Lo scopo del lavoro è di valutare l’efficacia e la sicurezza della vertebroplastica percutanea in soggetti con dolore focale provocato dalle fratture vertebrali da compressione. Materiale e metodi. In un periodo di 11 mesi, sono stati sottoposti a trattamento di vertebroplastica percutanea 49 pazienti, di cui 7 sono stati ritrattati, per un totale di 56 interventi su 108 vertebre. I pazienti erano affetti da fratture vertebrali determinate da cedimento osteoporotico (n=28) o da processi infiltrativi benigni e maligni (n=21). Tutti i pazienti sono stati visitati al momento della dimissione e successivamente intervistati telefonicamente a distanza di una settimana, uno, tre, sei e nove mesi dall’intervento per accertare l’evoluzione del dolore ed eventuali modifiche della qualità di vita. La durata media del follow-up è stata di 3,8 mesi. Risultati. A distanza di una settimana dal trattamento tutti i pazienti hanno riferito una completa scomparsa o un significativo miglioramento del dolore. In 8 pazienti su 49 (16,3%) si è verificata una recidiva del dolore; 7 di questi pazienti sono stati sottoposti ad un secondo trattamento ad un altro livello con pronta risoluzione della sintomatologia algica. Dopo l’intervento solo 3 pazienti (6,1%) hanno continuato ad assumere FANS, mentre prima della manovra tutti facevano uso di farmaci antidolorifici. Abbiamo inoltre osservato un’importante beneficio sulla qualità di vita, in quanto dopo il trattamento tutti i pazienti hanno riferito un miglioramento della capacità deambulatoria e solo 3 (6,1%) erano ancora costretti a portare un corsetto ortopedico (contro 15 nel preoperatorio). Abbiamo avuto una sola complicanza grave (1,7%), rappresentata dalla formazione di un ematoma sottocutaneo paravertebrale, risoltosi in una settimana circa. Otto pazienti (16,3%) hanno sviluppato dolore transitorio nella sede della puntura o radicolopatia nei giorni successivi alla manovra. In 63 su 108 delle vertebre trattate (58%) si sono verificati piccoli spandimenti asintomatici di cemento all’esterno del corpo vertebrale e in 2 pazienti asintomatici (3,5%) si è riscontrata la presenza alla radiografia del torace di un piccolo embolo polmonare di cemento. Discussione. La nostra esperienza conferma l’efficacia della vertebroplastica nel trattamento del dolore da frattura vertebrale. Se le indicazioni sono rispettate il miglioramento dei sintomi è spesso immediato, come il ripristino dell’attività motoria, e il gradimento per l’intervento elevato. L’utilizzo di metodiche adeguate di guida limita il tasso di complicanze. PAROLE CHIAVE: Vertebroplastica - Fratture vertebrali - Osteoporosi - Metastasi. Aim. The aim of this paper is to assess the effectiveness and safety of percutaneous vertebroplasty in patients with focal pain caused by compression vertebral body fractures. Materials and methods. Over an eleven-month period 49 patients underwent percutaneous vertebroplasty, of which seven were retreated, for a total of 56 operations on 108 vertebrae. The patients were affected by osteoporotic compression fractures (n=28) or by benign and malignant infiltrative processes (n=21). All of the patients were examined at discharge and later at one week, and one, three, six and nine months after surgery to ascertain the development of the pain and possible changes in the quality of life. The mean length of follow-up was 3.8 months. Results. One week after treatment all patients reported complete disappearance or significant alleviation of the pain. In 8 out of 49 patients (16.3%) there was a reoccurrence of pain; 7 of these patients underwent further treatment at another level with immediate pain relief. After surgery only three patients (6.1%) continued to take non-steroidal anti-inflammatory drugs (NSAIDs), whereas prior to surgery all patients were taking pain medication. We also observed an important benefit in the quality of life, in that after treatment all patients reported an improvement in functional abilities, and only three (6.1%) still had to wear a back brace (against 15 in the preoperative period). We only had one serious complication (1.7%) which involved the formation of a subcutaneous paravertebral haematoma, which was resolved in about a week. Eight patients (16.3%) developed transient pain at the site of the puncture or radiculopathy in the days following the operation. In 63 out of 108 of the treated vertebrae (58) there were small asymptomatic leakages of cement outside the vertebral body and in two asymptomatic patients (3.5%) the chest radiograph revealed a small pulmonary embolism of cement. Discussion. Our experience confirms the effectiveness of vertebroplasty in the treatment of pain caused by vertebral fractures. If the indications are respected the improvement of symptoms is often immediate, such as the return of mobility, and patient satisfaction with surgery is higher. The use of appropriate guiding systems limits the number of complications. KEY WORDS: Vertebroplasty - Vertebral fracture - Osteoporosis - Metastases. Il cemento acrilico più utilizzato è il polimetilmetacrilato (PMMA). Numerose pubblicazioni descrivono l’impiego del PMMA nel trattamento chirurgico delle fratture vertebrali patologiche e dei tumori primitivi dell’osso quali ad esempio gli angiomi aggressivi e i tumori giganto cellulari [1, 2]. In un tempo successivo, con l’affinamento dei materiali e della diagnostica per immagini, è stato possibile introdurre il cemento per via percutanea sotto guida radiologica senza dover