In this study we analysed the effectofsomeparameters on the collection efficiency (CE) of the Cobe-Spectra cell separator endowed withAuto-PBSC software as well as the influence that the patients’ age and diagnosis can have on the mobilisation and collection of CD34+ cells. Materials and methods. A retrospective analysis was made of 233 PBSC collection procedures carried out in 140 patients (80 males and 60 females aged between 17 and 70 years) affected by different haematological malignancies.All the patients underwent stem cell mobilising chemotherapy, which differed according to their underlying disease, and received granulocyte colony-stimulating factor (G-CSF). Results. The data revealed that the degree of mobilisation was influenced by the diagnosis, being greater in patients with lymphomas than in patients with the other diseases considered, and negatively affected by age, although there was a statistically significant correlation between the number of CD34+ cells mobilised into the peripheral blood and the number harvested. Furthermore, it was seen that higher levels of white blood cells (above 15.000/µL), of peripheral blood CD34+ cells and cycle volume were associated with lower CE. Conclusions. The optimal regulation of the procedural parameters, also taking into account the pre-apheresis white cell count, can translate into higher quality results, less stress for patients and better use of resources. Tralafinedeglianniottantael’iniziodeglianninovanta, oltre al progredire delle esperienze cliniche, due eventi fondamentali contribuirono a creare una nuova dinamica dell’attività di raccolta aferetica delle cellule staminali: - la scoperta della possibilità di aumentare notevolmente il numero dei progenitori emopoietici circolanti promuovendone la dismissione midollare (mobilizzazione) attraverso l’utilizzo dei fattori di crescita, in particolare del G-CSF, sia somministrati singolarmente sia in associazione alla chemioterapia per potenziare l’effetto mobilizzante3,4; - la caratterizzazione delle cellule staminali come cellule CD34+ in citofluorimetria che ha consentito in tempi rapidi (90 minuti) il monitoraggio del livello di queste cellule circolanti durante il periodo di mobilizzazione, nonché il dosaggio delle cellule ottenuto nel prodotto aferetico al termine della procedura di raccolta5. Di pari passo, in questi anni, anche lo sviluppo tecnologico ha contribuito a rendere sempre più adeguata ed efficiente la strumentazione disponibile per l’esecuzione tecnica delle procedure aferetiche6,7. Tutti i separatori cellulari di ultima generazione sono dotati di programmi specifici per la raccolta delle cellule staminali con la finalità dimigliorarel’efficienzadiraccolta(CE)dellecelluleCD34+, di ottimizzare le caratteristiche del prodotto aferetico in relazione a specifiche esigenze cliniche e di proporre una gestione più automatizzata della procedura rendendola 141 D Marenchino et al. Introduction In the last decade the infusion of stem cells harvested from the peripheral blood by apheresis has become a widespread practice and in some cases has replaced the use of bone marrow as the source of cells for reconstituting haematopoietic activity in patients undergoing autologous or allogeneic transplantation1,2. At the end of the 1980s and the beginning of the 1990s, besides progress in clinical experience, two fundamental events contributed to creating new dynamics in the activity of apheretic collection of stem cells: · the discovery that it is possible to greatly increase the number of circulating haematopoietic stem cells by promoting their release from the bone marrow (mobilisation) through the use of growth factors, in particular granulocyte colony-stimulating factor (GCSF), administered alone or in combination with chemotherapy to increase the mobilising effect3,4; · cytofluorometriccharacterizationofstemcellsasCD34+ cells, which has enabled rapid (90 minutes) monitoring of the circulating levels of these cells during the mobilisation period, as well as assays of the cells obtained in the apheresis product at the end of the collection procedure5. Gli obiettivi che ci siamo proposti sono i seguenti: - inchemodolacontaleucocitariapre-aferesi,lariduzione delvolumecicloelamobilizzazionedellecelluleCD34+ possano influenzare la CE, - quanto le differenti patologie nonché l’età dei pazienti possano incidere sulla mobilizzazione e sulla raccolta dellecelluleCD34+. Si pone anche, come ulteriore obiettivo, di ricavare informazioni utili per una regolazione ottimale delle singole procedure aferetiche. Materiali e metodi Pazienti Dall’aprile1999aldicembre2004pressoilnostroservizio sono stati trattati 140 pazienti affetti da differenti neoplasie ematologiche. Tutti i pazienti sono stati sottoposti a chemioterapia mobilizzante associata a fattore di crescita (Lenograstim, rHuG-GSF). Le caratteristiche dei pazienti sono illustrate nella Tabella I. In this study we analysed 233 procedures for collecting peripheral blood stem cells (PBSC) carried out using continuous flow Spectra (COBE BCT, Lakewood, CO, USA) cell separators provided with Auto-PBSCsoftware. The aims of this study were to determine: - in what way the pre-apheresis white cell count, the reduction in cycle volume and the mobilisation of the CD34+ cells can influence the CE, - how much the different underlying diseases, as well as the age of the patients, can affect the mobilisation and collection of the CD34+ cells. Finally, we also tried to collect information that could be useful for optimal regulation of the individual apheresis procedures. Materials and methods Patients FromApril 1999 to December 2004, 140 patients with different haematological malignancies were treated.All the patients received mobilising chemotherapy associated with growth factors (Lenograstim, rHuG-GSF). Nei primi tempi una parte delle procedure sono state effettuate utilizzando le impostazioni del software consigliate dalla ditta. Questi parametristandardprevedevanounvolumeciclo≥1.000µL per i pazienti con una conta leucocitaria pre-aferesi ≥10.000µLsenzaapportaremodifichesostanzialialnumero totale di raccolte previste dal programma. Successivamente, al fine di migliorare la CE, si è provveduto ad introdurre alcune variazioni specifiche per ciascuna procedura riguardanti il volume ciclo, il volume di raccolta e il volume di sangue processato. Le caratteristiche tecniche delle procedure aferetiche sono riassunte nella TabellaII. The haematopoietic stem cells were mobilised by daily administrations of G-CSF at doses of 510 mg/kg of body weight and the concentration of CD34+ cells in the peripheral blood was monitored in order to identify the best time to start the collection, setting the minimum threshold concentration for harvesting CD34+ Blood Transfus 2006; 4: 141-50 Il software Auto-PBSC (V6.1) utilizza 3 fasi per la separazione e la raccolta delle cellule mononucleate nella cintura a doppio stadio. 1- Fase di accumulo In questa fase le cellule mononucleate vengono accumulate e concentrate nella cintura. 2- Fase di raccolta Questa fase inizia quando viene raggiunto il numero 143 D Marenchino et al. cells at >10/µL. In most cases a central venous catheter was used in the procedures as the route of collection or return of blood on the basis of the characteristics of the vascular accesses of the patient: in no case was it necessary to interrupt treatment because of problems associated with poor flow rate of the collection. Collection of the CD34+ cells and the technique used Overall, 233 PBSC collection procedures were carried out in 140 patients (mean value 1.6/patients, range 1-3) using continuous flow Spectra cell separators provided with Auto-PBSC softeware (V6.1). Some of the earlier procedures were carried out using the software settings advised by the manufacturer. These standard parameters called for a cycle volume of ≥1,000mL for patients with a pre-apheresis white cell count ≥10,000/µL, without introducing substantial changes to the total number of cells collected predicted by the programme. Subsequently, in order to improve the CE, some specific variations were introduced for each procedure; these modifications involved the cycle volume, the collection volume and the volume of blood processed. The technical characteristics of the apheresis procedures are summarised in Table II. TheAuto-PBSC software (V6.1) uses three phases for the separation and collection of mononuclear cells in the double circuitry separation chamber. 