RAZIONALE - Progetto Seneca: background ed obiettivi
La BPCO è la quinta causa di disabilità
nel mondo. L’impatto maggiore della
BPCO non è sulla mortalità, ma piuttosto
sulla qualità della vita, specialmente
a causa degli effetti combinati di età,
fattori sociali e delle comorbilità che
sono spesso presenti nei soggetti con
questa patologia. La variabile interrelazione di fattori respiratori e non che
condizionano l’indipendenza e lo stato
di salute rendono gli effetti della BPCO
sullo stato di salute molto eterogenei e
solo debolmente correlati con la severità
dell’ostruzione bronchiale. In realtà,
diverse caratteristiche non respiratorie
della BPCO (sarcopenia, osteoporosi,
deterioramento cognitivo, depressione)
e le malattie coesistenti - soprattutto
cardiovascolari - contribuiscono a
peggiorare le qualità della vita. Mentre c’è
una crescente consapevolezza di questi
effetti della BPCO e delle condizioni
correlate, non esiste un accordo su come
valutare lo stato di salute dei pazienti
con BPCO nella pratica clinica. Persino
definizioni come handicap e disabilità
sono frequentemente usate in maniera
non corretta fuori della ristretta cerchia
dei geriatri e degli pneumologi. Chiarire
queste problematiche è essenziale per
pianificare interventi mirati ad alleviare
o prevenire la disabilità correlata alla
BPCO.
Il progetto Seneca 2008 fornirà all’uditorio
di pneumologi italiani un approccio ben
strutturato all’argomento: “Disabilità e
Handicap nella BPCO: interrompere il
circolo vizioso”. Ogni sessione inizierà con
una introduzione, fatta dal Prof. Pahor,
sullo stato dell’arte delle definizioni
di differenti misure di stato di salute e
performance, disabilità e handicap.
Verranno anche descritti i principi
basilari di valutazione multidimensionale
e di transizione dalla valutazione
al
trattamento
multidisciplinare
dell’anziano fragile in modo da fornire
all’uditorio una panoramica chiara di
questi argomenti.
L’introduzione del Prof. Pahor verrà
seguita dalla presentazione dei risultati di
una indagine Eurisko su come i pazienti
affetti da BPCO e i loro medici, sia di base
che pneumologi, percepiscono l’effetto
della BPCO sullo stato di salute. I risultati
di questa indagine verranno forniti
nell’ambito delle linee-guida scientifiche
presentate dal Prof. Pahor.
La correlazione tra BPCO e disabilità
verrà valutata dal Prof. Bellia per meglio
definire il fenotipo del paziente BPCO
vulnerabile, che è proprio quello con più
probabilità di diventare disabile.
Gli strumenti per valutare la disabilità
verranno presentati dal Prof. Antonelli
Incalzi in una prospettiva pratica e di facile
comprensione con lo scopo di fornire
prove di efficacia e di distinguere strumenti
completi da strumenti surrogati.
La relazione finale del Prof. Rick Hodder
discuterà la possibilità che la terapia
farmacologica possa rallentare sia
la progressione della BPCO che il
peggioramento della dipendenza
personale nella BPCO e riassumerà ciò
che può essere fatto con le terapie non
farmacologiche attualmente disponibili
al fine di modificare e prevenire la
disabilità e l’handicap nella BPCO
ed includerà una varietà di preziose
indicazioni potenzialmente utili e
frequentemente ignorate nella pratica
clinica.
Per chiudere la giornata, una sessione
interattiva guidata di“Opinion Capturing”
aiuterà a riassumere le opinioni e
i suggerimenti comuni da parte
dell’uditorio e degli oratori su argomenti
selezionati relativi alla disabilità e
all’handicap nella BPCO.
RATIONALE - The Seneca Project: background and objectives
COPD is the fifth leading cause of disability
worldwide. The biggest impact of COPD
is not mortality, but rather quality of
life, especially because of the combined
effects of age social factors, and the comorbidities often seen in patients with
this disease. The interplay of respiratory
and non respiratory factors conditioning
personal capabilities and health status
make the effect of COPD on health status
very heterogenous and only weakly related
to the severity of bronchial obstruction.
Indeed, several non respiratory features
of COPD (muscle wasting, osteoporosis,
cognitive impairment, depression) and
concurrent diseases, mainly cardiovascular,
contribute to affect the quality of life. While
there is increasing awareness of these
effects of COPD and related conditions,
there is no consensus on how to best assess
the health status of the COPD patient in a
practical manner. Even definitions such
as handicap and disability are frequently
misused outside of the restricted realm
of geriatricians and pulmonologists.
Clarifying these issues is essential to
plan interventions aimed at relieving or
preventing COPD-related disability.
The Seneca 2008 project will provide
the audience of Italian pneumologists
with a well structured approach to the
topic “Disability and Handicap in COPD:
Breaking the Vicious Cycle”. Each session will
begin with a state of the art introduction
by Prof Pahor on the definitions of different
measuresofhealthstatusandperformance,
disability and handicap. The basic
principles of multidimensional assessment
and the transition from multidimensional
assessment to multidisciplinary treatment
of the frail elderly will also be described to
provide the audience with a clearcut view
of this topic.
Professor Pahor’s introduction will be
followed by the report of a survey by
Eurisko on how COPD patients and their
physicians, both general practitioners
and pneumologists, perceive the effects of
COPD on health status.The results of this
survey will be provided in the framework
of the scientifically founded guidelines
illustrated by Prof Pahor.
The relationship between COPD and
disability will be assessed by Prof Vincenzo
Bellia in order to ty and better define the
phenotype of the vulnerable COPD patient,
namely the one most likely to become
disabled.
The instruments rating disability will be
presented by Prof Antonelli Incalzi with the
aim of providing proofs of efficacy and to
distinguish full spectrum instruments from
surrogate measures in a practical, userfriendly perspective.
Whether
pharmacological
therapy
can slow both the progression of COPD
decline and the worsening of personal
dependency in COPD will be discussed by
Prof Rick Hodder whose final discussion
will summarize what can be done with
currently available non-pharmacological
measures to modify and prevent handicap
and disability in COPD and will include a
variety of potentially useful and frequently
neglected pearls for clinical practice.
To close the day, an Opinion Capturing
session will help to summarize the
common opinions and suggestions of the audience and the speakers on selected
topics of handicap and disability in COPD,
using a guided interactive session.
The participants in the Seneca program are
expected to actively contribute to discuss
the topic and to draw a final short, but well
structured list of ready to use take home
messages for everyday practice.