ricorrere quindi ad un intervento «a cielo aperto». Il primo intervento di vertebroplastica per via percutanea è stato eseguito per il trattamento di un angioma aggressivo nel 1984 da Galibert e Deramond [3]. Successivamente le indicazioni alla vertebropastica sono state allargate al trattamento delle metastasi ossee, alle fratture su base osteoporotica e alle localizzazioni di malattia linfoproliferativa [4-6]. Scopo del presente lavoro è di valutare l’efficacia e la sicurezza della vertebroplastica percutanea in un gruppo di pazienti trattati consecutivamente presso il Servizio di Radiologia Inteventistica di un Centro Oncologico. Materiale e metodi Casistica personale Dal 1 agosto 2002 al 2 luglio 2003 sono stati sottoposti a trattamento di vertebroplastica percutanea 49 pazienti, 18 maschi e 31 femmine, di età compresa tra 36 e 91 anni (media 69±12 anni). Sette su 49 (14,3%) sono stati ritrattati, per un totale di 56 interventi su 108 vertebre. La patologia che più frequentemente ha posto indicazione all’intervento è stata l’osteoporosi (28 pazienti), mentre 12 pazienti erano affetti da metastasi da carcinoma (5 casi di origine polmonare, 4 gastrointestinale, 2 mammaria, e 1 renale), 5 da mieloma, 2 da linfoma e uno da una localizzazione di emangiopericitoma. Infine è stato trattato un caso di angioma. In 1 paziente il trattamento è stato eseguito nella stessa seduta su 4 vertebre, in 11 pazienti su 3, in 27 su 2 e in 17 su una sola vertebra. Il trattamento è stato associato ad ablazione mediante radiofrequenza nel paziente affetto da emangiopericitoma. La figura 1 mostra la distribuzione delle vertebre trattate. Criteri di inclusione La vertebroplastica percutanea è indicata in tutti i casi di frattura «da carico» del corpo vertebrale associata a dolore persistente non radicolare e non rispondente alla terapia medica. Per fallimento della terapia medica si intende la scarsa risposta alla somministrazione di analgesici, la necessità di utilizzare analgesici narcotici o di posizionare un corsetto ortopedico. Il dolore deve essere di tipo focale, intenso, localizzato lungo la linea mediana, con evidenza alla radiografia del rachide di una frattura del corpo vertebrale. La vertebra da trattare viene identificata dall’accentuazione del dolore durante la compressione delle apofisi spinose, condotta sotto guida radioscopica a livello della lesione. The most frequently used acrylic cement is polymethylmethacrylate (PMMA). Numerous studies describe the use of PMMA in the surgical treatment of pathological vertebral fractures and primary bone tumours such as aggressive angiomas and giant cell tumours [1, 2]. With the refinement of materials and diagnostic imaging, it was later possible to introduce the cement percutaneously under radiological guidance without the need of open surgery. The first percutaneous vertebroplasty operation was performed in the treatment of an aggressive angioma in 1984 by Galibert et al. [3]. Thereafter the indications for vertebroplasty broadened to include treatment of osseous metastases, osteoporotic compression fractures and the localisation of lymphoproliferative diseases [4-6] The aim of this study is to assess the effectiveness and the safety of percutaneous vertebroplasty in a group of patients treated consecutively at the interventional radiology department of a cancer centre. Materials and methods Patient group From 1 August 2002 to 2 July 2003 49 patients (18 males, 31 females) aged between 36 and 91 years (mean 69±12 years) underwent vertebroplasty. Seven of the 49 patients (14.3%) underwent further treatment, for a total of 56 operations on 108 vertebrae. The most frequent pathology indicating vertebroplasty was osteoporosis (28 patients), while 12 patients were affected with metastasis of a carcinoma (five cases with pulmonary origin, four gastrointestinal, two breast and one renal), five with myeloma, two with lymphoma and one with the localisation of a haemangiopericytoma. Lastly, there was one case of angioma. In one patient the treatment was performed on four vertebrae in the same session, in 11 patients on three vertebrae, in 27 on two vertebrae and in 17 on one vertebra. The treatment was associated with radiofrequency ablation in the patient affected by haemangiopericytoma. Figure 1 summarises the distribution of the vertebrae treated. Inclusion criteria Percutaneous vertebroplasty is indicated in all cases of vertebral body compression fractures associated with persistent non-radicular pain which does not respond to conservative treatment. Failure of conservative treatment includes the limited response to the administration of pain medication, the need for narcotic pain medication or a back brace. The pain must be of the intense focal variety, localised along the medial line, with evidence at spinal radiography of a fracture of a vertebral body. The vertebrae to be treated are identified by the increase in pain during compression of the spinous processes of the vertebrae, performed under radioscopic guidance at the level of the lesion. The causes of vertebral collapse are the loss of bone minerals owing to osteoporosis [2, 7-9] and benign and malignant infiltrative processes [2, 7, 8]. La vertebroplastica percutanea non è indicata: nei soggetti asintomatici, se vi è una prevalenza del dolore radicolare, qualora vi sia giovamento dalla terapia medica condotta con analgesici non narcotici, nei pazienti con tumori che si estendono allo spazio epidurale o che invadono più del 20% del canale vertebrale, nelle fratture traumatiche acute e come profilassi in soggetti osteopenici senza segni radiologici di frattura [2, 7, 8]. Sono controindicazioni assolute: le infezioni locali, la coagulopatia non correggibile e l’allergia a una qualsiasi delle componenti utilizzate nella procedura. 