni pozzetto. 6. Incubare a temperatura ambiente per 5 minuti. 7. Vuotare i pozzetti e lavare tre volte con il tampone di lavaggio. Rimuovere le gocce d’acqua rimanenti rivoltando il supporto sulla carta assorbente con una leggera scossa. 8. Aggiungere due gocce del cromogeno ad ogni pozzetto. 9. Incubare a temperatura ambiente per 5 minuti. 10. Aggiungere due gocce della soluzione d’arresto a agitare bene 11 RILEVAMENTO DEI RISULTATI con il fotometro ottica 12 LIMITI DEL TEST Risultati sierologici sono un aiuto per la diagnosi ma non possono mai costituire l’unico metodo per la diagnosi differenziale. 13 CONTROLLO QUALITÀ L’utilizzo di controlli permette di controllare la stabilità del kit. Il kit non deve essere utilizzato se uno di questi parametri sono fuori dall’intervallo di confidenza. Valori aspettati per i controlli sono: negativo: 0.0 a 0.3 unità di densità ottica positivo: 0.5 unità e più 14 POSSIBILI ERRORI I controlli negativi hanno un colore troppo intenso. Causa: lavaggi insufficienti Rimedio: lavare con più cura. Rimuovere le gocce d’acqua rimanenti rivoltando il supporto sulla carta assorbente con una leggera scossa. Non far seccare i pozzetti. Misurare i pozzetti a 450/650 a 620 nm. Positivo - Assorbanze maggiori di 0.3 unità di densità ottica (DO) Negativo - Assorbanze minori di 0.3 unità di densità ottica (DO) Un valore negativo indica che il paziente non ha valori rilevabili di anticorpi. Questo può essere dovuto o ad una mancata infezione o a una risposta immunologica insufficiente. 15.2 Interpretazione ottica dei risultati Confrontare i risultati con i controlli. Un campione è da considerare positivo se lo sviluppo del colore è obiettivamente significante. 16 VALORI ASPETTATI Il numero degli individui con risultati positivi può variare significatamente tra popolazioni diversi e regioni geografiche diversi. Possibilmente ogni laboratorio dovrebbe costruire il proprio intervallo di valori aspettati per la propria popolazione di pazienti. 17 STUDI EFFETTUATI Studio #1 – Canadian Reference Center Il centro ha comparato il HÖLZEL ELISA con un altro ELISA test commerciabile e ha trovato una concordanza del 84% (n = 82).
