Le modalità di trasmissione e prevenzione, i segni clinici ed il trattamento di questa purtroppo diffusa malattia dovrebbero entrare nella sfera d'attenzione di ogni proprietario di cavalli. Il ciclo di vita del Sarcocystis neurona coinvolge un ospite dominante, l'opossum, ospiti intermedi (gatti, puzzole, armadillo ed orsetti lavatori), ed il cavallo, che costituisce l'ospite finale. L'ospite finale non espelle l'organismo e quindi non può diffondere la malattia. Gli Opossum espellono l'organismo nelle loro feci, fatto che può quindi contaminare gli alimenti degli ospiti intermedi. Una volta ingerito dall'ospite intermedio, questo protozoo si stabilisce nei muscoli, dove causa pochi danni finché l'animale muore. Once eaten by the intermediate host, this protozoa takes up residence in the muscles, where it causes little harm until the animal dies. Sarcocystis neurona, and less commonly Neospora hughesi, are the two organisms that cause the disease. Per il momento le due principali misure di profilassi sono l'esclusione degli opossum dalle aree frequentate dagli equini e mantenere il cavallo in buona salute e quindi immunocompetente. Recinzioni e conservazione adeguata di fieno e granaglie aiuteranno ad impedire agli opossum di defecare nelle zone di pascolo. The protozoa may travel in the circulatory system to the central nervous system where it causes inflammation and tissue damage. Measures can be taken to reduce the risk of exposure however it is very difficult to totally eliminate the possibility of equine contact with the organism. Ideal prevention would be to effectively vaccinate horses against the two organisms that cause the disease. Currently there is no vaccine that provides adequate protection. At this time the two key preventative measures are exclusion of opossums from equine areas and maintaining a healthy immunocompetent horse. Adequate fencing and storage of hay and grains will help prevent opossums from defecating in feed areas. Attempting to maintain a healthy and strong immune system will also help horses fight infestation. It is critical to have a complete neurological evaluation performed by a veterinarian to rule out other diseases in an effort to make a correct EPM diagnosis. Diagnosis is made by the presence of clinical neurologic signs, ruling out other neurologic conditions, and diagnostic blood or cerebrospinal fluid tests. There are four to five different methods for laboratory testing in an effort to diagnose EPM. These methods search for evidence of the protozoa or assess the horse's immune response to the organism. Several different products are approved by the FDA for the treatment of EPM. Ponazuril (Marquis) comes in the form of a paste and is administered over a thirty day period. Diclazuril (Protazil) is the newest product on the market. It comes in a pelleted form and is top dressed on the feed. It is also given over a one month period. Treatment may be extended if the horse is improving but has not returned to normal. Alternative treatments for EPM exist and more research is being conducted in the field of antiprotazoals combined with products 270 | 271 AH Te n n e s s e e Ve te r i n a r y If the affected area is localized to the cervical region radiographs are examined to rule out developmental or degenerative arthritic conditions ni. Antiinfiammatori come Flunixin meglumina (Banamina) e dimetil solfossido (DMSO) sono importanti per aiutare a minimizzare l'infiammazione causata dal trasferimento e morte del protozoo nel sistema nervoso centrale. Immunomodulatori come il levamisole possono aiutare a rafforzare il sistema immunitario nella sua battaglia contro i protozoi. La somministrazione di supplementi di Vitamina E può inoltre aumentare la velocità e l'efficacia della guarigione dei nervi coinvolti. La prognosi per la guarigione dipende dalla gravità del caso e del danno neurologico. La conoscenza e consapevolezza dei segnali clinici è determinante per ottenere una diagnosi ed una cura precoci, e di conseguenza una guarigione completa.