25 22 Numero trattamenti 264 20 17 15 13 10 8 8 8 5 5 1 1 1 2 1 13 4 2 1 0 Tecnica I pazienti vengono informati dei benefici e dei rischi dell’intervento ed è richiesta la firma del consenso informato. Viene eseguita una visita anestesiologica, avendo cura di valutare attentamente la funzionalità respiratoria, che può essere compromessa dal decubito prono, obbligatorio per il trattamento, e dalla possibile tossicità della componente volatile monomerica del cemento [10]. La procedura viene eseguita nella maggior parte dei casi mediante guida fluoroscopica. Per il trattamento delle vertebre cervicali e dorsali alte, che sono una minoranza, è cautelativamente indicato l’utilizzo della guida combinata di TC e fluoroscopia. Con la guida TC si riconoscono meglio i peduncoli vertebrali ed è più facile stabilire l’inclinazione dell’ago sul piano assiale. Pressione, saturazione arteriosa ed elettrocardiogramma sono monitorati durante tutta la procedura. L’ago comunemente impiegato per la vertebroplastica è dotato di mandrino ed ha un diametro compreso tra 11 e 15 Gauge. La sua infissione avviene sotto guida radiologica dopo che è stata effettuata l’anestesia locale con ago Chiba, estesa in profondità fino al periostio (fig. 2A). L’avanzamento dell’ago oltre la corticale e all’interno dell’osso si ottiene con l’ausilio di un martello ortopedico. Il percorso è transpeduncolare per le vertebre lombari e dorsali inferiori, mentre viene preferito l’accesso parapeduncolare, attraverso l’articolazione costo-trasversaria, per il trattamento delle vertebre dorsali alte a causa dell’esigua dimensione dei peduncoli (fig. 2B). A livello cervicale si utilizza un approccio antero-laterale. Nell’unico caso di dente dell’epistrofeo trattato è stato utilizzato l’accesso anteriore transorale e un ago da 15 Gauge. Nella nostra casistica è stato utilizzato un accesso monolaterale in 95 delle 108 vertebre trattate (88%); è stata scelto un approccio bilaterale unicamente in quei pazienti in cui, trattando un solo lato, non è stata osservata soddisfacente diffusione del cemento [11]. Prima di iniettare il cemento è stata eseguita in tutti i casi biopsia ossea con ago tranciante da 16 o 18 Gauge, introdotto coassialmente attraverso l’ago da vertebroplastica. Questo perché, malgrado spesso vengano effettuate indagini diagnostiche preoperatorie approfondite, non sempre vi è la certezza della diagnosi di natura del cedimento vertebrale. La flebografia non è mai stata eseguita perché, in accordo con quanto riferito in letteratura, non considerata predittiva di un eventuale stravaso di cemento nelle strutture vascolari [2, 12]. Distribution of the vertebrae treated with percutaneous vertebroplasty (total 108 vertebrae). ty include: (1) the absence of symptoms; (2) prevalently radicular pain; (3) improvement following conservative treatment performed with non narcotic pain medication; (4) tumours extending to the epidural space or invading more than 20% of the vertebral canal; (5) acute traumatic fractures; and (6) prophylaxis in osteopenic patients without radiological evidence of fracture [2, 7, 8]. Absolute contraindications are: (1) local infections; (2) coagulopathy which cannot be corrected; and (3) allergy to any of the components used in the procedure. Technique The patients are informed of the benefits and risks of the procedure and asked to sign for informed consent. An anaesthesiological examination is performed, with particular attention to the assessment of respiratory functionality, which can be compromised by the compulsory prone position of the procedure and by the possible toxicity of the volatile monomeric component of the cement [10]. In most cases the procedure is performed under fluoroscopic guidance. For the treatment of cervical and upper dorsal vertebrae, which are a minority, the combined guidance of computed tomography (CT) and fluoroscopy is a precautionary indication. With CT guidance the vertebral pedicles are better identified and the inclination of the needle in the axial plane is easier to establish. Pressure, arterial saturation and electrocardiogram are monitored throughout the procedure. The most commonly used bone needle for vertebroplasty is beveled tip and has a diameter between 11- and 15-gauge. The Chiba needle is inserted under radiological guidance to the depth of the periosteum and the local anesthesia is performed (Fig. 2A). The advancement of the bone needle through the cortex and inside the bone is obtained with the use of an orthopaedic hammer. The route chosen for the lumbar and lower dorsal vertebrae is transpedicular, whereas in the treatment of the upper dorsal vertebrae a parapedicular approach through the costal-transverse process articulation is preferred owing to the limited size of the pedicles (Fig. 2B). Una volta verificato il corretto posizionamento dell’ago all’interno del corpo vertebrale, si procede all’iniezione del PMMA sotto guida fluoroscopica per monitorare la perfusione del cemento e per identificare precocemente l’eventuale stravaso in sede extravertebrale (fig. 2C-E). Per ottenere un sufficiente effetto consolidante è sufficiente l’iniezione di 2-6 ml di cemento. I preparati di PMMA da noi utilizzati sono l’Osteopal®V (Biomet Merck GmbH, CH-6460 Altdorf, Svizzera) e lo Spine-Fix (Teknimed S.A., Bigorre, Francia). Un solo kit può essere sufficiente per trattare più vertebre se il preparato, una volta miscelato e aspirato nelle siringhe, viene immerso in un bagno di soluzione fisiologica sterile ghiacciata, evento che determina un sensibile prolungamento del tempo di polimerizzazione [13]. Al termine della procedura viene effettuato un controllo TC, per valutare la diffusione del cemento e la presenza di eventuali complicanze (fig. 2F), e una radiografia del torace per evidenziare tracce di cemento embolizzato. Tutta la procedura, dal posizionamento degli aghi all’iniezione del cemento, è generalmente indolore e richiede un tempo variabile da 1 a 2 ore a seconda del numero di vertebre da trattare. Dopo il trattamento viene richiesto al paziente di mantenere il decubito supino per circa 1 ora. Le dimissioni avvengono generalmente in giornata. Analisi dei risultati Prima dell’intervento i pazienti trattati sono stati sottopo- lateral approach is adopted for the cervical vertebrae. An anterior transoral approach with a 15-gauge needle was adopted in the only case of C2 treated. In our case series monolateral access was used in 95 of the 108 vertebrae treated (88%). A bilateral approach was adopted only in those patients in which insufficient diffusion of the cement was observed after treatment on one side [11]. Before injecting the cement a bone biopsy was performed in all cases with a 16- or 18-gauge cutting needle introduced coaxially through the vertebroplasty needle. This was performed since the diagnosis of the nature of the vertebral collapse is not always certain, even though thorough preoperative diagnostic examinations are often carried out. In no cases was a phlebography performed in that in accordance with the literature it was not considered predictive of a possible extravasation of cement into the vascular structures [2, 12]. Once the correct positioning of the needle within the vertebral body has been verified, the PMMA is injected under fluoroscopic guidance to monitor the perfusion of the cement and to guarantee the early identification of a possible extravasation (Figs. 2C-E). The injection of 2-6 ml of cement is sufficient to obtain the necessary consolidating effect. The PMMA used by us are Osteopal V (Biomet Merck GmbH, CH-6460 Altdorf, Switzerland) and Spine-Fix (Teknimed S.A., Bigorre, France). Alla valutazione è stata aggiunta una stima della qualità di vita, intesa come capacità di deambulazione (con o senza ausili) e di svolgimento delle normali attività quotidiane, oltre che come la necessità di assunzione di farmaci antidolorifici non steroidei o oppioidi. Tutti i pazienti sono stati visitati al momento della dimissione e successivamente intervistati telefonicamente a distanza di una settimana, uno, tre, sei e nove mesi dall’intervento. La durata media del follow-up è stata di 3,8 mesi (±2,4). Al momento dell’intervista è stato chiesto al paziente di dare un giudizio indicando su una scala da 0 a 4 un valore di: 0 per la totale scomparsa del dolore, 1 per una sua significativa riduzione, 2 per una sua modesta riduzione, 3 se non vi erano variazioni sitomatologiche e 4 se il suo dolore era peggiorato. Il trattamento è stato considerato un successo nei casi in cui si è avuta una scomparsa o una significativa riduzione del dolore. Al paziente è stato richiesto di valutare se la qualità della vita si fosse modificata con l’intervento. I dati, raccolti su un database, sono stati elaborati complessivamente, per i soli pazienti neoplastici e per i soggetti con compressione vertebrale da osteoporosi. Facendo riferimento alle linee guida recentemente formulate dalla Society of Interventional Radiology, le complicanze sono state suddivise in lievi e gravi [12]. Sono state considerate lievi tutte quelle condizioni che non hanno avuto sequele ma che possono aver richiesto una blanda terapia medica o un periodo di osservazione in ospedale; come gravi quelle condizioni che hanno necessitato di terapie più aggressive e/o di un prolungamento dei tempi di ricovero o che hanno dato sequele importanti. Risultati Risposta sul dolore e sulla qualità di vita I risultati sono riassunti nella tabella I; la figura 3 rappresenta graficamente le variazioni del dolore locale nel tempo. In 41 su 49 pazienti (83,7%) si è verificata una completa scomparsa o una riduzione significativa del dolore entro le 24 ore dal trattamento, in percentuale simile nei due sottogruppi (neoplastici e non). Dopo una settimana dal trattamento tutti i pazienti riferivano di aver giovato della manovra. In 41 su 49 pazienti (83,7%), inclusi 8 pazienti deceduti per progressione neoplastica, la significativa riduzione o la scomparsa della sintomatologia algica focale si è mantenuta per tutta la durata del follow-up. In altri 8 pazienti si è verificata una recidiva del dolore dopo un periodo variabile di benessere. In una donna affetta da mieloma multiplo la procedura ha determinato inizialmente un miglioramento della lombalgia, mentre a 3 mesi si è verificato un peggioramento che soggettivamente ha portato il dolore ai livelli preoperatori. La paziente ha però rifiutato qualsiasi valutazione clinico-strumentale. I restanti 7 pazienti sono stati invece ritrattati per insorgenza di un dolore vertebale ad un altro livello. Nei 3 soggetti con osteoporosi la seconda procedura è stata resa necessaria dal cedimento dei corpi vertebrali adiacenti a quelli precedentemente trattati, rispettivamente