1- Accumulation phase In this phase the mononuclear cells are accumulated and concentrated in the chamber. 2- Collection phase This phase starts when the maximum number of cells that can be contained in the chamber has been reached. The predefined volume of cells collected in each cycle is 3mL, but this can be set from 1 to 5mL. 3- Washing phase Duringthisphasetheplasmapushesthecellsthroughthe collectionvalveinordertoguaranteethemaximumrecovery ofcellsinthefinalbag.Thepredefinedvolumeofthewash is7mL,butthiscanbechangedfrom2to20mL. Determination of the haematological parameters and of the CD34+ cells The cells in the peripheral blood were counted using a MICROS-60bloodcounter(ABXDiagnostics,Montpellier, France). The CD34+ cells in the peripheral blood and in the collection bags were analysed with a cytofluorimetric technique (Facs Calibur, Becton-Dickinson, San Josè, CA, 144 massimo di cellule che possono essere contenute nella cintura. Determinazione dei parametri ematologici e delle cellule CD34+ Il conteggio delle cellule del sangue periferico è stato effettuato utilizzando il contaglobuli MICROS-60 (ABX Diagnostics, Montpellier, France). Le cellule CD34+ presenti nel sangue periferico e nelle sacche di raccolta sono state analizzate con tecnica citofluorimetrica(FacsCalibur,Becton-Dickinson,SanJosè, CA, USA) utilizzando come reagente il "MonoclonalAb anti-CD34PEcloneHPCA-2(Becton-Dickinson),seguendo le linee guida della International Society of Hemotherapy andGraftEngeneering(metodoISHAGE)11. Calcolo della CE e analisi statistiche La CE è stata calcolata con la seguente formula: (CelluleCD34+totalipresentinellasaccadiraccolta)x100 CD34+/µL pre-aferesi x volume sangue processato Lo studio delle variabili che influenzano il valore della CEèstatoeffettuatoutilizzandoiltest"analisidellavarianza ad un criterio" ed il test "correlazione-regressione". Risultati Il numero totale di cellule CD34+ raccolte per singola procedura è stato di 0,12-43,5 x 108 (valore medio 5,5), corrispondentea0,26-62,1x106/kgdipesocorporeo(valore medio 8,1). È stata evidenziata, come già previsto, una correlazionestatisticamentesignificativatrailnumerodicellule CD34+ mobilizzate nel sangue periferico e quelle raccolte (p<0,0001). L’entità della mobilizzazione è risultata altresì influenzata dalla diagnosi raggiungendo i valori più elevati nei linfomi rispetto alle altre patologie considerate (p= 0,01) (Figura1),mentreèinfluenzatanegativamentedall’età(p<0,001) (Figura2).IdatisonoriassuntiinTabellaIII. Calculation of the CE and statistical analyses The CE was calculated using the following formula: (total CD34+ cells present in the collection bag) x 100 CD34+/µL pre-apheresis x volume of blood processed Blood Transfus 2006; 4: 141-50 310% (valore medio 68,9%). Il riscontro di valori superiori al 100%, seppur occasionale, è probabilmente attribuibile al rapido incremento delle cellule CD34+ nell’arco di tempo (5-6 ore circa) intercorso tra il conteggio iniziale effettuato su sangue periferico e l’inizio della procedura aferetica. Non si è evidenziata una correlazione significativa tra CEedetà(p=0,206),tra CEepatologie(p=0,775)etraCEe sesso (p=0,604). 145 D Marenchino et al. The variables potentially influencing the CE were analysed using one way analysis of variance and a correlation-regression test. Results The total number of CD34+ cells harvested in a single procedure ranged from 0.12-43.5 x 108 (mean value 5.5), corresponding to 0.26-62.1 x 106 cells/kg of body weight (meanvalue8.1). As expected, a statistically significant correlation was found between the number of CD34+ cells mobilised into the peripheral blood and the number harvested (p<0.0001). The extent of the mobilisation was influenced by the diagnosis, being greater in patients with lymphomas than in those with the other diseases considered (p= 0.01) (Figure 1), while it was influenced negatively by age (p<0.001) (Figure 2). These data are summarised in Table III. Collection efficiency (CE) ThecollectionefficiencyoftheCD34+cellswas9-310% (mean value 68.9%). The finding of values greater than 100%,albeitoccasional,wasprobablyduetoarapidincrease in the number of CD34+ cells in the period (of about 5-6 hours) between the initial count carried out on the peripheral blood and the start of the apheresis