Main subjects of research: 1) Correlations (morphological, immunohistochemical and molecular
biology features) between human developing peripheral sympathetic nervous system and neuroblasticassociated tumours; 2) Effects of pharmacological treatments on experimentally-induced type I diabetes

Pathology;
1) My research, until becoming P.h.D. in Pediatric Neuroscience, concentrated in the identification of
Schwannian-like cells encircling and guiding the migrating neuroblasts of developing peripheral sympathetic
nervous system. Soon after, I studied the comparative expression of molecules in the developing sympathetic
peripheral nervous system and in its associated neuroblastic tumours. The expression of cathepsin D in
neuroblastoma recapitulates that observed in embryonal and fetal neruoblasts. The understanding of the
molecular mechanisms regulating cellular differentiation of developing neuroblasts may provide crucial
informations for diagnostic and therapeutic strategies in human neuroblastic tumours.
2) I have studied the morphological changes induced in tissues from NOD mouse and DP-BB rats,
previously treated with several drugs in order to prevent diabetes mellitus. Other similar studies were
performed on experimental prevention of immunological disorders (Lupus EritematousSystemic-like
syndrome; hepatitis) in MRL-lpr/lpr mice. All these studies were crucial in establishing the efficacy, the dose
and time of treatment. My collaboration was crucial in identifying placental thrombosis as main pathogenetic
event in the fetal loss after passive transfer of mouse lupus monoclonal or human polyclonal anti-cardiolipin
antibodies in pregnant naive BALB/c mice.
3) The interest in tumour pathology is concentrated in identification of previously undescribed variant of
benign and malignant lesions. In this regard I first recognized the so-called “meningioma-like” variant of
follicular adenoma” and the atypical spindle cell metaplasia in nodular goiter. My current research interest
is concentrated on the identification of clinically significant biomarkers in thyroid tumours. I have identified
immunohistochemical markers highly sensitive and specific for papillary thyorid carcinoma (PTC). These
antibodies can be used as ancillary tools in those cases in which a differential diagnosis between a benign
lesion versus PTC is difficult on a solely morphologically-based approach. These studies, using antibodies
anti-mucins and anti-keratan sulphate, have shown, respectively, that mucin-1 and thyroglobulin and
transferrin undergo transformation-dependent post-translational modifications in their glycosilation pattern.
These novel findings represent a rational basis to exploit these changes as diagnostic and therapeutic
strategies in vivo.
4) In the field of breast pathology, I gave an important contribution in better defining the clinico-pathological
features of the fibro-myofibroblastic lesions arising from mammary stroma. In this regard I studied the
“benign spindle cell tumours of mammary stroma (myofibroblastoma; spindle cell lipoma; solitary fibrous
tumour)”, hypothesizing a unifying histogenesis from a common mammary stromal stem cell capable of
mesenchymal multidifferentiation along a fibro-myofibroblastic, leiomyomatous, lipomatous, osseous and
cartilaginous cellular line. Recent cytogentic studies have confirmed my morphologically and
immunohistochemically-based idea. These studies represented the basis for the current classification of such
tumours in the last W.H.O. edition dedicated to breast and genital neoplasms. In addition I first hypothesized
that this category of breast tumours show several clinical, morphological and immunohistochemical
overlapping with the category of
the “stromal tumours of the lower female genital tract
(angiomyofinroblastoma; cellular angiofibroma; superficial myofibroblastoma)”, suggesting that all these
neoplasms are histogenetically-related lesions, for which the comprehensive term of “benign stromal
tumours of the female hormonally-responsive stroma” is appropriate. With regard to other fibromyofibroblastic lesions, I first showed that mammary stroma, under particular conditions, can undergo
myofibroblastoma-like and deciduoid-like changes. Due to the worrisome morphological features acquired in
these circumstances, especially the deciduoid-like appearance in diabetic mastopathy, pathologist should be
aware of this possibility to avoid confusion with an invasive high-grade carcinoma.
Finally, Istudied the clinico-radiological and pathological features of a series of primary fibromatosis,
desmoids type, of the breast. Recognition of this unusual entity is crucial because it is a clinically and
radiologically mimicker of invasive carcinoma.
5) My studies were concentrated on the soft tissue lesions, primarily arising in visceral and parenchymal
organs. Accordingly, I have described several soft tissue tumour and tumour-like conditions in the breast
(solitary fibrous tumour; fibromatosis; spindle cell lipoma), kidney (cortical fibroma; rhabomyosarcomatous
differentiation in renal cell carcinoma), lung (low-grade fibromyxoid sarcoma), salivary gland (low-grade
myofibroblastic sarcoma), colon (well-differentiated liposarcoma), lymph nodes (angiomyomatous
hamartoa; primary amyloidosis), adrenal gland (angiosarcoma), thyroid (angiosarcoma).
6) In this field of pathology, I have first described two rare cases of “mesenchymal rhabdomyosarcomatous
hamartoma”in the oral cavity and a previously unrecognized myofibroblastic polypoid tumour of the
tonsillar pillar, labelled “angio-myofibroblastoma-like tumour” which looks like embryonal
rhabdomyosarcoma, botryoid variant. The recognition of both entities is crucial to prevent a useless
chemiotheraphy treatment. I collaborated in a large series-based clinico-pathologic study on anaplastic
lymphomas in pediatric age. I described the rare possibility that collecting duct carcinoma of the kidney may
present in pediatric patient, sharing a worse prognosis with similar tumours occurring in adults.
I first described the possibility that desmoplastic fibroblastoma (collagenous fibroma) is a benign tumour
that can be encountered even in pediatric patients. I collaborated with an international study acting to identify
striated muscle cells in non neoplastic embryo-fetal normal and pathological human lungs.
Finally, I collaborated with international research groups in identification of molecular profiles for childrens
affected by tuberous sclerosis complex.
7) Recently I concentrated my interest in the problems for a correct pathological classification (TNM) of
colorectal cancer. In this regard, in collaboration with other Institutions, we have shown that 12 lymph
nodes recommended by the current TNM is not a sufficient number to guarantee patient a correct stadiation.
In fact, if less than 26 lymph nodes are recovered in rectal cancer, there is the possibility that about 20% of
T3No patients are understaged. In addition, we have a critical view about the utilization of TNM5 in place
of TNM6, as recently suggested by U.K. pathologists, because the interpretation of pericolorectal tumour
deposits (PTDs) as lymph node metastasis is extremely arbitrary, and based only on PTD size (<3mm;
>3mm).Our opinion is contained in two recent “Letter to Editors” published on Virchows Archiv and
Histopathology.