a 1, 2 e 3 mesi dal primo intervento (fig. 4). Nei 4 pazienti oncologici il trattamento è stato effettuato per comparsa di nuove localizzazioni verte- ing a notable lengthening of the polymerisation time [13]. Once the procedure has been completed a CT examination is performed to assess the diffusion of the cement and the presence of complications (Fig. 2F) as well as a chest radiograph to identify traces of embolised cement. The entire procedure, from the positioning of the needles to the injection of the cement, is generally painless and requires between 1 to 2 h according to the number of vertebrae to be treated. After the treatment the patient is asked to maintain the supine position for about 1 h. The patient is generally discharged on the same day. Analysis of the results Prior to surgery all patients underwent a medical examination to assess the site and intensity of the pain. An assessment of the quality of life was also included, taking into consideration factors such as mobility (with or without aids) and to perform normal daily activities, as well as the need to take NSAIDs or opiates. All of the patients were examined on discharge and later interviewed over the phone one week, and one, three, six and nine months after treatment. The mean period of follow-up was 3.8±2.4 months. In the interviews the patients were asked to give a judgement indicating on a scale from 0 to 4 a value of 0 for total pain relief, 1 for significant pain relief, 2 for slight pain relief, 3 if there had been no symptomatological variations and 4 if the pain had worsened. Treatment was considered a success in cases in which there was complete or significant pain relief. The patients were also asked to assess whether their quality of life had changed after surgery. The data, which were recorded in a database, were also processed separately only for the patients with tumours and with vertebral collapse caused by osteoporosis. With reference to the guidelines recently formulated by the Society of International Radiology, the complications were divided into slight and serious [14]. Slight complications included all of those conditions which did not have sequelae but which may have required bland medical treatment or a period of observation in hospital. Serious conditions were considered those which required more aggressive therapy and/or prolonged hospitalisation or which led to important sequelae. Results Response to pain and quality of life The results are summarised in Table I. Figure 3 graphically represents the variations in pain over time. In 41 out of 49 patients (83.7%), there was complete or significant pain relief within 24 h of treatment, with similar percentages in the two subgroups (tumour and non tumour). One week after surgery all patients reported to have benefited from the procedure. In 41 out of 49 patients (83.7%), including eight patients who died owing to tumour progression, the significant or complete reduction of focal pain was maintained for the entire follow-up period. In the remaining eight patients there was a reccurrence of pain after a variable period of well 267 G. C. The patient, however, totally refused clinical and instrumental examinations. The remaining seven patients, on the other hand, underwent further treatment for the onset of vertebral pain at another level. In the three patients with osteoporosis the second procedure was necessary owing to the collapse of vertebral bodies adjacent to those previously treated, one, two and three months after the first procedure, respectively (Fig. 4). In the four cancer patients treatment was performed owing to the appearance of new symptomatic vertebral localisations one to four months after the initial procedure. Their was no further relapse in the seven patients re-treated (1 to 6 months follow-up). The parameters of quality of life also showed a significant improvement. All of the patients took pain medication before the procedure, including four who took opiates on a regular basis. After the procedure only three (6.1%) continued to take NSAIDs. Before the procedure 25 patients had limited mobility (51%) and 15 wore a back brace (30.6%). After surgery there was an improvement in mobility in all cases and only three patients (6.1%) were forced to continue wearing the brace, even though they reported a significant reduction in focal pain. One patient who was bedridden prior to the procedure was able to walk again without the use of aids. significativo miglioramento. The collection of blood cleared up spontaneously but the complication required hospitalisation of the patient for a week and the transfusion of blood derivatives. Una paziente costretta a letto prima della manovra è stata in grado di riprendere la deambulazione senza ausili. Complicazioni Nella nostra esperienza si è verificata una complicanza grave (1,7%) in un soggetto trattato a livello di tre vertebre dorsali che ha sviluppato un ampio ematoma sottocutaneo paravertebrale (fig. 5). La raccolta ematica si è risolta spontaneamente ma ha reso necessario il ricovero di circa una settimana e la trasfusione di emoderivati. Otto pazienti (16,3%) hanno sviluppato una sintomatologia dolorosa nei giorni succcessivi alla manovra. Si trattava di una modesta dolenzia cutanea nel punto di penetrazione dell’ago in 5 pazienti e di un dolore radicolare in 3. In 7 su 8 i sintomi si sono risolti entro una settimana. Nell’ultimo caso, in cui era presente un’estesa sostituzione neoplastica del corpo vertebrale, interessante