procedure. There was not a significant correlation between the CE and age (p=0.206), between the CE and the underlying disease (p=0.775) or between the CE and gender (p=0.604). Factors influencing the CE The CE was negatively affected by both the CD34+ cell count in the peripheral blood (Figure 3) and by the preapheresis leucocyte count (Figure 4). In a further analysis, patients were divided into two homogeneous groups on the basis of white cell count (less thanormorethan15,000/µL)andcyclevolume(500mLand ≥ 1000mL) and then the CE compared. The results are reported in Tables IV and V. On the basis of the statistical analyses it was found that the CE decreased in relation to higher levels of white blood cells (p < 0.0001), after having chosen a cut-off value of 15,000/mL, higher numbers of CD34+ peripheral blood cells (p<0.0001) and greater cycle volume (p<0.001). Side effects All the PBSC collection procedures were completed as planned with no interruptions due to technical problems or 146 Fattori influenzanti la CE La CE viene influenzata negativamente sia dalla conta delle cellule CD34+ nel sangue periferico (Figura 3), sia dalla conta leucocitaria pre-aferesi (Figura 4). Per un ulteriore approfondimento, i pazienti sono stati suddivisi in due gruppi omogenei sulla base della conta dei globuli bianchi (inferiore o superiore a 15.000µL) e del volume ciclo (500 mL e = 1.000mL) e sono stati messi a confronto i differenti valori di CE. I risultati sono riportati in Tabella IV e Tabella V. Sulla base delle indagini statistiche effettuate si evidenzia una diminuzione della CE in corrispondenza di valori più elevati di globuli bianchi (p<0,0001), dopo aver scelto un valore differenziale di 15.000µL, di CD34+ periferiche(p<0,0001)edivolumeciclo(p<0,001). Effetti collaterali Tutte le procedure di raccolta di PBSC sono state portate a termine come programmato senza interruzioni dovute ad inconvenienti tecnici o ad eventi avversi per i pazienti. Gli effetti collaterali più comunemente osservati riguardavano ematomi di grado lieve nella sede della venipuntura e parestesie prevalentemente in regione periorale (18%). Tale sintomatologia, legata ad una transitoria ipocalcemia indotta dalla presenza di citrato di sodio nella soluzione anticoagulante-conservante (ACDA), scompariva rapidamente con la somministrazione endovena di calcio gluconato. Discussione e conclusioni La raccolta di un numero sufficiente di cellule staminali da sangue periferico è determinante per la buona riuscita del trapianto e per una ricostituzione emopoietica stabile nel tempo. Alcuni studi recenti2,4 suggeriscono che una quotadicelluleCD34+>5x106/kgriduceiltempodiaplasia midollare ed assicura una ricostituzione emopoietica rapida e completa dopo trapianto di soli progenitori emopoietici circolanti. Per quanto riguarda la raccolta delle cellule CD34+, è indispensabile ottimizzare la fase di reclutamento attraverso lo studio della cinetica di mobilizzazione ed i protocolli in uso devono consentire una standardizzazione della procedura12. L’obiettivo da raggiungere, in termini di dose di cellule sufficienti al trapianto, è quello di effettuare 1-2 leucaferesi al massimo, limitando così l’impegno del paziente, i possibili effetti collaterali della procedura e i costi. These symptoms, related to the transitory hypocalcaemia, induced by the presence of sodium citrate in theADC-Aanticoagulant solution, disappeared rapidly following intravenous administration of calcium gluconate. Discussion and conclusions The collection of a sufficient number of PBSC is fundamental for the good outcome of a transplant and for stablehaematopoieticreconstitutionovertime.Somerecent studies2,4 suggest that a dose of >5x106/kg CD34+ cells reduces the time of bone marrow aplasia and ensures rapid, full haematopoietic reconstitution after transplantation of only circulating progenitor cells. As far as concerns the collection of CD34+ cells, it is essential to optimise the recruitment phase by studying the mobilisation kinetics and the protocols used must enable standardisation of the procedure12. The aim with regards obtaining a sufficient dose of cells for transplantation is to perform one or, at most, two leucaphereses, thus limiting the patient’s stress, possible