il forame di coniugazione con distruzione del muro laterale della vertebra, la sciatalgia si è prolungata per un mese. In 63 su 108 delle vertebre trattate (58%) si sono verificati piccoli spandimenti asintomatici di cemento all’esterno del corpo vertebrale. La fuga più frequente si è verificata attraverso la vena basivertebrale con coinvolgimento del plesso venoso interno anteriore del canale rachideo (29 casi su 108 vertebre trattate; 26,8%). Solo in uno di questi casi il cemento si è esteso ad una vena intervertebrale. In 23 vertebre (21,2%) si è invece verificata una piccola fuga di cemento nel plesso venoso esterno anteriore e in vene lombari (fig. 4C). In 3 casi (2,7%), peraltro coinvolti da estrema malacia del muro laterale vertebrale, si è avuto un piccolo spandimento nei tessuti molli contigui al soma. Infine in 7 casi (6,5%) si è verificata impregnazione dello spazio discale da parte del PMMA. In 2 pazienti asintomatici (3,5%) si è riscontrata la presenza alla radiografia del torace di un piccolo embolo polmonare di cemento, confermato dalla TC, in entrambi i casi localizzato in un ramo tributario per il lobo medio. Discussione Lo scopo della vertebroplastica percutanea è la riduzione del dolore provocato da frattura da compressione del corpo vertebrale. I meccanismi che determinano il miglioramento o la scomparsa dei sintomi non sono noti ma si ipotizza che il cemento ristori l’integrità biomeccanica della vertebra compromessa determinando il ripristino della sua rigidità e robustezza [13], riducendo così le sollecitazioni sulle terminazioni nervose intraossee e periostali accentuate dai movimenti nel punto di frattura [14]. È stato dimostrato che la stabilizzazione del corpo vertebrale avviene in pochi minuti come effetto del consolidamento del cemento e questo dovrebbe determinare la quasi immediata riduzione del dolore nel punto trattato. Infatti, i risultati del nostro studio mostrano che già a 24 ore dalla procedura si verifica una riduzione del dolore focale nella maggior parte dei pazienti (83,7%) e che dopo una settimana la totalità dei soggetti trae giovamento del trattamento (tab. I). This involved a slight skin irritation at the puncture point of the needle in five patients and radicular pain in three. In seven of the eight patients the symptoms cleared up within a week. The remaining patient, in whom there was an extensive neoplastic vertebral body substitution which involved the vertebral foramen with the destruction of the lateral wall of the vertebra, reported pain for a month. In 63 of the 108 vertebrae treated (58%) there were small asymptomatic leakages of cement outside the vertebral body. The most common site of escape was through the basivertebral vein with involvement of the anterior internal venous plexus of the spinal canal (29 cases out of 108 vertebrae treated – 26.8%). Only in one of these cases the cement extended to the intervertebral vein. In 23 vertebrae (21.2%), on the other hand, there was a small leakage of cement in the anterior external venous plexus and in the lumbar veins (Fig. 4C). In three cases (2.7%), affected by extreme softening of the lateral vertebral wall, there was a small leakage of soft tissue adjacent to the vertebral body. Lastly, in seven cases (6.5%) there was an impregnation of the intervertebral disc space by the PMMA. In two asymptomatic patients (3.5%) the chest radiograph revealed the presence of a small pulmonary embolus of cement, confirmed at CT, in both cases localised in a tributary branch of the middle lobe. Discussion The aim of percutaneous vertebroplasty is the reduction of pain caused by a compression fracture of the vertebral body. The mechanisms which determine the improvement or the disappearance of symptoms are not known, but it is thought that the cement restores the biomechanical integrity of the fractured vertebra, leading to a return of its rigidity and robustness [15], thus reducing the stress on the intraosseous 270 G. Anselmetti et al: Treatment of painful compression vertebral fractures with vertebroplasty no un parziale ripristino della capacità ambulatoria, si riflette nel grado di soddisfazione per l’intervento manifestato da parte della maggioranza dei pazienti. Risultati sovrapponibili sono stati riferiti da altri Autori [7, 10, 15]. In uno studio recente sui risultati di 100 vertebroplastiche consecutive il 97% dei pazienti ha riferito un miglioramento significativo del dolore già a 12-24 ore dalla manovra [7]. La riduzione del dolore si è manifestata con eguale frequenza nei soggetti con osteoporosi e nei pazienti con patologia tumorale (85,5% vs 81,0%) a conferma dell’utilità della manovra come ausilio nella terapia antalgica nel paziente oncologico. I risultati della casistica di soli pazienti oncologici del M.D. Anderson Cancer Center, pubblicata di recente, riferiscono un miglioramento o una scomparsa del dolore in 49 su 58 trattamenti (84%) entro le prime 24 ore [10]. Contrariamente a quanto riferito dagli Autori suddetti, nella nostra casistica hanno prevalso in percentuale i soggetti in cui il dolore è completamente scomparso rispetto a quelli in cui la sintomatologia si è ridotta (tab. I). La figura 3 raffigura la percentuale di soggetti che hanno tratto giovamento dal trattamento nel tempo. I risultati sono costanti a conferma del fatto che l’efficacia del trattamento è duratura. Ciononostante si possono osservare due modeste inflessioni delle curve relative ai pazienti benigni (a un mese) e maligni (a 3 mesi). Nel caso dei