side effects of the procedure and costs. This study showed that mobilisation of CD34+ cells is statistically significantly different, depending on the diagnosis and the age of the patients. In detail, mobilisation was significantly better in patients with lymphomas that in those with other pathologies, while increasing age had a negative effect. In contrast, the efficiency of the procedures was not statistically different in relation to the diseases treated, the age of the patients or their gender. Our analysis clearly demonstrates that the CE is influenced by both the cycle volume and by the preapheresis white cell count. In particular, reducing the cycle volume to 500 mL, for the same time used, improved the efficiency in all the procedures considered, reaching a mean value of 75%, while there was a strong inverse correlation betweenCEandpre-apheresiswhitecellcount.Theanalysis of the results having divided patients into two groups on the basis of white cell count (below or above 15,000/µL) andcyclevolume(500mLor ≥1,000mL)andcomparingthe 148 superioreneilinfomirispettoallealtrepatologieconsiderate, mentre è influenzata negativamente dall’età. Per contro, l’analisi dell’efficienza delle procedure non evidenzia variazioni statisticamente significative in rapporto alle patologie trattate, all’età ed al sesso dei pazienti. La nostra analisi dimostra con certezza che la CE è influenzata sia dal volume ciclo sia dalla conta leucocitaria pre-aferesi. In particolare, la riduzione del volume ciclo a 500 mL, a parità di tempo impiegato, ha permesso di migliorare l’efficienza in tutte le procedure considerate raggiungendo un valore medio del 75%, mentre esiste una marcata correlazionenegativatraCE econtaleucocitariapre-aferesi. Lavalutazionedeirisultatieffettuatasuddividendoipazienti in due gruppi sulla base della conta dei globuli bianchi (inferioreosuperiorea15.000µL)edelvolumeciclo(500mL e maggiore/uguale a 1.000mL) e mettendo a confronto i differenti valori di CE ha dimostrato che, nelle procedure effettuate su pazienti in cui la conta leucocitaria pre-aferesi risultavainferiorea15.000/µL,laCEharaggiuntounvalore medio del 67% nel gruppo trattato con volume ciclo maggiore/uguale a 1000 mL, valore che si è ulteriormente incrementatoriducendoilvolumecicloa500mL(V.M.=86%). In conclusione, il separatore cellulare COBE SpectraAutoPBSC, come già evidenziato in altri studi9,12, permette di ottenere elevate efficienze di raccolta delle cellule CD34+. A conferma di ciò, in tutte le procedure di raccolta di PBSC effettuate presso il nostro Centro, iltarget terapeutico è stato comunque raggiunto con un numero minimo di sedute ( valore medio 1,66/paziente, range 1-3). La regolazione ottimale dei parametri procedurali in considerazione anche della conta leucocitaria pre-aferesi, si può tradurre quindi in una più elevata qualità dei risultati e in un minor impegno per i pazienti trattati, insieme a un miglior utilizzo delle risorse. In questo studio abbiamo analizzato l’incidenza di alcuni parametri sull’efficienza di raccolta (CE) del separatore cellulare Cobe-Spectra dotato del software Auto-PBSC, nonché l’influenza che l’età dei pazienti e la diagnosi possono avere sulla mobilizzazione e sulla raccolta delle cellule CD34+. Materiali e metodi. Sono state analizzate retrospettivamente 233 procedure di raccolta di PBSC effettuate su 140 pazienti (80 maschi e 60 femmine di età compresa tra 17 e 70 anni) affetti da differenti neoplasie ematologiche. Tutti i pazienti sono stati sottoposti a trattamento chemioterapico a scopo di mobilizzazione, con protocolli differenti rispetto alla patologia di base ed a somministrazione di fattore di crescita G-CSF. Risultati. L’analisi dei dati dimostra che l’entità della mobilizzazione è influenzata dalla diagnosi, raggiungendo i valori più elevati nei linfomi rispetto alle altre patologie considerate, mentre è negativamente influenzata dall’età, pur esistendo comunque una correlazione statisticamente significativa tra il numero di cellule CD34+ mobilizzate nel sangue periferico e quelle raccolte. Inoltre, si è evidenziato che valori più elevati di globuli bianchi (superiori a 15.000µL), di CD34+ periferiche e di volume ciclo si associano ad una diminuzione della CE.. Conclusioni.