soggetti benigni ciò è stato determinato dallo sviluppo di altri cedimenti nelle vertebre adiacenti a quelle trattate, evento che è avvenuto complessivamente in 3 su 28 pazienti (10,7%), percentuale sovrapponible a quanto riferito in letteratura [16]. Una seconda seduta ha consentito di porre rimedio con successo alla sintomatologia dolorosa in tutti i casi. Nei soggetti neoplastici la recidiva del dolore osseo è conseguenza di una progressione neoplastica che può avvenire nella sede trattata o ad un altro livello in caso di insorgenza di altre lesioni sostitutive. Il dolore è determinato dal cedimento vertebrale e dall’infiltrazione delle strutture contigue (tessuti molli, canale vertebrale, forame di coniugazione) e pertanto può avere caratteristiche complesse. La strategia scelta consiste nel tentativo di trattare le sedi di recidiva del dolore se queste corrispondono a punti di progressione neoplastica. In questi casi parte dell’effetto antalgico può essere dovuto alla reazione chimica esotermica, che avviene durante la fase di consolidamento del cemento, con sviluppo di temperature comprese tra i 70° e 90° C che hanno effetto citolitico. Tutti e 4 i pazienti oncologici ritrattati hanno avuto giovamento dall’intervento. Nei pazienti neoplastici è anche possibile utilizzare una strategia in cui si prevede l’associazione tra diversi tipi di trattamento quali la radioterapia, la radiofrequenza e la vertebroplastica. Sembra che la vertebroplastica non riduca l’effetto di un successivo trattamento radioterapico [5], né che l’uso di radiazioni ionizzanti alteri in vitro il PMMA [17]. La riduzione dell’uso di famaci analgesici e il miglioramento della capacità deambulatoria sono altri indici di come la procedura abbia giovato sulla qualità della vita. Se vengono rispettate le indicazioni e la procedura viene condotta correttamente le complicanze della vertebroplastica sono un evento raro [2, 9, 17, 20-22]. Il rischio di stravaso di cemento nel canale vertebrale è maggiore quanto maggiore è la distruzione ossea; per questo la vertebro- and periosteal nerve endings accentuated by movement at the site of the fracture [16]. It has been demonstrated that the stabilisation of the vertebral body takes place within several minutes as an effect of the consolidation of the cement, and this probably determines the almost immediate reduction of focal pain at the treated point. In fact, the results of our study show that as early as 24 h after the procedure there is a reduction of focal pain in most patients (83.7%) and that all patients benefit from the procedure within a week (Table I). The rapid improvement in pain, which is often accompanied by a partial return to mobility, is reflected in the level of satisfaction expressed by most patients. Similar results have been reported by other authors [9, 12, 17]. In a recent study on the results of 100 consecutive vertebroplasty procedures 97% of patients reported a significant pain relief within 12-24 h after surgery [9]. The reduction in pain was similar among patients with osteoporosis and patients with tumours (85.5% vs 81.0%), confirming the usefulness of the procedure as an aid to pain reduction therapy in cancer patients. The recently published results of a case series of only cancer patients at the M.D. Anderson Cancer Center report the alleviation of pain or complete pain relief in 49 out of 58 treatments (84%) within the first 24 h [12]. In contrast to the results of these authors, in our case series the percentage of patients with complete pain relief was greater than the percentage of patients in whom there was simply a reduction in pain (Table I). Figure 3 summarises the percentage of patients who benefited from the treatment over time. The results are constant and confirm that the effectiveness of the treatment is long-lasting. Nonetheless, two slight troughs are evident in relation to patients with osteoporosis (1 month) and cancer patients (3 months). In the case of patients with osteoporosis this was due to the collapse of vertebrae adjacent to those treated, an event which occurred in 3 out of 28 patients (10.7%), a percentage similar to data reported in the literature [18]. A second session enabled the problem to be resolved with complete pain relief in all cases. In cancer patients the relapse of bone pain is a consequence of a progression of the tumour which can occur at the site treated or at another level in the case of other metastatic lesions. The pain is caused by vertebral collapse and infiltration of the adjacent structures (soft tissue, vertebral canal, vertebral foramen) and can therefore have complex characteristics. The strategy chosen consists of attempting to treat the sites of pain reoccurrence if these correspond to points of tumour progression. In these cases a part of the antalgic effect might be due to the exothermic chemical reaction, which takes place during the phase of consolidation of the cement, with temperatures reaching between 70° and 90°C and having a cytolitic effect. All four re-treated cancer patients benefited from the procedure. It was also possible in the cancer patients to use a strategy involving the association of different types of treatment such as radiotherapy, radiofrequency and vertebroplasty. It appears that vertebroplasty does not reduce the effect of a successive treatment with radiotherapy [7], nor does the use of ionising radiation alter the PMMA in vitro [19]. The reduction of the use of pain killers and the improvement in mobility are other indications of how the procedure benefited the patients’ quality of life. If the indications are respected and the procedure is correctly conducted, the complications of vertebroplasty are a G. C. Per ridurre il rischio di complicanze legate alla manovra è indispensabile disporre di strumenti di guida adeguati. Nel nostro centro ad esempio viene utilizzata la guida combinata di TC e fluoroscopia quando l’approccio al corpo vertebrale è complesso (fig. 2). Nella nostra serie si è verificata una complicanza grave. Si trattava di una donna che ha sviluppato un ematoma dei tessuti molli nella sede dell’intervento che ha richiesto un’emotrasfusione ed il prolungamento della degenza (fig. 5). Nei restanti casi i pazienti sono stati dimessi entro le 24 ore dalla procedura. Algie nella sede di puntura dell’ago e temporanee radiculopatie sono state trattate con successo con analgesici in 7 su 8 pazienti; in un soggetto con sostituzione neoplastica di una vertebra la comparsa di una sciatalgia ad una settimana dal trattamento era probabilmente determinata da una progressione neoplastica. Lo stravaso di PMMA è stato descritto come evenienza frequente in letteratura [22, 23] ma raramente esso determina sintomi. Nel nostro studio è stato possibile stabilire l’esatta frequenza con cui si verifica lo spandimento in quanto tutti i pazienti sono stati sottoposti a TC e a radiografia del torace immediatamente dopo la manovra. In 63 su 108 vertebre (58%) è stato osservato uno stravaso di modesta entità in prossimità della vertebra trattata; in 2 pazienti sono stati osservati piccoli emboli di PMMA nel lobo medio. Nessuno dei pazienti ha sviluppato sintomi come conseguenza della fuga. Per evitare l’eccessivo spandimento di PMMA è importante monitorare in fluoroscopia l’iniezione del cemento che deve essere sospesa, almeno temporaneamente, nel momento in cui viene visualizzata una fuga. L’evidenza di uno spandimento al controllo effettuato dopo la procedura non deve destare preoccupazione in assenza di una sintomatologia. rare event [2, 9, 17, 20-22]. The risk of extravasation of cement in the vertebral canal is greater the greater the osseous destruction; for this reason vertebroplasty is not indicated in patients with a massive neoplastic substitution of the vertebral body. To reduce the risk of complications linked to the procedure the use of appropriate guidance devices is indispensable. In our centre, for example, we use combined CT and fluoroscopy guidance when the approach to the vertebral body is complex (Fig. 2). In our case series there was one serious complication. This regarded a woman who had developed a haematoma of the soft tissue at the site of the procedure which required a blood transfusion and prolonged hospitalisation (Fig. 5). In the remaining cases the patients were discharged within 24 h of the procedure. Irritation at the site of the needle puncture and temporary radiculopathy were successfully treated with analgesics in seven out of eight patients. In one patient with neoplastic substitution of a vertebra the onset of pain one week after treatment was probably caused by progression of the tumour. The extravasation of PMMA has been described as a frequent event in literature [22, 23], but rarely does it cause symptoms. In our study the exact frequency of extravasation was able to be established given that all patients underwent a CT scan and a chest radiograph immediately after the procedure. In 63 out of 108 vertebrae (58%) there was a small leakage in proximity to the vertebrae treated. In two patients small emboli of PMMA were observed in the medial lobe, although neither of the patients developed symptoms as a consequence of the leakage. To avoid the excessive extravasation of PMMA fluoroscopic monitoring of the injection of the cement is vital, and the injection should be halted, at least temporarily, when a leakage is observed. The evidence of extravasation at the follow-up examination after the procedure should not be a cause for concern in the absence of symptoms. Conclusioni Conclusions La nostra esperienza conferma che la vertebroplastica è una procedura efficace nel trattamento del dolore da frattura vertebrale da compressione. Se le indicazioni sono rispettate il miglioramento dei sintomi è spesso immediato, come pure il ripristino dell’attività motoria ed il gradimento per l’intervento elevato. La vertebroplastica è efficace anche nel trattamento della recidiva, purchè il dolore conservi le stesse caratteristiche, e non controindica altri ausili terapeutici quali la radioterapia e la radiofrequenza che, specie nel paziente oncologico, rappresentano spesso un valore aggiunto. Se condotta con utilizzo di metodiche guida adeguate il tasso di complicanze, specie di quelle gravi, è modesto. Our experience confirms that vertebroplasty is an effective procedure in the treatment of pain caused by vertebral body compression fractures. If the indications are respected the improvement of symptoms is often immediate, as is the return of functional abilities, with patient satisfaction regarding the procedure high. Vertebroplasty is also effective in the treatment of pain reccurrence, provided the pain has the same characteristics and does not contraindicate other treatments such as radiotherapy and radiofrequency which particularly in cancer patients can offer additional benefits. If the procedure is conducted using appropriate guidance methods, the number of complications, particularly serious complications, is minimal.
